Form 20 is a common form used in workers' comp claims, but it can often be a little confusing for employers to fill it out properly. It is a statement of days worked and earnings of an injured employee which must be completed within thirty days after receipt of the claimant's hearing request or when weekly benefits are paid, as well as when a hearing or settlement conference is requested by the employer.
