Lisa Paladino discusses topics related to tongue tie and breastfeeding, from her unique perspective as a Midwife and Lactation Consultant with over 30 years of experience. If you are a parent looking for information beyond what google is telling you, or a professional who is curious to learn more than what you learned in school on this topic, this podcast is for you.
A gentle disclaimer. Please do not consider anything discussed on this podcast, by myself or any guest of the podcast, to be medical advice. The information is provided for educational purposes only and does not take the place of your own medical or lactation provider.
In this episode, Lisa talks with Dr. Fatima Montelongo Solís, a pediatric dentist, lactation consultant, professor, and tongue tie release provider from Mexico City. Dr. Fatima shares how her own journey into motherhood opened the door to lactation, infant care, and eventually tongue tie work.
They discuss what tongue tie care looks like in Mexico, including the limited number of professionals trained in release, rehabilitation, and functional assessment. Dr. Fatima shares how important it has been to collaborate with pediatricians, educate families without pressure, and support parents in making informed decisions.
This conversation is a beautiful reminder that although tongue tie care may look different in different countries, the goal is the same: helping babies feed, breathe, grow, and function well while supporting families with compassion and clarity.
In this episodeLisa and Dr. Fatima discuss:
Dr. Fatima Montelongo Solís is a pediatric dentist from Mexico City. She is also a lactation consultant, tongue tie release provider, and university professor at the National Autonomous University of Mexico. Her practice focuses heavily on lactation, tongue tie, oral restrictions, and supporting babies and children through a collaborative, functional approach.

In this episode, Lisa welcomes RN, IBCLC, and fellow podcaster Katie Oshita for a wide-ranging conversation about professional connection, interdisciplinary learning, emerging research, and telehealth in tongue tie and lactation care.
Katie shares how traveling across the United States and Canada inspired her podcast, Katie the Traveling LC, and gave her the opportunity to meet and learn from professionals working in different disciplines, communities, and countries.
Lisa and Katie reflect on their experience at the ICAP conference and what makes it such a valuable space for tongue tie professionals. They discuss the importance of curiosity, collaboration, honest conversations about complications, and being willing to learn from providers who approach care differently.
They also explore why stronger research is so important in this field, including emerging work related to tongue posture, whole-body function, aftercare, reattachment, measurable outcomes, and whether tongue ties should be released when feeding and function are going well.
The conversation concludes with a discussion of telehealth and how virtual care can connect families with specialized providers, expand access beyond their local communities, and help parents become active participants in assessing and supporting their babies.

In this episode, Lisa welcomes Jess Draper, a holistic infant occupational therapist, IBCLC, and pelvic floor therapist based in Eugene, Oregon. Through her practice, Hali’a Therapeutics, Jess provides concierge-style care for families from pregnancy through the first year of life, bringing a whole-body, nervous-system-informed lens to feeding, oral motor function, movement, milestones, and tongue tie care.
Lisa and Jess talk about the important role occupational therapy can play in infant feeding and tongue tie care, especially when babies are struggling with regulation, sensory processing, oral motor coordination, head shape, or feeding patterns that do not fit neatly into one category.
Jess explains sensory processing in a practical way, including how babies take in, filter, and respond to touch, sound, movement, and oral input. They discuss how sensory differences may show up during feeding, such as gagging, popping on and off the breast, difficulty reorganizing after switching sides, or becoming easily overwhelmed by movement, sound, or touch.
They also explore neuroplasticity, mirror neurons, and why babies often need support learning how to use new tongue mobility after release. Jess shares why infant care should include more than identifying restriction, and why supporting function, movement, and nervous system regulation can make such a difference.
The conversation also includes an important discussion about head shape, palate shape, birth positioning, time in containers, and how the body’s structure can influence feeding. Lisa and Jess emphasize the importance of looking at the whole baby, not just the mouth.
Lisa and Jess discuss:* How occupational therapy fits into infant feeding and tongue tie care * What sensory processing means in babies * Signs that sensory overwhelm may be affecting feeding * Why some babies struggle with oral input, movement, or switching sides * Neuroplasticity, mirror neurons, and infant oral motor learning * Why babies may need help learning to use new tongue mobility * How head shape and palate shape can affect feeding * The impact of birth positioning, prematurity, and time in containers * Why whole-body assessment matters before and after tongue tie release * Supporting both baby and parent as a connected dyad

