Episode 99. When applying to medical school, programs will send you a set of about 2-5 prompts for essays after they receive your primary application. It is just as important as the primary app when deciding who to interview! Some schools screen applicants and only send secondaries to selected applicants while other schools send secondaries to everyone. The purpose of secondaries is help schools decide if you are a good fit for their incoming class, so it's important to personalize your responses for each school.

Factors to consider when choosing schools: public vs. private, rural vs. suburban vs. urban, state/region preference, admission requirements (premed coursework, MCAT and GPA minimums), MCAT and GPA averages, tuition and financial aid, mission statement, quality of the education, match rates, how happy the students are, grading system, lecture attendance requirements, lecture vs. independent learning vs. team-based learning, research opportunities
Factors to consider when deciding number of schools: your competitiveness (MCAT, GPA, clinical experience, research, leadership), how competitive you are for your top programs (adding more if your top programs are reaches)
Include about 1/4 schools that are "safety" (public schools in your state, MCAT and GPA more than average), match/fit (public in state or private out of state, within 5 points on MCAT
For most applicants, aim for 15-25 schools, which is the average. Involve mentors in this process because this is highly individual and depends on numerous factors. For example, you may not need to apply to this many if you are applying to osteopathic medical schools with an application that has a strong osteopathic medicine focus. You also may not need to apply to this many if you are competitive for your top programs.

Episode 96.
This episode is based on the 2024 application cycle. Some dates for deadlines may change from year to year; however, many of the details of the application itself will remain the same, so this episode is helpful if you applying during future application years as well!
Pay attention to the date when schools can start reviewing your application. Aim to have your application submitted and verified by that date so that you can be in the first batch of applications. It may take weeks for your application to be verified, so the earlier you can submit, the better. If you don't have everything ready in time for that date, it is still worth applying that cycle, but you are likely not to receive as many invitations for secondaries and interviews. Most schools have rolling deadlines and will continue to review applications after the initial release.
Work on your personal comments essay (personal statement) and the work & activities section ahead of time. You don't have to wait for the application to open before drafting these statements. These essays are one of the most important if not the most important part of your application, so it is very much worth it to invest in professional services to look over this part of your application. See episode 92 for more tips for the personal statement and a timeline of the application season.
Make sure you enter your coursework correctly by referring to your official transcript so that there are no errors that could delay your application's verification.

Episode 94. There are two main applications used by medical students who are exploring residency programs to apply to: Residency Explorer and FREIDA. In this episode, you'll learn about how to use each site and take advantage of the filters and program information. As a fourth year medical student, I share insight into what information to focus on and what information may not be as accurate.

Episode 90. The personal statement is the most important part of your application for many specialties. It is also the part of your application that you uniquely have control over. Many students overlook this important element of the application.
The 2021 program director survey cited that in all specialties, the personal statement is more important when selecting who to interview than other factors including 1) volunteer and extracurricular experiences, 2) awards and honors, 3) the prestige of your medical school, and 4) research experience.
The personal statement was even more important than 1) USMLE/COMLEX scores and 2) your school grades for applicants applying to many specialties including child neurology, family medicine, neurosurgery, PM&R, psychiatry, and vascular surgery. Now that Level 1 and Step 1 is pass/fail, many more specialties are likely to join the ranks of these specialties that look more at the personal statement for a more holistic review process.

Episode 89. There are three types of intelligence: thinking intelligence, feeling intelligence, and doing/instinctual intelligence. Everyone has all three types of intelligence, but everyone has one dominant center of intelligence. The Enneagram numbers can be sorted into triads, and these triads also line up with a center of intelligence.
Head triad = thinking center of intelligence (5, 6, and 7), motivated by fear
Heart triad = feeling center of intelligence (2, 3, and 4), motivated by shame
Gut triad = doing/instinctual center of intelligence (8, 9, and 1), motivated by anger

Episode 82. Most lesions you'll have on your skin are benign. While waiting for your dermatology app (because most people should see a dermatologist!), here are some descriptions of the most common "spots" you might have. Don't be scared into thinking you have skin cancer because that's what Dr. Google (or Dr. TikTok) told you.
Benign skin lesions are non-cancerous growths. Diagnosis requires individual consideration of the lesion (by a doctor, in person) as well as your overall condition. Biopsy or excision is common when there could potentially be a malignancy. True benign lesions can still be treated but usually just for cosmetic purposes.

