Neurofibromatosis type 1 (NF1) treatment options and management strategies are evolving, and the multiplicity of clinical manifestations, both near- and long-term, makes diagnosis and management of the disease complex. The development of plexiform neurofibromas (PNs) further negatively impacts patients’ clinical outcomes and quality of life. These challenges necessitate an integrated, longitudinal management approach that encompasses the totality of the interprofessional, multidisciplinary team of clinicians in pediatric and adult care settings.

| Neurofibromatosis type 1 (NF1) presents unique diagnostic and management challenges due to its multisystem involvement and complex neuro-oncologic manifestations. Despite recent advances, including the use of mitogen-activated protein kinase inhibitors, many neurology and neuro-oncology clinicians face difficulties translating novel and emerging clinical trial data into everyday practice, particularly in identifying candidates for mitogen-activated protein kinase inhibitors, managing adverse events, and coordinating multidisciplinary care. Limited experience with NF1 further contributes to gaps in long-term disease monitoring and personalized treatment planning. To close these gaps, clinicians must be equipped with practical, case-based strategies to improve early disease recognition, evidence-based management, and interprofessional collaboration that includes a team-based approach to address both clinical and psychosocial needs in the journey of patients with NF1. In this CE Concepts live symposium, expert faculty will assess neuro-oncologic features of NF1, including tumor types and neurological signs, to support early recognition and guide clinical decision-making; evaluate the use of mitogen-activated protein kinase inhibitors in the management of NF1-associated plexiform neurofibromas in pediatric and adult patients, including team-based care to manage adverse events; and develop evidence-based, interprofessional care plans that reflect current clinical standards and support individualized, longitudinal management.

| Desmoid tumors (DTs) are rare, locally invasive neoplasms that present significant challenges due to their variable presentation, diagnostic delays, and the evolving therapeutic landscape. Oncology nurses and nurse practitioners often lack sufficient training in identifying early signs of DTs, understanding novel systemic therapies like tyrosine kinase and gamma-secretase inhibitors, and managing their distinct adverse event (AE) profiles. As frontline providers in symptom management and patient counseling, these clinicians urgently need education on evidence-based strategies for monitoring and mitigating treatment-related toxicities. As the use of immunotherapies for the treatment of cancer expands, emergency medicine (EM) clinicians will need a further understanding of their mechanisms of action (MOAs) to better guide diagnosis and treatment of patients who present to the emergency room with symptoms of adverse effects (AEs). While T-cell-retargeting therapies have revolutionized the treatment of hematological malignancies, they are also associated with symptoms of cytokine release syndrome (CRS), immune effector cell-associated neurotoxicity syndrome (ICANS), cytopenias, infection, and cardiotoxicity. While most cases of CRS and neurotoxicity are self-limiting and only require supportive care, some patients can progress to needing ICU care. To address these risks, EM clinicians require education on the relationship of MOAs of T-cell-directing therapies to potential AEs and improve their confidence and competence in diagnosing and treating CRS, neurotoxicity, and other toxicities associated with these agents in emergency settings. Peripheral T-cell lymphomas (PTCLs) are a diverse collection of non-Hodgkin lymphomas that make treatment and patient-centered care challenging, and highlight the need for targeted therapies based on molecular and genetic profiling to enhance treatment efficacy and reduce treatment-related adverse events (TRAEs). The role of CD30 expression further complicates patient care, because its variable expression among PTCL subtypes prevents establishing a uniform treatment approach. Though randomized clinical trial data is limited, these data can help refine treatment protocols and reduce the risk of relapse by identifying patients who may benefit from maintenance therapies or alternative treatment approaches.

Janus kinase/signal transducer and activator of transcription (JAK/STAT) pathway inhibition is the standard of care for patients with symptomatic myelofibrosis (MF); however, a significant number of hematologists/oncologists lack knowledge, competence, and confidence about JAK inhibitors (JAKis) in MF. These clinicians and their interdisciplinary collaborative care team need a deeper understanding of JAKis in MF, guideline-recommended risk-stratification algorithms, molecular indicators of disease prognosis, clinical features of MF, symptom burden, the impact of adverse events on treatment adherence, and health disparities in MF.

