About the Guests
Are you a patient who has received pelvic health care and is interested in contributing to the development of trauma informed treatment guidelines?
Researchers at the University of Calgary (led by Dr. Lauren Walker, RPsych), have been developing trauma informed care guidelines for pelvic health care with a group of multi-disciplinary health care professionals and patients who have experienced trauma histories and accessed pelvic health care. We are now seeking feedback on a proposed draft of the guidelines. The guidelines are meant to inform providers from a variety of backgrounds including gynecology, urology, oncology, family medicine, sexual health, pelvic floor physio and midwifery.
We are currently seeking feedback on these guidelines from patients who have received pelvic health care in the past 5 years, who also have a history of experiencing medical or sexual trauma. The survey should take about 15-20 minutes and is best completed on a computer. The survey is approved by the Conjoint Health Research Ethics Board. Thank you for considering participating.

Would you like to contribute to the development of trauma informed guidelines for pelvic medicine?

Kary’s professional roles include being an occupational therapist, entrepreneur, consultant, and educator/guide to future OTPs, to name a few. A role central to today's conversation is her role as the Director of Community Engagement at SOLACE Foundation.
Finding a way (and ways) together is central to her work. She credits a small town upbringing and starting her career in healthcare as an addiction counselor with laying the foundation for her passion for working in settings and on projects where resources may be less obvious (or even limited) and yet infinite possibilities exist by noticing, building and maintaining relationships and networks.
**It also has guided her focus in the occupational therapy profession. Kathryn Ellis (she/her) is an international change agent for sex and intimacy conversations in healthcare settings. She is a Doctor of Occupational Therapy and an AASECT Certified Sexuality Counselor. She received the American Occupational Therapy Association Roster of Fellows Distinction in 2024. Kathryn is the founder of continuing education company, the Institute for Sex & Occupational Therapy and developed the first certification program within Occupational Therapy to enhance OTPs preparedness to address sex in all clinical practice settings. Through education and building community, her mission is to dismantle the restrictive norms related to sex that restrict discussions of sex in healthcare settings.
She developed the United States’ military’s first Occupational Therapy Sexuality Clinic at Walter Reed National Military Medical. She continues to educate on veteran sexual health through her work with the United States’ Veterans Health Administration and the Ukrainian Olena Pinchuk Foundation. She is the co-editor of textbook “Sexuality and Intimacy: An Occupational Therapy Approach” and the Amputee Coalition and Veterans Health Administration’s workbook “Let’s Talk About It: Your Sex and Intimacy Guide.” Kathryn has a sex therapy private practice based in Delaware.

Courtney Trocinski
Since her doctoral capstone at a birth center in graduate school, Courtney found her lifelong passion and career in the birth world. As a doctor of occupational therapy specializing in pelvic health, she works primarily in fertility, prenatal and postpartum care. Courtney co-founded her practice, OWN Your Pelvic Health, in Denver, in 2021, where she manages a small team focused on women's health. Her co-founder focuses on pediatric pelvic health. They offer pelvic floor therapy, birth and postpartum doula services, personal training, childbirth education, and they also regularly host various workshops and support groups for expecting and new parents. OWN Your Pelvic Health is also home to a fabulous lactation consultant and massage therapist. When she's not treating clients, attending births, or teaching childbirth education, Courtney loves to spend time outside with her husband, taking advantage of everything Colorado has to offer.

Kaylee Johnson has gravitated toward providing compassionate OT working with people, and their pelvic floor, after neurological injury in her 13 years of experience. She became an expert in the SCI and TBI field through empowering patients at two level one trauma hospitals- in neuro ICU, neuro floor, outpatient, and most recently she worked at a national center of excellence for TBI and SCI.
When she moved from Colorado in 2020 she decided to pivot into SCI & TBI consulting, and pelvic floor therapy and maternal wellness, another story for another day

Shari’s passion for healing has driven her to create CoreRehab, a women’s health program designed to help women restore their bodies post-childbirth and prevent common chronic conditions. The goal of this program and its mission is to erase the stigma surrounding pelvic floor conditions and allow women to rehabilitate with dignity.
CoreRehab was born from a personal experience and mission. Shari is a mom of 2 and had a diastasis recti and pelvic floor dysfunction following her 2 childbirths. In Shari’s attempt to get better, she was met with subpar healthcare and statements such as “It’s normal you need to accept it.” Her answer was HELL NO! In the journey to cure herself, she came up with the CoreRehab program. CoreRehab is a fusion of pelvic floor therapy, postpartum corrective exercises, and Pilates.