Episode 79. This episode is great for everyone, whether you care about your own health or if you are in the healthcare field! Learn more about the two most common types of primary headaches (tension headaches and migraine headaches) as well as red flags to watch out for that may mean a more serious cause of your headache.
Headaches are common and are one of the most common causes of functional disability. Over 90% of people experience a headache at least once in their lives. Most headache are harmless. Tension headaches make up about 80% of headaches, and migraines account for about 10% of headaches. The remaining headaches include rarer types, and some are life-threatening.
Identifying the cause of headaches requires a doctor to take a thorough history and perform a physical examination. Additional testing like imaging (CT or MRI) is only indicated if headaches persist after first line treatment or if red flags are present.

Episode 78. This episode is extremely important for 3rd year medical students (and also 1st and 2nd medical students looking ahead) for tips on preparing for 4th year when it comes to selecting a specialty and scheduling away and audition rotations. Topics include sub-internships, VSLO/VSAS, Clinician Nexus, when to schedule rotations, when to ask for letters of recommendations, and when you really need to decide on your specialty (and what to consider when making this important decision).
Away rotation- a rotation not at your core rotation site (where you did 3rd year rotations). It usually means you travel.
Audition rotation- a rotation that has a residency program you are interested in. You being at the site and rotating means you have face time with residents and attendings. If you do well, you are more likely to be offered an interview there. If you don’t do as well, they may not invite you.
Sub-internships or sub-Is- rotations in 4th year in which you are treated like an intern. You often have many of the responsibilities and workload like an intern would have. If an away rotation or audition rotation is distinguished as a sub-I, they may be more intense than if it was not designated with this term. What a sub-I means at different institutions varies widely.
Auditions and sub-Is can be home rotations or away rotations. Away rotations are pretty much always audition rotations. Away rotations and audition rotations are sometimes sub-Is.
December and January of 3rd year: start thinking about where you'd like to complete away rotations and start applying.
February: VSLO opens for most programs on that site. The process ends once you fill your schedule!
You’ll probably want to have your specialty chosen by the mid-point of 3rd year since you will want most if not all of your away rotations in the specialty of your choice.
Most residency program websites will also include information on how to apply for 4th year away rotations. They will likely say if you have to apply on VSLO, Clinician Nexus, their own application, or to simply email their coordinator. If you don’t see specific info, you can always find the contact us area and reach out to their program coordinator to ask.

Happy holidays! Whether you are listening to this episode in December, January, or any other time of year, I hope that this is helpful to any goal you set (including New Year's Resolutions). If you are listening to this episode after the 1st, it is not too late for you. Starting a goal in the new year is an arbitrary date. You can make changes to your life at any point. Also, if you feel like you already failed at your new year's resolution, don’t let yourself take that all or nothing approach. Take a day or two to reassess. Analyze why you fell short and address that issue. Maybe you can consider revising your goal to something more attainable. Be patient and be forgiving of yourself. For any lasting change in your life, you are likely not going to get there on your first try. Learning from your mistakes takes you a step further to where you want to be.
Exercise: run a 5k, set a new PR, take the stairs, set a step goal, create a stretching routine
Diet: eliminate junk food from your home, set a goal for no snacking after dinner, drink more water, limit how often you eat out
Sleep: establish a new bedtime, create a wind-down routine, eliminate screen use before bed
Finances: save money, create a budget, pay off debt
Spiritual: read the Bible in a year, be intentional about prayer
Self-care and Mental Health: practice gratitude, meditate daily, do something for yourself everyday
Family and Friends: reaching out once a week, make plans to see them more often, spread positivity by giving one compliment a day
Things You Should’ve Been Already Doing: floss, brush your tongue, go to the dentist, clean your fridge, cleaning out your car, go to a well check
Hobbies: practice an instrument a half hour a day, starting something new, attend a class, read one book a month for fun, start journaling, start a blog
Bad Habit Elimination: quit smoking, quit watching porn, limit social media use

Episode 75.
You may be looking to choose a new diet to follow because you want to lose weight, manage a chronic condition like diabetes or hypertension, or just to become healthier. There are so many choices of diets out there, and you are not alone if you feel overwhelmed by the amount of choices. Some diets seem to contradict each other, leading you to ask questions like the following: Is high fat or low fat the way to go? Are carbs good or bad? Should meat be the largest or smallest food group in my diet? Your doctor also may not be the most helpful when it comes to this. A common thing I hear in healthcare is to "follow whatever diet you can stick to". To be fair, there is truth to this since it seems like almost anything is an improvement compared to the modern Western diet.
In this episode, I discuss the foundations of what makes a "good diet" and what red flags to watch out for, since I can't possibly go into detail in all the hundreds (if not thousands) of diets that can be read about online. I then take time to go through some of the common ones I am asked about with a straightforward recommendation for or against the diet.

