We’re joined by Ann Slee, Associate CCIO, Medicines at NHS England and Dr Afzal Chaudhry, CMIO at Cambridge University Hospitals NHS Foundation Trust who will be putting the NHS Digital’s newly published medications guidance into action.

There are currently 237 million medication errors every year. While the safety risks are small for many patients, for others there are serious risks because of errors in prescribing, dispensing or monitoring medications.

Patients may find they are having to repeat their medication information to different clinicians, and for those on several medications this can be confusing and stressful.

When this information can be easily transferred between systems, it will lead to safer and better care. In this podcast we talk about the real benefits this will bring, and how it will impact both clinicians and patients.

From October 1, 2018 onwards, only direct digital discharge summaries will be permitted under the NHS England Standard Contract or providers must have a plan in place to implement digital discharge summaries by December 2018.

This includes discharge summaries from hospitals to GPs, as well as outpatient letters. To gain the full benefit of digital transmission, it is important that discharge summaries and outpatient clinic letters are constructed using coded data and standardised information using PRSB’s standards so that data can be automatically extracted into GP records.
