Using Fault Trees to Conduct Root Cause Analysis (RCA)podcast episode with speaker Chris JacksonWhat is a root cause' of failure? It is something that you did (or didn't) do that ultimately resulted in failure. It's not corrosion (that's a physical process that follows the laws of nature that you can't change). It's not a porous seal that allows moisture to enter your product to cause corrosion (that's a defect or fault that you or your team made happen). Root Cause Analysis (RCA) is the process where we systematically work out what things we did or didn't do that allowed failure to occur, and Fault Trees are often very helpful in finding this root causes. Understanding (how bad) the Exponential Distribution (is)podcast episode with speaker Chris JacksonWe often use probability distributions to help us characterize the likely values a random variable will have. This includes the random variable we call time to failure' or TTF, which is how long something works before it fails. Failure is a random process, but just because it is random, doesn't mean it's unpredictable. Products that wear-in will have vastly different probability distributions for TTF when compare do products that wear-out. So you need to have a good idea of the probability distribution of the TTF of your product when it comes to reliability engineering. But we often see in textbooks and standards the exponential distribution' simply being assumed as a good' TTF probability distribution. But is it? And what happens when it isn't but you use it anyway? Join this webinar to find out!

Using Monte Carlo Simulationto Make Hard Reliability Analyses Easypodcast episode with speaker Chris JacksonSometimes the equations we need to model reliability are just so complicated that we simply avoid them, or use other equations that are simpler but model the wrong thing (see some of my earlier webinars!). But it doesn't have to be this way. Monte Carlo simulation is so simple that anyone who has access to Microsoft Excel can use it. What this means is that we don't need to use complicated equations (like those use to model things like switching systems) and instead can use Microsoft Excel to help us get approximate (but really accurate) answers. Fundamentals of Control Chartspodcast episode with speaker Fred SchenkelbergOn one of my first vendor visits, they proudly showed us the control charts behind glass near the conference room. They were nicely printed, all in perfect control, and rather odd. A control chart belongs on the production line, where the operators make the measurements. They are of little value when automatically collected and printed out once a month for display-only purposes.
Statistical control charting (SPC) is a method for monitoring and determining whether a process is in control or stable. It permits the team to identify opportunities for process improvement and provides the necessary evidence that a process is in control, if it actually is, as a prerequisite for calculating process capability.
Let's discuss control charts and how best to employ them. We will also examine how valuable an SPC program can be for the organization and customers.

Fundamentals of Safety Marginspodcast episode with speaker Fred SchenkelbergOne way to create a robust product or system is to consider all the stresses and design to withstand those stresses. We often do not know all the stresses, magnitudes, and ranges of variation. We know that many materials degrade with time, yet we do not know precisely how much or how fast. We also do not know what new stresses may arise or how stresses will change over time.
With so many unknowns, we turn to safety margins to address the unknowns. The process is simple enough, meaning we will design a product to withstand more stress than we expect. For example, if designing a bracket that is expected to carry a 100-pound load, we design a bracket capable of withstanding a 200-pound load. That doubling of capability is a 2x safety margin.
Let's examine safety margins and why they are one method to design robust products that also improve product reliability performance.

Fundamentals of Lessons Learnedpodcast episode with speaker Fred SchenkelbergWithin an organization or a team, we collectively know a lot. We, again collectively, have a wealth of information. And, if you follow my Mom's advice, we learn something new every day. We also forget a lot every day. When we forget something that we learned by making a mistake or error (learning the hard way), we are prone to repeat that mistake or error.
Organizations and teams also learn the hard way at times. Or someone recognizes a better way to accomplish something. Organizations also forget what they have learned. Either collectively, the organization's members forget' the lesson learned, or the folks that do remember the lesson move on to a new role elsewhere. For an organization, there is significant value in avoiding repeating previous mistakes or in continuing to benefit from a better way.
Let's discuss the idea and basics of a lessons-learned system. Plus, a few examples of how such systems go wrong. We will examine how FRACAS and CAPA systems are different. And finally, we will outline the best lesson-learned system I've seen in action.

