This week Dr. Cruse talks about seeing the lowest numbers we’ve had since the end of July. Cases are really down and we are in what the CDC considers low community transmission and no longer recommends routine masking. He reminds us that we should still wear a mask if we are sick or have had a Covid exposure. He also reports that Wayne Memorial Community Health Centers have received the new Pfizer pill Paxlovid that can be used to treat Covid in the first five days of symptom onset. He talks in details about the risk benefits and data. We now have numerous treatments available for Covid.

This week Dr. Cruse talks about the improving but still high numbers here. He talks about Wallenpaupack school district ending their mask mandate which he thinks is a little too soon. He also talks about a new monoclonal antibody therapy called Evusheld that is is available at Wayne Memorial Community Health Centers to prevent COVID infection in people with severe immune suppression such that they are unlikely to get adequate protection from the vaccine, or for people that are allergic to the vaccine. However it does carry significant cardiac risk and should be used in caution and people at high risk for heart disease.

This week Dr. Cruse talks about improving numbers here but points out that it’s improving only because it has been so bad. We still have some of the highest numbers of the pandemic right now. 35% of all cases of the pandemic and 12% of all deaths have occurred in the past two months. Things are looking better, but it definitely isn’t over yet.

This week Dr. Cruse reviews the numbers which are very high still but seem to be peaking. Hospitalizations and deaths are still high from Omicron. He reports that the vaccine mandate for healthcare will go into affect for Wayne Memorial Community Health Centers and Wayne Memorial Hospital this week. They hope to get more free home test kits this week. He also talks about returning to a new normal with vaccines, masking, home testing and isolation, and treatments for those at higher risk.

This week Dr. Cruse talks about the very high cases here and around the country. Also hospitalizations and deaths are increasing with Omicron. He discusses the two new pills to treat COVID. Molnupiravir by Merck and Paxlovid by Pfizer. He focuses on Molnupiravir because they are getting that this week at Wayne Memorial Community Health Centers. There is significantly concern about it causing birth defects and should be used with caution in women of childbearing years. This week Dr. Cruse talks about the incredibly high number of cases in Wayne and Pike county. There have been 1800 cases in the past week which is 11% of all of our cases of the entire pandemic. Omicron is spreading like wildfire. While it is milder than Delta, it is not mild and is leading to hospitalizations and deaths. It is affecting younger kids and toddlers more. This week Dr. Cruse also talks about the current monoclonal antibody infusion they are giving at Wayne Memorial Hospital and Wayne Memorial Community Health Centers.

This week Dr. Cruse talks about record breaking case counts across the country. He goes into a more in-depth analysis of studies on Omicron Variant. He also reviews new CDC guidance on isolation of those infected with Covid being shortened to five days. Omicron is definitely affecting people with prior Covid infection as well as vaccinated but not boosted individuals. If you’ve not gotten your booster you really need it.
This week Dr. Cruse discusses very high and still increasing numbers here. He addresses some peoples concern that the numbers are being inflated. He reviews the data on all positive test done to the community health centers and they account for around half of all numbers reported in Wayne in Pike county. Their numbers do not include numbers from testing done through the ER or hospitalized patients or done by providers that are not part of the community health centers or done outside of the Wayne Memorial system. Therefore the states numbers are very consistent with what they’re seeing in their office numbers. He reports that 36% of their cases are currently in the under 18 year old category. He also discusses monoclonal antibody treatment, indications, risks and benefit. They are now doing that through the outpatient clinics. He also reminds us of Omicron spread and concern for holiday gatherings. Stay safe.

This week Dr. Cruse talks about continued high numbers here and strain on our healthcare system. He discusses numbers in the schools and particular that Delaware Valley school district had to close one day last week due to cases in bus drivers. Delaware Valley, which has a looser masking policy has a higher rate of Covid cases among students and staff than the districts with stricter policies. He believes that we should continue with masking in schools in January, with a tiered approach to loosening mask requirements. He also discusses more data on Omicron spread, increased contagiousness and vaccine resistance. He recommends boosters and all adults six months after their second dose.

