On this episode of Dentist Brain Candy, we continue our discussion of the book, Limitless by Jim Kwik. Today I'm discussing Part IV: Limitless Methods. Chapter 11 of the book is devoted to the discussion of focus. We all have so many things competing for our attention; it can cause anxiety and, worse yet, loss of focus, leaving us scattered and in a place where we accomplish less.
I walk you through Dr. Andrew Weil's method for breathing to recenter yourself. Then we'll look at your to-do list, and how you might use some anti-procrastination tools to complete a task you’ve been putting off so you can move into a more productive flow.
We'll also talk about Chapter 12, which is about studying. Jim Kwik teaches that the most successful people are lifelong learners. Listen in as I walk you through seven of Jim's simple habits to unlimit your studies, including how you can use Baroque music for better concentration when memorizing facts or for reading retention.

It can be hard to be the cheerleaders in our own lives, but it’s true that positive emotions broaden our attention and thinking, seed human flourishing, and trigger upward spirals towards greater well-being.
The book Limitless by Jim Kwik is all about “limiting beliefs” and how they might be holding us back from achieving our biggest dreams.

Robotic assisted drilling systems (RADS) are cutting-edge and very interesting. I share my thoughts on how they could create breakthroughs in dental surgery and also cause potential complications.

In 2018, Joe DiMeo fell asleep at the wheel. In the resulting accident, he suffered third-degree burns over 80% of his body. He was left without eyelids, ears and fingertips. But in August of 2021, a team at NYU Langone Health performed a face and double hand transplant on Joe. Was the procedure successful? How has his life changed since the surgery?

This week we discuss some pretty interesting articles from the Journal of Oral & Maxillofacial Surgery the first one discussing the Akinosi nerve block technique versus the inferior alveolar nerve block technique for beginners to see whether there's an ease in learning and decrease complication rates for beginners. I didn't ask us the use of recombinant human bone morphogenetic protein for use in grafting of the Alveolar cleft. I then proceeded on to discuss where there's a correlation between an impacted mandibular third molar, the type of impaction and mandible fractures and then the type of mandible fractures and believe me when I tell you there is. You're going to have to listen to this episode to find out what that correlation is.
This week in Dental news to abuse will start off by discussing how dental offices can win back lost patience as well we'll talk about a new retraction paste that's made out of a combination of 15% aluminum chloride and kaolin Clay will also discuss a new product available called Moisyn they can help patients with xerostomia or dry mouth. I will also talk about an implant post-operative care system and a handheld intraoral x-ray system and a new water line filter guaranteed to keep your water lines clean and bacteria free.
This week we go over weight loss with jaw fractures and whether or not doing a closed reduction or an open reduction makes a difference in this weight loss. The average patient with a mandibular fracture loses approximately 5% of their body weight but if it made it to the ICU they lose up to 9% of their body weight. We also will discuss obstructive sleep apnea and whether there's an increased incidence of obstructive sleep apnea with a mandibular setback surgery using a vertical ramus osteotomy. We will also discuss the use of dental implants to facilitate jaw surgery in a complex restorative case in an edentulous maxilla.

In this episode I discussed three interesting articles the first article discusses the quality of life of patients with jaw and facial deformities before and after jaw surgery and the notable unremarkable quality of life improvements from both a psychological and functional component. I also discuss whether antiplatelet drugs need to be stopped and whether the increase the risk of bleeding after dental implant surgery and a very good crossover study supporting the conclusions of this and I talked about spiking your local anesthetic with dexmedetomidone in an attempt to increase the onset of action the total length of duration and improve the properties of lidocaine local anaesthetic
In this episode, I discuss the increasing use of the Intensive Care Unit for a. Digenic infections over the last decade in Australia. I also discuss the conservative treatment of medication-related osteonecrosis of the Jaws and the success rates and spontaneous resolution of this process that you can expect from this modality and this treatment course. I also discussed the use of a C-Arm intraoperatively vs CT scans for complex facial fractures.
In this episode, I start by discussing the periodontal stability on the distal of the mandibular second molar after a coronectomy on mandibular third molars. This is a three-year study and over the last several years the popularity of a coronectomy for high-risk third molar situations with close proximity to the inferior alveolar nerve begs the question as to success and possible risks. This study aims to discuss whether or not there is peridontal pocketing on the distal of the second molar when you leave the roots behind with the extraction of third molars. I also discuss the use of intravenous ibuprofen versus intravenous Tylenol to reduce postoperative pain and narcotic consumption after third molar surgery. You don't want to miss this episode and I think you'll find it very useful and applicable to everyday practice.

Orthognathic surgery has an interesting progression through time that any dentist or surgeon should know about and will find well summarized in this podcast.

