What about hormones for prevention? On a national level the answer is no. But everyone needs their own analysis.
National guidelines on how to give testosterone to women for low desire.

NAMS 2022 guidelines: According to NAMS, “the benefits of hormone therapy outweigh the risks for most healthy symptomatic women who are aged younger than 60 years and within 10 years of menopause onset.” For women with primary ovarian insufficiency and premature or early menopause who are at higher risk of bone loss, heart disease, and cognitive or affective disorders, “hormone therapy can be used until at least the mean age of menopause unless there is a contraindication to its use.”

Estrogen Matters with Dr. Avrum Bluming Avrum Bluming received his MD from the Columbia College of Physicians and Surgeons. He spent four years as a senior investigator for the National Cancer Institute. He organized the first study of lumpectomy for the treatment of breast cancer in Southern California in 1978, and for more than two decades he has been studying the benefits and risks of hormone replacement therapy administered to women with a history of breast cancer. Dr. Bluming has served as a clinical professor of medicine at USC and is the co-author of “Estrogen Matters: why taking hormones in menopause can improve women’s wellbeing and lengthen their lives – without raising the risk of breast cancer” with Dr. Carol Tavris. Hear his story about how he became the advocate for hormones and hormones after breast cancer. So many people think correlation = causation – case with E and breast cancer – just poor education in our population? “we don’t know what causes breast cancer” – can people handle a grey world? Estrogen receptor positive doesn’t mean estrogen caused the cancer – discuss Dispel the myth that early puberty and late menopause is a breast cancer risk – dogma – “lifetime exposure” – but women with lots of pregnancies have lower risk. Should we just work on a re-brand? Go on HRT because if you do get breast cancer you will do better than if you weren’t on it? How do aromatase inhibitors work? People call them “estrogen blockers” so people think estrogen causes cancer and recurrence. Why only in post-menopausal women? What’s with adipose tissue? People always say it is because adipose tissue increases estrogen – is it estrone ?? Or inflammation? Doctors don’t want to be sued. Do you know of any doctor getting sued for giving hormones in general or after breast cancer? If estrogen is FDA approved for prevention of osteoporosis – why did the USPSTF say no to prevention and why are we screening for osteoporosis so late (age 65?) Do you agree with the USPSTF about no for prevention. Your opinion of Progestin causing Breast cancer" Is the placebo arm bad? Is the risk not actually that high – from 2-4% Is it the synthetic progestin? Everyone’s talking about dementia now – Dr. Bluming’s take There are 25 studies showing E is safe after breast cancer and 20 review studies– and the HABITS trial is the only one that shows local recurrence and is flawed – how many studies are enough? Advice for younger physicians who care about this work? Do you think the world is just crazy? How does Dr. Bluming cope? Dr. Jill Krapf is a gynecologist specializing in vulvovaginal disorders in Tampa, FL

Often times with pain with sex there is a physical cause, but you can’t always see it.
Pelvic pain and pain with sex is very isolating and people feel very alone. People don’t know how to talk about it.

What does “all in your head” mean and why people don’t like that.
“Those who perceive their partners as kind and understanding report more intense pain than women whose partners have responses that promote adaptive coping.” – what! What is adaptive coping?
How to communicate your pain to your partner and your doctor.
What if you are a partner of someone with dyspareunia - - “best partner response is having a partner who engages in what researches call facilitative responses – affection and encouragement to cope with the pain in an adaptive way, such as by focusing on pleasure, deep breathing or positive thoughts while experiencing the pain.”
We talk about stress, sex and the gut – things like irritable bowel syndrome related to pain with sex?
The role of birth control pills and pelvic pain and health. In her book, “When Sex Hurts” they talk about Yaz (drospirenone) and pelvic pain – a prospective study of women’s genitals and sexual function prior to starting Yasmin – and then after three months- decreased clitoris, decreased blood flow, decreased labia minora, decreased libido, fewer orgasms and more dyspareunia.
The role of testosterone in pelvic pain – one study showing women with higher androgen levels have less work related neck and shoulder pain then women with low androgen and differing levels of Testosterone may explain some of the gender related differences in pain perception

