En este episodio conversamos con el Dr. Domenech Asbun, cirujano especializado en cirugía hepatobiliar, pancreática y del tracto gastrointestinal superior, con una práctica enfocada en técnicas mínimamente invasivas. Formado en cirugía general en UCSF Fresno y posteriormente en cirugía hepatopancreatobiliar, foregut y mínimamente invasiva en Mayo Clinic Florida, actualmente desarrolla su actividad clínica y académica en Baptist Health, en Miami.
Más allá de revisar aspectos técnicos, en esta conversación buscamos conocer a la persona detrás del cirujano: su trayectoria, sus influencias, su filosofía de trabajo y las experiencias que han definido su compromiso con la educación y la seguridad quirúrgica.
Una parte central del episodio está dedicada a su extraordinaria labor promoviendo la colecistectomía segura. El Dr. Asbun ha impulsado, a través de SAGES, el curso International Safe Cholecystectomy, una iniciativa dirigida especialmente a cirujanos de países de ingresos bajos y medios, con el objetivo de difundir principios reproducibles que ayuden a prevenir lesiones de la vía biliar y mejorar la seguridad de una de las operaciones más frecuentes en cirugía general.
Durante la conversación reflexionamos sobre la visión crítica de seguridad, la identificación correcta de la anatomía, la pausa antes de dividir cualquier estructura, las estrategias de rescate frente a una colecistectomía difícil y la responsabilidad de reconocer cuándo es necesario cambiar el plan quirúrgico.

Dr. Clarke is an Associate Professor of Surgery at the Medical College of Wisconsin, surgical oncologist and division chief of surgical oncology at the Froedtert Hospital Cancer Center, and this past year’s president of the Association for Academic Surgery. Her research interests include personalized cancer care, surgical workforce diversification as a means to ameliorate cancer disparities, and targeted approaches in pancreatic neuroendocrine tumors. We're excited to discuss updates in neuroendocrine tumors and mentorship in HPB!

Dr. Tran Cao is a surgical oncologist, HPB surgeon, and associate professor of surgery at MD Anderson, where he also serves as the MIS training curriculum director. El Dr. Roberto Hernandez-Alejandro ha sido Jefe de la División de Trasplantes en la Universidad de Rochester desde agosto de 2016. Bajo su liderazgo, expandió las oportunidades de trasplante mediante el aumento del uso de órganos donados después de la muerte cardíaca. Recientemente, el Dr. Hernandez abrió un programa de trasplante hepático para metástasis hepáticas de cáncer colorrectal irresecable, siendo actualmente el centro líder en volumen y resultados en América del Norte. También tiene experiencia en resecciones hepáticas y es uno de los pocos cirujanos en Estados Unidos que realiza el procedimiento ALPPS.

She is the program director of the HPB fellowship at her institution, with a passion for surgical education.
Dr. Babicky is accompanied by her HPB fellow, Dr. Anushi Shah. Dr. Shah comes to us with a unique perspective in HPB training as a just recently graduated HPB fellow (after this episode was recorded) at Providence Portland Medical Center after graduating in 2022 from a transplant fellowship at MGH.

Today’s episode features Dr. Ching-Wei Tzeng. Dr. Tzeng is a hepatobiliary surgeon and Associate Professor of Surgical Oncology at MD Anderson. Among many roles, he is the Department Vice Chair of Clinical Operations and Program Director for the HPB Surgery Fellowship.
Dr. Tzeng is an accomplished researcher. His interests focus on multimodality therapy for GI Cancers and perioperative quality improvement, including cancer surgery standards, patient-centered opioid reduction protocols, and quality of life measures. Dr. Tzeng is a passionate surgical educator and has been widely recognized with numerous teaching awards.