More about my guest:Brianna Murosky was drawn to pelvic heath when she experienced her own symptoms of pelvic floor dysfunction while in collage. She holds a certification as a Pre and Postpartum Corrective Exercise Specialist (PCES) and Trauma Inform Pelvic Health (TIPH) in addition to completing a divers variety of pelvic health courses. Brianna is especially passionate about getting to the root cause of each individual's pelvic floor dysfunction which involves assessing nervous system regulation, biomechanics, lifestyle habits and more. She believes it is her job to empower her clients to better understand what's going on with their own bodies, and walk alongside them in their journey to recovery. In her free time she enjoys spending time with family and friends while cooking a homemade meal. She is always down for an outdoor adventure including water and snow sports. Jeanice is a board certified pelvic health physical therapist. She became interested in the field of pelvic health in 1999 after the birth of her first son. Since that time, she has seen the power of pelvic floor physical therapy in her own life as well as countless others that she has connected with over the years. She is the owner of a 10 location therapy practice in central Texas as well as a medicare certified home health agency. In 2018, she started a nonprofit pelvic health awareness initiative, myPFM.com, to reach more people with hope and help. In 2024, she launched a line of pelvic supportwear clothing. Rakhi Srivastava, MOT, OTR/L (she, her, hers)
Rakhi has been an OT for over 13 years. In April 2022, Rakhi established Queen City Pelvic Health and Wellness, LLC, where she treats all genders holistically from head to toe and believes that “nothing is TMI.” She treats in Anderson Township near Cincinnati, OH and has a second location in Fort Thomas, KY. Rakhis experience includes 11 years at The Christ Hospital, where she worked in inpatient rehabilitation and acute care prior to outpatient pelvic floor therapy. In her last 4 years at Christ, Rakhi planned, developed, and executed the hospitals pelvic floor therapy program. From 2015-2019, she was also an Adjunct Professor of OT at Xavier University. She was a speaker at the OTs in Pelvic Health Summit, has been a guest on the OTs in Pelvic Health Podcast, and was part of the small group of pelvic health OTs who established the Pelvic Health CoP through the AOTA. Alyson Stover
A childhood family trauma introduced Alyson Stover, MOT, JD, OTR/L, BCP to the power of occupational therapy. A subsequent lifetime of study and practice convinced her that patient advocacy was every bit as essential as her technical practice of occupational therapy. That would be a pretty full slate for most OTs, but it doesn't begin to describe all of Stover's accomplishments. Stover founded and owns Capable Kids, a private outpatient pediatric practice north of Pittsburgh that provides physical and speech and language therapy services in addition to occupational therapy. She's also an associate professor in the Department of OT at the University of Pittsburgh School of Health and Rehabilitation Sciences. In July 2021, she will begin a four-year term as president-elect/president of the AOTA, representing over 100,000 OT practition

Rebeca Segraves
Dr. Rebeca Segraves, PT, DPT is an ABPTS Board-Certified Women’s Health Clinical
Specialist who practices in acute care and home health, serving individuals immediately after birth and pelvic surgery. She earned her Doctor of Physical Therapy degree from Western Carolina University and completed residency training in Women’s Health Physical Therapy at Duke University. Clinically, her focus is on standardizing inpatient and early outpatient pelvic health rehabilitation immediately after birth and pelvic surgery, including cesarean section and hysterectomy. She lectures in multi-disciplinary clinical and academic settings with an emphasis on earlier utilization of occupational and physical therapy to enhance recovery after procedures impacting public health and quality of life.

Rebecca Hurst / Where to find her: WebsiteWith 22 years experience primarily in the inpatient setting, she recently opened a small business mentoring therapists on neurogenic bowel/bladder. This spring, she left inpatient and negotiated the first OT position in her hospital based pelvic health outpatient clinic. Her passion is continence training no matter what the diagnosis but her background is training SCI patients, caregivers, and staff. She is thrilled to specialize in pelvic health. Her claim to fame is being on the Oprah Show to promote women's safety on college campuses. In her free time, she loves making home grown bouquets, learning to quilt, mountain biking, and hanging out with her dog.

She has been an OT for 23 years, and have worked in almost every setting all over the country. She discovered Pelvic floor therapy through her own issues specifically after her daughter was born trying to get back to running and started getting plantar fasciitis, knee and low back pain. Ironically enough, she has been peeing her pants since She was in her20’s. She is now a proud former pants pee’er! However once she started my deep dive, she fell in love with PFT, it made changes in me that she had not expected and the best part is that it is a whole body approach. She admits the field can be difficult to feel confident in a times but it’s just such a perfect fit for OT. She now has my own business which allows her to take a little more time to evaluate patients. She constantly learning and adapting her evaluations/sessions to meet the needs of her clients.