The reduced incidence of cervical cancer over the past few decades was primarily due to robust screening guidelines and vaccinations. However, despite the proliferation of screening programs, the diagnosis of cervical cancer is associated with inefficiencies and inconsistencies and has not decreased cervical cancer incidence further over the past decade. Additionally, numerous health care disparities persist as unaddressed medical needs, particularly for individuals with marginalized sexualities and genders. An exciting array of new immunotherapy options are further emerging for patients with locally advanced, recurrent, or metastatic cervical cancer, with promising potential to heighten treatment effectiveness. It is imperative that clinicians and the multidisciplinary care team are appraised on these novel therapies and are conscious of health care disparities and unique patient needs.

Cytomegalovirus (CMV) remains a major concern in both solid organ transplantation (SOT) and hematopoietic stem cell transplant (HSCT), primarily due to challenges related to treatment selection and monitoring, management of drug-related adverse events, and resistant/refractory disease. To minimize the impact of CMV infection in patients with HSCTs or kidney transplants, the interdisciplinary care team managing these patients must have guidance on incorporating novel antiviral therapies into prevention and treatment schemas for CMV.

Patient prognosis for endometrial cancer (EC) is highly benefited by molecular profiling, and national and international guidelines robustly recommend its integration into practice. Characterization of DNA mismatch repair (MMR) status identifies clinically significant molecular subtypes. This information assists clinicians in creating treatment plans tailored to specific tumor phenotypes and individual needs, including those patients with advanced and recurrent EC. Despite ringing endorsements and clear clinical applications, however, molecular profiling remains underutilized due to logistical challenges and a lack of clinician confidence. It is imperative that clinicians, in collaboration with the multidisciplinary team (MDT), become knowledgeable about this exciting technique and utilize it to enhance treatment decisions for patients.

Reducing barriers to adequate anticoagulation for patients with cancer improves outcomes for those with cancer-associated venous thromboembolisms (CA-VTEs). Advances in cancer treatments and the resulting improvements in patient survival have led to an increase in the number of patients with cancer who develop a VTE at some point during their treatment (around 15%). CA-VTE is associated with decreased survival, increased morbidity and hospitalization, and the potential delay or interruption of systemic anti-cancer therapy. During this series, our faculty will expand clinician knowledge of VTE risk factors and management strategies, discuss optimal anticoagulation therapy selection and guideline adherence, and explore approaches to working with multidisciplinary teams.

To facilitate timely diagnosis and rapid treatment initiation for anemia in CKD, the multidisciplinary and interprofessional CKD-anemia treatment team should be aware of the significant impact of anemia on patient quality of life, progression of CKD, and cardiovascular events. Erythropoietin-stimulating agents (ESAs) are associated with significant cardiovascular safety-related concerns, but the emergence of the oral hypoxia-inducible factor prolyl hydroxylase inhibitors (HIF-PHIs) may improve erythropoiesis without the undesirable downstream effects of ESAs. However, HIF-PHIs place in therapy has not yet been fully elucidated. Here, join the interprofessional team of Dr. Ajay Singh, Dr. Jane Davis, and Dr. Calvin Meaney as they explore an incisive review of current CKD-anemia treatment and diagnostic challenges, as well as the evidentiary base for HIF-PHI utilization in CKD-anemia that establishes context for the future of these agents. All clinicians managing patients with non-dialysis-dependent chronic kidney disease or heart failure must be aware of the deleterious impacts of concomitant anemia and the established, emerging, and practice-changing role of intravenous (IV) iron therapies. The vicious cycle of cardiorenal anemia syndrome (CRAS)—the pathologic triad wherein the three conditions are each capable of causing and/or being caused by the other—can be effectively mitigated and managed with IV iron. Unfortunately, clinician hesitancy in prescribing IV iron, largely due to adverse effects associated with earlier formulations, must be overcome before the full benefit of IV iron in CRAS can be realized.
