Dr. Dan Brillhart and Army Nurse Erick Thronson discuss our experiences integrating a 2 person Damage Control Resuscitation (DCR) Team into the manuever element Special Operations Forces (SOF) in austere environments in Africa. This DCR Team moved with the lead ground element on operations lasting from several hours to several weeks and provided advanced resuscitative care as the initial treatment to dozens of casualties at point of injury. We describe how this model worked for us and advocate for expansion of similar programs both within SOF and conventional units.

Dr. Meaghan Keville is an Active Duty EM physician stationed at CSTARS Baltimore at the R. Adams Cowley Shock Trauma Center. Graduated USU in 2010 and then completed EM Residency at Wright Patterson AFB/Wright State University 2010-2013. She has been forward deployed with CCATT, TCCET and with a field surgical team in support of USSOCOM.
In this lecture, she discusses the importance of mindset and mental fitness in medicine and as she has recovered from injury.

Col (Ret) Edward Fieg spent one of his multiple deployments as an embedded advisor to an Afghan hospital, focused on improving the nation's capability to care for patients. He shares his reflections on his time in Afghanistan and how the end of the Afghanistan war has affected him.

Col(s) Kristen Silvia is a board certified Emergency Medicine Physician, Flight Surgeon and currently Chief of Bioastronautics at Detachment 3, Human Space Flight Support Office, Patrick Space Force Base, Florida. She leads the DoD’s contingency medical operations for NASA and U.S. Space Command in support of U.S. crewed space flight programs. She also directs training of space medicine to all DoD forces for worldwide astronaut rescue/recovery and nominal/contingency landing site support. Prior to her current position, she trained the Air Force’s Pararescueman (PJs) in all aspects of combat trauma and critical care as the Medical Director for the Pararescue & Combat Rescue Officer School.

Welcome to our first episode of the GSACEP Career Corner! We will highlight an individual's career path, with tips on how to succeed in varied military/federal assignments!
Lieutenant Commander Stuart earned his Bachelor of Science degree in Microbiology at the Georgia Institute of Technology, where he commissioned through the Navy ROTC program. He received his Doctor of Osteopathic Medicine from the Philadelphia College of Osteopathic Medicine in 2009. After graduation, he reported to Naval Medical Center Portsmouth, VA as a transitional intern, completing the program in 2010.
LCDR Stuart reported to 3rd Battalion, 3d Marines where he served as the Battalion Surgeon from 2010 to 2013. During this time he deployed twice in support of Operation Enduring Freedom. He also served as the Director of Medical Readiness and Immunizations for 3d Marines. LCDR Stuart was then assigned as the Deputy Regimental Surgeon at 3d Marines where he coordinated medical support for the development of Marine Rotational Force – Darwin and RIMPAC exercises. Additionally, he pioneered the development of a combat training curriculum, the creation of a training tactical simulation laboratory and served as the Director of Operational Training.
In 2014, LCDR Stuart returned to Naval Medical Center Portsmouth to begin his emergency medicine residency training. His involvement in combat trauma research resulted in 18 presentations and publications, numerous awards, including 2nd place at the American Academy of Emergency Medicine’s research competition and was recognized for creating the largest scholarly output in the program’s history. In addition, he also oversaw the expansion of the Simulation, Vivarium and Biotechnical Skills programs.
After graduating, LCDR Stuart assumed the role of MEU Surgeon for the 13th Marine Expeditionary Unit and deployed in support of WESTPAC 18.2 and Operation Inherent Resolve.
In 2019, LCDR Stuart returned to Naval Medical Center Portsmouth as academic faculty. He served as the Interim Chair and now concurrently as the Senior Medical Officer for the Emergency Department where, in addition to serving as the medical director, he is responsible for their COVID-19 responses, and the Director of the Combat Trauma Research Group, which was awarded the 2019 Military Health System’s Team Award.
LCDR Stuart is board-certified in Emergency Medicine, holds a faculty appointment as Assistant Professor of Military and Emergency Medicine at the Uniformed Services University of the Health Sciences, and is a Fellow of the American College of Emergency Physicians, American Academy of Emergency Medicine and the Academy of Wilderness Medicine. His military decorations include two Meritorious Service Medal and two Navy and Marine Corps Achievement Medals.
LCDR Stuart is married to Lisa Stuart and has a daughter, Lily and son, Benjamin.