What is WeiBayes Analysis?podcast episode with speaker Chris JacksonSome of us might have heard of the Weibull distribution. Some of us might have heard about Weibull analysis. Weibull analysis uses the Weibull distribution to help us visualize failure data in a really useful way. We can see' if our product is wearing out. We can see if our product is wearing in. We can estimate how many things will have failed by certain times. We can do all manners of other wonderful reliability engineering' things through Weibull Analysis. But then there is WeiBayes Analysis.' It combines Weibull and Bayesian analysis ( I know that doesn't mean much to most people). It can be really useful if we know how' something we are testing fails. Because if we do, then we can essentially feed a certain parameter into the analysis to help it on its way. This means that we don't need as much data (always a good thing in reliability engineering). But there are quite a few catches. Interested in learning about Weibayes analysis? Dealing with Small Sample Sizespodcast episode with speaker Fred SchenkelbergIt's rare to have sufficient samples for all the desired reliability work. It's common to have very few samples. So, how does one manage to learn what is necessary to learn from the few samples? How can you maximize the value of reliability work in the scant sample situation?
Let's discuss a few approaches and a process that enable you and your team first to have the right amount of samples. And what can be done when there are too few samples available? The trick is to start with what future decisions need better information and not start with how many samples you have.
There are three strategies to deal with small sample sizes. First, focus on the critical information only samples can provide and how valuable that information is to the team. Second, use other methods to create the needed information without using samples. Third, organize the various testing and measurements of the sample allocated to reliability testing along with samples or units assigned to other sections of the team to extract as much information as possible.

What is ‘fatigue’? … and no I don’t mean feeling ‘tired’.podcast episode with speaker Chris JacksonMany reliability engineers know what fatigue is (in addition to feeling tired). Fatigue is an example of a failure mechanism.' A failure mechanism is the physical, chemical or electrical process that results in failure. Fatigue is interesting in that it allows something to fail without it ever being exposed to stresses that are beyond its strength. How does this happen? Fatigue is based on cracks. Cracks magnify stresses at their tips. So cycling stresses can have the effect of incrementally (and sometimes very slowly) increasing the crack length at the tip. Fatigue has caused all manners of failures that have surprised us throughout the history of engineering. But now we have learned so much more about this really fascinating failure mechanism can cause steel, aluminium and other material to fail, even when the stresses they experience are tiny. Carl and I have found that there isn't a set of methods that we always use or recommend. There is no one-size-fits-all plan. We have found there exists a set of steps that we tend to follow as a guide. A framework for thinking through what needs to happen that improves the outcomes. Steps that add value.
Let's discuss the process of achieving reliability objectives. This may be a change in approach for some of you, for others, a chance to enhance what you already do. The idea is to explore in detail why we think achieving reliability objectives is best done using a process approach.

What is Supportability' and How Does it Relate to Reliability?podcast episode with speaker Chris JacksonWhat is supportability? Is it working out how many spare parts you will need (and when)? No. Is it working out how many maintainers or technicians you will need to keep your system working? Still no. What about working out what tools these maintainers or technicians need? No again. But many people think that this is what supportability' is. Supportability' is actually a characteristic of your product or system. Is the ability' for it to be supported.' And this can mean different things in different scenarios. It is also not a number.' If you have two comparable systems, but one needs fewer spare parts, is easier to maintain, needs fewer tools, has lots of sensors that give plenty of warning for maintenance, and so on than it might be more supportable. But the main thing you need to think about' regarding supportability is that because it is a characteristic of a product or system it has to be baked INTO the design. Then review the many possible ways to find or generate the information. Finally, select the method to create the best possible information within the given constraints.
As reliability engineers, we generate information for the use of decision-makers. It is how we influence decisions that create value. Having the correct information at the right time for the right people is the key to success.

Design for Reliability – Stressorspodcast episode with speaker Chris JacksonAny product is a collection of materials that have been engineered, shaped, and modified to become components and joints (e.g. weld and braze joints). The materials can degrade due to exposure to use conditions steel screws corrode when exposed to water, some plastics become brittle when exposed to sunlight, and coatings on surfaces can wear away.
Components and joints will stop functioning as required if their materials degrade too much. This is a problem if it leads to a reduction or loss of product performance before the end of a product's expected life.
Identifying the conditions to which materials are exposed and selecting materials that can withstand the exposure is a critical part of designing products that have good reliability.
During this event, I will discuss the identification of the conditions that can cause materials to degrade.

It's just a piece of paper that conveys you mastered some body of knowledge. You most likely also committed to abide by a code of ethics. Plus you may have committed to continuing education to maintain the certification.
Having a certification means you know the terms, definitions, techniques, and concepts concerning reliability engineering. That’s all.
Does it mean you are a professional? No.
So, why pursue a certification? Because it is good for you and reflects well on you.
Understanding the range of tools available to reliability engineers permits you to select and use the right tool at the right time. You, with some practice and experience, become efficient and effective as a reliability professional.
Will you immediately receive a pay raise? No. Will you handle bigger problems? Most likely.
Being a professional reliability engineer is more about what you accomplish than what is on the wall. Master the knowledge and use it wisely. Solve problems and help create reliable products and systems. That is what being a pro is all about.

