I see many people making the mistake of using the terms hospice and palliative care interchangeably; however, they have distinct differences. I wanted to take some time today to break down those differences, explore OT’s unique role in each setting, and share my tips and personal stories to help practitioners navigate these challenging but rewarding settings. Providing OT in the home or doing mobile outpatient has become so much more popular in the last few years, especially for individuals who are branching out into their own private practices. But if you’re working for one of these companies or considering starting your own, it’s important to understand the differences in billing and scope of practice between the two, as it greatly impacts patient care and reimbursement. That’s why this week on The Amplify OT Podcast, I am talking about home health OT vs OT in the home: key differences in billing and practice!

  • The fundamental differences between home health (Medicare Part A, provided by home health agencies) and OT in the home (mobile outpatient, typically billed under Medicare Part B).

Addressing Fraud And Upholding the Professional Integrity of OTs [Strengthening Occupational Therapy from Within]As clinicians, it is our job to find the balance between operating in a legal, ethical, and professional manner while always keeping the client’s well-being a top priority. As you may have experienced, these things don’t always line up and we find ourselves navigating incredibly difficult situations. We can be so quick to point out abuse from other professions, family members, etc. but can be hesitant to call out abuse when it comes to our colleagues, bosses, or other practitioners. But if we want to continue to strengthen our profession from within, it is our responsibility to recognize and address these red flags of fraudulent or unethical behavior, regardless of the consequences we may face.   • Historically, the United States has leaned closer to a volume-based care system, only adopting value-based care alongside the Affordable Care Act of 2010.
  • Pros and cons of volume-based and value-based care models.

  • Less common for OT practitioners, Capitated payments (value-based) can be compared to a gym membership where you pay a set dollar amount per month regardless of how many times you go to the gym.
  • Bundled payments (value-based) are like getting a paycheck where a certain amount goes towards paying necessities like bills and groceries and the rest is up to you to spend as you choose.

Spring 2024 Amplify OT and Healthcare Advocacy Updates2023 was a big year for passing healthcare legislation. A lot of what came through Congress in 2023 is starting to be implemented early in 2024, impacting OT in a big way. That’s why this week on The Amplify OT Podcast, I am talking about OT advocacy updates for spring 2024.

In this episode, I cover:

  • With the passing of the 2022 Omnibus Bill, Congress established a new Medicare mental health benefit known as the intensive outpatient program (IOP).
  • With the passing of the Lymphedema Treatment Act in 2022, Medicare Part B will now cover standard and custom-fitted lymphedema compression treatment items for each affected body part for patients with lymphedema.
  • CMS rolled out new Caregiver Training codes that can be used as of January 1, 2024. This is the first time OT practitioners can be reimbursed for services that do not directly involve a service provided directly to the client.
  • In a rare turn of events, CMS is increasing Medicare Part A payments to home health agencies by 0.8% in 2024, equating to $140 million.
  • Home health will see some changes to their quality reporting program and value-based purchasing in addition to the payment increase.
  • OT services paid for under Medicare Part B will face a 3.37% decrease compared to 2023.
  • For 2024, the updated KX Modifier therapy threshold is $2,330 for OT and $2,330 for physical and speech therapy services combined.
  • General supervision of OTAs in private practice was unexpectedly extended until the end of Calendar Year 2024.
  • Aetna is delaying the implementation of the OTA payment differential from December 2023 to March 2024.
  • The Consolidated Appropriations Act of 2023 confirmed telehealth coverage in hospital-based outpatient clinics for therapy under the Medicare Physician Fee schedule through December 31, 2024.

We just ended the 117th Congress with some wins, some losses, but most importantly, a plan to keep moving forward to improve the Occupational Therapy profession for both patients and practitioners! That’s why this week on The Amplify OT Podcast, I am talking about the 2022 OT advocacy wins and losses from congress!

Congress ended its term with a massive 1.7 Trillion Dollar legislative package- The Omnibus Reconciliation Act of 2023 (otherwise referred to as the Omnibus Bill). Within this bill, there are quite a few healthcare policies that impact our practice.

To recap, here are some of the wins and areas for improvement that resulted from the 117th Congress:

The Wins:

  • The Allied Health Workforce Diversity Act was officially passed!
  • Another extension was passed to allow Occupational Therapy practitioners and others to continue providing services via Telehealth for Medicare beneficiaries and receive reimbursement until the end of 2024.
  • The Lymphedema Treatment Act was passed to expand the coverage to include Lymphedema garments as well as other commonly prescribed items.
  • A new Medicare mental health benefit was passed for intensive outpatient services that are provided at Federally Qualified Health Centers (FQHC) or rural health clinics.
  • A correction was made to a grant administered by the Health Resources and Services Administration (HRSA) to expand their funding that allows OT programs to apply for behavioral health workforce education training to doctoral-level programs.

Areas for Improvement:

  • Congress mitigated the conversion factor in Medicare Part B payments (or the Medicate Physicians Fee Schedule) which, in turn, leads to cuts.
  • Revised supervision and exemptions for rural and medically underserved areas under the S.M.A.R.T. Act were not included in the bill.

You might be wondering what’s next. AOTA is going to continue analyzing this bill to look for opportunities we can benefit from (& spoiler alert: they have already identified a few) and now it’s time to reintroduce the bills that did not pass and look toward introducing new legislation.