Episode 72. Low back pain is very common. It is the 2nd most common reason to see a doctor and also the 2nd most common reason to take time off work. In general, doctors don't have much to offer patients with back pain. They may prescribe pain medications like NSAIDs or muscle relaxants. They may also refer the patient to see a physical therapist. Osteopathic doctors have another tool they can use with osteopathic manipulative medicine (OMM), also known as osteopathic manipulative technique or osteopathic manipulative treatment (OMT). Osteopathic care follows a holistic, whole person approach that utilizes evidence-based medicine and knowledge of anatomy and physiology. Osteopathic care can treat back pain by targeting the underlying cause (alongside medication if needed). Osteopathic doctors are fully licensed medical doctors who receive all the training that allopathic physicians (MD physicians) do but also with the additional training in OMM and a curriculum that emphasizes the interconnectedness of the human body.

The healthy defense mechanisms or mature defense mechanisms discussed include anticipation, humor, altruism, suppression, and sublimation.
Coping strategies are skills that can be used to deal with life stressors. Adoptive coping focuses on responding in a constructive way while maladaptive coping focuses on short-term stress relief that may increase stress long-term (anger, drug use, "eating your feelings").
Adaptive coping can be problem-focused coping (targeting the underlying cause), emotion-focused (responding with acceptance), and/or meaning-focused coping (mindfulness, gratitude, prayer).

Episode 69. This episode covers the different types of neurotic defense mechanisms, ways that we unconsciously deal with stress and anxiety. Neurotic defenses are at a higher level than the unhealthy defense mechanisms I described in the previous episode, but there are also mature defense mechanisms that are at an even higher levels. The neurotic defense mechanisms include externalization, control, displacement, intellectualization, isolation of affect, rationalization, reaction formation, repression, dissociation, and inhibition. Anyone may have these defense mechanisms but it is especially common in people with personality disorders or other mental health diagnoses, especially anxiety-related disorders.

Episode 68. This is the first episode of a series on defense mechanisms and coping strategies. To start off, this episode covers level 1 and level 2 defense mechanisms, the pathologic defense mechanisms and the immature defence mechanisms. Collectively, I refer to these as the unhealthy defense mechanisms. Just because you use one of these defense mechanisms does not mean that there is anything wrong with you; we have probably all used one or more of these at one time or another. What is important is to acknowledge that these are unhealthy defenses, and there are healthier defense mechanisms and coping strategies that you can use instead. These defense mechanisms, however, are unconscious, so many people need therapy in order to work on effecting their defense mechanisms.
Defense mechanisms covered in this episode include splitting, projection, denial, acting out, blocking, idealization, identification, passive aggression, undoing, regression, and fixation. Splitting, acting out, and passive aggression are often found in borderline personality disorder, or BPD. In episode 8 (Season 1), I talk more about BPD as well as antisocial personality disorder. Denial is often found in people with substance use disorders. I talk about Alcohol Use Disorder in episode 16 (Season 1) and Opioid Use Disorder in episode 65 (Season 2 episode 15). Regression is often found in dependent personality disorder, which is covered in more detail in episode 13 (Season 1). Additionally, I talk more about therapy in episode 62 and 63 (Season 2 episode 12 and 13).