1) What advice would you have for other students who have identified this interest and may be considering pursuing a pelvic health position directly after graduation?
Emily talks about being a generalist vs specialist and that there was not not one right path to get there. We are not in an escape room - we are growing a garden.
2) I love that you shared that you went straight into Pelvic Health --did this require you to step outside your comfort zone? If so, how?
3) Based on your experience, do you have any "lessons learned" that you would want to share with other OTs who are preparing to apply for positions in pelvic health for the first time, and perhaps any tips specifically for new grads?

Emily is a native Michigander, mom of two, and lover of chocolate chip cookies. Her favorite meal is brunch, her favorite word is “whimsical” and her favorite leisure occupations are thrift shopping and losing track of time in nature. Emily came to OT as a second career and has a previous master’s degree in Educational Leadership; her prior work focused on adult education, coaching and organizational development in community and higher education settings. She became interested in pelvic health through her own experiences as a client, and she is particularly motivated to extend trauma-informed____________________________________________________________________________________________

Jessica became interested in acute OT post childbirth after being an acute care OT for 15 years and seeing a need and perfect fit for OT after having her 3 children. She also started an acute post-partum program and is now a full time Outpatient pelvic health OT. A little bit more about Alyson Stover:
A childhood family trauma introduced Alyson Stover, MOT, JD, OTR/L, BCP to the power of occupational therapy. A subsequent lifetime of study and practice convinced her that patient advocacy was every bit as essential as her technical practice of occupational therapy.
That would be a pretty full slate for most OTs, but it doesn't begin to describe all of Stover's accomplishments. Stover founded and owns Capable Kids, a private outpatient pediatric practice north of Pittsburgh that provides physical and speech and language therapy services in addition to occupational therapy. She's also an associate professor in the Department of Occupational Therapy at the University of Pittsburgh School of Health and Rehabilitation Sciences. In July 2021, she will begin a four-year term as president-elect/president of the American Occupational Therapy Association (AOTA), representing over 100,000 OT practitioners. Stover is the first Pitt faculty member to hold this national leadership position.
Stover earned a bachelor's degree in rehabilitation science and her master's degree (MOT) from Pitt. Like all licensed OTs, she passed the National Board for Certification in Occupational Therapy (NBCOT) certification exam and met other Pennsylvania commonwealth licensure requirements. As if all that wasn't impressive enough, she also holds a Juris Doctor and a post-graduate certificate in health law, both of which provide a solid foundation for championing patients' rights.

Danielle Nation is a pelvic health OT in Rochester, Minnesota, with over 1 year experience. She earned her bachelor's degree in psychology and entry level OTD at St. Kate's in St. Paul, Minnesota. Danielle is passionate about equitable healthcare for trans and gender expansive individuals, as well as integrating trauma informed care as an essential piece of gender affirming care.

Rakhi Srivastava, MOT, OTR/L is a Pelvic Floor Occupational Therapist and owner of Queen City Pelvic Health & Wellness, LLC., which she launched in April 2022 in Cincinnati, Ohio. Rakhi graduated with a Masters in Occupational Therapy from Xavier University in 2010 and began her career at The Christ Hospital working in inpatient rehabilitation and acute care. In 2018, Rakhi helped establish and grow a very effective pelvic floor therapy program for the hospital system. Rakhi served as an adjunct professor in Occupational Therapy at Xavier University for nearly 5 years where she taught lectures and labs.

