You hear it in 90% of press conferences...head coaches expressing some variation of "it's all on me". So when your game plan stinks, your "all everything" tight end doesn't get the ball, your line can't give your QB time, and your offensive coordinator looks lost (again)...you either have a hard time identifying issues or you're unwilling to do something about it.