Dr. Steve Sample and Dr. Torree McGowan reflect on the differences in their deployments to Iraq and Afghanistan, drawing on their shared experience as emergency medicine residents at SAUSHEC and their overlap in Balad.
Dr. Stephen Sample, MD graduated from University of Louisville Medical School 2005, and completed his emergency medicine residency at the San Antonio Uniformed Services Health Education Consortium (SAUSHEC) in San Antonio, Texas, in 2008. He deployed to Balad in 2009 and Bagram, Afghanistan in 2011. He is currently an emergency medicine physician and Dubois County EMS medical director in Jasper Indiana.
He has been married to his wife Tobi for 24 years next month. His daughter Haley is 23, working at Stitch Fix in SF, and his other daughter Riley is almost 21. She is a junior at Indiana University, pre-med. He is a frequent guest on MSNBC with Brian Williams and has a large Twitter following at @supermansings. His dogs Lucy the Newfoundland and Charley an asshole of a Lab/golden mix are the stars of his Twitter.

Dr. Regan Lyon and CPT Brian Thebaud share perspectives from their roles on embedded medical teams in some of the furthest forward medical care in current combat operations.
Major Regan F. Lyon is an emergency medicine physician and Defense Analysis graduate student at the Naval Postgraduate School in Monterey, CA. Prior to starting graduate school, she served with the Special Operations Surgical Teams, 720th Special Tactics Group, Hurlburt Field, Florida. Major Lyon received a direct commission after graduation from Texas A&M University in 2006. She completed medical school through the Uniformed Services University of the Health Sciences (USUHS) and graduated from the Emergency Medicine Residency at San Antonio Military Medical Center. In 2014, she deployed as the medical director of the 83rd Rescue Squadron at Bagram Airfield, Afghanistan. She served as the Special Operations Surgical Team’s emergency medicine physician in 2017 and Team Leader in 2019 in support of Operation INHERENT RESOLVE. Major Lyon has specific interests in the employment of medicine on the battlefield, extending from point of injury to austere surgery, and its impact on operations. In recognition of her academic contributions, she was appointed an Assistant Professorship at the USUHS Department of Military and Emergency Medicine.
Captain Brian E. Thebaud is a student at the Army Acquisition Professional Education Program in Huntsville, AL. Prior to transitioning to the acquisition career field, CPT Thebaud served as a Special Forces Detachment Commander from 2017 - 2020. CPT Thebaud received a direct commission as an Infantry Officer from the University of Alabama in 2010, prior to attending Special Forces Assessment and Selection in 2014. He has deployed three times in support of operations in the Central Command (CENTCOM) area of responsibility.

Dr. Andrea Austin interviews Dr. Maximillian Lee on the impacts of the current events in Afghanistan on the physicians who served in that theater of war. Col (ret) Lee is the former Air Force Special Operations Command, Chief of Aerospace Medicine and an influential leader in the development of our current physician forward operating stance. He deployed to Afghanistan multiple times as an emergency physician, on Critical Care Air Transport Teams and also in Forward Surgical Teams.

Al Giwa is a former attorney who entered medicine to become a bioethics professor; in the process he has started and led several successful ventures spanning the healthcare and legal consulting world to entertainment companies and healthcare startups.
He studied philosophy as an undergrad at Clark University, Law at the University of Ilorin, and attended Columbia University for his MD. He obtained his MBA from GWU and an MBE from Clarkson University/Icahn School of Medicine. During his journey, ethics and the desire to learn and teach others have instilled in him a leadership style that is truly transformational in nature. Dr. Giwa is currently on deployment in CENTCOM as part of Operation Freedom's Sentinel with the US Army. He is an Associate Professor of Medicine and Bioethics.
