In our field, we hear and use the word attachment so often. And, in the community of trauma-informed clinicians, I'd always felt like we had a deep understanding of what this word means.

But when I sat down with Dr. Mara Tesler-Stein for the second time on The Zero Disturbance Podcast, we got to unpack the lack of understanding that lies within this concept for our clients... and possibly even for the folks in our clinical communities.

A lot of research in the perinatal field suggests that we jump to a "disturbed attachment" diagnosis with our NICU babies. However, Mara helps redirect us to what she sees as the real key to fostering secure attachment for parents and children.

And the answer is not so reliant on the NICU experience that the research suggests. In fact, it's actually quite simple.

If parents of NICU babies are able to respond with attunement, responsiveness, and sensitivity, then secure attachment will find its way. And yes, that means that parents need to put in their own work, spending time reflecting on their own attachment behaviors and styles, but when that work is navigated, they're able to give their babies the secure attachment they so lovingly want to offer.

And that's the magic in the trauma-informed therapy we offer our adult clients.

This interview was a powerful reminder of how the narratives we create about parents or kids are sometimes an interpretation (or misinterpretation) of behaviors and urges. As we explored attachment and parenting throughout the lifespan, even touching on complex diagnoses such as neurodivergence, the vulnerable child, and RAD, Mara brought me "back to home base" in the NICU.

Mara Tesler Stein, Psy.D., PMH-C is a clinical psychologist practicing in Chicago and Jerusalem. She is the founder and director of The Touchstone Institute for Psychotherapy and Training where she offers both EMDR Foundational trainings for perinatal specialists & advanced EMDR workshops which take a deeper dive into the intersections and applications of EMDR and perinatal mental health.

Dr. Stein is a staunch advocate for trauma-focused care and is trained and certified in a range of integrative trauma therapies. A certified EMDR therapist, EMDRIA Approved Consultant and Trainer for the Institute for Creative Mindfulness, Dr. Stein is also certified in Emotion-Focused Family and Couple Therapy, is a Gottman Certified Therapist, and continues to pursue training in Clinical Hypnosis, Yoga-Informed treatment, Advanced applications of EMDR, Brainspotting, and Ego-State therapies.

She is the co-author (with Deborah Davis, Ph.D.) of Parenting Your Premature Baby and Child: The Emotional Journey (Fulcrum, 2004) and Intensive Parenting: Surviving the Journey Through the NICU (Fulcrum, 2013).

Her trauma work is grounded in her personal perinatal journey, which began 25 years ago taking her through infertility, twin pregnancy, prolonged hospital bedrest, the NICU, and years of raising NICU graduates.

Supporting families, even under the best circumstances, can be challenging. So what happens when we need to support families when the world is not set up for their kids to succeed... or even survive?

I had the opportunity to sit down with author Wendy Jones to discuss what it means to show up not only as a therapist for our young, trans clients but also what it means to show up for their parents.

Not unlike any other new or different experience, folks benefit from getting support from those who have a similar thing happening in their life. And that's exactly what Wendy has set out to do with her new book, Transformed.

In our conversation, we dive into our shared experience as moms to trans kids. We share stories about what our kids said and did that helped guide us in understanding their truth. For us, it looked similar; for other parents to gender non-conforming kids, it can look different. But one thing remains true; the value in following our kids' lead, in each singular moment, as they show up in front of us.

And, for a therapist, or any other adult in a trans kid's life, it's the same strategy.

Did you know that 41% of gender non-conforming individuals will attempt to take their own lives? That's 10 times higher than their cis-counterparts. And, trust me, I know that's a harrowing statistic, but there is good news.

Wendy helps us understand the power we hold as therapists and how gender affirming adults are a key piece in the preservation of young, trans lives. Studies show that with at least one supportive adult, suicide rates decrease.

How powerful is that?

With research like this growing rapidly, it's clear that it's time for clinicians to get trained in how to offer gender-affirming care for trans and gender non-conforming clients.

Y'all, Wendy and I get vulnerable in our conversation. We talk about how we tried, and sometimes failed, at finding resources, how we grieved the loss of the futures we had visualized for our kids, and how we navigated losing friends and family members who just couldn't get on board.

I'm feeling empowered to share this with you and my hope is that you'll listen, learn and grow with us.

As a gender therapist and mom to a transgender child, Wendy felt there was a strong need in the world to support parents of trans kids.

