The government has announced a new plan to include a two-dose chickenpox vaccine in the childhood immunization schedule for all babies born on or after October 1, 2024. The vaccine will be administered at 12 months and again when the child enters junior infants. There is no catch-up program for those born before this date, who will need to get vaccinated privately. Despite initial concerns about the duration of efficacy, authorities, including HICWA, have determined the benefits outweigh risks. A vaccination uptake of 80% is necessary to achieve herd immunity and protect vulnerable groups. Common side effects are minimal, and the vaccine aims to significantly reduce chickenpox and associated complications in the community. The podcast encourages all eligible people to get vaccinated to ensure broad protection for the population.

A Gay Dads Journey From Childhood in Rural Ireland to Marraige and Family Life with Seamus Kearney Martone.
I found this episode hugely insightful, to think we grew up in the same town but had such different experiences. Castlerea is a wonderful locality and community, but like any rural Irish town in the 1980's and 1990's, fear of anything other than a conventional relationship was an underlying current. It wasn't out of hate in my view, it was out of lack of exposure and diversity. Anyone who was in any way 'not the norm' left town as soon as they could to find a peer group they felt at home with, so people never got used to diversity within the community.
Castlerea is a place I am proud to call home, and like most towns it is now mostly a diverse and welcoming community. I was always unaware of how Seamus or anyone else may have felt growing up in Castlerea, it didn't occur to me and I remained oblivious. The world has moved on, technology supports awareness and diversity, I love to see people expressing themselves truthfully and openly. I am concerned that with rising levels of hate globally we may end up going backwards - and that is why pride month is so so important! Never tolerate hate. Love between one person and another is a beautiful thing and quite frankly doesn't concern you unless you are one of them! Lets embrace differences of sexuality, culture, and beliefs so that no community feels oppressed - there is room for us all and we learn from each other.

I wanted to be happy when I saw the headlines... I've campaigned for RSV immunisation a lot over the last year since they were approved by the EMA.
I did NOT think I'd have to revisit this topic after I read the finer detail.
Unfortunately Minister for Health, Stephen Donnelly has launched a half baked plan which will leave hundreds of babies still facing hospitalisation and ICU again this Winter.
I don't like to give criticism without being constructive, so in this episode, I speak to Minister Donnelly to try to implore him to see the solutions to any impediments he foresees as an unnecessary challenge. If he is not answerable, then it's worth asking the question...who exactly is?

Amazon Apple Podcasts Google Podcasts Spotify Will we have RSV immunisations in Ireland this Winter? The headlines would have you believe that RSV immunisation is imminent in Ireland. Will we actually have RSV vaccines in Ireland by next Winter?
Call me a sceptic but the time lines are just not making sense..
In this episode I explain why it seems highly unlikely that we will have vaccines this Winter and what action we should take to change the outlook. Our health system is already broken, our population is growing and our health needs are becoming more complex. RSV Vaccines or immunisations would enable a small amount of very necessary breathing room in seasons of poor respiratory health.

Amazon Apple Podcasts Google Podcasts Spotify I chat with Sinead Hingston-Green all about her baby Alby’s recent experience of RSV and Bronchiolitis.
Sinead shares her story of love, life and loss. It is no wonder why Sinead feels such deep fear when a child gets sick. Any parent knows that feeling, but after Sinead’s experiences it could only be majorly amplified.
Despite all of the trauma that Sinead has endured, her love for her children is so clear and pragmatic. Sinead is a down to earth lovely human and helps me to raise bronchiolitis awareness so well in this episode.
We demystify the confusion around RSV and Bronchiolitis and hopefully help you to know when it’s time to seek medical help and when you can care for your baby at home.

Amazon Apple Podcasts Google Podcasts Spotify Lil Red’s Legacy – Sepsis AwarenessThis episode is so important. Up and coming rap artist Sean Hughes from Finglas in Dublin was only 15 years old when he died from sepsis on 12th January 2018.
Today I explain the signs and symptoms of sepsis and chat with Sean’s parents Joe and Karen about his brief illness and devastating death.
Joe and Karen have selflessly worked to raise awareness of sepsis in Ireland since the sad death of their son Sean.
We hear Sean’s music and his amazing self expression through rap. I am so moved by his lyrics and am so sad that his time with his family was so sort.
Thank you to Lil Red’s Legacy Awareness Campaign for all of the amazing work that you do – Karen and Joe you are saving lives and that is some amazing legacy for Lil Red to have.

We discuss the medium to long term potential impacts of a child contracting the measles, mumps, rubella or chicken pox viruses. We also discuss the benefits of vaccination with the MMR Vaccine and the potential need for a national roll out of the Chicken Pox Vaccination. It is a difficult area for parents to navigate currently as the benefits of private vaccination do not include herd immunity in 15-20 years when there is currently no guarantee of immunity beyond that period according to the vaccine manufacturer Varivax.

