There are as many types of caregivers as there are stars in the sky, but we thought it would be useful to give an overview of the types we’ve come across. You may recognize these as your family members or friends, or even yourself.
THE PRIMARY CAREGIVER This is a total guess, but chances are if you’re reading this, you are a primary caregiver. This is the person who typically is the first line of care. They’re the person who gets the call in the middle of the night. They’re the person who makes casseroles every week to share. They’re the person who drives their loved one to doctor’s appointments and picks up medication. They’re also the person who burns out first.
The primary caregiver in a family may be decided due to proximity to the loved one, natural caregiving abilities, or because no one else steps up.
Being the primary caregiver can frequently lead to resentment of other family members (which we’ll cover in a later chapter) but in the end the primary caregiver knows that they are the first line of defense, and sometimes the only line.
In some extremely rare families, there is actually more than one primary caregiver. These two or more people share all of the work evenly and for the most part agree on how things should be done. If you’re in this situation, stick a horn on your head because you, my friend, are a unicorn.
THE SECONDARY CAREGIVER If there’s a primary, there must be a secondary, right? Not always. In some lucky families, there are two people who are willing to share the heavy lifting of giving care, and this is a blessing. As they say, teamwork makes the dream work.
In some very rare instances, this “secondary” caregiver is actually a true partner to the primary, doing just as much of the heavy lifting but still taking a backseat when it comes to the difficult decisions or things like schedule-making. In most cases the secondary caregiver helps, but not near as much as the primary. For instance, they may come over once a week rather than three or four times a week.
Another issue that can come up with this situation is if the secondary caregiver is jumping in out of a sense of duty, but really isn’t particularly good at the job. In this case, it’s best to give the individual a defined set of duties that they’re good at, like dealing with insurance companies or managing grocery pickups.
THE BACKUP CAREGIVER This is the person in the family who is around when needed, but rarely volunteers. They’re essentially like a backup to the starting quarterback. If everything is going smoothly, they don’t feel like they need to do anything at all. If the shit hits the fan, they will do as they’re told, but in some cases no more.
THE UNCOMFORTABLE CAREGIVER The discomfort here isn’t necessarily due to a lack of desire to help. It has more to do with the individual’s confidence or natural caregiving ability. The discomfort may present itself in some of the less desirable cleanup duties that are sometimes needed, or in things like transfer assistance. Whatever the specifics, the uncomfortable caregiver is there, but sometimes not all that useful. And, the loved one may pick up on the discomfort and not be comfortable themselves.
The good news is that because the uncomfortable caregiver wants to help, they can sometimes help out in other ways. If the “hands on” duties are an issue, they can be quite useful organizing doctor appointments or making grocery runs.
THE EMOTIONAL CAREGIVER This individual takes themselves out of the running for being useful mainly because they can’t control their emotions. There are going to be times when you cry. It’s completely natural and will hit you when you least expect it. But, when things are going sideways a good caregiver becomes stone calm. The emotional caregiver isn’t able to get to that state of mind, and can be a hindrance in emergencies.
THE TORNADO This person means well, and many times they actually are quite useful. However, their frenetic nature may cause the loved one to be uncomfortable and tense. The tornado swoops into a situation and tries to fix everything at the same time. If their energy can be directed toward one thing, such as cleaning, they can be very helpful.
THE DISTANT CAREGIVER The “distance” we’re talking about is usually in miles. Quite frequently, families have one sibling that’s close in proximity to the loved one, and one or more siblings that live hours away. The primary duties naturally fall to the closer sibling, but how much the other siblings help is up for discussion. The hope is that the distant caregiver will step up, take some time off of work or away from their family, and help as much as physically possible. However, quite frequently the distant caregiver uses their distance as an excuse not to help at all. They may not even check in via phone, figuring the primary has it covered. The hope is that, if that’s the case, the distant caregiver will at least be willing to help out with money for care if it’s needed.
THE NON-EXISTENT CAREGIVER This person is the least useful of this entire list, and it may be a conscious decision on their part. Basically, they’re not there in person, they don’t call, and they don’t try to help. They may have decided, “that’s not for me – my sibling can handle it.” This is the type of caregiver that causes the most resentment among those who are making an effort.

Whenever we do an episode that is based on one of those chapters, we'll post the text in the show notes so you can read it if you want.
What’s a caregiver? There are tons of definitions, because no two situations are the same. Caregivers can run the gamut from being a live-in assistant for all daily functions to being the person who drops in once a week to deliver a casserole and take out the trash. In the end, the best definition we can come up with is “one who helps make life easier for someone else.”
This podcast is designed for a broad swath of caregivers who are taking care of an elderly parent or loved one. Our experience is very much first-hand. Let us tell you how our lives were turned upside down and changed dramatically over a short six months, all due to being primary caregivers.
Steve started his caregiving “career” while his father was fighting a losing battle with brain cancer between the fall of 1999 and January 2001. In that case, he was the secondary caregiver (see the episode later in this podcast for a definition of that) while his mother was the primary. His job was basically to come over several nights or afternoons a week to give his mother a break and let her get out of the house. After his father passed away, he took a long break, as his mother was quite independent. That all changed when the COVID-19 pandemic began. Not only did it require him to assist with groceries and other things that would allow his mother to stay safely at home, but that time also began his mother’s slow progression to becoming wheelchair bound.
Jennifer’s experience with caregiving is much longer. In fact, it was already beginning to some extent when she and Steve married in 1995. Jennifer’s mother has a variety of medical problems, including rheumatoid arthritis. While modern medicine has allowed Jennifer’s mother to stay somewhat independent, RA is a brutal disease that can make everyday activities painful and sometimes impossible.
The true impetus for this podcast, however, came in the second half of 2021, when Jennifer and Steve received a crash course in intensive caregiving and experienced many of the issues that are covered in this show.
In July, Jennifer’s mother needed emergency surgery to remove a colon blockage. This led to weeks of hospitalization, weeks more in a skilled nursing facility, and weeks more of home care. It was a horrible and messy situation for everyone involved. As Jennifer’s mother returned home and was trending toward becoming somewhat independent again, Steve’s mother went down (literally) with a back issue that compressed her spinal cord.
Previous to the spinal cord problem, Steve’s mom was somewhat mobile and independent (despite having no cartilage left in her knees) but the compression eliminated control of her right leg and necessitated emergency surgery to avoid total paralysis. During the preparations for the surgery, doctors discovered that she also had congestive heart failure that had to be controlled before surgery could be attempted. This led to several weeks in the hospital, more than two weeks in physical rehab, a month in skilled nursing, and eventually a move into assisted living.
While all of this was going on with Steve’s mother, Jennifer’s mother caught COVID-19 despite all of our precautions. Fortunately, she recovered without requiring a hospital stay, but it was an added twist that kept things interesting.
Oh, and just for fun, right in the middle of all this Steve needed elbow surgery that couldn’t be put off. It was to treat a raging infection that he likely caught at the hospital while sitting with his mother. That was a bonus we didn’t expect.
If you’re listening to this show, you’re likely taking a journey that’s similar to the one we’ve been on. We wanted to lay out our experience, but not to elicit sympathy or make you think we are somehow experts. It’s more to say, “we’ve been where you are,” and if there’s one thing we’ve learned in this process, it’s that having someone around who knows what you’re going through is invaluable.
One more thing before we get started in earnest – there are going to be a few things in this show that may offend some readers. Whether it’s salty language, inappropriate humor, or simply hitting a little too close to home on personality tendencies … don’t say we didn’t warn you.

Hosts Jennifer and Steve Mullen are both GenXers navigating taking care of aging parents while also taking care of kids.
