Studies show that genetics and the environment play a role in the population becoming more “myopic”.
In the next 30 years, 50% of the total population will be myopic.
Your eyeball continues to grow as you age. When the eye is growing faster than it should, we can’t correct it….we can compensate for it. One of the symptoms of being myopic is having distance vision or being nearsighted.
While our devices have been in our hands over the last 13 years or so, this epidemic has been growing for 30+ years.
The larger the eye gets, the higher likelihood of rise for eye disease; like cataracts and glaucoma.

Styles change. What people like and don't like, change. But our focus on patient care will never change. Our services must continue past the exam room and we have to meet our patients where they are in their life. That may present itself in green frames or stark white cat-eye frames or they be readers. Regardless, we have over 1,200 pairs of glasses for you to choose from.

Dr. Jeff answers those questions and describes
Spoiler - it's free!
Dr. Jeff & Cole discuss FSA and HSA.
FSA = flexible spending account
HSA = health savings account
FSA are not very flexible - majority of them to term. They will not roll over and when the year is over, your benefits are gone.
Most HSAs will roll over.
People get busier around the holidays and with benefits running out - we are seeing an additional number of patients in the office. Eye care practices see an influx of patients in the last few weeks of the month (December) as ‘some’ people wait until the last minute to get their annual eye exam.
Lesson 12 - A Conjunctivitis Did Not Just Walk In The DoorWe make it a practice of addressing our patients, not the condition, disease or issue. We care for patients, not their disease.
Lesson 13 - I Can Hear - Why Aren’t You Taking Care Of Me?People are busy and no one likes to wait.
We strive to not laugh and banter outside of the exam rooms or around patients - as they are here to be seen and perception is reality. If the patient perceives that you are playing around and not making them a priority - it quickly becomes their new reality.
Don't lose sight of the patient being in the room.
We should never lose when it comes to patient care.
What is DDD?

Dr. Jeff and Cole discuss Glaucoma, what signs may be presented when you get Glaucoma.
What’s your cupping? It may not be what you think it is. Dr. Jeff introduces the visual of the doughnut and why your cupping is so important.
Dr. Jeff answers questions that our patients are asking us on a monthly basis.
Can you get Glaucoma from your mom and dad?Yes
Can you get Glaucoma from your neighbor? No
What conversation am I having with my mother / grandmother, that will help us notice signs?Patients, for the most part, don’t have pain. They also won’t have symptoms - it’s less symptomatic for high blood pressure.
What’s happening with your optic nerve? This is what we are looking at every time you come in for an exam.
When you have Glaucoma (garden variety type), it’s bi-lateral (in both eyes) but asymmetric disease - one eye proceeds the other. General, people treat both eyes at the same time.
The invert to Glaucoma is Age-Related Macular Degeneration (ARMD).
Glaucoma loses vision around the circumference of the eye and conversely ARMD loses vision in the center of the eye.
Is my vision going to be full restored? With Glaucoma, when you lose your vision; we can’t bring it back.
We’re looking at millions of nerve wires and seeing if they are thinning to see if you are trending to Glaucoma, over time. We’re watching the nerve, structure and function by the OCT and side-vision test are part of it as well.
12 to 20 is the average eye pressure - it varies day to day and time of day.
4 Types of Glaucoma
You won’t feel pressure in your eyes until it’s in the mid-40s range.
What does Glaucoma suspect mean?
It means you have risk factors (age, African American, Rx, Diabetes, thickness of your cornea, performance of side-vision test) and that we are watching your measurements, pressures and other risk factors.
Does it mean I will get Glaucoma? No.
9 out of 10 people will not get Glaucoma
Will I get Glaucoma because I’m Diabetic?
No.
Having your annual eye exam is so important to consistently watching the health of your optic nerve, watching your high pressure and the general aging and health of your eyes.
We can see so much more today, just by looking into your eyes.

Dr. Jeff Kegarise introduces Performance Vision Therapy. Dr. Kegarise introduces Holli Weaver
who works with Performance Vision Therapy at Cool Springs Eye Care and Donelson Eye Care.
Performance Vision Therapy is essentially physical therapy for the eyes.
There are three main areas where a patient would need vision therapy. Those are, lazy eye
issues, muscle or functional issues in the eye, commonly associated with reading, and the eye
brain relationship, which has to do with visual processing.
Dr. Jeff Kegarise has Ron Kegarise on As I See It. Ron Kegarise is Dr. Jeff’s father. Ron