So the good ol’ bowel, bladder and food diaries…. It’s a powerful trio! And they are amazing for examining how our clients' habits may be contributing to their symptoms. This is so important because whoever takes the time to review things like this with them? It is so valuable for the big picture, combined with all the other tools we bring to the table.
I love giving them the diaries to fill out before the first session and it's ideal for the diaries to span 3-5 days to get an accurate read of their habits, and I love it to span the weekday and weekend. I wanna see it all!
When I talk with my clients about why I am asking them to fill it out, because it is a big ask, I share the following with them:
A Diary Gives us A Good Snapshot Of What’s Happening With their Body
For example, if they are leaking. Knowing how often they leak, when, and how much can help us create voiding habits that work with their body, as well as better assess what we need to address the leakage. Do they Always have a problem at 10 in the morning? Perhaps they need to examine what foods they are having for breakfast or how much time they are allowing yourself to use the toilet. Keeping a diary will help illuminate these patterns so we can investigate the whys.
Another example is if they are having FI, keeping a diary Helps us To Identify Triggers That May Be Causing them To Have it. By keeping a record, you can start to uncover trends that may be contributing to the issue. For instance, that cup of coffee first thing in the morning may be irritating their bowels more than they thought before, hinting that it’s time to rethink your java habit.
Lastly, It Provides them With A Roadmap For A Discussion with us. Recording leaks and daily habits gives us a chance to outline everything they have been experiencing can really help detail the discussion. For many, when and how they eliminate is not something they really record and recall with incredible accuracy --it’s really not the highlight of their day, right? So having this roadmap really makes these patterns black and white, which helps us create a customized tx plan for them.

The wide world of pelvic health -- if this is a new specialty for you, then this episode may be illuminating because I remember when I first learned about Pelvic Health, the field seemed quite focused and specific. Many of the OTs I speak with assume that to work in PH, you mainly work with pregnant or expecting people.
I Have to admit that most of my stories and points of reference are also around pre and postnatal ppl. The FP, my company, was the first pp in Manhattan owned by an OT and the pre and postnatal community was my sole focus. So naturally it was my point of ref for everything . I get a lot of questions from OTs about whether that’s the only population you can work with in this field, so that's what I want to focus on today. This episode is Meant to arouse your curiosity.. And Inspire you to think outside the box. You can implement Fundamental pelvic health techniques in a variety of communities and settings. It’s not an all or nothing approach. After all, we are OTs, and we are only limited by our ability to problem solve with our clients. And we can problem solve with every single one of them.
This is by no means will this be an exhaustive list but I will do my best to present a wide array. If there is something I have left out, please reach out to me via email or via social media and I will gladly add your additions to a future episode!
There are a few different ways we can look at the subspecialties of PF. The first is from the communities we can serve. The ones we will be reviewing today are “ people going through puberty, prenatal, postnatal, perimenopause, men, pediatrics, oncology, elderly, men's health, LGBTQIA+ athletes, ostomy care. Basically if you have a pelvis, we can support you! Let’s get into the details of this now.
The first was those of us working with clients in a period of life where there are in transitions: this would include puberty and pre puberty clients, there's the pre and postnatal communities and my heart is absolutely here with this vastly underserved population. Likewise, there’s perimenopause and menopausal people. Actually, all of these are underserved individuals who could use our support. These are periods in our life when our Hormones are in such flux and that has an influence on our pelvic floor function. For example, both with lactating people and with perimenopausal people, estrogen is traditionally in decline, and this affects our PF’s ability to be as buoyant, responsive and supple. During the menopause years specifically, These tissue changes and their symptoms are often referred to as genitourinary syndrome of menopause (GSM), and some tell-tale signs include pain with sex, vaginal dryness, and urinary incontinence! (Side-note: some providers may use the term vaginal atrophy instead of GSM). A very similar albeit temporary process happens to people who are lactating.
In terms of supporting pre and postnatal people, we can offer birthing rehearsals that help prepare the birthing person for optimal delivery such as optimizing the uterus and ab wall to help push the baby out vs what many people think which is that the vagina pushes the baby out. Quite the opposite, the vagina actually relaxes and gets out of the way for optimal delivery. Postpartum care can help with the ADLS of being a new parent and all that goes with that. Such an important part of being a pelvic health OT is making sure you have clients! And this is a topic I am very passionate about because as OTs, we are innately good at building relationships. And that’s all, at it’s core, marketing is. So let’s build relationships within our community with ppl who interact with our clients and can send them our way!
I wanted to dig deep so in preparing for this podcast epi, I reflected back to what I’ve done over the past 9 years with the Functional Pelvis (ya know, what worked, what flopped) in order to share my top tips in today’s talk.
We are going to talk about how to authentically hustle. The sales and marketing aspect is hard for so many of us, which is why I use the word “authentically hustle.” And what’s authentic for me may be different than what’s authentic for you. So after we discuss some key ideas today, spend a few mins brainstorming and jotting down some ideas that inspire you. What feels right for you? How can marketing feel less salesy? Only you can answer this question but I hope by hearing my stories, you will see that the authentic hustle is entirely possible.
Ok so let’s get into it now.
My first tip -- Start getting your name known. Whether you have a strictly online business, or you offer in-person services, you should have an on-line presence that helps people meet you and get to know your personality and brand. This means making yourself visible and showing up with consistency. It’s important for you to be seen as the expert where your client and/or referring parties hang out.
Here is an example of this in the virtual world.
Join FB groups where your potential clients or potential referring parties are. For me, this meant a doula FB group for the NYC area, a perinatal connection networking FB group, some popular fitness programs FB groups such as RYC, Mutu, etc. Join them and listen. Listen to their needs, pay attention to the specific language they are using to describe their concerns. You can mirror their language and concerns back to them or in posts. This will help them see you “get them.”
Once you are a fly on the wall and start to see patterns with their language and how they are speaking to their concerns, start to step-up and be seen as the expert in the room. Reply to their concerns with tangible, compassionate, concrete answers. And friends, be consistent with showing up so they continue to see your name in the forums. Ways to make this easier include Scheduling something on your calendar weekly to review your top groups. When you consistently relay tips and tools you will be seen as the expert and a dependable source of wisdom. From my experience, You will see referrals from this.
The same things apply to the real world.
So let’s talk about that now.
The goal in marketing your business is to make connections so that people (be it referring parties or clients who talk to their friends) think of you. Basically, You are top of mind when anyone mentions something remotely related to your specialty.