In this episode, Lisa talks with Maya Bundesen-Magier, occupational therapist, mother of two, and founder of MapMyMilk, a digital tool created to help breastfeeding families track maternal diet and infant symptoms with more clarity.
Maya shares how her own experience with food reactivity in her babies led her to create a better way for families and providers to look for patterns without relying on fear, extreme elimination diets, or guesswork.
Lisa and Maya cover the overlap between tongue tie symptoms and food reactivity, including reflux, fussiness, poor sleep, stool changes, breast refusal, and feeding discomfort. They also talk about why symptoms should not be viewed in isolation, and why a functional, whole-baby approach matters before assuming that one issue explains everything.
This conversation highlights the importance of clinical integrity, individualized care, and using better tools to support breastfeeding families who are trying to understand what is really going on with their baby.

In this episode, Lisa discusses one of the most common concerns after tongue tie release: reattachment. But rather than focusing only on whether tissue has “grown back,” she widens the conversation to include aftercare, feeding function, healing, follow-up, and the importance of an individualized care plan.
Lisa explains why persistent or returning symptoms after frenotomy do not always mean reattachment. Feeding challenges may be related to incomplete release, healing patterns, oral motor habits, body tension, milk supply, latch mechanics, reflux, or the baby needing help learning to use new tongue mobility.
She also reviews current research and guidelines on revision, recurrence, massage, stretching, and follow-up care, highlighting that the evidence is still evolving and that not all studies or professional organizations define aftercare the same way.
Using her CAREFUL™ framework, Lisa explains how professionals can think through these cases more clearly by listening to parent concerns, assessing function, relating symptoms to possible causes, educating families, focusing on function, understanding scope and referral needs, and looping back with follow-up.
The key message: Preventing reattachment is not just about keeping tissue apart. It is about helping feeding function improve.Mentioned in this episode:Lisa’s course, Professional’s Guide to Tongue Tie in the Breastfeeding Infant, teaches the CAREFUL™ approach and helps professionals move beyond “Is there a tie?” into a more functional, dyad-centered way of supporting breastfeeding families.

In this episode, Lisa talks with Dr. Sergio Suarez, an osteopathic physician, about how osteopathic medicine can support babies with feeding challenges, tongue tie symptoms, and body tension.
Dr. Suarez explains what Osteopathic Manipulative Medicine actually is, how DOs are trained, and why the structure-function connection matters so much in infant feeding. He shares how gentle hands-on care can help identify and release tension patterns that may affect latch, tongue movement, digestion, comfort, and overall feeding function.
Together, they explore where osteopathic care may fit before and after a release, why some babies need more support than a frenotomy alone, and how families can make more informed decisions when they are feeling caught between conflicting opinions.
This conversation is a thoughtful look at tongue tie care through the lens of function, body tension, and collaborative support for the breastfeeding dyad.
In this episode, we discuss:
Dr. Sergio Suarez is a board-certified osteopathic physician with training in family medicine, neuromusculoskeletal medicine, and osteopathic manipulative treatment. In his current practice, he works with patients of all ages, including infants and families navigating feeding challenges and tongue tie. He also serves as an adjunct professor of osteopathic manipulative medicine at Touro College of Osteopathic Medicine.

