Personal protective equipment (PPE) is currently a topic of discussion on a global level. The COVID-19 pandemic has brought infection control procedures and the use of PPE under intense scrutiny, especially in long-term care. On a daily basis, we hear about the shortcomings of PPE supply and use, along with the lack of proper infection control in healthcare. Do not let the negative publicity paralyze you. As a leader, it is imperative to analyze what occurs in the facility and find ways to improve. By investigating competency and compliance, directors of nursing or other nurse leaders, such as infection preventionists, can improve PPE utilization in their facilities.
History of PPE
Since Leonardo DaVinci invented the first respirator to prevent contamination from chemical warfare in the 16th century, healthcare personnel (HCP) have been attempting to protect themselves from illness (Segal, 2016). As understanding of pathogens increased, PPE have evolved in response. PPE as we know it today originated in the 1970s when the Centers for Disease Control and Prevention (CDC) published the manual Isolation Techniques for Use in Hospitals. In the 1980s, PPE use intensified due to the human immunodeficiency virus (HIV) pathogen, which led to universal precautions being introduced in 1985 to prevent the transmission of infection. In addition to the gowns and gloves already being utilized, facemasks and eye shields were implemented to protect mucous membranes. In 1989, the Occupational Safety and Health Administration (OSHA) proposed a rule on occupational exposure to bloodborne pathogens. The rule on bloodborne pathogens was then finalized and published in 1991. The CDC issued an isolation guideline in 2007 which addressed PPE, including the donning and doffing procedures (Segal, 2016).
Transmission-Based Precautions and PPE
The route a pathogen spreads (air, droplets, contact, or a combination of these) dictates the transmission-based precautions that are appropriate. Once the type or types of transmission-based precaution have been selected, the PPE that will provide a barrier specific to that route of transmission is used to protect the HCP and resident. PPE may consist of face masks or respirators, gowns, gloves, face shields, and goggles.
Due to the current pandemic, the CDC issued the Interim Infection Prevention and Control Recommendations for Patients with Suspected or Confirmed Coronavirus Disease 2019 (COVID-19) in Healthcare Settings, which were updated on May 18, 2020. This guidance states facilities should use transmission-based precautions for residents with suspected COVID-19 or test-based positive results; it includes the use of gloves, gowns, face masks, goggles, and, in some cases, a N95 mask or respirator.
PPE competency
Merriam-Webster defines competency as the quality or state of having sufficient knowledge, judgment, skill, or strength. In other words, competence is based on performance and whether the HCP can properly complete the job or task. In order to achieve competency, the HCP must be educated regarding both the purpose and the procedures.
OSHA requires that each HCP know the following regarding PPE:
When it is necessary
What kind is necessary
How to properly put it on, adjust, wear, and take it off
The limitations of the equipment
Proper care, maintenance, useful life, and disposal of the equipment
In general, when observing an HCP for PPE competency, the observer should make sure the staff member is performing hand hygiene correctly, correctly determining which type of PPE to use, and donning and doffing each type of PPE correctly and in the right sequence. Below is a breakdown of what the observer should look for regarding each type of PPE to determine competency.

It's a busy and challenging time for the nurse assessment coordinator. COVID-19 is requiring all hands on deck in some facilities and CMS is providing some leeway on paperwork. In this podcast, AANAC Curriculum Development Specialist, Jessie McGill, RN, RAC-MT, RAC-MTA, discusses current challenges and some tips on getting caught up on MDS assessments if you have fallen behind.
Directors of nursing services (DNSs) and other managers can support and motivate staff in two critical ways: individually and organizationally. Taking the following steps can help staff deal with the anxiety of caring for residents in a pandemic:
Teach staff how to help each other—and themselves
Judy Davidson, DNP, RN, FCCM, FAAN, a nurse scientist at UC San Diego Health and co-chair of the Strength Through Resiliency Task Force at the American Nurses Association, offers the following two stress reduction techniques that DNSs can use themselves and teach staff as well: * Tap Out and Take a Lap. “If you see someone at work whose anxiety level is rising, you figuratively tap them out and invite them to take a lap of the unit with you. A lap of the unit usually takes about one minute, and that is all it takes to do this stress reduction technique,” says Davidson. “While walking, you coach them through a little breathwork. If they are really panicked, just do four breaths in and four out. Doing that for five cycles will add up to a minute—which is all it takes to break the stress response that the person is under and ground them again so they can get back into the game.” If the person isn’t totally panicked, “they may be able to tolerate a more complex breathing pattern, four-seven-eight: Breathe in for four, hold for seven, and breathe out for eight. Four to five repetitions of that will give you a minute that you have walked around the unit, and now both you and your colleague are feeling better,” says Davidson. “I recommend anyone to do that for helping out in-the-moment stress.” * Scanning for Normalcy. “When you are fearful, you reflexively scan your environment for threats. It is a basic human instinct, but we are going to break the fear response by turning that on its head,” says Davidson. “When you leave work and you want to leave it behind you, do this exercise,” she suggests. “Turn your head all the way to the left. That’s important because turning your head will activate the vagus nerve to help calm you down. Once you have your head all the way to the left, you slowly turn your head [back toward the right] and notice everything beautiful and normal that you can see. For example, ‘There is a bird in the tree. There is a leaf on the ground. That flower is just opening.’ “By the time you get to the other side, you have gone away from that stress response and you have brought yourself back to recognizing what is normal in your environment,” she continues. “Do that when you leave work and then any time you are walking outside for a little bit of exercise, and it will bring you back into normal moments. You can’t be in fear-based mode 24/7.”
Connect staff with key resources
The Centers for Disease Control and Prevention (CDC) has established a resource page, Healthcare Personnel and First Responders: How to Cope with Stress and Build Resilience During the COVID-19 Pandemic. This page reviews symptoms of stress (e.g., irritation, anger, or denial); identifies stress-related disorders (e.g., post-traumatic stress disorder), as well as compassion fatigue and burnout; and provides contact information for free resources (e.g., the Lifeline Crisis Chat). In addition, the CDC offers the following tips for improving coping skills and enhancing resilience:
Communicate with your coworkers, supervisors, and employees about job stress.
Talk openly about how the pandemic is affecting your work.
Identify factors that cause stress and work together to identify solutions.
Ask about how to access mental health resources in your workplace.

One-third of SNF residents experience an adverse or temporary harm event, and the majority of those are preventable. As part of CMS’s focus on raising awareness of nursing home safety and to support safer nursing home care across the nation, CMS and the Quality Innovation Network National Coordinating Center released a new resource: a Change Package to prevent all cause harm in nursing homes. The Change Package is a compendium of successful practices of high-performing nursing homes, illustrating how they prevent harm while honoring each resident’s rights and preferences.