She knew that her own journey was challenging at first, and finding support wasn't always an easy thing. As she had been working with transgender people and their parents over the past five years, Wendy came to find that the parents were oftentimes scared and felt alone in the process of accepting their child's gender identity.

Due to the time constraints of therapy, she wanted to find a way to reach more parents with her own hard-earned strength and wisdom. Writing seemed to be just the right medium to accomplish this goal. Books were one of the main ways that her family got the necessary knowledge to start their gender journey eight years ago. However, there weren't many books out there that offered both lived/personal and clinical experiences.

Wendy hopes that her book can reach parents around the world that are in need of education, support, and encouragement. Her overreaching goal is to help trans kids by helping their parents and to be a strong positive force in today's gender revolution.

Anyone else feeling irritated by how clinicians love to tell each other what is right or wrong? Ever notice how scripts often don’t thoroughly address client preferences or the process of client engagement? Where is the role of autonomy for the client?

To remedy our irritation, today we zoom out and get curious about how we make clinical choices, and how we engage with our clients about making those clinical choices.

For EMDR clinicians, this can be tricky. Questions may already be coming up for you like: How do we make a shared decision with our clients when we have a protocol, step-by-step process, and/or script that we are to follow to do it right and be effective?

Herein lies the balancing act between positive health outcomes and patient autonomy. How and when do we make clinical choices with our clients? How do we follow the script or protocol, and stay client-centered? What’s important to the client? How do we triage what is clinically relevant? The questions are important, and the answers feel challenging.

Good news: physicians have been researching this for decades. And it’s time for us to catch up with the rest of the healthcare literature. Today, as we talk about shared decision making, we realize that we cannot say we are trauma-informed if we are not client-centered. I said what I said.

You deserve to have richer conversations with your clients, in consultation, and even in Facebook groups. As we dive into models of shared decision making, get clarity on which shared decision making model you’re using when you make clinical choices. By identifying your orientation along the shared decision making spectrum, you'll get to have more productive conversations with your colleagues and clients.

And you can finally understand how to make EMDR a client-centered therapy. Perhaps you were also clinically trained in a paternalistic model of shared decision making. I was, and boy did it mess me up. The paternalistic model tells us the rules about EMDR, like talking during Phase 4, what target to start from, and what form of BLS to use.

Carol Miles MSW, LCSW, a clinical social worker specializing in individual psychotherapy, maintains a private practice with adolescent and adult clients. She also provides EMDR training and consultation under her Three Rivers Training Center. She currently specializes in offering intensive EMDR therapy to address trauma and healing.

A graduate of LSU School of Social Work, Carol specializes in Clinical Social Work, with over 30 years of experience as a social worker in clinical, administration, and marketing with the public and private sectors. Among her areas of expertise are working with clients who have eating disorders and trauma. Carol has often presented at statewide conferences and to general audiences on topics ranging from Eating Disorders, Assertiveness, EMDR Therapy, DBT, and developing a private practice. She taught as an adjunct professor at the Tulane School of Social Work from 1998-2018.

Carol is an EMDRIA Certified Therapist, and an EMDRIA Approved Consultant and Trainer. She's been a member of EMDRIA since 2008, shortly after being trained in EMDR Therapy in New Orleans. As a volunteer for EMDRIA, she served as a member and co-chair of the Conference Committee and chair of the University Special Interest Group. She founded and is the Regional Co-Coordinator of the South Louisiana EMDR Network, stepping down in 2022. In 2017, she was elected to serve on the EMDRIA Board of Directors, 2017-2021. She served a 2-year term as the President of EMDRIA, 2019-2020. She currently serves as the Past-President of EMDRIA, 2021. She was appointed to fulfill a one-year term 2022.

I sat down with the one and only Dr. Jamie Marich and had the opportunity to hear from a clinician whose transparency about their own identity is the very thing that makes them a great trainer, therapist, and overall human being.

In our conversation, we talk about how the messaging and training many therapists receive about dissociation creates an unfounded fear and misunderstanding about clients with dissociative minds.

Jamie's ability to take a critical look at the therapy community's fears while sharing her own lived experience as a person with a dissociative mind, is the key in helping us understand that our community is better off with a modified approach to dissociation.

We examine the value of disclosure and advocacy as an effort to decrease the stigma around this identity so that therapists may actually serve their dissociative clients.

It's time that the trauma-informed community understand the identities of the folks who seek out our help in a way that goes beyond protocol. And our hope is that instead of seeing the dissociative mind as something to be feared, we'll all begin to see it as a super power instead.

As therapists, we all want to feel that we are helping others and feel secure in our own lives. Especially during a pandemic.

