Sometimes we stay in jobs, relationships, or situations long after they’ve stopped serving us — not because we don’t see the truth, but because walking away makes the loss feel real. That’s the power of sunk cost fallacy, one of the most common (and painful) psychological traps we fall into.In this video, I explain why our brains resist letting go, how the “pain of realized loss” keeps us stuck, and why that pain doesn’t actually mean what you think it does. You’ll also learn about inverse sunk cost fallacy — the belief that you’ve missed your window — and how to break out of both.If you’ve ever wondered why it’s so hard to leave something behind or start something new, this video will help you understand what’s really happening in your mind — and how to move forward with less guilt and more clarity.

I tested three top-rated AI therapy apps to see how they'd handle a severe depressive episode. Spoiler alert, I'm not worried about being replaced yet.AI therapy shows promise for addressing accessibility and cost barriers in mental health care. These digital options could provide immediate support for those who can't access traditional therapy, though they currently lack the emotional connection of human interaction.None felt fully equipped to handle serious mental health issues, with responses ranging from overly optimistic to appropriately cautious about their limitations. Most parenting advice creates broken adults. Society pressures us to raise "convenient" children, but this approach severely damages their emotional development and mental health.

Therapist credentials and education don't matter as much as you think. What truly matters is their ability to connect, understand, and implement effective treatment strategies.

Through my experience as a clinical psychologist, I've identified three essential questions that separate effective therapists from those who just wing it.

I'm teaching you exactly what to ask your therapist and what answers to look for. You'll learn to spot red flags and identify the right therapist who can genuinely help you heal.

Facing emotional pain and grief is incredibly hard.I spent years running from my childhood grief, thinking time would magically fix everything. But avoiding pain only prolongs the suffering and stunts our growth. It's understandable to want to avoid it, ignore it, run from it. The reality is, sometimes facing that pain, trauma, or grief turns out to be as bad as you think it will be.The critical thing here is that you do force yourself to face it, feel it, and work through it. It's going to sting and hurt. Sometimes you have to let it hurt to heal.

Managing 1 or more chronic mental health conditions is a full-time responsibility.

If you push this to the bottom of your priorities it will be extremely difficult for it to ever improve significantly. But since you have a life filled with other responsibilities, your time, attention, energy, and resources are probably already spread thin.

Here are 5 you can do if you have just 5 minutes.

Thought patterns don't define you, and they're not fixed. The thing about depression is it traps you in narrow, repetitive thinking.

Your thinking is actually quite malleable. But reshaping your thoughts, often takes a lot more than just trying to think differently.

I'm talking with Dr. Moshe Bar of Hedonia about how they've been able to gamify Facilitating Thought Progression (FTP) in order to rewrite thought patterns.

I'm all-in on alternative, science-based methods for managing depression and anxiety. We explore how gaming can reshape cognitive patterns, improve mood, and complement both therapy and medication.

I have partnered with Hedonia, and both believe in, and regularly use their app-based game for depression and anxiety, Mood Bloom.

Life doesn't always involve constant growth - it has dormant phases too.

We sometimes experience events that halt progress unexpectedly, invisibly.Trauma, resource scarcity can end growing seasons prematurely.

Like trees, grass, we aren't fragile for needing dormant periods occasionally. Unseen circumstances dictate when growth stalls, despite others' journeys. Don't judge the pause as failure - it's natural cycles.

Would you describe your life as a rollercoaster? This probably looks like sustained periods where you're extremely high-functioning and then prolonged periods where you struggle to do even the most basic functions.

Most people suffering from depression and anxiety experience this to some degree. If this is you, you're probably like me, a "high ceiling, low floor" person - highly functional sometimes, unable to do basics at others.

I'm sharing the 3 keys that help me move through the lows - and highs - for more consistent functionality.

There's an epidemic of unhelpful responses when someone opens up about their struggles. My take: Most well-meaning people inadvertently make things worse because they simply don't know what to say.

When we see loved ones suffering, it triggers distress in us too. Our instinct is to quickly make their pain go away, for our sake as much as theirs. But rushing them through emotions or invalidating their fexelings creates isolation.

I want to look at four common unhelpful responses, and give you some new tools to use. These tools also apply to your self-talk - be your own supportive friend.Get Practical tools for navigating life with depression and anxiety, delivered weekly.https://mailchi.mp/90ccaf44c876/self-hope-psychology

Insecurity is a universal human experience, not a personal failing. Most coping methods actually reinforce insecurities rather than resolving them.

Embracing insecurities as identity reinforces negative thoughts. Denying weaknesses leads to reality harshly exposing them. Overcompensating through impression management is exhausting.

I'll show you how to overcome insecurity through incremental self-improvement. Facing insecurities through gradual exposure therapy diminishes their power.

