Opinion:Oh Andrew Little, how you frustrate me... Let me count the ways. 

First and most obviously, not answering a basic question is frustrating. 

Did the overloaded and under-pressure health system fail a woman who died after presenting at Middlemore?

Because a report into her death says in black and white, “the overloaded and under pressure health system failed her”.

Andrew Little refused to say. Sure he provided context but so does the report and within that context, this is the conclusion it drew. 

You either agree or contest the findings of the report. Either of those things are fine but you don’t get to do neither.