El Dr. Frans Serpa es un destacado cirujano general especializado en el aparato digestivo y hepatobiliopancreatico. El Dr. Serpa se graduó como Doctor en Medicina y Cirugía por la Facultad de Ciencias Médicas de Cuenca en 1997. A lo largo de su carrera, ha buscado una formación continua y enriquecedora, lo que le ha llevado a adquirir experiencia en instituciones de renombre internacional.
Entre sus logros destacados, se encuentra su experiencia en gastroenterología quirúrgica en la Real y Benemérita Sociedad Portuguesa de Beneficencia - Hospital de Sao Joaquim (HBPSP), en Sao Paulo, Brasil, durante los años 1999 a 2001. Asimismo, su especialización en cirugía hepatobiliopancreática y trasplante hepático tuvo lugar en la Real y Benemérita Sociedad Portuguesa de Beneficencia - Hospital Sao Joaquim (HBPSP), también en Sao Paulo, Brasil, entre 2006 y 2008.
Además de su experiencia clínica, el Dr. Serpa también ha desarrollado habilidades en el ámbito de la gerencia de salud, obteniendo su especialización en Gerencia de Salud de la Universidad San Francisco de Quito en 2017.

Oscar Imventarza nació y creció en Buenos Aires, Argentina. Se graduó de la Universidad de Buenos Aires (UBA) en 1982. Realizó su entrenamiento quirúrgico entre 1983 y 1986 en el Hospital Argerich y el Hospital Finochietto.
En 1987, comenzó su entrenamiento en Cirugía Hepatobiliar y Trasplante de Hígado en la División de Trasplantes de la Universidad de Pittsburgh, bajo la supervisión del profesor Thomas Starzl. Se convirtió en investigador (1987), becario clínico (1988), instructor de cirugía (1989) y profesor asistente de la misma universidad en 1990.
Oscar Imventarza regresó a Argentina a fines de 1992 y abrió como Presidente los primeros dos Programas Públicos de Trasplante de Hígado de Argentina en el Hospital Juan P Garrahan (para niños) y el Hospital Dr C Argerich (para adultos). Ambos se convirtieron en los centros de HPB públicos más importantes de Argentina. También desarrolló el primer Programa de Trasplante de Hígado y Páncreas de la Región del Litoral, ubicado en la ciudad de Formosa.
El profesor Imventarza ha sido miembro activo de diferentes sociedades quirúrgicas. Fue miembro fundador y presidente de la Sociedad Argentina de Trasplantes, miembro del Consejo del Capítulo Argentino de la IHPBA (CA-IHPBA) desde 2007 y presidente electo del mismo capítulo para el período 2011-2013. Es miembro activo de la Academia Argentina de Cirugía. Su práctica se centra en cirugía de hígado y trasplante para pacientes adultos y pediátricos.

Today we're excited to share our interview with Dr. Flavio Rocha. Dr. Rocha is a surgical oncologist and HPB surgeon at OHSU, where he serves as a professor of surgery and the Hedinger Chair and the Division Head of Surgical Oncology. Dr. Rocha’s clinical and research interests include pancreas cancer and cholangiocarcinoma, as well as other cancers of the upper GI tract.
We had a great discussion about biliary tract cancers, and Dr. Rocha shared his expertise on a variety of related topics to include gallbladder cancer and the OPT-IN trial, as well as intrahepatic cholangiocarcinoma and the NEOGAP trial. Enjoy!

In this episode, we interviewed Dr. Alice Wei. Dr. Wei is an HPB surgeon at Memorial Sloan Kettering Cancer Center. She is the Co-Director of Surgical Initiatives at the David M. Rubenstein Center for Pancreatic Cancer Research and an associate professor of surgery at the Weill Cornell School of Medicine.
We discuss Dr. Wei's initial academic career as an HPB surgeon in Toronto, followed by a mid-career transition to her current position in NYC at MSKCC. During the podcast we talk about the importance of having distinct initial, mid, and later career goals. We additionally examine differences in centralization of care for pancreatic surgery between the Canadian and the United States healthcare systems, and finally hear about developing trials investigating definitive radiotherapy for pancreas cancer.