As highly trained, trauma-informed therapists, our mission is to guide our clients in developing secure attachment and healthy relationships. Alongside them, we also seek to develop a security in our private practice that provides us with healthy choices in our careers.

And how we learn about what gives us security depends on who teaches us what real security means.

For many of us, we’ve learned that being on insurance panels gives us security; insurance companies send us lots of clients and requires very little marketing on our part to reach “full.”

But does full feel good? Do low reimbursement rates from insurance companies allow us to feel the type of security we really crave? Our series of innovative thinking continues, and today we are honored to be joined by Thomas Zimmerman, EMDR Consultant & Trainer.

Thomas helps us think about how we resolve traumatic memories, and introduces us to The Four Blinks Version of Flash. What I loved most about talking with Thomas was how he got me thinking about how we translate EMDR and other trauma-informed therapies so they are accessible to clients and clinicians. Healing trauma is a social justice issue, and we are all a part of it.

I'm excited to journey with you this year as we learn about Flash/ Four Blinks, EMDR 2.0, and Brainspotting to expand our abilities beyond EMDR to serve our clients. These other modalities that tax the working memory deepen our understanding of how EMDR works, giving us more clinical reasoning ability.

Thomas Zimmerman, Ms.Ed., LPCC (he/him) is a highly trained and experienced trauma therapist who has worked with hundreds of severely traumatized clients and is familiar with the wide range of ways that trauma may express. He specializes in attachment wounding and working through problems with resourcing with severely complex trauma. Much of his work is organized around the central “Dip Your Toe In” approach in all phase of working with complex trauma.

Thomas has attended graduate programs at Western Kentucky University, Bowling Green State University, and Youngstown State University.

Do you have an innovation mindset?

You know who did? Francine Shapiro.

She first created EMD in 1987 after her famous walk in the park, and later created EMDR in 1991 with standard protocol (8 phases) and the AIP model. EMDRIA wasn't established until 1995.

Over the past 30 years, many clinicians have gone on to expand our understanding of healing trauma and EMDR, even bringing in new theories about taxing the working memory (EMDR 2.0).

When we hear the word imagination, a variety of things may come to mind. Maybe it’s two kids playing a pretend game or maybe it's a fantasy television show or maybe it’s even a comic book with some creative characters.

What we don’t typically think about is EMDR therapy. In fact, when we think about imagination and EMDR, our training might even tell us that what we’re looking at is dissociation.

That's why today, I want to open up a conversation about the relationship that we, as clinicians, have with imagination.

We know that, for children, imagination holds the key to so much of what we know to be human development. Imagination is what kids use to play, and to learn, and to try on different identities and cognitions. It’s a powerful tool that so many children use to process their day to day life.

So, y’all, when our adult clients are reprocessing childhood trauma and a cartoon character or some other imaginative figure comes in, I want you to pause before you write this off as dissociation. Take a moment to recognize that you may actually be exactly where you need to be: in their childhood neural networks and narratives. Then take a moment to recognize that your client may be doing exactly what they need to be doing in Phase 4: connecting those childhood narratives to the present, adult narratives.

We put so much pressure on ourselves to identify when our clients are dissociating in Phase 4 that we’re quick to react with the assumption that we need to bring them back to target or back to the present. But what if, instead, we were to take some time to let imagination do the job it was meant to do?

I imagine we’d be offering a resource to our clients that we didn’t even know they had.

Y’all know that I believe EMDR is magic. And I think, among the many reasons, the magic of EMDR lies within its structure.

It can feel safe to hear and repeat the message of “stick to the script and trust the process.” And while the message has a positive intention, it's just not good enough.

In fact, it's basic. And EMDR is too sophisticated to be basic.

At first we see the benefit of this message with our peers in basic training exercises and single-incident trauma clients in our practices; it feels reassuring and so we internalize it.

But then something troublesome can happen.

When a complex trauma client walks through our door and the line-by-line script doesn’t offer what that client needs, we get cold feet with EMDR.

And not only do we lose trust in the process, but we lose trust in ourselves.

Sticking to the script and trusting the process should no longer be the default message in basic EMDR training or Facebook comments for those seeking consultation. We deserve more nuance than that because we are far beyond basic.

So, I’d like to offer something different. What if this idea just meant that we believe in the power of EMDR, the value of the 8 Phases, and that we trust the AIP model? And what if we used this as encouraging blueprint instead of a line-by-line script to be repeated?