Dr. Cruse informs us that this past week was the second highest case count here of the entire pandemic. Based on last year’s pattern he expects things will get worse from here. The rate of cases and deaths are very high. The hospital is still overwhelmed and is running short on some Covid treatments. He also discusses the Omicron variant. It is in the US and is in Pennsylvania, but not in our area yet that we know of. Preliminary data shows that it is much more contagious, but less deadly than the Delta variant. Still he discusses that high transmissibility even with lower virulence (deadliness) can lead to higher deaths overall. Get your vaccine, wear your mask, and avoid large holiday gatherings.

this week Dr. Cruse talked about how numbers are still very high here. They are lower than the week before but he feels that is due to less testing and delay on data collection over the Thanksgiving holiday. He feels the number will be very high next week. He discusses studies on transmissibility versus vaccine resistance and that transmissibility has a much larger impact on the pandemic than vaccine resistant does, and the combination of both can be catastrophic. The omicron variant recently discovered in South Africa is extremely concerning because it has both characteristics and particularly may be five times more transmissible than delta. However vaccines do still work and masks definitively work. So get your vaccine and wear your mask, this may be a long ride.

This week Dr. Cruse discusses the dramatic surge in cases locally to one of the highest case counts of the entire pandemic. He also reports that 25% of all deaths in Wayne in Pike County during the entire pandemic have occurred in the past two months. He discusses Thanksgiving and travel safety. He also discusses lifting the mask mandates in schools, now that the decision is returning to local school boards. He feels that with vaccines now approved in children down to age 5 that it is much more achievable. However with 32% of our cases in children under 18, the overall spike in cases here and the high death toll currently that this is not yet the time to end school masking.

This week Dr. Cruse reports that cases are increasing here, in Pennsylvania, and nationwide. He discusses a major spike that is now occurring in Europe and may predict what will happen here as the weather turns colder. He reviews data on vaccination versus natural infection versus both. He also discusses the upcoming change in state wide mask mandate requirements. He strongly recommend school districts take a data driven approach, where they decide on mask mandates based upon local community transmission and not on public opinion. He encourages then following the data and being willing to change strategies based upon community and school transmission. This decision of school boards affects the entire community
This week Dr. Cruse tells us that the Covid numbers here are still bad. Last week saw probably the highest death toll here of the entire pandemic, 20% of all of the deaths in Wayne and Pike County during the entire pandemic have happened in the past six weeks. The Pfizer vaccine has been approved for 5 to 11 year olds and Wayne Memorial community health centers started vaccinating them this past weekend. He also discusses the vaccine mandate rules for companies with over 100 employees and for healthcare workers. Get your vaccine.
This week Dr. Cruse reports that cases are down a little here but at 224 cases per 100,000 people per week, they are 10 times higher than in June. He discusses safety and effectiveness of masking in school children and some of the studies on that. He reports that they are analyzing the numbers in schools with strict masking policy versus looser masking policy and if that affects the number of cases in children. He hopes to have real data on that soon so that we can make good informed decisions based on it. He also discusses the recommendation from the FDA advisory committee that the vaccine be approved in 5 to 11 year olds. It is not yet available but hopefully will be within a week.
This week Dr. Cruse tells us that numbers are still high here. The big news is the hospital is overwhelmed, with the hospital full, the ER full and diverting patients. The medical workers here are overworked and burn out. Please get your vaccine, wear your mask, and do all the things we know prevents Covid transmission. He also discusses that Johnson and Johnson and Moderna boosters were approved and how to get yours.
This week Dr. Cruse updates us on the still high level of cases in Wayne in Pike County and how that is putting a strain on the hospital system here. Pennsylvania is one of six states that still has increasing cases. Also transmission in school age kids is very high and we are seeing more kids sick with Covid than at anytime during the pandemic. He reviews data on the recommendation by the FDA advisory committee on Johnson and Johnson boosters. They did recommend boosters for both Johnson and Johnson and Moderna, however that still has to go to the full FDA and CDC this week for final approval. We should know by the end of the week and be able to start scheduling Moderna and Johnson and Johnson boosters next week. He reminds us how to schedule boosters through Wayne Memorial Community Health Centers.
This week Dr. Cruse reports that despite improving numbers across the country, the pandemic is worsening here with increasing numbers and some of the highest numbers we’ve seen in the entire pandemic. He reviews that people in the know including doctors, nurses, politicians, and news agencies are vaccinated. He also discusses vaccine misinformation on social media and being skeptical of anything you read on social media. Finally he updates us on Wayne Memorial Community Health Centers vaccine clinics and how to schedule your vaccine or booster.
This week Dr. Cruse reports that even though cases are decreasing across the country, they are not yet decreasing here. He is concerned about new surges in less vaccinated pockets of well vaccinated states and is concerned that we may not have peaked yet. He also reviews the limited information on Merck’s pill to treat COVID and reminds it that it is not available yet and is not nearly as effective as vaccines. He also reviews information about COVID vaccine recommendations in pregnant women and that their vaccine team will be at the Midwifery Mingle at the Cooperage on Thursday Oct 7. This week Dr. Cruse talks about continuing increase in cases here and a significant number of cases in school age children since the beginning of the school year. He talks about new strain on the healthcare system here primarily due to burn out of healthcare personnel and staffing shortage. The burden of caring for unvaccinated Covid patients is impacting the healthcare of other patients here Northeast Pennsylvania. He reminds you that if you’re not vaccinated you definitely need to wear a mask to decrease transmission and decrease strain on the healthcare system. The choice to not get vaccinated does not just affect you but affects everyone. He also discusses the FD advisory committee recommendation for Pfizer vaccine boosters only in patients over the age of 65, those with underlying health conditions, and people at high risk of contracting Covid such as healthcare workers. This is based upon data showing that even though breakthrough cases are occurring the vaccines are very protective against hospitalization and death. Wayne Memorial is currently doing third doses for immunocompromised patients, but it’s not doing these boosters yet. More information will be coming out about when we will start doing boosters and about possible boosters for Moderna and Johnson and Johnson vaccines.