What happens when you step into a room full of people who truly understand your work?
In this episode, I’m sharing reflections from the International Consortium of Ankyloglossia Professionals Conference in Portland, and why experiences like this matter so much in a field that is still evolving.
Because if you’ve ever felt like you’re the only one asking deeper questions…
the only one noticing complexity…
or the only one trying to connect the dots in tongue tie care…
you’re not alone.
This episode is about connection, growth, and what changes when we come together as professionals - open to learning, willing to question, and committed to doing better for families.
In This Episode, We Talk About:* Why tongue tie work can feel isolating, and how that impacts clinical confidence * What shifts when you’re surrounded by professionals who “get it” * The value of respectful disagreement and staying curious in a changing field * How real growth happens through conversation, not just coursework * The role of community in shaping better clinical care * Why fun, laughter, and connection are essential, not optional, in this work

What happens when everything you thought you knew… isn’t enough?
In this episode, Lisa is joined by Patty Reiger, RDH and Orofacial Myofunctional Therapist, who shares the story of her granddaughter’s tongue tie journey - and how it changed the way she sees this work.
Although Patty has decades of experience in oral health, she quickly realized that her professional knowledge didn’t fully prepare her for what families face when concerns are dismissed, answers are unclear, and support is fragmented.
Together, they explore the moment many providers eventually face:
recognizing the limits of your own scope, and the need for a truly collaborative approach.
In this conversation, they discuss:
This episode is a powerful reminder that:

In this episode of the Tongue Tie Experts Podcast, Lisa Paladino, CNM, IBCLC shares a clinical thinking framework she has primarily taught inside her professional trainings - until now.
One of the most common questions Lisa hears from professionals is not simply “How do I identify a tongue tie?” but something deeper:
How do I think through these cases?
How do I avoid missing something important?
How do I support families without creating fear or confusion?
The challenge is often not a lack of knowledge - it’s the absence of a clear framework for clinical reasoning.
In this episode, Lisa introduces the CAREFUL™ framework, a model developed from more than three decades of working with babies and families. This approach is designed to help professionals slow down, consider the full feeding picture, and provide thoughtful, family-centered care.
Lisa also discusses:
• Why feeding challenges cannot be understood through anatomy alone
• The importance of caring for the parent–baby dyad
• How structured clinical thinking improves confidence and decision-making
• Why interdisciplinary collaboration is essential in tongue tie care
• How a careful, measured approach builds trust with both families and colleagues
This episode offers a look into the thinking process Lisa teaches in her professional education programs and reflects the philosophy behind Tongue Tie Experts.
If this episode resonates with you, consider sharing it with a colleague. The more we strengthen how professionals approach these cases, the better outcomes we create for babies and families.

What happens when a tongue tie is released - but feeding doesn’t improve?
In this episode, Lisa Paladino, CNM, IBCLC, explores an important clinical truth: a frenotomy releases tissue, but it does not teach a baby how to feed.
Inspired by an actual professional discussion across disciplines, this conversation highlights why feeding is functional, not just structural, and why preparation, assessment, and follow-up matter just as much as the procedure itself.
Lisa discusses:
This episode isn’t about whether releases are right or wrong. It’s about improving the process around them, and delivering better care for babies and families.
If this resonates with you, share it with a colleague in another discipline. That’s how change begins.

In this episode of the Tongue Tie Experts Podcast, I’m joined by Sarah Hornsby, dental hygienist, educator, entrepreneur, and one of the early pioneers in bringing myofunctional therapy into modern interdisciplinary care.
Sarah shares how her clinical curiosity led her from traditional dental hygiene into the world of oral function, airway health, and ultimately the creation of an online training program that has educated more than 2,000 providers worldwide.
We talk about what happens when professionals step outside their silos, why function must be part of the conversation when we discuss tongue tie, and how collaboration between lactation, dentistry, therapy, and airway-focused providers changes outcomes for families.
We also discuss the realities of building something new - the uncertainty, the persistence, and the deep belief that better care is possible when we learn together.
This is a conversation about growth - clinically, professionally, and as a community.
What We Discuss* How Sarah’s career evolved from dental hygiene into myofunctional therapy * Why oral function and airway development must be part of tongue-tie conversations * The importance of true interdisciplinary collaboration * How online education expanded access to this work globally * The challenges (and rewards) of building a new clinical pathway * Creating professional community instead of practicing in isolation * Where myofunctional therapy is headed as awareness grows