I also discuss how our National Association will help with an application for the American College of Surgeons fellowship and the requirements for that. I also discuss the changes in the American Board of Oral & Maxillofacial Surgery certification process and for maintenance of that board certification which is profound and significant. I also discuss the significant impact that vaping has on healing and the rationale and study that helps prove that healing from vaping and cigarette smoking are equivalent. This is an action-packed episode full of interesting information that you must check out!
In this episode, I start with a discussion about low-level laser light therapy and helping nerve recovery after a bilateral sagittal split osteotomy. There is a significant risk of permanent numbness after jaw surgery particularly lower jaw surgery and this study looks at a novel therapy of low-level laser light therapy to see if recovery can be optimized for patients I also discuss the fact that the impacted third molars can damage and cause external root resorption of mandibular second molars and what those risk factors are. I also discuss the incidence of this occurring in the study. Finally, in this episode, I discuss the psychological impact of doing surgery first for patients with Orthodontics jaw surgery versus the conventional way.
In this episode, I discuss the misdiagnosis of a swelling on the roof of the mouth that turned out to be cancer. This was misdiagnosed twice and can have serious implications for patients. In addition, we talked about the incidental radiographic finding with a panorex showing calcifications in the Carotid artery and its association with obstructive sleep apnea heart attacks and strokes. We also discuss using dental implants to help a patient with severe erosive lichen planus. Lichen planus can be a painful mucosal condition making Dentures very difficult or impossible to wear. Dental implants offers the opportunity to elevate the appliance off of the mucosa making impossible to wear Prosthetics without pain.
This week I discuss articles out of the Journal of Oral & Maxillofacial Surgery in December of 2018. We will discuss a pork larvae infestation of the oral mucosa in three separate patients including both the clinical presentation and management of these lesions. We shall discuss how to prevent periodontal defects distal to mandibular third molars after the extraction of wisdom teeth as well as how to predict the recurrence of regional disease after primary resection of oral squamous cell carcinoma. In addition to this, we shall also discuss the utility of viewing a third molar video particularly a third molar informed consent video and whether this helps to decrease patients anxiety and whether an Emergency Room Physicians are able to accurately diagnose, order CT scans and consult for specialists for head and neck infections and particularly odontogenic infections.

The administration of Botox is well within the scope of practice and I think dentist, in general, are fantastic with giving shots and should consider offering the service with the proper training. In this episode, I discuss the all on four concepts and exactly what that means and review an article from the International Journal of Oral & Maxillofacial implants 2018 titled the biomechanical comparison of different implant inclinations and cantilever links in All-on-4 treatment concept by three-dimensional finite element analysis. I don't want you to worry, I do get into this article but I will promise not to bore you to death and make this too heavy but more clinical in nature.

There is now an alternative fixed restoration that is not a hybrid restoration. Zest International has developed an abutment and a system called the locator FTX system. In this episode we discuss using four or more implants in either the maxilla or mandible and specifically 13 cases that were studied for 3 years showing that this is a successful and reasonable treatment modality. Anybody treating patients with fixed full Arch Restorations needs to be aware of this treatment concept.

We've all had or soon to have a patient with a fixed restoration on four more implants and one of those key implant fails. In the past the only choice and option available was to toss out the final restoration and place a new implant and make a brand new fixed hybrid restoration. In this episode, I discussed a novel new treatment approach that is a salvage procedure so that the final restoration does not need to be remade.

Patience with biopsy proven oral lichen planus we're given a questionnaire in this Prospect of study to see what the triggers were and if avoidance of the triggers was helpful for these patients. A list of the typical and most common triggers is also given for your review. I also discuss the odds and likelihood and reasons for plate removal with jaw surgery. This discussion will help you have a better feel for when patients will have plates removed and if they are likely to have an improvement in signs and symptoms with removal of those plates.

I also discuss an article that did a benchtop study on the neurotoxicity of articaine versus lidocaine and found that there was no difference in toxicity which would suggest that Articaine may be acceptable for inferior alveolar or mandibular nerve blocks

Their survey asks questions about prescribing patterns and attitudes of oral surgeons. Over 300 people responded to the survey and the results are quite interesting.

In this episode, I go over last month's International Journal of Oral & Maxillofacial implants and discuss three articles that I thought were relevant. One article was on the effectiveness of platform switching to preserve bone levels. Another article was on whether or not bone levels and success rate of implants were affected by moderately controlled diabetics and the third article was on two different sized elevation techniques one using a trephine and one using an osteotome and bone grafting material to see if there was a difference in success rates and the amount of bone that was generated.