Let’s discuss 9 uncommon questions that make a huge impact on healing. Why are they uncommon? Because they aren’t typical, you won’t find them in most intake questionnaires and well because….. they are all about bowel habits.
Here’s why this matters….The majority of our clients who come to see us, they are often coming due to urinary issues, painful intimacy, pelvic stability issues, prolapse, abdominal wall concerns…..often not directly concerning their bowel habits.
But most of these issues often come back to a healthy relationship with bowel movements. Even if it wasn’t the primary reason they came to see us. In fact, it may not even be something that they flagged on their intake forms. So how does this affect our session with our clients? How does this affect their outcomes?
Let’s focus on our ability to ask quality questions and develop the rapport we have with our clients. These two powerful things enable us to see if there is a relationship there between the symptoms they are coming to see us for and what's really driving it. Very often it comes back to bowel issues.
As Jane Kauffman said, my mentor from biofeedback training and incontinence solutions, the poop drives the pee.
**We just mentioned that the majority of our clients come to see us for anything other than bowel movement issues so I have learned to be a bit of a detective to find out what's really going on. I have learned to ask better subjective questions, even if the intake didn’t direct me there. and maybe even especially when the intake doesn’t bring me there.
I am going to tell you what I have learned to do…**
I use the Australian pelvic floor questionnaire as one of my evaluation forms that our clients fill out. There are sections devoted to urinary fx, sexuality, bowel function. Many of us, including myself, will kinda glaze over asking more in-depth questions about bowel function if there are no red flags on the form. Notice I said I used to do this.
So much of pelvic floor function is not discussed in everyday life so our clients don’t even know what optimal function is , and this couldn’t be more true than with BM. Some of our clients will come to see us because of a newish symptoms. … Urinary Incontinence, urinary freq, etc. But many of them have had BM issues proceeding this for years if not decades that have contributed to their newer symptoms. For so many, what is more shameful than pooping?…. So our clients come to accept many of these issues as common, or normal . They buy Imodium or laxatives but not much else. Most people experiencing these issues don’t even bring them up with their GPs.
This is why the intake is crucial! Your job is to help them to share things that they don’t even realize are an issue. As they start to see how interrelated their stories, concerns, and experiences are, they will continue to connect the dots in their own life, leading to so much self-efficacy!
So the questions we reviewed on the Australian pelvic floor questionnaire don’t resonate with our clients very much. They are clinical and undefined .