Pharmacy's Day Out at the DáilMany of my listeners follow me on Instagram and know that I have been campaigning hard for the expansion of pharmacy services in Ireland. Yesterday there was a significant update and this seemed like the easiest way to explain where we stand at this time!
Yesterday, the Irish Pharmacy Union met with the Oireachtas Committee for Health to discuss the Irish Pharmacy Unions proposals. Specifically the Minor Ailments Scheme or Pharmacy Triage service. Whilst mine and the IPU's proposal align I do want to say that other than being a normal member I do not sit or take part in any of their committee work of government lobbying.
I decided to campaign separately as I became increasingly frustrated with the lack of patient access to healthcare in Ireland. I felt the public needed to understand what services Pharmacy are providing in other countries and question why our government are so slow to engage and act on proposal put forward by the IPU.
I hope you find this commentary useful in understanding the current lay of the land. I would also like to thank the IPU representatives who very ably put forward their proposals yesterday. I'd also like to extend my gratitude to the members of the health committee who really took their time to understand and appreciate the proposals. A further thanks is definitely due to Pat Kenny and his Newstalk team for bringing forth my fight to the Minister for Health Stephen Donnelly live on air yesterday.

A Disney Paris Review January 2023I recently shared on my Instagram that I booked a last minute family holiday to Disneyland Paris and got lots of requests for a review! So here it is – A Disney Paris Review January 2023 – from a Pharmacist, not a travel blogger!
In this episode I discuss:* How I booked. * What kind of package I chose and why. * How much it cost for a family of five. * Disneyland access and rides reviewed (kids age 10, 9, and 7 at the time of travel!). * Walt Disney Studio’s Park reviewed. * How we managed a day trip to Paris with three exhausted kids! * How we managed to feed our Coeliac 10yr old at Disney and in Paris! * Why we were happy with the company we booked with.
I really hope that all of you dreaming of a trip to Disney get their at some point soon! We had put it off for a few years but the time was just right and while my bank balance isn’t thanking me I am so happy to think back on the memories that we have made. I hope this episode ‘A Disney Paris Review January 2023’ is useful if you are thinking of travelling!
We booked with Cassidy Travel who had no idea that I would be doing a public review!

LETTER TO LEO – 16th December 2022
Dear Taoiseach,
It’s not often I’m embarrassed to be Irish, but as a pharmacist working within the confines of the Irish
system today, I am. I spent five years in university developing my medical expertise in pharmacy.
Despite this training my skills and those of my pharmacy colleagues, are being chronically
underutilised in an Irish healthcare system at breaking point.
I find this very frustrating, as my time spent working as a pharmacist in Scotland has shown me that
there is a very simple answer to quickly alleviate the massive pressure frontline doctors are currently
facing. We are in the eye of the storm, and you are a doctor in a position of leadership and legislative
power. You know that the daily figures counting the infection of influenza, RSV, coronavirus, and
Strep A will only continue in an upward trajectory, as these viruses always do in late Winter and early
Spring.
Let us help. The community pharmacist is your biggest ally, we are open from early morning until late
at night, and in many cases 7-days a week. A patient does not require an appointment to see a
pharmacist, and almost half of the population live within 1km of a pharmacy. You led us through a
pandemic, the healthcare system can’t take any more, and the patient always has to come first.
Instead of making promises, chairing working groups, and discussing policy, now is the time to act.
Pharmacists want to help, we are qualified, our range of services has so much potential to
significantly improve access to patient healthcare. What is more, this model of patient care is already
working successfully in Scotland and further afield. Every day I see patients suffering who cannot
access a doctor due to the extreme pressure that the healthcare system is under. Together we can
make a big impact to improve this situation.
Today, I am asking you for urgent action on both the implementation of a Minor Ailments Scheme and the introduction of Patient Group Directives (PGDs).
1. Minor Ailments Scheme:
2. Patient Group Directives:
Pharmacists could work to carefully developed Patient Group Directives (PGDs) to assess and supply
medicines to treat the following conditions:
As a newly qualified pharmacist working in Scotland in 2006, I was able to supply antibiotic eye drops
for conjunctivitis. Sixteen years later and Ireland still hasn’t caught up, it’s insulting to our profession
and injurious to patient care.
Taoiseach, surely you can see how important this is. You have an important job to do, going back to
your GP role once a week was admirable during the pandemic but not a long term solution. Now we
need you to lead this country and act to swiftly to ease the pressure on doctors. Introducing a PGD
does not need to be difficult, we have a roadmap from Scotland. We can’t afford years of planning
and preparation by people who don’t work in community pharmacy. We need cooperation between
healthcare professionals, their governing bodies, the HSE and the Government, so that patients can
get the care they need now.
Pharmacists go into the profession because they are passionate about health and their communities.
Pharmacists are skilled, and caring healthcare professionals who are more than capable of
implementing PGDs and working within the boundaries of them.
There is no legislation to support the supply by pharmacists of prescription medicines without receipt
of a prescription. By law we can administer certain medications in certain circumstances, however
the administration and the supply of routine treatment are two very different things.
We need an exception put into the Medicines Act that allows for the provision of certain prescription
only medicines, approved as part of a Patient Group Directive. This requires emergency legislation,
to avoid further delays to accessible healthcare.
Every PGD would be developed, assessed, and signed off by a senior doctor. This will facilitate
pharmacists to use their existing skills to treat common, uncomplicated ailments in patients, without
any contraindications to treatment. This is in the best interest of patient care and is well within the
scope of community pharmacists.
Pharmacists are NOT seeking to be doctors, but we ARE looking to be able to improve patient access
to medicines and healthcare in predefined, safe, and approved settings.
Sincerely,
Sheena Mitchell MPSI