El Dr. Mercado estudió la Licenciatura de Médico Cirujano en la Universidad Autónoma de San Luis Potosí y en 1981 ingresó a la residencia de Medicina Interna en el Instituto Nacional de la Nutrición Salvador Zubirán; posteriormente a la residencia de Cirugía General con reconocimiento de la Universidad Nacional Autónoma de México. Ha publicado más 200 Artículos en revistas especializadas nacionales e internacionales con temas relacionados a la Cirugía del Hígado. Ha escrito 7 libros de la especialidad además de publicar más de 170 capítulos en libros nacionales e internacionales y su trabajo científico ha sido citado en más de 1,140 ocasiones. Actualmente se desempeña como Directo de Cirugía de su querido Instituto Nacional de la Nutrición Salvador Zubirán.

El Dr. Asbun nació en su amada Bolivia, recibió su título de médico de la Universidad de Chile en Santiago, Chile. Su formación postdoctoral incluye una beca de cirugía general en el Hospital de La Santa Cruz y San Pablo, Universidad Autónoma, en Barcelona, España, y posteriormente una beca de oncología quirúrgica en la Universidad de California en San Diego. Se graduó como el mejor de su clase en la Universidad de La Plata, Buenos Aires, y obtuvo el título de doctor en medicina de la Universidad de Buenos Aires. Javier C Lendoire graduated with the Honor Degree at the University of Buenos Aires. His general surgical training was completed at the Hospital Español of Buenos Aires. In 1989 he was awarded with the Rotary Foundation Scholarship for International Understanding and began his training in Hepatobiliary Surgery and Liver Transplantation at the Transplant Division of the University of Pittsburgh in charge of Thomas Starzl.
In 1993 Javier Lendoire as Vice Chairman opened with Oscar Imventarza the first Public Adult Liver Transplant Program of Argentina in the Hospital Dr C Argerich of Buenos Aires. Also, they developed the first Liver Transplant Program of the Litoral Region located in Formosa City and the Liver Tx and HPB Unit in Sanatorio Trinidad Mitre of Buenos Aires. Dr Lendoire became Chairman of Liver Transplantation at ITAC, Buenos Aires in 2019.
In 2003 he received his Ph.D from the University of Buenos Aires and in 2007 he was appointed Professor of Surgery at the same University.
Dr Lendoire is founding member and Past-President of the Argentine Society of Transplantation, Council Member since 2003 and Past President of the Argentine Chapter of the IHPBA (CA-IHPBA). He served as President of the AHPBA (2015-2016), member of the Scientific Committee of the IHPBA since 2008 and was recipient of the 2019 AHPBA Nonie Lowry Distinguished Service Award.
In this episode, we had the honor of interviewing Dr. Mike D’Angelica, program director of both the HPB and Surgical Onocology fellowships at Memorial Sloan Kettering. He is the Enid A Haupt Chair in Surgery at MSK and the current president of the AHPBA.
In this episode, we took at deep dive into the use of hepatic artery infusion pumps and, more broadly, Dr. D’Angelica’s approach to the treatment of patients with CRLM. This is a real crash course into the use of this modality, and should be of interest to everyone from early trainees to attending surgeons wanting to learn more about HAIPs. We really enjoyed this interview, as is evidenced by the fact that it went for over an hour, and we hope you enjoy it as well.

In this episode, we discussed a recent article in HPB entitled, "Patients’ treatment preferences for potentially resectable tumors of the head of the pancreas." We discussed this interesting paper (link to paper below), which centered around patient decision making when faced with a difficult decision of which therapy to receive in the setting of resectable pancreas cancer. This paper contained some very surprising insights into the thought process of patients, and we had a very interesting discussion around this topic. Enjoy!

In this episode, we discussed the development of collaborative clinic trials in surgery with Dr. Marc Besselink, who has been a true leader in this effort. Dr. Besselink is the PI of multiple important trials and has led collaboratives that are conducting practice-changing trials. We discussed the results of the DIPLOMA, LEOPARD and LEOPARD trials and how they have affected his current practice as well as his advice for MIS HPB surgeons around the world. Enjoy!