With decades of experience guiding families and supporting nurses, Olena offers a grounded, compassionate, and deeply informed perspective on the current landscape of infant feeding and postpartum care.
Together, Lisa and Olena explore how maternal-child health has evolved within traditional medical systems, and what has been lost along the way. They discuss the commercialization of breastfeeding support, how language shapes our understanding of infant feeding, and why individualized, relationship-centered care matters.
Key Topics Discussed
Takeaways Breastfeeding is not simply a biological act; it’s relational, emotional, and deeply contextual. * Healthcare systems often prioritize efficiency over connection—yet connection is where healing and confidence emerge. * Families need more than instructions, they need clarity, support, and validation. * Professionals must listen not just to symptoms, but to stories*.
About Our GuestOlena Dobchansky is a clinician, educator, and advocate in maternal and child health. Her work centers on empowering families and elevating the role of nurses and lactation professionals in supporting holistic, evidence-informed, compassionate care.

In this episode of Tongue Tie Experts, IBCLC and Certified Nurse Midwife Lisa Paladino shares two real-life case studies that uncover the most common - and most misunderstood - breastfeeding challenges. Learn how to recognize the signs of tongue tie, why maternal pain should never be ignored, and what truly needs to happen before and after a tongue tie release to ensure optimal results.
Lisa breaks down the importance of a full feeding assessment, explains why babies often compensate during breastfeeding, and highlights how preparation, aftercare, and collaborative support can transform outcomes.
Whether you're a lactation professional, a healthcare provider, or a parent seeking answers, this episode offers practical insights and evidence-informed guidance to help families navigate tongue tie and breastfeeding with confidence.
Key Takeaways Maternal experience matters. A parent’s pain, intuition, and observations are essential clinical clues that should always be taken seriously. * Breastfeeding pain is not normal. Persistent discomfort signals that something in the feeding relationship needs attention. * Babies often compensate for feeding difficulties. These compensations can mask underlying issues while contributing to maternal pain and inefficient feeding. * Tongue tie is only one piece of the puzzle. Breastfeeding challenges are multifactorial and require a full, holistic assessment of the dyad. * Proper preparation leads to better tongue tie release outcomes. Pre-release guidance, expectations, and support make a significant difference. * Post-release care is essential. In this episode, Lisa Paladino shares her top five takeaways from the American Laser Study Club’s Breathe & Thrive Conference. While tongue tie is often discussed in the context of breastfeeding, this conference reinforced its lifelong impact on airway health, speech, sleep, and overall function. Lisa breaks down key insights from leading experts and discusses how professionals can improve outcomes for infants and beyond.
Key Takeaways:
1️⃣ Tongue Tie Is More Than a Breastfeeding Issue – It impacts oral development, airway health, and lifelong function, not just infant feeding.
2️⃣ Collaboration Is Essential – No single provider can fully treat tongue tie. Success comes from teamwork between lactation consultants, SLPs, bodyworkers, and release providers.
3️⃣ Function Over Form – A release alone isn’t enough. Pre- and post-procedure therapy are critical for proper tongue function.
4️⃣ Airway Health Starts Early – Mouth breathing, poor oral habits, and sleep issues often stem from oral restrictions that should be addressed in infancy.
5️⃣ Misinformation Still Exists – Many outdated myths persist, and ongoing education is essential to ensure families get the right care.

This week's episode features a warrior parent.
The true experts on tongue tie are the parents - especially those who have struggled to find answers for their child's feeding challenges, and wind up learning so much that they can educate the providers who are supposed to be caring for them.
In this episode, Lisa interviews Sarah B. about her journey to feed her fourth child.
Sarah is a mom whose journey was difficult. As you'll hear in this episode, she was told by an MD that her daughter didn't have a tongue tie. Her daughter was having such struggles to feed, by breast and bottle, that it took a feeding tube to rescue her. But still no one answered Sarah's question of why her daughter couldn't feed, until at age 7 months when a lactation consultant pointed her in the right direction for functional assessment and release of tongue and lip tie.
It is my wish that by hearing stories such as Sarah's, families will feel heard, know they aren't alone, and be inspired to always search for answers.

