The objectives of these guidelines are to establish the best evidence-based management of VSs, including initial otologic evaluation, imaging diagnosis, use of surgical techniques, assessment of tumor pathology, and the administration of radiation therapy. Because management of these tumors remains imperfect, it was also recommended that information on promising emerging therapies be assessed in the same manner to determine the possible application of these findings. Authors also discuss the planned update of these guidelines.

We now present the updated set of data and studies regarding targeted therapies as well as immunotherapy with and without targeted therapies. We present a systematic review and evidence-based practice guideline to help practicing physicians to determine the role of these treatments in progressive glioblastoma.

During the past 20 years, the brain trauma community’s approach to guideline development has evolved as the science and application of evidence-based medicine has advanced. This new iteration of the guidelines reflects the most current methodologic standards and establishes more rigorous procedures for future work. As a result, the guidelines include changes in the evaluation of previous work, an increase in the quality of the included studies, and essential improvements in the precision of the recommendations.

The purpose of this work is to systematically review the literature to form evidence-based guidelines regarding the relationship between duration and dose of preoperative opioids and patient-reported outcomes and adverse events after elective spine surgery for degenerative conditions. We also review the literature regarding the association between preoperative opioid weaning and these outcomes.

This clinical guideline was created to improve patient care by outlining the appropriate information gathering and decision-making processes involved in the treatment of patients with perioperative spinal disease. One objective of this review is to determine preoperative diagnostic studies that predict increased risk of reoperation or SSI in patients with diabetes. In addition, the published literature was assessed to determine if an increased BMI correlates with an increased risk of reoperation or SSI. Finally, the impact of preoperative smoking and risk of reoperation after spinal fusion was evaluated and if smoking cessation decreases risk.

This systematic review was conducted to evaluate all available evidence to aid clinicians and guide clinical practice by determining the best options for management of pediatric MM. Guidelines authors aimed to systematically review the literature and make evidence based recommendations about the timing of closure after birth, and to investigate the evidence concerning persistent ventriculomegaly and cognitive impairment.

The use of bilateral anterior limb of internal capsule (ALIC) DBS for OCD was approved by the US Food and Drug Administration (FDA) under a humanitarian device exemption (HDE) in 2009.9 Following this HDE approval and publication of the 2014 CNS guidelines, there have been additional published studies reporting the use of DBS for OCD, although most do not meet criteria to be included in this guidelines update. The purpose of this update is to review the literature following publication of the original guidelines and to update the recommendations as appropriate. Based on the availability of new literature, the current guideline is a major update resulting in modification and update of prior recommendations as well as an updated discussion.

The thoracic and lumbar (“thoracolumbar”) spine are the most commonly injured region of the spine in blunt trauma. Trauma of the thoracolumbar spine is frequently associated with spinal cord injury and other visceral and bony injuries. Prolonged pain and disability after thoracolumbar trauma present a significant burden on patients and society. Ultimately, this clinical guideline was created to improve patient care by outlining the appropriate information gathering and decision-making processes involved in the evaluation and treatment of patients with thoracolumbar spine trauma. The surgical management of these patients often takes place under a variety of circumstances and by various clinicians. This guideline was created as an educational tool to guide qualified physicians through a series of diagnostic and treatment decisions in an effort to improve the quality and efficiency of care.

Managing cancer pain, once it is refractory to conventional treatment, continues to challenge caregivers committed to serving those who are suffering from a malignancy. Although neuromodulation has a role in the treatment of cancer pain for some patients, these therapies may not be suitable for all patients. Therefore, neuroablative procedures, which were once a mainstay in treating intractable cancer pain, are again on the rise. This guideline serves as a systematic review of the literature of the outcomes following neuroablative procedures.

Surgery is recommended for brain metastases that are large, have significant perilesional edema, result in neurological deficits, and present with uncertain pathology. In addition, surgery provides tissue diagnosis, when needed. Smaller targeted craniotomies and an emphasis on minimizing postoperative deficits have led to faster operations and discharge a few days after a craniotomy. Given the limitations of radiation therapy and other targeted therapies, surgery plays a critical role for patients, the timing of which is discussed in this guideline.

The efficacy of bilateral deep brain stimulation (DBS) for the treatment of motor symptoms and levodopa-induced dyskinesias in Parkinson’s Disease (PD) patients is well established. However, the effectiveness of selecting different stimulation targets is less clear. While there is evidence to suggest that the subthalamic nucleus (STN) and globus pallidus internus (GPi) are effective targets, when combined with best medical treatment versus best medical treatment alone, the circumstances in which one target should be selected over the other are still disputed. It is also unknown whether STN and GPi DBS induce similar benefits. In this guideline, the authors systematically review the literature and grade the quality of evidence for the use of STN and GPi in DBS in patients with PD.

Surgical resection is central to treatment paradigms in combination with radiation therapy and chemotherapy for treatment of newly diagnosed glioblastoma. Authors discuss the evidence in newly available literature since the original guideline was published in 2008, and the recent updates to the recommendations regarding cytoreductive surgery.