This week Dr. Cruse discusses continued rise in cases in our area. He reminds us we should all be masking at the store. He reviews the importance of students masking at school and that really there is no medical reason your child cannot mask at school. Don’t bother asking for a mask exemption. They don’t do them. Finally he reviews the very shaky data on benefit of ivermectin, an anti-parasite drug used mainly in animals, and the dangers of ivermectin and ivermectin overdose. If it does have benefit, it is small and does not nearly approach the proven benefit and safety of vaccines. “You are not a horse. You are not a cow. Seriously y’all stop it.”
This week Dr. Cruse talks about the approval of the Pfizer vaccine, and the largest safety study ever done on a vaccine which was done on the Pfizer vaccine and published in the New England Journal of Medicine last week. The safety and effectiveness of the vaccine is remarkable and it is 300 times safer than COVID. He discusses vaccine mandates in industries across the US. He also discusses the school mask mandate and the evidence of both efficacy and safety of face masks. Be happy that the schools are mandating masks and get your vaccine if you’re eligible.
This week Dr. Cruse talks about breakthrough infections, explains why the rate of breakthrough infections is difficult to determine but that hospitalizations and deaths due to Covid in the vaccinated it’s still rare. He discusses why we get breakthrough infections and that this was expected. He also discusses the CDC’s recommendation that people with moderate to severe immunocompromise are recommended to get a third dose of an mRNA vaccine and explains who this applies to. He also discusses the importance of getting 12 to 18-year-olds vaccinated before return to school and that masks should be worn during school based upon our current numbers. He urges everyone eligible to get vaccinated so that both life and school can return to normal.
This week Dr. Cruse corrects a mistake he made last week saying that Wayne County was in substantial transmission category. He did the math wrong, but three days later we were there anyway. He talks about the impact of the Delta variant on his home state of Louisiana and about overwhelmed hospitals turning away patients. He discusses how misinformation has led many people to not get the vaccine and to get sick and die. He talks about increasing numbers here in Wayne County and the importance of everyone masking indoors. He also discusses that if you’re vaccinated and are exposed you should be tested. He updates information on times that they are at the fair giving free vaccines and that all three vaccines are available there. Come out to the fair and get your vaccine or at least get your questions answered.
This week Dr. Cruse talks about the dramatic increase in Delta Covid cases across the country and here. He discusses new CDC guidance that vaccinated people in areas with substantial and high transmission should wear a mask indoors. While Pike County fits the definition of substantial transmission and Wayne for moderate, he recommends that vaccinated people wear masks. The CDC also recommends that vaccinated people who are exposed to Covid be tested, but they still do not need to quarantine unless they develop symptoms. He reviews data about increased risk of infection in the vaccinated as well as increased chance that the vaccinated will spread the virus to others to support these new recommendations. He believes we're at a crossroads where we could develop a completely vaccine resistant variant unless we vaccinate everyone rapidly and have everyone masking and socially distancing again. He also reports that Wayne Memorial Community Health Centers and Wayne Memorial Hospital will be at the Wayne County Fair giving out vaccines and educating the public about vaccination. This week Dr. Cruse talks about continuing increase in cases hospitalizations and death across the country due to Delta variant. He reports that we have seen vaccine breakthrough infections here in Wayne County but they’ve been mild. He talked about vaccine safety and reassured that no vaccine has had delayed long-term side effects and we should not expect any from Covid vaccines. He also reminds the unvaccinated that they need to mask anytime they are in public and that even the vaccinated may want to consider starting to mask again in indoor public places.
This week Dr. Cruse talks about increasing cases in all 50 states as well as increasing hospitalizations in most states. This is due to a combination of the more aggressive Delta Variant as well as low vaccination rates in certain states and counties. Covid case rates are still low here but are increasing. He’s concerned that even though our vaccine rate is better than many that it’s not enough. He also discusses vaccine misinformation and who you should trust. He argues that you should trust your doctor and other doctors in the community over most other sources. If you haven’t gotten vaccinated yet ask your doctor about it.
This week Dr. Cruse reports that cases have increased 59% nationwide. While this is mostly in states with low vaccination rate, it is also occurring in counties with low vaccination rates in otherwise vaccinated states. He feels that this is very concerning for Wayne County. Cases do appear to be increasing here and we have one of the highest rates of Covid in the state right now. He discusses that data shows the vaccines may not be working as well as against the Delta variant and there’s certainly concerns that in these hotspots around the US we will see even more dangerous mutations occur. Finally he compares the death rate from Covid to many other illnesses that we readily vaccinate for and points out that there’s no vaccine that is as likely to save your life as Covid vaccine is and possibly there is no other single intervention as likely to save your life as a Covid vaccine is.
This week Dr. Cruse talks about the new increasing rate of Covid across the country due to the Delta variant. Cases are increasing in 27 out of the 50 states, mainly in areas of lower vaccination rate. While Pennsylvania is doing well, Wayne County is labeled as a high-risk area because of its low vaccination rate. He urges everyone who is eligible but has not yet been vaccinated to get their vaccine now.
This week Dr Cruse reports that cases are no longer decreasing in Wayne and Pike County, even though they are for the rest of Pennsylvania. Cases are actually increasing in 12 states. That is related to low vaccination rates as well as increase in the more contagious and more deadly Delta variant. Our vaccine rates here are lower than both the state and national averages. He believes that unless we increase vaccination here we will also be seeing increase cases due to Delta variant in the next month. He also discusses preliminary results on their vaccine hesitancy survey.
This week Dr. Cruse reports that despite decreasing numbers here there is a slight increase in cases in the US, especially in states with a low vaccination rates. The Delta variant is playing a part in that and is increasing here in the US. He talks about the Delta variant and that if we do not get our vaccination rates up here our summer reprieve may be brief. He also talks about the importance of us helping to vaccinate the rest of the world, not just because it is a humane thing to do, but because it may protect us from further waves of infection.
This week Dr. Cruse Reviews how numbers continue to improve here from our winter peak, but are still much higher than they were a year ago going in the summer. He talks about the more contagious and deadly Delta variant now making up about 10% of cases in the US and what that means for a worse spike in the fall if we do not get more people vaccinated. He also talks about the new focus in healthcare on post Covid syndrome and the beginnings of a multidisciplinary approach to help those suffering from it.