Lesson 5 is titled “Welcome To The Office”. Dr. Jeff discusses the importance of knowing your patients names, and calling your patients by their name. Dr. Jeff and Cole Evans discuss the importances of personalization with every patient that walks into the practice. This is to make every step of the way feel comfortable and personal for the patient.
Dr. Jeff puts a significant emphasis on the amount of time a patient spends in his offices. He wants to minimize the amount of time patients spend waiting to show them that their time is valued and that the practices will make every effort to not waste the patients time.
“One Patient At a Time should be required reading for students at every medical, dental, and optometry school” - Dr. Tuck
Dr. Tuck is a hospitalist, he specializes in hospital medicine. He recognizes the concepts discussed in One Patient At a Time are relevant to not only the outpatient sector of healthcare but hospital care and primary care as well. Dr. Tuck appears in a previous episode.
Cole Evans discusses lessons 3 & 4 of One Patient At a Time… “Smile When You Say That” & “Greet Everyone With a Smile and Make Eye Contact”. Dr. Jeff believes in the power of simplicity, this lesson is simple to act upon and implement, yet it can have a large impact on how patients feel about their experience. Dr. Jeff strives for excellence in his eye care practices.
Dr. Jeff and Dr. Susan in their book say “Expect it, Demand It”. This means that the expectation for this must be set with the team, and then adherence to this expectation needs to be demanded to ensure that the level of excellence within the practice is met.
Cole Evans, Director of Marketing introduces Jeff Kegarise, and discusses the previous episode that introduces Dr. Jeff and Dr. Susan’s book “One Patient At a Time”. Cole and Dr. Jeff begin to discuss relationships within the field and how the provider patient relationship begins from the very first interaction with the patient. Dr. Jeff is passionate about having an impact on patients before they even enter the office.
Cole and Dr. Jeff discuss how important patient reviews are to them at Cool Springs and Donelson Eye Care. Dr. Jeff and Dr. Susan want their patients to know that they come first as a person and Dr. Jeff and Dr. Susan put respect for their patients first and it is the priority of every visit before eye care even begins.
Dr. Jeff discusses his plan of action for what occurs when he deals with dissatisfied patients. He tries to reach out to patients and find out what has happened and what can be done to fix the issue.
Dr. Kegarise introduces Cole Evans, president of the Evans Company, and director of Marketing
for Cool Springs Eye Care and Donelson Eye Care. Dr. Kegarise brings up the topic of social
media in healthcare with Cole. Dr. Kegarise and Cole discuss the importance of social media
not only to physicians but to medical practices and the field of healthcare as a whole. Social
media for doctors can have a great impact, there are countless channels and platforms for
doctors to reach out and enter the market, engaging with their patients.
Cole Evans discusses the importance especially for doctors, through the channels of social
media, letting people into their businesses and practices, by telling their story. Marketing is
relevant even in the health industry. Media and especially social media are important platforms
for healthcare professionals to be involved in because of the high volume of customers reached
through these channels. Dr. Jeff and Cole discuss the evolution of marketing through social
media and how this process has evolved over the years.
Cole Evans introduces Dr. Susans and Dr. Jeff's book and new publication, “One Patient at a
Time”. Dr. Jeff discusses why himself and his wife Susan decided to write a book in the midst of
running multiple eye care practices. Dr. Jeff explains that he wanted to share his experiences in
his quest to make every single one of his patients feel special. Dr. Jeff and Dr. Susan have built
their practices on making patients feel as if they are the only patient every time they visit the eye
care practices. Dr. Jeff and Dr. Susan decided to co-author a book to share those experiences,
and share that passion in delivering a great and courteous experience for patients. Dr. Jeff and
Dr. Susan wanted to share how and why they have built and ran their eye care practices the
way that they have, with patients in mind. Dr. Jeff and Dr. Susan’s passions led to the creation
of their book One Patient at a Time.
Dr. Jeff brings on a special guest, his father Ron Kegarise. Dr. Jeff Kegarise discusses
metallurgy and its uses in the field of eyecare. Dr. Jeff and Ron Kegarise discuss the main
elements and metals used. They discuss the properties of stainless steel and the features that
make stainless steel the material of choice for most.
Dr. Jeff and Ron discuss the different materials used and the transferability of COVID-19 among
these materials. Stainless steel is commonly used in the medical field. Ron informs us of the
unique property that copper possesses, in which COVID-19 has tremendous difficulty surviving
on the surface of this material vs other materials such as stainless steel and aluminum. The
antibacterial properties of copper make it especially attractive to use during a global pandemic.

Dr. Jeff and Stephanie discuss the craftsmanship and creativity that go into creating every frame.
Dr. Jeff discusses the Neurolens and its dramatic effect on the improvement of headaches and migraines in patients. Dr. Evan Shields discusses his experience with the Neurolens and his own patients. In his experience some of his patients' migraines were completely eliminated. Some patients experience other symptoms aside from headaches such as light sensitivity, dizziness, and neck stiffness. Patients are often surprised that some of these symptoms are due to eye issues and nerve pain.
The Neurolens help alleviate discomfort that comes from the eyes not being properly aligned and focused. The Neurolens helps the eyes interact the same way.
Dr. Evan Shields practices at both Cool Springs Eye Care and Donelson Eye Care. Dr Shields is the lead doctor over the Neurolens Program. Dr. Evan Shields grew up in Kentucky. Dr. Shields obtained his Bachelor's degree from Middle Tennessee State University. He went to Optometry School in Memphis Tennessee at the Southern College of Optometry. Ming grew up during China’s Cultural Revolution – during which millions of innocent youth were deported to remote areas to face a life sentence of poverty and hard labor. Ming had to play the Chinese violin erhu and learned to dance, in order to escape the labor camp.
He eventually made his way to America with only $50 and graduated with the highest honor from Harvard Medical School and MIT. Dr. Wang founded a 501c(3) non-profit organization, the Wang Foundation for Sight Restoration, which to date has helped patients from over 40 states in the U.S. and 55 countries, with all sight restoration surgeries performed free-of-charge.
He was named the Kiwanis Nashvillian of the Year for his lifetime dedication to helping blind orphan children from around the world. Dr. Wang is the co-founder and president of the Tennessee Immigrant and Minority Business Group, and co-founder of the Common Ground Network, a non-profit that focuses on Dr. Wang’s life-long mission to help people find common ground and solutions to our society’s problems.