We are going to be talking about getting centered through interoception today. I love this topic because it really highlights our strengths as OTs and as holistic thinkers. This type of perspective helps the nervous system feel supported, and the longer I am a pelvic floor therapist, the longer I see the benefit of starting with this.
We are going to kick off this topic with discussing permission vs informed consent – Permission is simply, may I touch you? Can I do an internal exam now? They reply yes or no. Informed consent is a bit different – its sharing with them what you are going to do, and where you will touch them so that they are fully aware of what they are agreeing to.
Everything is an invitation – this is what creates a feeling of being empowered – we can do this right from the beginning—starting with where they have to be in your space – such as saying “make yourself comfortable” sit wherever you’d like. Instead of “you can sit right there.” Even the physical space can create a feeling of safety – we can do tangible things like this to encourage them to connect to their nerv system – by responding to you and the environment. And just by walking in, and choosing where to sit, they already feel empowered. They start their experience this way. Many of our clients have had too many experiences of being disempowered. And that stops with us. Practice in a way that fosters a sense of ownership of their own body – empower through choice by using the language of invitation and cuing.
Here are few other examples of how Safety begins with the phy envt – we always ensuring clients are not on display, they are in a private space both so they aren’t seen but also aren’t overheard talking and sharing their story with you, also ensuring no one will interrupt the tx while in progress. Consider things like Do you answer the door during tx? Esp while the client is laying face down.
We should be Mindful of our fascial expressions and the position of our body. This is even more impt with covid and us all wearing masks. We need to be mindful of our eyes and they convey safety and trust.
Also, Let the indiv know where you are in the room. If you are moving around (esp if they are lying prone, or if approach from behind). Think about this in assessment and treatment. Especially if they can’t see you.
Another important aspect that’s imprtamt, especially with masks is our Tone of voice – how you are saying (not just what saying)
As pelvic floor therapists, we are all trauma therapists –– The intention often determines the approach. Why you do something will inform how you do it. It’s about the Mental and emo and physical – this is a very multifaceted approach. And there are layers of trauma. There could have been the original trauma that resulted in a secondary trauma. For ex, my client Runs for stress relief- but she injured herself – she can no longer run – her stress levels increase more. This is simple example of the Physical affecting the mental aspects and demonstrates the layers of trauma.
The response and experience the person has is more important than the event. For ex, take two War veterans -one who returns w/ ptsd and the other doesn’t. Their History, wiring, learned behavior, coping mech, what strategies do they have to reg a dysregulated nervous system? All of this has an effect on their response.

Let’s get right into it… this is a common question for anyone interested in becoming a pelvic health OT. What courses do I take? What trajectory do I follow to become the best practitioner I can be?
The truth is that there is not one right approach for anyone, and this especially applies to us as we take more and more specialized courses.
Let’s face it, the world of pelvic health is a bit of the wild west. Especially as OTs, there is not a linear path we have to take. This of course can cause confusion for us as we learn about the field and it can be confusing for our clients as well because it's hard to put an experience in a box when we all take diff paths to get there. And all the paths are ok… I encourage you to take the one that resonates the most with you. How awesome is it that we can be in a field where we can literally carve out our approach to helping clients? You can do this through taking a fitness based approach, a hands on approach some as visceral manipulation, a trauma based approach (which I think all pelvic floor therapist's should have, but that’s a topic for another episode), a behavioral based approach, an educational based approach, etc. Take the courses that speak to you and you will create a practice that is deeply authentic to you. Your bag of tools will be plentiful and you can customize the treatment session to the client in front of you. I don't know about you…. But does it get any better than that?
So back to being a beginner in pelvic health… what steps can you take? I suggest Taking a foundational course or 2.
I started offering OT Pioneers: an intro to pf therapy for OTs in 2018 and I have had the honor of supporting over 500 OTs at the time of this recording. This is a self paced online course with group mentorship and you have lifelong access to the material and updates.
Another introductory course that I recommend after OTP is fellow OT Laura Rowan’s PRmatt course, it’s a 2 day in person lab where you get to practice your hands on manual therapy skills.
I would be remiss if I didn’t mention fellow OT tiffany lee’s course called, “biofeedback training and incontinence solutions” This is a beginner to intermediate course designed to provide a basic working knowledge of biofeedback assisted behavioral therapy for bladder, bowel, and pelvic pain disorders. Pam’s document?
There are also intro courses offered through H+W such as PF1. This is a large format course, often over 50 students and while this is changing, we are often the only OT enrolled in the course. While they have always been welcoming to OTs, the curriculum is primarily PT based.
What’s next? After your intro courses, what do you do? My top suggestion is to join a supportive community such as the OTs for PH FB group. You can find and connect formally or informally to mentors (I have a list of mentors who offer services on the page). Mentorship is undoubtedly an incredible way to propel your learning to the next level.