This week Dr. Cruse reviews case numbers with us which are the best we’ve seen since October. There’s really been a dramatic drop off in Covid cases. Comparing this to Israel’s data gives us evidence that vaccines are a big part of this. We are at the same point in vaccination when Israel saw a big drop off in their cases. He reminds us that COVID is still here and we still need to keep vaccinating. We are not at the level to get herd immunity yet. He also reviews data about heart damage and young men after receiving Covid vaccine and points out that this does occur with Covid and at a much higher rate than with the vaccine. He sees this data as a reminder of what COVID does even in young people and the importance of vaccinating them.
This week Dr. Cruse talks about the dramatic improvement in numbers here, but the concern that we may see a rise in cases again from the Indian variant unless we continue to aggressively vaccinate everyone. He discusses the Increase in Covid in England with the Indian variant. He reminds us of how to get the vaccine and talks about your vaccine card allowing your to return to unmasked activities.
This week Dr. Cruse talks about decreasing numbers. He reminds us that Covid is still out there and while improving has not gone away. In fact the numbers here are still five times higher than they were a year ago. He expresses a concern that if we do not get enough people vaccinated in the summer we will see another wave in the fall and winter. He updates us on vaccine clinics in the schools. He also talks about testing and vaccine initiatives WMCHC is working on with the local summer camps. Finally he addresses concerns that the unvaccinated are taking off their masks also and does that pose a danger to the vaccinated.
This week Dr. Cruse talks about new guidance that those that are fully vaccinated against Covid no longer need to wear a mask in most indoor settings. He explains why you still need to mask in some settings such as healthcare settings and public transportation. He also discusses that the Pfizer vaccine is now approved down to age 12, and the reasons to get your teen vaccinated. Wayne Memorial Community Health Centers has vaccine clinics scheduled at the schools. The bottom line is get your vaccine so your life can return to normal.
This week Dr. Cruse talks about finally seeing decreasing case numbers in Wayne in Pike County. However the pace of vaccination here is slowing and he is concerned that we will not get to the point of really suppressing the virus at this rate. He addresses common concerns in people hesitant to get the vaccine. He announces plans to set up vaccine clinics at the schools and the anticipation that the FDA will approve the Pfizer vaccine down to age 12. Finally he updates us on WMHCHC’s vaccination schedule this week and that they are now taking walk-ins.

