Affidavit from a registered pharmacist or competent person 8. Affidavit of Non-Conviction, List of Equipment and Machinery for manufacturing, and List of Equipment Provided for Testing 9. Letter of appointment along with Bio-Data of Registered Pharmacist in case of Retail License. 10.