If we can alter the way that we value the structure of EMDR, we can alter the relationship that we have with ourselves as EMDR clinicians.

We know that every experience has feelings attached to it. And, as therapists, we’re apt to listen to and validate those feelings for our clients and (hopefully) for ourselves.

And learning, just like all of the other experiences in life, has feelings attached to it. And while we would typically create space for the feelings that come up, in Phase 4 we tend to pick and choose what feelings we think are appropriate for this time.

Reprocessing in Phase 4 is just learning, y’all. And for our clients (and for us), there are going to be feelings that show up as this happens.

Our job is not necessarily to interfere with these feelings because we believe that something else should be happening. Our job is to understand if that frustration, or other feeling, is clinically relevant.

And spoiler alert, oftentimes, it is.

I’ve identified the 5 questions that I ask myself whenever I’m feeling “stuck” in Phase 4 and my hope is that this framework will help you understand that sometimes “stuckness” is the clinical stepping stone you need to understand for your client.

If your experience as a newly trained EMDR therapist was anything like mine, you probably started to see that “sticking to the script” to navigate through standard protocol was confusing when working with complex trauma clients. Why? Because the beauty of standard protocol often needs interweaves, parts work, and more Phase 2 preparation (things not in "the script") in order to flow. And while some basic trainings teach us these things, many of us don't get to learn these more advanced topics in the beginning of our EMDR training.

And far too often, the lack of understanding to work with these complex clients creates a fear in us. We worry that we can’t do EMDR because we don’t know what to do when reading the line-by-line script isn't working.

I want to offer a framework for you to think about case conceptualization in a way that lowers the stakes when it comes to choosing a target with your complex trauma clients.

Every clinician, whether they think they work with reproductive issues or not, needs to hear this discussion! Why? First, even if your clinical work doesn’t focus on these topics, you’re still likely to find them showing up in your office. Next, how we think about incorporating EMDR therapy during the reproductive period, especially during a client’s pregnancy, can be applied conceptually to every client! By exploring the hopes, fears, and hesitations that arise when incorporating EMDR during the reproductive period, we illuminate what comes up across clinical practice areas.

There are a lot of alarming messages in the therapy world about EMDR and pregnancy. And for many EMDR clinicians, this messaging has instilled an aversion to doing EMDR with our pregnant clients.

We're taught to fear the "dangers" of doing EMDR in the first and third trimesters, the threat EMDR has on high-risk pregnancies, and that we should, at all costs, avoid any reprocessing in Phase 4 with our pregnant clients.

And while this messaging has a good intention, it loses sight of one of the biggest questions associated with EMDR and pregnancy: what's the risk if we don't do EMDR with our pregnant clients?

I sat down with perinatal mental health advocate and expert Dr. Mara Tesler Stein to learn more about EMDR and pregnancy to unpack the past, present, and future behind this misunderstood question.

Who do you think you are? You’re too big for your britches. Wait your turn; you’re too young. You may have grown up hearing similar messages that were meant to help you, but a lot of those messages are just the result of generational trauma. Instead, these messages did more harm than good.

Even as adults, people often want to put their fears onto us, and give us a plan that works for them but not for us. We can no longer hold on to beliefs that do not serve us. We cannot be effective therapists or teachers having listened to these voices and choosing to continue to listen to these voices.

As adults, we have choices. We can make intentional decisions about the voices that we let into our heads and into our hearts. Of course, sometimes we listen to the wrong people, but it’s important that we take responsibility and make a choice on who we are going to listen to and who we aren’t.

If you have any desire for other people to listen to you, you have to be very intentional about who you are listening to and who is guiding you. Surround yourself with people who believe in you and who allow you to let your unique light shine.

I was recently speaking with a consultee about her experience with veterans and I asked if she would be willing to share her unique perspective. Her response was, “what am I going to say that hasn’t already been said?”

This got me thinking. Why don’t people create content? It’s not because they don’t have ideas or because they’re not smart. I realized it’s not just about the content, but it’s about how it is delivered.

Each of us is a combination of all of the people and voices that we have taken in. This creates a unique perspective within us that no one else has. Each of us has different content to share based on our own unique perspectives, and each of us will deliver that content in a unique way.

Similarly, content is received in very different ways by different people. Even if what you are sharing has already been said, no one can say it in exactly the unique way that you can, which means that it has never been received in the same way either.

When I came up as a therapist, I was taught to offer sliding scale or pro bono spots as a way to be generous. Maybe you were taught this, too.