Wayne and Pike County have very high rates of Covid compared to the rest of the nation right now. Even with that happening the rate of new people getting vaccinated is low. He discusses that the more contagious and more deadly B117 variant now accounts for over half of all cases in Pennsylvania. He reemphasizes the urgency to get more people vaccinated as quickly as possible here. He reminds us that everyone over the age of 16 is eligible for vaccination in Pennsylvania and tells us how to get that done.
This week Dr. Cruse discusses the gradual decrease in cases in Pennsylvania, Wayne in Pike County, and the nation over all, while deaths are still increasing. He discusses the B117 variant outbreak is increasingly affecting younger adults with more severe illness and how that points to the need to get younger adults vaccinated ASAP. He tells us that new vaccine rates are slowing just as we are nearing the tipping point where we should start seeing vaccine benefit in case numbers. He also reviews vaccine safety, including the Johnson and Johnson vaccine which the CDC and FDA just cleared to resume use due to the high benefit versus the risk.
This week Dr. Cruse focuses on vaccine safety, specifically the data around the Johnson & Johnson vaccine and the pause of its use due to cerebral venous sinus thrombosis and why he believes that was an over reaction. He reminds us that everyone 16 and older is now eligible to receive the vaccine and encourages everyone to go out and do so.

He reviews some of the multi organ system damage that current can occur from Covid and evidence of chronic lung damage even in young healthy people with mild symptoms. Covid-19 rates in Wayne and Pike counties are extremely low, but some states are seeing an increase in new cases. These new cases can be due to an increase in testing, but the rise in hospitalizations in at least 8 states show that there is an increase in the severity of cases.
Wayne Memorial is testing asymptomatic individuals. Everyone who comes into the ER or who is admitted to the hospital will be tested regardless of whether or not they are showing symptoms. Dr Cruse reports that asymptomatic individuals with little or no exposure risk are turning in negative test results.
At least 12 cases in PA were linked to an individual who had been at several indoor parties, however after an outdoor party elsewhere in the country, new cases were not forthcoming. Evidence therefore suggests that spread is more likely indoors than outdoors.
Dr James Cruse discusses Wayne County transitioning to the "green" phase and how other states which never fully closed, or who re-opened early, are experiencing a spike in Covid cases. He explains how a pair of hairstylists who tested positive managed to NOT spread the disease to more than 100 clients who they worked closely with. Wayne County will move to the "Yellow" phase on Friday May 22nd and Dr Cruse explains what that will mean in terms of masking, social distancing, and working from home. Wayne Memorial Hospital has opened a 2nd drive-thru testing facility in Pike County, and we hear how testing will bounce between Wayne and Pike counties during the weekdays. More services are opening at the hospital and its satellite locations including physical therapy and non-emergent surgeries.
