In this episode I speak with James Greenblatt about orthomolecular psychiatry. A pioneer in the field of integrative medicine, James M. Greenblatt, MD, has treated patients since 1988. After receiving his medical degree and completing his psychiatry residency at George Washington University, Dr. Greenblatt completed a fellowship in child and adolescent psychiatry at Johns Hopkins Medical School. Dr. Greenblatt served as the Chief Medical Officer at Walden Behavioral Care in Waltham, MA for nearly 20 years, and is an Assistant Clinical Professor of Psychiatry at Tufts University School of Medicine and Dartmouth College Geisel School of Medicine.

An acknowledged integrative medicine expert, educator, and author, Dr. Greenblatt has lectured internationally on the scientific evidence for nutritional interventions in psychiatry and mental illness. Through three decades of practice and research, Dr. Greenblatt is a leading contributor to helping physicians and patients understand the role of personalized medicine for mental illness.

He is the author of eight books, including the best-seller, Finally Focused: The Breakthrough Natural Treatment Plan for ADHD. His updated edition of Answers to Anorexia was released in October 2021 and his newest book, Functional & Integrative Medicine for Antidepressant Withdrawal is available now.

Steve is the author of The Cannabis Manifesto and an originator of the wellness approach to understanding cannabis. He played a key role in the passage of Initiative 59, Washington DC’s medical cannabis law; the passage of Prop 64, California’s adult-use law; and successfully defended multiple legal attempts by US federal authorities to shutter the doors of Harborside.

DEFINITIONS

  • Psychotomimetic: Outdated term used to describe substances that mimic psychosis
  • Hallucinogen: A substance that generates hallucinations (does not accurately describe psychedelic experience)
  • Psychedelic: an experience in which the mind (soul) is fully manifested
  • Entheogen: a psychedelic substance specifically used for spiritual practice
  • Microdosing: the practice of using sub-psychedelic doses (5-25% of a full dose) for enhancement of mood, creativity, and energy

RECREATIONAL VERSUS THERAPEUTIC USE

  • These substances enhance sensory experiences and opens one to new ways of being in the present moment
  • Intention makes the experience what it is
  • We refer to this as Set and Setting
  • Mindset: what is your present mental and emotional state? What do you want from this experience?
  • Setting: Where are you? Do you feel safe? Supported?
  • Harm reduction is a safer approach than prohibition

CLASSIC PSYCHEDELICS

WHAT DO THEY TREAT?

  • Cancer related anxiety and depression
  • Treatment-resistant depression

  • A synthetic product of lysergic acid, which is naturally occurring in the parasitic rye fungus, Claviceps purpurea
  • An ergoline tryptamine with a rigid structure
  • Very potent with only 75-150 µg p.o. to illicit a psychedelic effect
  • Peaf effect in 1-2.5 hours
  • Total duration is 8-12 hours
  • 〖5-HT〗_2A and 〖5-HT〗_1A Agonist
  • Also interacts with dopamine and glutamate pathways

PSILOCYBIN

AYAHUASCA

  • “Vine of the sould”
  • A shamanic tradition originating in the Amazon region of South America
  • A tea brewed from the vine of Banisteriopsis caapi which contains β-carbolines which act as monoamine oxidase type-A inhibitors, and the leaves from the bush Psychotria viridis which is rich in dimethyltryptamine (DMT)
  • DMT can be smoked or intravenously injected for a 10-20 minute psychedelic experience, but it is not orally active. The MAO-I allows the DMT to reach the brain where it agonizes 〖5-HT〗_2A receptors producing a 4-6 hour experience.
  • When asked how they knew to combine these two plants, the indigenous people will say that the plants themselves guided them
  • The practice of ceremonially ingesting ayahuasca is protected as a religious sacrament in many parts of the world
  • Nausea and vomiting are common

MDMA

  • 3,4-methylenedioxymethamphetamine
  • Not a classic psychedelic because it does not produce vivid mystical experiences
  • Classified as an entactogen or empathogen
  • WARNING: Most classes of antidepressants lower the effects of MDMA and increase the risk of serotonin syndrome
  • Affects Alpha-2 receptors impairing thermoregulation, but also e

Saffron for ADHD

  • Baziar S, Aqamolaei A, Khadem E, et al. Crocus sativus L. Versus Methylphenidate in Treatment of Children with Attention-Deficit/Hyperactivity Disorder: A Randomized, Double-Blind Pilot Study. J Child Adolesc Psychopharmacol. 2019;29(3):205-212.
  • The standard of care in treating ADHD is the use of stimulant medications, the most popular of which is methylphenidate (Ritalin).
    • It works by blocking the reuptake of norepinephrine and dopamine in presynaptic neurons in the cerebral cortex and subcortical structures. NE and DA are the primary neurotransmitters responsible for learning, rewards, memory formation, and stimulation.
    • Side effects include insomnia, nausea, loss of appetite, anticholinergic effects (dry mouth, pupil dilation and blurred vision, hyperthermia), orthostatic hypotension, and arrhythmias.
  • Saffron (Crocus sativus) has been known as the world’s most expensive spice. About 90% of the world’s supply comes from Iran. In addition to its culinary application it is also used for its antispasmodic, antiseptic, antidepressant, anticancer, memory enhancement, anxiolytic, neuroprotective, and anticonvulsant effects.
    • The primary mechanisms of saffron are thought to be reuptake inhibition of NE and DA, antagonism of NMDA receptors, and agonist of GABA-α.
  • How the study was done
    • Randomized, double-blind, parallel-group clinical trial.
    • Tehran University of Medical Sciences
    • 50 Males and females between the ages of 6 and 17 with ADHD completed.
    • Patients were randomized to receive standard-dose methylphenidate (tid, at 30mg/day for kids >30kg) on a titration schedule or 30mg/day of saffron in three doses.
    • Outcome measure was a parent and teacher rating scale measure at baseline, week 3, and week 6 of treatment.
  • Results:
    • There was no statistically significant difference between parent or teacher ratings of either inattentive or hyperactive behaviors between the two groups. In other words, both saffron and methylphenidate improved ADHD symptoms equally well.
    • The saffron group reported fewer although not significantly fewer side effects.

My thoughts:

  • Stimulants lead to significant improvement in the quality of life for many people, however, they don’t work in everyone and many have undesirable side-effects.
  • Coming down for the medication is hard and can lead to mood disturbances
  • People can become tolerant to stimulants
  • They can become dependent on them in place of developing skills to manage their symptoms
  • This study was small, the first and only study of saffron for ADHD, and there was no placebo group
  • More studies should be done to confirm the findings
  • Saffron is safe, give it a try, and let me know what you think