How to Create an ALT Plan We cheat time. As reliability engineers, we are asked to peer into the future and predict the time to failure for our products and systems.
So, how do you go about setting up an accelerated life test? There are options that work, and some that do not.
Focus on the failure mechanism. That is the best advice I ever received learning about ALT design.
It is easy to apply a stress for some set time and declare the product is reliable. Yet, the result most likely is not useful. Applying a stress that actually accelerates the desired failure mechanism will get your meaningful results.
So, how do you go about designing an ALT that actually yields valuable information?
ALT takes many forms yet the results of the testing should reveal time to failure information. We should answer the question of how many units will fail over some duration? Unfortunately, even very poor tests will create an answer.
Let's discuss how you can craft meaningful ALTs that provide meaningful results that the science and engineering support.

For the past 75 years, we read books, gone back to campus, attended workshops, traveled to conferences, and participated in evening meetings. Today we have more options from more sources for our professional development.
Reliability is an engineering discipline that encompasses a broad array of tools and techniques, useful for answering durability and robustness type questions.
Product development teams often rely on reliability engineering professionals to guide, advise and manage reliability programs.
Reliability is a facet in nearly every function of an organization.
This implies the knowledge and skills required for the reliability engineer are comprehensive. The knowledge breadth may span aspects of material science to design constraints to warranty reverse logistics.
How do engineers become reliability professionals? What are the knowledge transfer options available to the reliability profession? How do we get started and maintain our knowledge?

Exploring Alternatives to MTBF You may already know my position on MTBF.
If not, in short, do not use MTBF at all, ever, in any form. So what should we use instead?
I suggest using reliability, the probability of success over a specific duration.
Let's discuss what will work for you.
Let’s explore MTBF in two parts.
First, let's share some stories about how the use of MTBF leads to misunderstandings and poor decisions. How bad can the use of MTBF really be? What have you seen and how much time have you spent educating others about MTBF?
Second, let me propose that we simply use reliability as a replacement metric.
The probability of success over duration. Right from the definition for reliability. It straight forward, the math is easy, and very few can misinterpret its meaning. For a bonus ass the cost per failure to tie the reliability metrics into business objectives language (money).
Finally, what to do if your organization or vendors are mired in the use of MTBF.
Let's discuss the next steps and what you can do today to minimizes the adverse impact of MTBF. Plus, let's share some success stories of steps various organizations are taking to improve their reliability programs simply by avoiding the use of MTBF.

Creating Meaningful Reliability Predictions Early and often during product development, the team needs to know the expected reliability performance of the current design. The values provide information to make decisions.
We have the ability to create meaningful estimates. Or, not.
The best time to estimate the reliability of a design is after all units have shipped and failed.
Thus we know when and why the units failed. This allows us to create an estimate that is fairly accurate. In practice we do not have this luxury, thus rely on a range of tools to create reliability predictions.
Let's start with the notion that parts count predictions are pretty much fiction. They do not provide a meaningful estimate of field performance.
Using an industry database to find and tally failure rates is a futile exercise.
Engineering judgment, field data, vendor experimental evidence, physics of failure models, and accelerated life testing provide a range of tools that we can use to create an estimate that has value.
Sure, these approaches may take more work than a quick parts count prediction, yet provide meaningful results.
Over the course of a project, we may have the large uncertainty of only engineering judgment.
Later we refine the estimates using improved models and analysis. The intent is to provide feedback to the team in an ongoing manner to enable the team to meet or exceed the reliability requirements.
Let's talk about what works and how to find the information you need to make reliability predictions.

Asking a Vendor for Reliability Data Selecting as a supplier for components or subsystems involves many aspects including the desired reliability performance.
Once selected the ability of the supplier to provide items that meet or exceed the reliability requirements relies on their understanding of the requirements and operational conditions related to the specific item within the system.
It also relies on the supplier’s knowledge of their own design and manufacturing processes as it related to the reliability performance.
Specifying reliability tasks, such as FMEA, HALT, ALT, demonstration testing, etc.
Does little to actually guarantee the item actually meeting requirements, unless the implementation of the activities is of value to the supplier. The specific activities described in other parts of this section outline how to evaluate the effective application of the activity.
One way to assist suppliers is to not make specific activities requirements.
The objective is to receive items that meet requirements for the end users, not to practice the use of various reliability related tasks.
Instead ask the supplier to provide the accumulated evidence, at specific points in the program, detailing the current estimates related to the reliability requirements.
The evidence presented is at the discretion of the supplier and evaluated by the program for suitability. Does the evidence come to the natural conclusions presented?
They should provide a logical, coherent argument.
