We visited a festival in Ibri in the afternoon on the 23rd of February 2024. The festival was held for a few weeks only. The opening time was 5 pm and the closing time was 11 pm. We heard that in the evening it became too congested. So we decided to go earlier.
The festival wasn't far from our house. It took only 15-20 minutes to get there. We reached there at around 5 pm. Hence, it wasn't crowded still. It was a free festival. We didn't need to buy any tickets.
At first, we enjoyed the clown show (Video at the end of this post). The children enjoyed it very much on the stage, except for my child :) He was too afraid to go to the stage. There was a colorful bus parked beside that stage.
Then, we found the circus people who were showing various acrobatic stunts. Most of the visitors were enjoying their performances. (We can add only one 3speak video in one post! So we will add the video in another post).
In the following photograph, we see two rides; driving vehicles, and sliding. All were paid rides!

Having health insurance is somewhat comforting for a patient because if (and when) they are sick, they can get the benefit of having the insurance. That's true in most cases. But, there are some exceptions well. Besides many benefits, we having some troubles with the insurance patients as well.
Some investigations, tablets/syrups and injections are not covered by insurance companies. E.g. any hormone tests, vitamin tests, serum iron profile tests etc. aren't covered by insurance. For many insurances, we need to get approval from them before doing an investigation. Many times, what we (the GP doctor) feel about doing some tests upon history and physical examination of a patient, the insurance doctor doesn't feel the same way and rejects the approval request. Some patient pay cash to do the tests, while others shout and create problems.

"Breaking bad news" to the patient is something that needs to be handled very carefully. In some higher examinations, there is a station to check the student's (doctor's) skills in breaking bad news to a patient. But, telling a patient about some simple deviations from the normalcy of the vitals, is not like telling a patient that you are having cancer. However, again, some people are too sensitive to hear that the BP is 90/60 mmHg, instead of the normal 120/80 mmHg!
Regarding this issue, should we (the doctor) write the abnormal vitals in the patient's fine but tell him/her verbally that "Everything is Ok"?

On Tuesday (6.9.22) morning I turned into an ENT doctorπ Three people came with ear wax. One of the three was having pain as well l. So, his ear wasn't cleaned. The other two patients' ears were cleaned.
One patient came with an acute attack of Bronchial asthma with reduced oxygen saturation.
Another patient came with a renal stone. But, his pain was minimum.
One angry follow-up patient came because previously given medications failed to reduce his headache.

On Saturday (3.9.22) I was super busy in my chamber. I came in contact with 90 people in my clinic. The majority of them were sick people. One of them had an acute MI (heart attack) and I had to escort him to the higher centre. For the whole duty time (10 hours and a half) I didn't have any free time. And, to finish consulting the registered patients, I had to close the new patient's registration before the clinic's ending time.

If you are involved in the POSH (proof of sharing) project and following @poshtoken as well as @acidyo you can see that according to the latest posh stat, 498 tweets received the dividends from the 400 $POSH tokens. Also, you are getting the liquid hive if the comment gets upvotes. Recently, some big name has made their posts about this project. E.g. POSH: A great way to earn HIVE by @taskmaster4450 or Have you noticed your POSH passive income? by @belemo etc.
These all collectively attract more hive users to share hive links on Twitter. I think the "liquid hive rewards" is more influential than the $POSH tokens. Can acidyo single-handedly (mostly) continue supporting it by upvoting on the comments?

Sometimes, besides treating a patient, I need to meet the demands (!) of a patient's attendant. Many people who come to my chamber as attendants along with a patient, ask me to check their blood pressure or write a medicine for their headache/fever/any problems or show a prescription to adjust the medications they are already on etc. It is sometimes bothersome to have such demands from them.

I am working in the polyclinic for almost 6 years. One of the few problems I have faced here is the lack of some necessary investigations to finalise a diagnosis. Many young and middle-aged patients come to us with the history of rapid weight loss over last few months. Later, following investigations, it reveals that the patient was having a very high blood sugar level, i.e. Diabetes mellitus! Many of them feel fatigued most of the time, even though they eat a lot.
Why does a diabetes patient develop these symptoms?

Sometimes, a patient comes with a constant/frequent symptom to us. But, during the physical examination, the symptom completely disappears. On the other hand, Sometimes a patient comes with a very infrequent symptom but in front of us, the symptom prevails prominently. In a case of acute lower back pain following lifting a heavy object, avoiding lifting heavy weight and taking some rest should be a priority.
In a case of acute appendicitis, which is an acute surgical emergency, an appendectomy should be done first.
In a case of allergic dermatitis in a palm, the allergens need to be avoided and (if related to work) work needs to be changed as the priority.
However, the reality is we are always going for alternate options.