You’re ready to start serving your people! You are ready to get a j-o-b as a pelvic floor OT. Let's talk about the how's now. I have been offering ot pioneers my online intro course since 2018 and my students tell me this is one of their biggest questions, so I thought let’s kick off our third episode spot lighting it!
I want to share the story about how I got my first job as a pelvic health ot.
1 year job open.
HR denied it.
Went to office administration…. Met director.
Shadowed…. Stayed out of the way, while being attentive. Asked questions after (appropriately).
Many realize after shadowing it isn’t right for them.
Breath of fresh air…
“You really enjoy this,” your energized and it’s infectious.
2 weeks later -- full time job offer.
Things I learned:
- Gatekeepers (in my case, this was HR but it really be anyone in the process) - don't let them slam the door in your face.
Find a creative way around them.
Be respectful of the process but don’t be shy about kicking down whatever doors stand between you and the opportunity.
- Passion will take you far. Remember what Erika said to me… your energized and it’s infectious. People crave to be around people who are inspired and who inspire. You are no doubt one of those people. So show up, ask questions, add to the conversation and demonstrate why you should have a seat at the table.
- Move on if it’s not a match. Not every open job out there will be a match for you. That's 100% ok. Just like you aren't a match for every potential partner you may have dated, or a match for every potential client, it’s all good to recognize when it’s time to move on. In the case of dating, this means it’s time to have the it’s not you, it’s me conversation. For clients, it’s when you refer them to another caring practitioner that may be a better fit due to their expertise, their approach or their style. And in the job market, it’s about recognizing that the environment may not be one where you will thrive. Unfort, there are probably more like this than not. While it’s fantastic to shine a light for those who simply may not know that OTs are incredible fits for ph, it’s not up to you to fight a battle when you are not seeing eye to eye or not getting the support you need, or having to constantly prove yourself. In these cases, the person is not open to learning. You will find a home where you thrive and when these situations present themselves, I simply say to myself, thank goodness I found out now. It’s time to move forward and find or create an environment where I feel like a valuable part of the team. Because you are!