Feeling Broken In The Bedroom? What You Need To Know About Your Sexual Health - With Dr. KellyCasperson
This is an interview I did with LesleyMcShane for her podcast “Redesigning Midlife” and it was just such a goodinterview I asked her if I could share it again.
1.Basics: a. Menopause means it’s been one year and one day since your lastperiod. b. The average age of menopause is around 51. and c. for ten yearsprior to menopause, women go through peri-menopause, where hormones are doing areverse puberty.
3.Even if you don't have menopause symptoms, if you are over the age ofapproximately 51, you have gone through it.
4.The most sexually satisfied people aren’t young people, it is people in their50s, 60s, 70s.
5.Great sex is not a passive thing. It’s an active process, just like someonethat is physically fit. It has to be prioritized.
6.Use of lube increases your success of orgasm by 70-80%. Silicone based lube isbest because you don’t absorb it like you do the water based.
7.There is no “how long” should I use Vaginal Estrogen cream. The use of it islike using sunscreen or wearing a seat belt.
8.You don’t need a “sex drive”. We do things because they are awesome. Ifyou aren’t having awesome sex, don’t wonder why you don’t desire it.
9.We feel entitled to a spontaneous sex drive that has been sold to us by media.And when we don’t achieve that, we end up feeling broken.
10.Putting something in the vagina only leads to female orgasm 30% of the time sospecial focus need to be paid to the female body. Your largest sex organ isyour brain.
11.Don’t sit passively by waiting for spontaneous desire to happen. Get dressed upand go to the party even if you don’t think you want to. Chances are you’llhave a good time once you get there.
12.Control of stress and cortisol levels in our bodies will help us desire moreintimacy.
13.If there is a desire mis-match in your relationship, normalize it by talkingabout it.
14.Communication with our partner about menopausal symptoms and what we are goingthrough is important to our relationship.
15.Get educated on hormone replacement therapy as an option. When women startthinking about how they want to live as they get older and get educated, thenthey see how beneficial HRT could be for them.
16.Women between the age of 50-60 that are on hormone therapy live 3-4 yearslonger than women in the same age group not on hormone therapy.
17.For our young adult children that are starting their own sexual lives, thebiggest thing they need to know from the start is about consent.
18.They need to be aware of is orgasmic equality.
19.We have to educate our girls that pleasure is for everybody.

Hi I have a question! I just saw my gynecologist , she has me on vaginal estrogen. :the WHI has walked back virtually all of their early alarmist findings. In recent publications, they reported that estrogen does not increase “all cause mortality” or deaths from heart disease and cancer. Actually, they said, it increases longevity, most notably when begun within ten years of the last menstrual period. It is the best preventative for osteoporotic hip fracture. It is safe and effective when applied vaginally for local symptoms. And, in the most striking about-face from their 2003 headlines that HRT “did not have a clinically meaningful effect on health-related quality of life” for women in menopause, they stated in 2019 that “Hormone therapy is the most effective treatment for managing menopausal vasomotor symptoms.”

· What are other options for testosterone? This is the first time I’m hearing that pellets are harmful. I’ve done two rounds of HRT and getting ready for my third. Genetic testing in 2017 and was positive for Lynch Syndrome. I was just diagnosed with uterine cancer yesterday. 43 years old and total hysterectomy in my very near future. Terrified about trying to get my hormone levels correct post surgery!! Any suggestions or recommendations?
I’m a 38 y/old breast cancer survivor. I’m on estradiol, got all the fancy lubes, and now I’m curious about testosterone… People only seem to talk about estrogen for women, but my libido is nonexistent and penetrative sex is excruciating. Curious if testosterone also takes a hit while on hormone blocking meds post-cancer. I discuss this stuff with the oncologist but it feels trivial compared to the work they did to cure my cancer. However, my PCP won’t touch the hormone topic with me and defers to the oncologist because of my cancer history… thoughts/suggestions?
I’ve gathered so much info from you about the estradiol cream and so much more than I got my gyn to prescribe the cream for me. I’d like to note that I am 59 and I have never had any problems with painful sex or any vaginal issues in the past. I applied the first dose both inside and a bit on the outside areas, it was fine with no noticeable changes. But 4 days later I applied the second dose, this time just inside and within a day or so I felt changes. My clitoral area was sensitive with discomfort and I experienced dryness in the outer labia like never before - not really painful but noticeable discomfort. I have not taken a third dose but I really want to be able to use this so I don’t shrivel up and go away LOL. I also was getting a few hot flashes and they really decreased after the first dose but are back now, although not as much. Is it possible that the discomfort will diminish after a while if I keep taking it? Would it be effective if I only applied it once weekly?
I’m 58 and 2 years in menopause. I use an estrogen patch, progesterone, and a testosterone cream - thanks to you - but it’s not clear to me if my transdermal estrogen will protect the vulva from atrophy. Should I also be using an estrogen cream on my vulva in addition to my HRT regimen? I asked my doctor and she said creams for the vulva are only from lichen sclerosus, but that doesn’t sound right.