One family (father, mother and 10-year-old boy) visited me. Yesterday they returned from Salalah, a well-known tourist spot in Oman. The mother and the boy were suffering from upper respiratory tract infections. A quite common problem the returning visitors from Salalah are suffering from.
Another couple visited with a similar kind of illness. But, they returned from Makkah, after performing their Umrah, instead of from Salalah.
One Bangladeshi came with a constant cough and some sort of breathing problem for the last 10-15 days.
Another Omani visited with pain in his anal area pain, constipation and bleeding with stools.

On Saturday, I was quite busy in the evening at the office because more patients came together at that time. Other times, the patient load was less.
I treated and referred a few injury cases.
Several patients came with upper respiratory tract infections.
One staff nurse working in a government hospital came to do a rapid antigen test for COVID19. He was suffering from diarrhoea, fever and fatigue for the last 2 days. The test result came back negative.

Thursday morning shift was a quite bit of relaxing duty time.
One Egyptian came to take medicine for his sleeplessness and headache.
One Omani came to take a fitness certificate to continue working in his workplace after being diagnosed with asymptomatic hypertension.
One Omani Lady came to clean her ears, blocked with impacted wax. It was a successful procedure. She was happy at the end. However, she cried during the procedure!!

One patient came to me this morning. It was his third visit in the last 5 months. He was having the same symptoms as before; sneezing, running nose, and occasional nasal blockage. That means he was suffering from allergic rhinitis. He came for an injection for his problem, just like the previous visits.

Sometimes, a patient comes and complains about passing urine more than before and becomes more thirsty. Following the test, it reveals that the patient is having diabetes (newly diagnosed). Sometimes, a poorly controlled or uncontrolled diabetes patient comes and complains the same, along with or without other symptoms. These two are among the most common symptoms of raised blood sugar aka hyperglycemia.

One Bangladeshi worker came with pain in his left wrist and lower end of the left forearm area for 15 days. He sustained an injury to the affected area 15 days before. On the day of the Injury, he didn't do an X-ray. He just took some painkillers. As the pain wasn't subsiding, he came to me today. An X-ray showed a fracture. He worked with a fractured wrist joint for the last 2 weeks! Because An X-ray wasn't done.

I got a diabetic patient in my chamber today. He didn't have any idea about his disease. Even though his blood sugar was very high, he wasn't even taking any medication for the disease. Hence, he started developing complications. As he was unaware of the extent of his disease, he was wondering why all of this was happening to him. He wasn't educated about his disease.
Another patient came with allergic rhinitis. Similarly, he didn't have some basic knowledge about it. Because he wasn't counselled properly before. This means he wasn't educated about his disease either.

Anyway, today, Sunday, 17th of July, after performing my Fajr prayer I went out cycling, as a daily routine. Towards the end of today's workout, I stopped at a small park-like place and made this video. I just showed some surroundings of that place.

If a patient knows about his or her disease to some extent, the management would be much easier. Today 19th of Ramadan. Thursday. The last day of Single duty In Sha Allah. Another GP is returning back today after enjoying his annual leave. Tomorrow he is supposed to join his duty, In Sha Allah.
Today morning I found that my old office desk has been replaced with a new one. Looking good so far π.
I had an easy and relaxing morning duty. 10 patients in my chamber and 9 people in the visa medical section.

Hi there! Greetings from Oman. Assalamu Alaikum. How are you? Hopefully, you are okay. I am fine too. Here is today's Alive video. It is also my submission number 387 for the I am alive challenge contest.
Today, I consulted 45 patients in my chamber. One Indian lady came with anaphylaxis following ant bite. But, with injection and nebulization the symptoms resolved. So, didn't need to rush to the higher centre, Alhamdulillah. Another 24 people came for the visa medical check-up. I had to examine all of them.
According to the latest (on 21 Mar 2022) COVID19 updates from the Ministry of Health, Oman, in the last 24 hours, new confirmed cases were 122 and no new death. So far, the total number of COVID19 related death is 4250.

The Supreme committee in Oman dealing with COVID19 has announced some new rules. There is some relaxation in the COVID19 measures or regulations. From today onwards, the travellers coming to Oman do not need a PCR test, only a full vaccination certificate is enough. There are a few other changes as well. In this video, I have talked about two common allergic conditions patients present with. Allergic rhinitis affected patients present with sneezing, running nose, nose blockage due to polyp, throat irritation or foreign body sensation. On the other hand, allergic dermatitis patients present with severe itching and rashes on the skin. The treatment principle of any allergy is to avoid exposure to the allergens.

Today two things happened to my Listnerds account. I have invested a small amount of hive and bought some Listnerds tokens. And, one of my mail got verified giving me some Listnerds tokens as well. As @achim03 told, the mail got verified after 7 days.
