Our new Chair of the Department of Medicine, Dr. Kim Williams, presents his first Grand Rounds on the topic of Nutrition, Health Equity, and CV Mortality. First, Dr. Williams discusses some of the main issue in the United States including being its expensive and broken healthcare system. With the expectance of an increase in cardiovascular disease and stroke mortality in the US in the upcoming decades, particularly in minority groups, Dr. Williams believes our poor nutrition promotes and accentuates health inequities and ethnic disparities. Dr. Williams encourages the implementation of education around plant-based diets which are associated with lower rates of of systemic hypertension, hyperlipidemia, obesity, and diabetes.

Dr. Laura Bishop presents "Opiate Use Disorder in the Inpatient Setting: To Bup or Not to Bup with Dr. Bishop" by first examining the practice for adjustments to create space of destigmatizing patients with SUDS. She then discusses the symptoms of opiate withdrawal as well as the pharmacology of buprenorphine and its efficacy/safety in treating opiate use disorder. We here at Louisville Lectures are excited to share a new project with you that has been nearly two years in the making. When working with our incoming residents on procedure training, we noted that there were no resources that fit our needs precisely. We set about trying to define what the ideal training resource would look like. This included a first person perspective because, as everyone says, there is simply no substitute for seeing (and doing) it for yourself. We have attempted to give learners an opportunity to just this in the first installment of our Procedure Series. The production of this series with a GoPro Dual Hero allows us to also provide an optional 3D experience to allow for the viewer to perceive the depth of field that can be experienced in few other ways.
I hope that you will find the Procedure Series as interesting and as much of a learning experience as we have. It could not have been done without the expertise and hard work of our team. We have a great deal more to follow this, as I have dedicated a portion of my Chief year here at Louisville to obtain similar perspectives of several other procedures. As I step back from my role as Managing Director to pursue a fellowship in Pulmonary and Critical Care Medicine, Drs. Laura Bishop and Christopher Migliore will be stepping forward to help manage the long-term growth of Louisville Lectures and continuation of our current projects. This is the first lecture in a research lecture series (initially provided as a symposium by the University of Louisville Division of Infectious Diseases). He then covers important principles regarding conducting research in a university setting from the initial clinical question to publication and establishing a database for continuing research.
In an update of his popular lecture, Dr. Luis Marsano delves into providing adequate nutrition for patients with cirrhosis to prevent hepatic encephalopathy and alcoholic hepatitis. In this lecture, Dr. Craig McClain discusses the pathogenesis of alcoholic liver disease, treatment, and patient cases. Dr. McClain uses his expertise in nutrition to discuss risk factors and advanced pathophysiology that predispose patients to excess harm from alcohol. He closes with a focus on where the research on treatment is and what the most recent evidence tells us about how we might improve clinical management.
In this lecture Dr. Raghuram takes a look at traditional and new definitions of FUOs as well as a practical algorithm for their workup. She takes a comprehensive look at common non-infectious causes including often used drugs that can produce fevers in patients. Dr. Raghuram finishes with several cases presented in a board review style and walks through her thought process with evaluating the questions.
In this case-based lecture, Dr. Krishansamy walks through several real-life examples of patients and their indications for novel treatments for diabetes with a focus on mechanisms of action. For internists not inclined towards cardiology or critical care, an ultrasound might be merely a diagnostic test to be ordered. Research and expertise in this field are expanding rapidly, however, and an affordable handheld ultrasound that can easily be carried in a white coat is likely not many years away.
