Prof. Michael Böhm explains how SGLT-2 inhibitors became a strong pillar of treatment in HFrEF and HFpEF patients. Their antidiabetic effect and kidney protection make them a widely used tool in cardiovascular diseases. We learn where GLP-1 agonists stand in cardiovascular risk protection and how beneficial they are in overweight HFpEF patients. The new mineralocorticoid receptor antagonists (MRAs) also proved their role in HF treatment. The new, non-steroid MRA, finerenone improved CV outcomes in the FINEARTS-HF trial in HFpEF patients, presented at ESC2024. The MOONRAKER program will further investigate finerenone’s place in HF therapy. He stresses that we must use all 4 pillars of HF treatment.

Professor Eric Quinton Klug shared his insights on the LIBerate-HR trial. Among patients at high or very high risk for a heart attack or stroke, the addition of the investigational drug lerodalcibep to standard cholesterol-lowering medication for one year reduced LDL, or "bad" cholesterol, levels by more than half on average, compared with a placebo. In addition, 90% of patients treated with lerodalcibep, versus 16% of those on placebo, attained the newer, more stringent guideline-recommended LDL targets set by the American College of Cardiology and other expert organizations.

David Austin, the study's lead author shared his insights on the PROACT trial. Patients who took the ACE inhibitor enalapril while undergoing cancer treatment with high-dose anthracycline chemotherapy did not show any difference in troponin T levels at one month after their last chemotherapy dose compared with those who did not take it, according to results from the PROACT study presented during a Late-Breaking Clinical Trial session at ACC.24 in Atlanta.

Professor Gregg Stone provides a summary from a real-time updated network meta-analysis comparing OCT, IVUS and Angiography guidance in percutaneous coronary intervention procedures. The OCTOBER and ILUMIEN IV randomised trials both studied OCT-guided versus angiography-guided PCI. This analysis compared the overall effects of intravascular imaging in improving outcomes of the PCI procedure as compared to angiography, as well as IVUS vs angiography, OCT versus angiography, and IVUS versus OCT. Results showed that IVUS-guided PCI was associated with lower rates of target lesion failure as compared to angiography-guided PCI.

Professor Amany Elshorbagy explains, that obese children and adults with heterozygous familial hypercholesterolemia have a higher risk of coronary artery disease compared with HeFH patients with normal weight. A similar trend is observed in patients with homozygous FH. These are the findings of a cross-sectional analysis on 35,919 patients from 46 countries of the EAS FH Studies Collaboration Registry.
Overweight and obesity are common in people with FH and contribute to their already elevated risk of CAD, even from a young age. Holistic management of people with FH should include body weight management where needed, in addition to the lipid-lowering medication, to minimize the risk of CAD events in this population.

Prosessor Eliot A. Brinton explains the results of the CLEAR Outcomes trial, the first study to assess whether bempedoic acid could reduce cardiovascular outcomes. The cholesterol-lowering drug bempedoic acid reduced the combined rate of major adverse cardiovascular events by 13%, meeting its primary endpoint in a large study of patients with high cholesterol who were unable to tolerate statins, according to research being presented at the American College of Cardiology’s Annual Scientific Session Together With the World Congress of Cardiology.

Professor Gregg Stone explains the results of the FREEDOM COVID Anticoagulation Strategy Randomized Trial. The main conclusions are among non-critically ill pts hospitalized with COVID-19, at 30 days the lower incidence of the primary outcome for therapeutic dose compared with prophylactic dose did not reach statistical significance. However, significantly fewer pts treated with therapeutic-dose died or required endotracheal intubation within 30 days. Outcomes with therapeutic-dose enoxaparin and apixaban were similar, and bleeding was infrequent with all three regimens. Based on these findings, the use of therapeutic-dose anticoagulation may improve outcomes in non-critically ill patients hospitalized with COVID-19 who are at increased risk for adverse events but do not yet require ICU level-of care.

Searching for potential cardiovascular risk factors led to many discoveries in the last decades and radically changed our view on CV prevention. Professor Nathan Wong took part in an expert discussion about Lipoprotein(a) at the recent ACC23 Congress. He explained that Lp(a) is an underrecognized genetic risk factor for cardiovascular disease. Elevated levels are seen in approximately 20% of the population so this really should be a call to action to mesure this at least once in a person's life. Particularly in those with a personal or family history of ASCVD and for those who have elevated levels. A more intensive approach at risk factor management is warranted, including more intensive efforts to reduce LDL cholesterol. At present and hopefully in the future therapies currently in development will to lower Lp(a) and will be proven to reduce ASCVD risk in those individuals elevated Lp(a).