Episode 67. In this episode, I discuss Obsessive Compulsive Disorder (OCD), a mental health diagnosis characterized by obsessions, compulsions, or both. I cover what symptoms OCD is associated with, how it is diagnosed, and how it is treated. OCD is very different from OCPD, which is discussed in a different episode (season 1, episode 13). Episode 62. The word "therapy" is often thrown around but can be used to refer to a variety of different types of therapy. First of all, there is psychotherapy (which is probably what you think of first and is what this episode is about), but there are other types of "therapy" like physical therapy, occupational therapy, and speech therapy, to name a few. When talking about psychotherapy, there isn't just one type either; there are many! This episode doesn't even cover all the types, but I am starting out talking about the most common types that you are likely to encounter if you see a counselor, a therapist, a psychologist, or a psychiatrist. There are also types of therapies that your primary care physician may even utilize! Episode 60. There are a lot of misconceptions about depression during pregnancy. Is depression "normal" during pregnancy?... NO! A woman who is depressed during pregnancy should seek medical care. There are several medications available that are not associated with risk of congenital anomalies, and the risks of leaving depression untreated has a much greater impact on the growing baby. Even if a woman does not want to be put on medication, there are other treatment options available that you can talk to your doctor about. In this sequel to my previous episode on diabetes, I focus most on diabetes mellitus type 2, speaking about the specific management and monitoring that patients do themselves (like checking fasting and postprandial glucose) and monitoring done by physicians (like monofilament testing). Episode 55. Now that Level 1 and Step 1 are scored pass/fail without a numerical score, other factors in students' residency applications are becoming much more important. Whether you want to match into a surgical subspecialty, primary care, or any other specialty, the following are going to be important to work on starting on day 1 in medical school: research experience, extracurricular involvement, and leadership. Episode 47. This episode is part 4 of a series on clinical rotations. I am speaking from my medical school experience, but you may find this helpful as a PA student or nursing student as well. This episode's topics include how to prepare for surgical rotations (especially general surgery and OB/GYN), how to get and take advantage of opportunities to learn hands-on skills and procedures, and how to do OMM/OMT while on rotations. I also cover a few important points about professionalism that you probably haven't thought about. The end of the episodes contains some important tips about pimping.
Here are some highlights from this episode:
On preparing for surgical rotations: It may help to read up on the surgery/procedure and review relevant anatomy the night before.
On procedures: Be around and ready to help. If there is something you want to do, ask the preceptor if they would like help with it or if they can show it to you. Asking ahead of time is better than in the room in front of the patient. Many times opportunities will be offered. Make sure you react enthusiastically and gratefully so that more opportunities will then be given to you. If you see your preceptor put on gloves, put on gloves (even if they get wasted). In surgeries, put on regular gloves before scrubbing in; you might get the chance to help with anesthesia.
On OMM: I will take time to diagnose a somatic dysfunction and report it to my preceptor with recommendation to do OMM in my plan. It is then up to the preceptor to decide if they are comfortable with supervising me and having me assist with the treatment. I’ve found that many DO preceptors will let you perform OMM, especially DO residents. Do not do OMM without approval first. You wouldn't be able to give a patient medication or perform a procedure without your preceptor, and OMM is no different.
On professionalism: Don’t ask where someone went to med school. Remember computer etiquette. Ask if there anything else you can help with at the end of the day.
Pimping: Remember that this is a teaching method. Anki is the best preparation for this.

Episode 44. If you are in medical school (or in training for any health profession), this is for you! I just finished my core rotations of my third year of medical school and wanted to share with you some tips that worked for me and things I wish I knew before starting rotations.
If you want perfect scores on your preceptor evaluations, there are two elements I would argue are most important: staying professional and reacting well to feedback. The word professionalism is likely to make medical students roll their eyes, but professionalism really does capture what should be in the forefront of your mind when on clinical rotations. It doesn't just apply to how to interact with your preceptor but also the nursing staff, other students, and the patients, too. While focusing on these goals, I also share in this episode what you should bring to rotations, what you must do on day 1 of a new rotation, how to work well with other students, and how to take advantage of downtime.

Episode 43. This episode I specifically focus on the time period that students take to exclusively study for Level 1 and/or Step 1 (after classes end for the year). I have some tips about how to navigate the new pass/fail grading based on frequently asked questions: What can I do to maximize my likelihood of passing? What resources should I prioritize in the weeks leading up to the exam? How do I know if I will be competitive for my specialty of choice?
The previous episode I mention that discusses general studying for the first board exam (COMLEX Level 1 and USMLE Step 1) is episode 21 entitled "How to Study for COMLEX Level 1 and USMLE Step 1". I recommend listening to that episode too, especially if you are going into your 2nd year of medical school (preclinical curriculum). The timeline I base my episodes on often assumes that your school follows the same format as my school's curriculum. Just know that what I am basing my timeline based on year 1 and 2 preclinical curriculum with the first board exam(s) taken at the end of the second year. Years 3 and 4 are clinical rotations with the second board exam(s) taken at the end of year 3 or the beginning of year 4.

Episode 26. Enneagram 1s are near and dear to my heart because I am one myself. I didn't want to confuse you by jumping around, but there is no rhyme or reason why the Enneagram 1 is first or why the Enneagram 9 is last. Take a look at what the Enneagram looks like (yes, the shape), and you'll see the numbers and interconnected and sorted around a circle instead of nine numbers in any order. The connections (lines) are what is important as well as which numbers border each other.
Listen to my previous episode (the Enneagram) for an overview of the series even if you are already familiar with the Enneagram.

