The ReSPECT process has been widely implemented. It stands for the Recommended Summary Plan for Emergency Care and Treatment and, when used well, it provides a guide for clinicians to ensure people's wishes and values are respected in an emergency situation. I am delighted to open the new season with Professor Jugdeep Dhesi, geriatrician and President of the British Geriatrics Society. Listen in as we explore the changing demographics of dying and the vital role geriatricians play in joining the dots in Advance Care Planning.

In this episode it is my pleasure with Radha Sundaram an NHS consultant in intensive for the last 16 years in Scotland. Radha is an advocate for organ donation and improving awareness amongst our diverse society. She has had a long-standing interest in end of life care and has been responsible for the guidelines in NHS Greater Glasgow and Clyde.
Radha also does pro bono work for a charity called Pallium in India which was described in the Lancet as model for end of life care and for the Mohan Foundation which is India’s largest organ donation advocacy organisation

In this episode it is my pleasure to talk with Peter Berry and Deb Bunt. Peter was diagnosed at 50 with early onset Alzheimer’s.
Previously the owner of a thriving timber business, Peter’s life changed upon the diagnosis. He has emerged through depression and two suicide attempts to be a passionate advocate of living well with dementia. A keen cyclist, Peter has used his cycling to raise money for and awareness of Alzheimer’s. He is often seen cycling around rural Suffolk on his penny farthing bike.
Deb retired to Suffolk from London and found a new purpose in life when she befriended Peter. One of Deb’s ambitions had been to be a publisher author and meeting Peter has enabled her to achieve this. Her friendship with Peter has also instilled a new-found confidence in Deb which she describes as a wonderful gift from Peter.
We talk about what living well means to Peter and the legacy he is creating through his work.

In this episode it is my pleasure to talk with Dr Barny Hole and Dr Emma Murphy. Barny is a kidney doctor currently studying for a PhD. Barny’s research looks at how older people with kidney failure decide between dialysis and comprehensive conservative care.
Emma is an Associate Clinical Professor in Nephrology Nursing with a combined clinical and academic role. Emma is currently leading the QUALYCARE Kidney study which aims to understand the variations in the quality of end-of-life care for patients with kidney disease on dialysis.
We explore the trajectory of kidney disease, conversations around stopping dialysis and what Advance Care Planning with kidney disease looks like.

In this episode it is my absolute pleasure to talk with Dr Julian Able who became a consultant in palliative care 2001, working initially at a district general hospital and a hospice. For more than a decade he has become increasingly involved in finding ways of building compassionate communities around people at end of life. He has run projects at local, regional and national levels. He was Vice President of Public Health Palliative Care International. He is an international keynote speaker and has published regularly on models of public health palliative care. He is the editor, along with Professor Allan Kellehear, of the Oxford Textbook of Public Health Palliative Care.

Clair Fisher advocated for the benefits of early intervention palliative care, more open conversations around death and dying and normalising Advance Care Planning. Clair made these aims part of her retirement project after she was diagnosed with stage 4 bowel cancer. I had worked with Clair and was aware of how thoroughly she had addressed her Advance Care Planning, to hear her shout out on Twitter that her wishes were not being heard inspired the podcast. We recorded in the final weeks of Clair’s life and the episode has since been downloaded over 500 times.
Many of the podcast’s subsequent guests have spoken about the impact Clair’s work has had on them.
I am proud to have known Clair professionally, and to have had the honour of recording this episode with her. Clair talked about how Advance Care Plan helped her prioritise quality of life, the challenges she had in ensuring her wishes were heard and the wellbeing advantages of planning ahead. Clair shared the strategies and tools she used to document her wishes and what mattered most to her.

In episode five I talk with Jim about what he learned from working in a hospice and how he walks two roads between life saving active interventions and supporting people at the end of life. Jim talks about what can happen when Advance Care Planning is not in place, describing some of the calls he has attended to demonstrate the reality. We know that many people express a wish to be in their own home at the end of life, yet often the planning to facilitate this is missing; Jim describes what can happen without these important plans in place.