Welcome to a special episode of our podcast, a little different than most. Actually, it may be my most vulnerable episode ever. The tables are turned, and you’ll hear me, Lisa Paladino, being interviewed by Lo Nigrosh about my personal experiences with birth and breastfeeding. If you’ve ever wondered how I got from someone born into, and working in, a medicalized and bottle feeding family and culture, to being an advocate for natural birth and breastfeeding, please listen.
Lo Nigrosh was featured on Episode 303 of the Tongue Tie Experts Podcast discussing overcoming breastfeeding pain. The episode that you’ll hear today is from her own podcast, the Milk Making Minutes.
Special thank you to Lo for lending me this episode, for being such an advocate, and for allowing me the time and space to share more about my journey. Let me know your thoughts.

In this episode, Lisa discusses the controversy of timing of tongue tie release, including hospital identification of tongue ties and the possible downsides of early treatment without a complete functional assessment.
She discusses why her practice has evolved from recommending immediate release, to a slower approach, utilizing a holistic, team view, and how this is leading to better experiences and outcomes.
This raises the question - What is a successful outcome of tongue tie treatment? Is it release of frenum or is it successful breastfeeding?

How do we know if releasing the tongue or lip frenum will solve breastfeeding problems?
One of the most complex questions and, honestly, a dilemma, is when a parent asks me if they “should” have their infant’s tongue or lip tie released, a procedure known as frenotomy. They want to know if it will be worth it, if it will solve the problems, if it is really going to make a difference for their infant. These are great and necessary questions. Sometimes I wish I had my crystal ball to see into the future for these families and tell them for sure.
But over the years, I have become better at discerning which infants will benefit the most. In this episode, I share you 4 aspects to consider, beyond the typical assessment checklist.

Although frenotomy is considered a minor, quick procedure, it may be stressful. Being prepared keeps nerves calm and allows for a smoother experience.
As a lactation consultant, I have guided many families to and through the day of frenotomy.
In this episode, I share some key takeaways from this experience - such as questions to ask a provider before you trust them with your precious baby, what to have on hand for after, and how to feel the least trauma because you'll have the confidence that you are ready to handle any challenges that may arise.
Please note: Being prepared is multifaceted. During this episode, I discusses preparation for the actual day of the surgery. Of course, the prior step is being sure that the procedure is necessary.

Listen as Elyssa describes her journey with nursing her daughter, Ophelia. From the beginning, Elyssa sensed that something wasn't right, but was told by multiple medical professionals that everything was "fine". It wasn't fine, and it took a mother's drive to provide nutrition for her daughter, overcoming obstacles, to set things on the right track.
Let's learn from this mom's story, because it's so similar to what I hear all of the time. Stories of families who believed their medical professionals understood breastfeeding management, and weren't guided to the right interventions. Breastfeeding parents need and deserve a complete evaluation from an IBCLC.

In this episode, Lisa interviews Dr. Martin Rosen, a chiropractor, author, lecturer and teacher, regarding the safety, efficacy, and advantages of chiropractic for infants and children, and the reasons why chiropractic is helpful for infants with tongue tie.
We discuss how infant symptoms such as tummy time difficulty, feeding challenges, and colic are signs that shouldn't be ignored, and how compensations that develop in the first year can affect the child's whole life.
Perhaps my favorite part of this discussion, was about how our healthcare system is normalizing dysfunction, as it becomes more common. Common does not equal normal.

The true experts on tongue tie are parents - especially those who have struggled to find answers for their child's feeding challenges.
In this episode, Lisa interviews Karin Ashley NP. Karin's story began with her first child, 10 years ago, and has continued through four children, each with different presentations and treatments. It is interesting to realize, as Karin has, the evolution and improvement of treatment options.
Karin and I are both graduates of Dr. Aviva Romm's Functional and Integrative Medicine Professional Training Program, and Karin has combined that knowledge with her own experience, to create programs to support women in the preconception, pregnancy, and post partum stages.

Tongue tie treatment can be complex, and those who expect relief of symptoms with "a quick snip" or a laser release, even by an experienced provider, are usually disappointed.
The teamwork model of care, discussed in this episode is presented for parents to have a better understanding of treatment, and for professionals to guide their patients towards effective outcomes.