This weekend I was on call and so I discuss three cases that I came across. The first case had a history of a dental extractions approximately 3 weeks ago that did not resolve and was admitted with gas in the pterygomandibular and the buccal space. The second case was a trauma case of someone that was assaulted with fists and had oss of consciousness and a small subdural bleed. This patient was taken to the main operating room and had open reduction internal fixation of a lefort 1, lefort two and right side zygoma fracture. The third case that I discuss is a young man that tripped and fell on a cable and broke his jaw in two places and was taken to the operating room where he was wired closed and then fixated with plates and screws. Using an opioid-sparing technique in your office is expensive and has a significant financial impact. In this episode, I start by discussing antibiotic resistance and its implications in society as well as the reasons for this increase in antibiotic resistance. We discussed the causes including easy access in developing nations over administration prescription by healthcare providers as well as providing antibiotics to prophylactically livestock. I then transition nicely into clostridium difficile infections and discussed the most likely antibiotics and the odds ratio of developing a C. difficile infection from antibiotics and then treatment for a C. difficile infection. This includes an FMT or poop transplant.

In this episode, I discuss a study that compares digital impressions with traditional PVS impression with dental implants. They use an open tray impression technique and compare accuracy. The second article I review is on Intra Operative CT scans with Orbital Fractures. This is a great article showing that in this retrospective analysis they change the management 44% of the time. I also review an article that supports the knowledge and what you probably assumed; taking 3rd molars out on people over the age of 65 has more complications than those between 15-20 years of age.
This weeks episode discusses white patches and their presentation in the oral cavity. We discussed hereditary sources we discussed the who definition of a leukoplakia and frictional chemical and other sources of white patches on the oral mucosa. We discussed the incidence of conversion into a dysplasia or carcinoma in situ or Frank cancer. There is quite the section on Hormone Replacement Therapy for women that everyone should hear. I then discuss a large oral antral communication that took two attempts at closure. The first attempt was with a simple buccal sliding flap and that quickly broke down and failed. The second successful surgery including using a buccal fat pad flap to advance over the opening. This is a must read for all Americans. I discuss and interesting article about surgical site infections with jaw surgery and come away with actionable points to help prevent those. The interesting case shows a cool radiograph of an impacted #20 that destroyed #19 and the challenge of that extraction.

I met this nice fella through the emergency room from an ATV accident who sustained a right mandible fracture and right zygoma fracture as well as an ear laceration. The ear laceration was repaired by the emergency room and I noted that after the sutures were removed and the head wrap was removed that there was a hematoma. I proceeded to evacuate the hematoma and the patient did well with all of his injuries post operatively.

I then move on to a literature review from the Journal of Oral and Maxillofacial Surgery on central giant cell granulomas. This gets us all up to date on some of the new novel approaches and factoids in a very direct, rapid fire manner. The interesting case is worth a visit to the website to check out the pictures and radiographs. This crazy wisdom tooth with a large cyst was up at the floor of the orbit on the right side and was removed, biopsied and came back as a dentigerous cyst. Patient is fantastic during and after the procedure.

Thursday of last week was an interesting day and I thought I’d share what I was up to that day. I completed a Le Fort I procedure and did a modification of her previously LeFort I that needed some expansion after some poor healing. The cuts of wisdom teeth as well and then I had an interesting trauma case that ATV accident with a right zygoma and the right mandible fracture.

This week I go over two articles that are very relevant to everyone in dentistry today. Lichen planus, especially the erosive variety is very annoying and affects the quality of life for patients and we can help them. Anyone doing 3rd molar surgery or seeing patients and referring them for wisdom teeth should and now will know that flap design does not affect the health of the second molar long term.
I start off with an Irish Proverb for the quote of the day just to get everyone loosened up and then discuss the fifth and final installment of Wheelbarrow Profits by Jake and Gino. Right now the market is hot for Multi-unit Family Investing and so getting in may not be a good time but learning about this is never a bad time. The article about lip cancer and specifically about squamous cell carcinoma is over a 25-year span and has some solid take home points. The interesting case is that of a traumatically fracture tooth number 9 and a dentoalveolar fracture as well and how I approached the treatment with a significant gingival injury.

INTERESTING CASE
This week I have several items to discuss including a new company that is making a 3D identical titanium and Zirconium dental implant that is designed to fill the socket of an immediately extracted tooth. This would be and will be an amazing technique and I am fascinated by the idea. The episode ends with some dental fun facts!
In this episode, I continue with our book review of Wheelbarrow Profits and then move to a very interesting study that used 5% Fluorouracil instead Modified Carnoy’s solution to decrease the recurrence rate of keratocystic odontogenic tumors with a decrease in paresthesias. If you treat these lesions you will not want to miss this discussion. The Interesting Case shows the surgical approach to removal of a mandibular bicuspid that is an extra tooth.
