I call it the "Frankenstein procedure"! There's a company that is processing dead people's fat and selling it to plastic surgeons who will inject it into your face or buttocks...or anywhere. Wow. This raises a lot of questions. First - the ethics. You or your family donates your organs, expecting that your kidneys, liver, heart, and lungs will help another person live a normal life. But do you expect that your fat will be harvested and used for cosmetic reasons? And do you expect that the fat will enrich the finances of the company that sells it and the plastic surgeons who inject it? I'll be not. And then there's the question of safety - the company can be certain the "donor" didn't have HIV or hepatitis, but they can never be certain the person didn't harbor Mad Cow Disease (Creutzfeld-Jacob disease in humans - same "prion", different animal). Botox is the most popular procedure in all of cosmetic surgery. About 10 million people in the US have Botox every year. While the term "Botox" has become a generic term, there's actually 6 drugs that have Botox like effects - Xeomin, Dysport, Jeuveux, Daxxify, and Letybo are the others. These drugs all work basically the same way. But now Abbvie, the company that markets Botox, has applied to the FDA for approval of a new short acting Botox-like drug. This one, called TrenibotulinumtoxinE (TrenibotE), works within hours (not days), but only lasts 2-3 weeks (instead of 3-4 months). The only way this one makes sense to me is to mix TrenibotE with Botox to create a hybrid drug that works immediately and lasts 4 months. But I would expect the costs to be significantly higher than either drug alone. Watch for this combo in 2026! Cosmetic surgery must be pain free. To have painless procedures, plastic surgeons often use general anesthesia, where a breathing tube is placed and your brain is basically anesthetized. This type of anesthesia is important for liposuction, tummy tucks, and most breast procedures. But general anesthesia has risks, particularly cardiac problems and postoperative nausea. So, when possible, intravenous sedation, using propofol and versed, is used. Face and eyelid lifts are procedures that often are done under IV sedation. And finally, some procedures can be done with you wide awake, using only lidocaine local anesthesia. Mole and scar revisions can be done that way, and with the proper patients, eyelid lifts, small liposuctions, and procedures like chin impalnts can also be done under local anesthesia. COVID19 is a terrible scourge.  But it has allowed many people to have cosmetic surgery because so many people are telecommuting - performing their jobs from home.  And so many procedures now can be performed with just a day or two lost from work - as long as no one sees you.  And with masks, even procedures that cause bruising of the face can be hidden.  Those are some of the reasons cosmetic surgery is so popular now.

PRP - or platelet rich plasma is one of those mythological procedures in cosmetic surgery.  Sometimes called the Vampire facial, this procedure takes your own blood, removes cells, and reinjects it into your face or scalp. When I perform cosmetic rhinoplasties (nasal reshaping surgery), I rarely touch the septum. Most patients have a slightly deviated septum (the cartilage between the nostrils) but unless the septum is obstructing the nose or tilting it significantly to the side, it should not be operated on.  That's because the septum is the structural support of the nose and it can collapse if it is operated on.  So many septums are operated on just to get the patient insurance coverage.  That's wrong at so many levels  - medically and socioeconomically.

Barbed sutures have been the noninvasive facelift hope of plastic surgeons for over a decade.  The first generation failed miserably and were taken off the market by the FDA.  A new generation of dissolvable threads using barbs or cones arrived about 2 years ago.  I was convinced that this generation was better.  Early on, I gave up on the procedure, realizing that it was not a good procedure.  Last week, a paper was published in Aesthetic Surgery Journal that discussed the over 30% complication rate and the total failure of the procedure by a year, and even earlier.  On tonight's show, I discuss this procedure.  Also, in graphic detail, I explain how I perform a rhinoplasty - nasal reshaping.

A French billionaire died last week during a penile augmentation.  We won't debate the merits of that procedure, but tonight we discussed how to stay safe during any plastic surgery.  Of course, that means selecting a plastic surgeon certified by the American Board of Plastic Surgery and having surgery in an accredited operating room with a board certified anesthesiologist.  And my advice is to see a board certified internist.  Consider having a stress test if you are over 60.

Tonight, my guest was Dr. Robert Grant, chief of plastic surgery at both Columbia and Cornell. We spoke about a new study that documented just how easy it is to cause blindness when wrinkle filler is injected in the upper portion of the face.  This is particularly important since States are now allowing non physicians such as dentists and nurses, to inject filler. I hate to say "I told you so" but...I told you so. Over a year ago, I discussed how plastic surgeons were being enticed to perform vaginal rejuvenative procedures with lasers or radiofrequency devices. Plastic surgeons have sent out a warning that the chance of dying with the Brazilian Butt Lift (BBL) is 1 in 3000. You may not think that is high, but it is the most dangerous procedure in plastic surgery. In the BBL, many ounces of fat are injected into the buttocks to create a chubby buttock. (Interestingly, even 10 years ago, I was asked to REMOVE fat from the buttocks via liposuction....) Fat can enter the veins and travel through the heart into the lungs. The advisory did not tell plastic surgeons not to perform the procedure, but, reading between the lines, my feeling is that this procedure should simply not be performed. Period.