How sexual function is biopsychosocial and the meds that affect the “bio” part of this
There are two FDA approved meds for women with low desire. They work in 50-60%of people that take them. They work by boosting dopamine.
Addyi - filbanserin - came out in 2015. It is an oral nightly medication. Side effects are sleepiness and may include weight loss.
It is safe to use if you are on antidepressants.
Vyleesi – bremelanotide – an on demand injection – likely works by increasing dopamine.

Desire is Shaped by our Lived Experiences – The Heteronormative Theory Paper
Dr. Sari van Anders is the Canada 150 Research Chair in Social Neuroendocrinology, Sexuality, and Gender/Sex, and Professor of Psychology, Gender Studies, and Neuroscience, at Queen's University.
Dr. van Anders has published over 80 papers with research that sets out new ways to conceptualize, understand, and map gender/sex, sexual diversity, and sexuality, and also provides unique tools and theories for feminist and queer bioscience, especially within social neuroendocrinology and studies of testosterone.
Can we talk about Gendered behavior for a fast minute – a lot of people – majority people – might not know or believe that they are “socially gendered” – she explains this to us.
How does being gendered as a female affect our desire?
how some aspects of sexuality widely assumed to be hormonal, might actually be better understood with or alongside a sociocultural lens.

You propose the heteronormativity theory of low sexual desire in women partnered with men, arguing that heteronormative gender inequities are contributing factors.
Four Hypothesis
o Inequitable divisions of household labor
Women have less free time
o Blurring of partner and mother roles – its not that mothers aren’t sexual – it is that the relationship between mothers and those they mother is not a sexual one.
Nurturant experiences decrease testosterone and increase peptides like oxytocin and vasopressin.
Thinking sexual thoughts can change testosterone levels
How Cortisol affects our sexual response
o Objectification of women
Proximity to the norm matters for their desire
Women’s focus on appearance and being sexy instead of sexual

On social media A woman asked – my vagina is so dry that sex is painful, how can I fix this? Wikipedia: Gaslighting is a colloquialism, loosely defined as making someone question their own reality. The answer provided on social media to this question: - Regular sexual activity, self stimulation and pelvic floor exercise can help keep your vagina health. - IS THIS ACTUALLY TRUE? - This doctor said “I tell women to avoid sugar and use probiotics to make sure their entire ecosystem is “humming along” – NO evidence that oral probiotics affect vaginal microbiome – then she plugs her diet and her probiotic products - She then goes on to say – two additional ways to counteract vaginal dryness is to strengthen the pelvic floor by doing kegel exdrcises and using a “holistic formulation” that helps maintain optimal levels of natural moisture, and “supports a healthy vulva and vagina”. – this is her product – which states it “prevents leaks” and costs 70 dollars. Also claims to “reignight desire” – WTF I searched in J sex med - does intercourse delay or treat vaginal atrophy? – can’t find a damn thing I found on an internet site - “Regular sexual activity, with or without a partner, can help maintain healthy vaginal tissues through increased blood flow and avoid or lessen the effects of vaginal atrophy.” - No citations, so again I went to J Sex med – can’t find a thing On menopause.org - Regular vaginal sexual activity is important for vaginal health after menopause because it stimulates blood flow, helps keep your vaginal muscles toned, and maintains your vagina’s length and stretchiness. - No citations, and furthermore, the vaginal muscles are smooth muscles controlled by the autonomic nervous system – you can’t tone them with sexual activity - I will buy that stretching the vagina frequently probably allows it to be stretched frequently without pain – but is there evidence and why sex, why not just dilation? Current treatment options for postmenopausal vaginal atrophy Iuliia Naumova1 and Camil Castelo-Branco2 Int J Womens Health. 2018; 10: 387–395. It is important not to forget about the positive effect of sexual intercourse on the improvement of vaginal health – no citations (JAMA 1983;249:2195-2198) Vaginal Atrophy in the Postmenopausal WomanThe Importance of Sexual Activity and Hormones - But this study was not randomized – perhaps the less atrophy allowed the women to be sexually active and not vice versa? How do we know the sexual activity caused less atrophy? We don’t have any studies. You talk about how the clitoris is lost and found and lost and found throughout documented history. “17th-century under-the-table narrative about sex has a strong emphasis on mutual pleasure that is noticeably lacking in many of today’s sex-ed books and in too many sex-ed classes.” Why don’t we tell girls and boys about this aspect of a woman’s anatomy? We discuss the female condition of being frigid – now we have a medical disorder if we don’t orgasm with PIV sex. In 1994 U.S. Surgeon General Dr. Jocelyn Elders suggested sex should be mentioned as safe and healthy in school curricula. In answer to a question at the UN Conference on AIDS as to whether she thought teaching about masturbation in schools might reduce unsafe sex, she replied: “I think that is something that is part of human sexuality and it’s part of something that should perhaps be taught. Today I report on a paper that interviewed individuals with great sex lives. Yes, surprise, they tended to be older! Older and wiser!
Dr. Kleinplatz is the lead author on this 2009 paper and I run through their six-ish top traits that people with optimized sex lives share. Hint - good sex is not just the absence of dysfunction. AND - NONE of it was "I have a spontaneous sex drive"! This is good news for all of us!