So a question I get a lot is what is a pelvic floor OT, right? Are you wondering that same thing? I think it's a really natural question. And the reason for that is, is that many, many of us, the public even OTs have, have heard about PTs in pelvic health, right? The natural question that arises from that is, well, then what's a pelvic health OT. Can we even do this work? I find that it all comes down to your approach, not your credentials. There really isn't any skill that PTs have that OTs can't acquire. And likewise, there's no skills that an OT has that a PT can't acquire. It's literally about skill acquisition and desire. That's it. Now that being said, I am by, towards a pelvic health approach that let's say looks more at the person as a whole, including the physical, emotional, spiritual, and mental wellness aspects.
We aren't beings that are in silos. We're a whole person, and it is absolutely a essential to learn how to connect with our clients and ask questions about these areas of their life, how to use motivational, interviewing how to use non-verbal communication and how to approach the relationship with a facilitator versus a fixer approach. In the decade that I have been a pelvic health OT, I have found that addressing everyday activities in a person's life while taking into account their environment, their relationships, their level of functionings and their goals are key. Why, why is this approach key? Well, the thing is that pelvic health addresses the most intimate aspect of our client's lives, right? I mean the three main jobs of pelvic health are elimination. And by the way, this means keeping it in when it's not time to go, but also letting it out when it is completely second job intimacy.
So this means participating in whatever intimate act you choose and having it be pleasurable with the ability to experience orgasm and last up support. So that's elimination, intimacy and support. And when I talk about support, this includes the organs within the pelvis, but also the support that goes all the way up to the top of the head, right? Thanks to gravity, the pelvic floor support everything on top of it, right? So the Vista and our abdomen, our hearts, our lungs are 10 pound skull and everything in between, right? What a huge job for such an underrated body part, all of the jobs that the pelvic floor have often happen behind in closed, right? Particularly intimacy and elimination. And what happens when an activity happens in secrecy? Well, we don't have many opportunities to talk about how it functions optimally. So for example, have you ever asked someone, Hey, uh, so when you start peeing to you, like ever push a little bit to get it going, or, Hey, uh, it kind of hurts when my partner and I start fooling around.
Does that ever happen to you? Nah, I don't think this happens very often, even with our closest to friends. So this means we don't know things like it is optimal to not use effort to start or stop our urine or that we should pee once every two to four hours or that just in case peeing is an optimal for bladder and brain communication. And of course that intimacy of every kind should not hurt. These are the kind of, of myths that we're gonna be busting on this podcast. Are you in, is this interesting to you? -- Transcript --No matter if you're in OT or PT, so it doesn't matter whether there's an O or a P in your credentials - what's most important is if there's a T. We are all therapists at the end of the day.
All of us have received emails and phone calls for people’s lives that we have transformed. That's what's most important and that's what brings us to something like continuing education.
We are so hungry to learn more, to develop new skills, to expand our knowledge. By nature, therapists are intellectually curious and compassionate people.
There is just so much that OT's and PT's have to learn from one another. I personally have trained with some amazing PTs such as Diane Lee- her ISM series has been tremendous for my clients and at the same time PTs have taken my online pelvic health course and felt that it brought such a value to their practice. There's a definite need to cross collaborate and interest in serving our clients while expanding our knowledge base. While I know things are changing- there is a huge gap of knowledge around OT's role within the pelvic health field. Most people associate pelvic health with PTs- so many are surprised to see us – to see ots in this specialty.
Although it is within our scope of practice, it is up to each one of us to create our own path to competency to be able to work with our pelvic health clients. I think most of us can agree that with the majority of therapists specialized learning comes post graduation. Both PT's and OT's come to pelvic health from such varied backgrounds that this is what creates a rich individualized experience for our clients. This however also adds to the confusion around what a pelvic health session is like. It's hard to place an experience in a box when we each have our own way of getting there. like OTs take their own paths, pts take their own when to comes to specializing in pelvic health. the APTA even has their own women’s health section which is incredible and while there are certifications available out there, such as PRPC through Herrmann and Wallace to streamline the path professionals can take, in most areas of the world these certifications are not required for practicing within pelvic health, again adding to the confusion around who's doing what. however there are more clients in need of this very special area of rehab and there's room for us all. Just as OT's and PT's work side-by-side in other fields of rehab, there's a huge need for us to work collaborating in ph, especially when you consider the sensitive taboo nature of this area of practice. It's been my experience that the general population has such poor understanding of pelvic floor function and particularly how it impacts us from a whole body, whole person perspective that I think the more of us -OT and PT- working together in this field to spread the message, the better. pelvic floor function is a crucial part of a much broader functional tasks of toileting and intimacy,
Both of which have broader connections within a person’s emotional cognitive and social abilities. It's also important to consider the social implications for people with pelvic floor issues -withdrawal from social and recreational activities, social isolation, disempowerment, lack of self-esteem, anxiety, depression, and the impact on close relationships such as with our spouse, our friends and our children. I love today’s topic. I think it’s essential for us in pelvic health to have a framework that we can feel grounded in, one that we can come back to when we can see multiple ways that a treatment plan can go. When we get analysis paralysis and are just not sure where to go. This is when it’s helpful to come back to our model or our framework. It my opinion, if I had to sum up the framework of a pelvic floor OT, it’s 2 fold:
We embrace a facilitator vs a fixer mentality
We take a Whole person/BPS approach
Now there’s a lot more that goes into it than just these two things but keep in mind this a podcast not a dissertation, so give me some slack. Also, this framework is not exclusive to OTs. Many PTs have this same approach. It’s just that I am a proud pelvic health OT, and underdog most of my career and I am so firmly rooted in supporting and encouraging OTs to get out there.and strut their stuff.
Let’s kick off today’s episode with a quiz! Here’s the Question: How do you view your role with your client?
While some of us listening to this episode are newer OTs and some of us are more seasoned, I think no matter where you are, it's extremely relevant to stop to check in and ask yourself this question. We all have an internalized viewpoint of how we see our role as the OT. This viewpoint is usually unarticulated, it's usually subconscious, but here is the thing - this internalized viewpoint is going to have an immense impact on our words and actions and our relationship with our client.
So How do you view your role with your client? There are 3 options to this multiple choice quiz.
Option A: I carry my clients up the mountain
Option B: I lead the way and they follow.
**Option C: I walk alongside my clients.
Full disclosure: Standardized tests have never been my strong suit. Good thing we’re not grading this one. Let’s review each of these options.**
Option A: I carry my clients up the mountain.
If I carry up my client - I am thinking of them as weak, I am doing for them, I am doing all the hard work and they are being passively transported. I am not educating, I am accomplishing the task (getting up the mountain) but here I am treating them as someone weak and incapable. I can check the box, I did it. But they are not an active participant as someone who has gotten better because of it, they don’t have more knowledge they probably won’t be able to get up the mountain themselves again.
In this option I am viewing myself as superior which automatically means I am viewing them inferior. “I am the practitioner here. I know what's right. I am in charge here.”
If I had this view then I probably would just go in and manually release them, I wouldn’t talk to them about why I was manually releasing them, wouldn’t give them any home exercise programs that would reinforce what I’m trying to build in their body, they are just coming and getting done. The experience can be a little bit like a massage, or experiences I have had with massages. You know the ones when You are not being told which muscles are tight, you are not being told why they are doing what they are doing, and what you can do about it yourself when you get home.