Y'all, I did a thing: took my twins on vacation in Pacifica, stayed at a new house we've never stayed in, and I did it ALL BY MYSELF. Talk about a parenting milestone!

In addition to creating new neural pathways on our inaugural mommy vacation, I noticed old neural pathways. My brain was linking up associations with a relationship I had in Pacifica nearly 15 years ago, and it was giving it to me in pieces, just like someone throwing memory fragments of confetti.

These confetti moments gave me a palpable reminder of how our brain is always trying to give us what we need by rapidly linking up associations and bringing them to the front burner. And for that, I have a deep respect and appreciation for the brain.

By noticing myself noticing my process, it got me thinking about how doing EMDR as a client has literally changed my brain. And perhaps completing EMDR targets is not just about decreasing the target disturbance, but, more importantly, it's about changing the way the brain processes information forever. Is there anything more incredible than that?

As EMDR therapists, we know that we never want to overwhelm our clients with feeling too much or too fast when it comes to trauma. Less is often more when creating safety. We know this because we are taught to spend time in Phase 2 with resourcing and preparation before going into trauma networks. And when we do go into Phase 4, we know to keep clients in their window of tolerance. And, if they leave that WOT space, we bring them back so they can process the trauma effectively.

As people living through an almost 2-year pandemic, we know that we have been and remain firehosed by trauma. And we are often not in our window of tolerance. And taking breaks from trauma (and the painful feelings that go with it) is responsible and wise as we live to survive.

When we talk about wealth, success and boundaries in our field, it can feel like a controversial topic. And that's because, for therapists, tons of negative cognitions have been assigned to our financial accomplishments.

If we don’t accept insurance or our rates are “too high,” we’re demanding. If our generosity isn’t deemed substantial enough, we’re unfriendly. And if we attempt to create offers that financially benefit us, we’re greedy.

But y’all... if we had a client come into a session with these negative cognitions, we’d buzz that right out of the room. So why don’t we allow ourselves that same shift in our mindsets?

Maybe it’s the exhaustive language of rate shaming or the illusion that we should always be available to help everyone or possibly, it’s the fallacy that our one and only goal should be for our practice to be “full.” Perhaps it’s all of these things and more. Whatever the cause, the problematic culture that shapes what is deemed “appropriate” in terms of our success is.... well, it’s bullshit.

I want for other EMDR clinicians what I have discovered for myself: protecting my time, being intentionally generous, and creating high-value offers doesn’t mean that I’ve traded off what it means to be a good therapist. I’m done letting anyone else determine what success in my practice looks like. And you should be, too.

I opened up my laptop the other day and ended up in one of the many private practice Facebook groups. I stumbled across a post I’d seen 1,000 versions of before. And it went something like this... “I’m an exhausted parent and I don’t know how to fit my client schedule into my personal life. I’m so tired and so burnt out and I don’t know what to do.” I scrolled through the comments to see what our community had to offer and I won’t lie to y'all... I was infuriated.

Comment after comment I read: “I’m so sorry!” “It’s so hard, but hang in there. It’ll get better!” “Such a bummer. I feel you!” I read over and over and over as folks in our community offered up empathy and nothing else.

Y’all we have normalized burnout and exhaustion to a point where clinicians in our own community don’t even believe they’re in control. We’ve accepted a communal collapse of morale to a point where we’ve encouraged each other to lean on a crutch of learned helplessness.

Our empathy has bled into a laundry list of excuses as to why it’s “normal” to feel the way we do. If, as EMDR clinicians, we understand the importance of support therapy and change therapy, why aren’t we living that value within our own community??

It’s time that we come forward against the acceptance of burnout in our community by taking back the agency we so desperately need. When my twins were two years old, my client schedule wasn’t working for me and I’ll be honest, my life was miserable. And listen, I get that there are some things in life that you just have to get through like teething or cold and flu season.

But y'all, running your own private practice is not one of those things. I changed my practice to something that made sense for me and I won’t lie to you... it was hard. And I had to ask myself questions about what I wanted from my career that I was afraid to answer and I had to set goals that I was afraid I wouldn’t reach. But guess what? I did it anyway. And I learned that in our practices (and in our lives), there isn’t failure.

There is simply forward movement with data collection, interpretation and experimentation. It’s a lather, rinse, repeat kind of formula until your practice gets to a place where you get what you need, when you need it and how you need it.

As therapists, we know that things don’t get better when they get ignored. So, let’s move beyond just offering support for each other because there’s a moment when empathy isn’t enough.