Episode 21. Board exams are a huge part of medical school! Even with the switch to a pass/fail grading system, it is essential to pass on your first attempt. Second year medical students have been obsessively stressing over Level 1/Step 1 board exams for years. Residency, and therefore your future career as a physician, is on the line. Students have struggled with balancing their classwork with board review, but I am here to say that it is possible to succeed with both. I won't publicly share my scores, but I certainly did well enough on both COMLEX Level 1 and USMLE Step 1 and improved my class rank during my second year with the strategies I am choosing to share with you. You don't have to risk burnout or sacrifice your good habits (taking breaks, exercising, spending time with loved ones, etc.) if you plan ahead with a study schedule that works for you. I was in the last medical school cohort that had three-digit score exams. I realize that subsequent cohorts now have even more uncertainty in this process, so I hope to lessen the stress with a few guiding words. Thanks for listening! This is the last episode in the personality disorders series on First Line. Join me for a discussion on Cluster C disorders (Avoidant, Dependent, and Obsessive Compulsive), also known as the anxious and fearful cluster.
A few notes about personality disorders that I want to cover or reiterate:
Unspecified personality disorder can be diagnosed in an individual who meets the criteria of rigid personality traits that impair function and cause distress while having characteristics of more than one personality disorder without meeting the minimum criteria for any one.
Psychodynamic therapy is first line treatment for personality disorders. It focuses on making the person aware of their traits and then taking steps to modify his or her behavior and thought process to take on healthier ways of interacting with the world. This is especially effective for histrionic PD, avoidant PD, dependent PD, and obsessive compulsive PD. Antisocial PD is unfortunately unlikely to benefit. Other options for therapy include CBT, supportive psychotherapy, and group psychotherapy.
Medications DO NOT lead to personality changes, but it is a good idea to treat co-occurring anxiety and depression to better support individuals for personality disorders.
Many of the PDs start to mellow out when individuals are in their 30s and 40s (especially borderline), but this is less true for schizotypal and obsessive compulsive PDs.

September was my month completing an underserved medicine rotation during my third year of medical school. I worked at a family medicine clinic in a county that is over 75% rural. This episode, I speak about the rotation in general and introduce the social determinants of health. Then, I share a recording of my thoughts after completing the first two days of the rotation. This week, the host of First Line (Aubrey Ann Jackson) discusses two of the personality disorders in Cluster B: antisocial and borderline. Cluster B is known as the dramatic and emotional cluster with emotional reactivity, poor impulse control, an unclear sense of identity, and possible aggression. Individuals with a Cluster B disorder have lives marked with stormy and problematic relationships, and affected individuals are also at risk for self-destructive behavior. It is so important for all of us to learn more about personality disorders to identify individuals who may need help. Individuals affected by personality disorders most often do not realize there is a problem, often shifting the blame onto external sources. Additionally, these disorders are unfortunately often misunderstood and excessively stigmatized.

The shift from 2nd year to 3rd year medical school is a huge change from the classroom setting to the hospital and clinic setting. For my class, the change was even greater because our 2nd year was largely completed virtually.
I started my first clinical rotation of medical school in psychiatry from August 2nd to August 27th. In this week's episode, I am including audio that I recorded after my first day (August 2nd) that I have not released until now. I also have a recording from the last week that reflects on how the rotation went and what I learned. It is estimated that 15% of the general population has at least one personality disorder. This week, we will be discussing the personality disorders in Cluster A: Paranoid Personality Disorder, Schizoid Personality Disorder, and Schizotypal Personality Disorder. This is not only an interesting topic but one that is incredibly important, as personality disorders deeply affect the daily functioning, interpersonal relationships, and mindset of individuals who are affected. Unfortunately, personality disorders are often misunderstood, but it is my goal to provide more information that is easily accessible. It is likely that you or someone in your life has a personality disorder. In most cases, they do not receive professional help through therapy because they are unaware of their personality traits or, if they are aware, they do not see their maladaptive traits as a problem, even if it causes them distress or leads to impairment in their life.

Injury and accidents are the 3rd leading cause of death according to the CDC and is the leading cause for people ages 1 to 44! We will be also touching on chronic lower respiratory diseases, stroke (cerebrovascular diseases), Alzheimer’s disease, and Diabetes (ranked 3-7).