Featured in this week’s episode of You Are Not Broken host Kelly Casperson, MD interviews OBGYN Dr. Meedlen Charles. Dr. Charles will discuss the effects of Postpartum Depression and how women can be embarrassed talking about the issues they have after giving birth.
Dr. Meedlen talks about the importance knowing that women with previous conditions of anxiety and depression are more likely to be affected by Postpartum. However, most women will have effects after birth, and this is a normal part of giving birth that many do not want to discuss. In some cultures, it is frowned upon to talk about what women experience after pregnancy and birth.
Most women will experience many difficult side effects after birth including lack of desire, body issues, anxiety, depression, vaginal dryness, and more. Some women may feel as if these effects are uncommon, but the reality is that many women experience side effects after giving birth. Women may have a difficult time having sex with their partner or breast feeding for a long period of time. When returning to the work force, women might endure a hard time “wearing multiple hats” and managing their role as a new mother. These feelings are normal and common in most cases.
We talk about pain and dryness with sex. Hormones can be a reason for this, so can birth trauma. Tips and tricks and our favorites!
Typically, there are appointments a few short weeks after giving birth to observe if a woman is experiencing postpartum. Unfortunately, many believe that postpartum will stop after 30 days or 3 months, but the reality is that it can last up to a year after giving birth. The highest risk for perinatal suicide occurs between nine months to a year after giving birth.

We talk about low libido post-partum. So common and there are so many reasons why.
The best cause of action is to talk honestly to your doctor because while it may feel embarrassing, they are there to help you. They can recommend lotions or oils to make your body feel more comfortable during sex, therapists to help you deal with your anxiety or depression, and more.

Impossible goal for 2021 – how about intimacy? It is the journey, not the destination – a journey of 1,000 miles starts with a single step.
It is not the YOU when you get there – but the journey
What is your reason for your goal? If it is so you feel better about yourself, ask yourself why you can’t feel good about yourself now. You are worthy now, just as you are. Nothing you can get/obtain will CREATE the worthiness. Feeling of worthiness comes from thoughts, not circumstances.

Today I am here with sex educator and sex therapist Tracy Dalhstead-Rienstra. She wanted me to talk about this important piece of legislation coming out on the November ballot for people who live in Washington state.
As an expert in the field of sexuality, she talks about the current standard of education that she sees in adults in her practice and what adults who didn’t get a good sex education look like in their struggles in intimate relationships.
BACKGROUND Senate Bill 5395 (SB 5395), passed by the Legislature in 2020, would require Washington’s public schools to provide comprehensive sexual health education instruction that is medically and scientifically accurate, evidence-informed, age-appropriate, aligned with health and learning standards, and inclusive of all students regardless of their protected class status.
A referendum called Referendum 90 will be on the November ballot asking voters to approve or reject Senate Bill 5395.
A 2017 Centers for Disease Control and Prevention (CDC) survey indicates that nearly 40 percent of all high school students report they have had sex, and 9.7 percent of high school students have had sex with four or more partners during their lifetime. Among students who had sex in the three months prior to the survey, 54 percent reported condom use and 30 percent reported using birth control pills, an intrauterine device (IUD), implant, shot or ring during their last sexual encounter.
The birth rate for women aged 15-19 years was 18.8 per 1,000 women in 2017, a drop of 7 percent from 2016. According to CDC, reasons for the decline are not entirely clear, but evidence points to a higher number of teens abstaining from sexual activity and an increased use of birth control in teens who are sexually active. Though the teen birth rate has declined to its lowest levels since data collection began, the United States still has the highest teen birth rate in the industrialized world.
Sex Education and States All states are somehow involved in sex education for public schoolchildren.
As of October 1, 2020:
§ Thirty states and the District of Columbia require public schools teach sex education, 28 of which mandate both sex education and HIV education.
§ Thirty-nine states and the District of Columbia require students receive instruction about HIV.
§ Twenty-two states require that if provided, sex and/or HIV education must be medically, factually or technically accurate. State definitions of “medically accurate" vary, from requiring that the department of health review curriculum for accuracy, to mandating that curriculum be based on information from “published authorities upon which medical professionals rely.”
Research on sexual abuse shows comprehensive sexual health education is an important and effective prevention strategy. When students learn about and develop skills related to affirmative consent, they are more able to set personal boundaries and respect the boundaries of others. (Schneider, M., & Hirsch, J. S. (2018). Comprehensive sexuality education as a primary prevention strategy for sexual violence perpetration. Trauma, Violence, & Abuse.)

When you empower someone with how their body works, everybody wins and confidence happens!
Dr. Gerig is owner and founder of www.pelvicsolutionscenter.com. She sees men and women who have pain in the pelvic region, which may include the lower abdomen and back, gluteal area, genitals and rectum. She is an expert in interstitial cystitis (IC). Learn that not everyone that comes into her clinic with interstitial cystitis actually has IC. The classic IC symptom – pain perception in the bladder when it fills up and it is relieved when you urinate. There usually is associated urinary frequency.
Pelvic floor muscle tension (high tone pelvic floor) is more common than IC and is often confused with IC.
IC, pudendal neuralgia and pelvic floor muscle tension are the three main things Dr. Gerig treats.
What a PT and a pelvic floor expert finds on physical exam cannot often be seen on a CT scan.
She integrates the role of physical therapy in her practice. She thinks 95% of her patients need PT. PT is the primary treatment except for ulcerative IC. Ulcerative IC is a condition where there are ulcers in the bladder (Hunner’s ulcers).
Physical therapy is vital in educating women about their bodies and bodily function.
Clean eating is helpful. Your primary fluid intake should be water. Integrate an anti-inflammatory diet. Integrate movement every day into your life. Slowly increase your activity as you are able. Dr. Gerig thinks movement and stress management are very important when you have muscle tension.
Tips for stress management!!!
Healing from pelvic pain takes time. There is no magic wand! It takes time and work.
Medications for IC and pelvic pain. Elmiron side effects. Supplements used – glucosamine and quercetin, aloe vera. Other medications used: histamine blockers, neuromodulators, vaginal muscle relaxants.
Dr. Gerig’s goal is to get her patients OFF medication eventually.
Switching topics, Pudendal neuralgia. This is a pelvic nerve called the pudendal nerve, it feeds the rectum, urethra, genital. Symptoms are burning, throbbing pain, worse with sitting. People often have urinary frequency and a “foreign body” sensation in the pelvis. It can cause pain with urinating, pain with sex, pain with bowel movements.
Post childbirth child pain. Prolonged pressure of the baby’s head on the pudendal nerve can cause urinary issues. Women can have pain with sex, pain in tailbone. Find a PT who is well trained in treating pelvic pain.

When estrogen goes away, the vulva and vagina loose their blood flow, elasticity and resistance to injury and infection.