Psychological testing is not a perfect science by any means - it can have many barriers to entry including cost, access, and availability. But self-diagnosis is not the answer! This episode unpacks the very real concerns with self-diagnosis. Even when struggling to access professional care, firmly holding self-assigned labels carries risks. Most think acceptance means settling for misery. That's not true. Acceptance empowers you to change what matters. It acknowledges some pain is unavoidable - but suffering is optional.Life has inherent discomforts we cannot control - that's "pain." Trying to force away that pain often backfires, causing more issues. That's "suffering." Radical acceptance separates the two categories. This video teaches evaluating life's pains objectively. Some are changeable with effort. Others aren't worth the cost of change. Radical acceptance means tolerating unchangeables, focusing energy on high-impact areas instead of fruitless struggles. Emotions are not light switches. You cannot simply turn them on or off just because you (or somebody) would like to.

Being told (or telling others) to calm down, cheer up, or get over it is dismissive and unhelpful. There is no direct mechanism for selecting your emotions.

The biggest problem is that this type of command is outcome based - become x new emotion now. But without a process for changing your emotions you may do more harm to your current emotional state.

I'm sharing with you that process here.Get Practical tools for navigating life with depression and anxiety, delivered weekly.https://mailchi.mp/90ccaf44c876/self-hope-psychologyMy book: For When Everything is Burninghttps://bit.ly/forwheneverythingisburning Organize your day around your mental health goals with Sunsamahttps://bit.ly/DrScottSunsama (affiliate link)The app I use to learn core principles from thousands of nonfiction books in minuteshttp://shortform.com/drscott (affiliate link)Connect with me on TikTok:https://www.tiktok.com/@dr.scott.eilersTherapy with me (Iowa residents only)http://www.northstarpsychcenter.com/Work with me (Non-Iowa residents)http://www.drscotteilers.com/Disclaimer: This content is not intended to be a replacement for receiving treatment. Not all negative thoughts are bad. But constantly thinking about negative things can lead to a downward cycle psychology calls rumination.Rumination is when we have certain thoughts, memories, or ideas, that get stuck in our brain. And trying to relax, unwind, or think about something else can simply make the thought more ingrained. There's a reason for this, and it's the reason why doing nothing is actually dangerous for rumination.Get Practical tools for navigating life with depression and anxiety, delivered weekly.https://mailchi.mp/90ccaf44c876/self-hope-psychologyMy book: For When Everything is Burninghttps://bit.ly/forwheneverythingisburning Organize your day around your mental health goals with Sunsamahttps://bit.ly/DrScottSunsama (affiliate link)The app I use to learn core principles from thousands of nonfiction books in minuteshttp://shortform.com/drscott (affiliate link)Connect with me on TikTok:https://www.tiktok.com/@dr.scott.eilersTherapy with me (Iowa residents only)http://www.northstarpsychcenter.com/Work with me (Non-Iowa residents)http://www.drscotteilers.com

Can introverts with mental health conditions succeed in a world seemingly built for extroversion?

When real psychological conditions are applied to people without those conditions because of pop-culture-misappropriation, bad things happen.Case in point: the terms "narcissist," and "depressed." The secret is out: gratitude is good for your mental health. If you struggle with chronic anxiety, your life tends to revolve around three phases.

This pattern happens because of what's called the "worry bin" in your brain, a space for stress and worry.

Understanding those phases, and how to manage them, will help you reduce anxiety by shrinking the worry bin.

You can't eliminate stress entirely, but you can learn skillful distractions to train your brain to focus on more engaging and less stressful activities. The key is not just solving problems but changing how much mental space you allocate to stress and worry.

Managing your mental health is complex enough on its own without adding in the fact that every day can bring new challenges. What do you do when new depressive episodes and stressors while you're managing existing ones? Recovery hinges on recognizing and addressing the changes these bring to daily routines, social interactions, and self-care. The key is balancing 'inputs' and 'outputs' in your life. Inputs are activities that replenish energy and enhance well-being (like sleep, nutrition, and exercise). Outputs are tasks that drain energy, such as work, school, and chores. Focus on building and maintaining a strong foundation of inputs. This can prevent the unsustainable cycle of being overwhelmed by outputs.Get my book: For When Everything is Burninghttps://bit.ly/forwheneverythingisburning Connect with me on TikTok:https://www.tiktok.com/@dr.scott.eilersSee the Podcast:https://www.youtube.com/@dr.scotteilerspsydlp529Here's what's inside:00:00 Navigating Mental Health Challenges 02:21 Recovering After a Depressive Episode 04:40 Balancing Inputs and Outputs in Mental Health 06:52 Rebuilding Inputs for Mental Health 10:23 The Importance of Rebuilding the Foundation Disclaimer: This content is not intended to be a replacement for receiving treatment. Learned helplessness is a phenomenon where individuals feel that no matter what they do, they can't escape pain or negative outcomes.

A lot of times this comes from abusive situations, harsh criticism, or even internal struggles like depression and anxiety. These beliefs can stick with you, even when circumstances change.

If you've experienced inescapable pain before, you might not realize when you have opportunities for safety and happiness now. Don't assume you're still in the same situation; test the floor of your life for safety and change your perspective.

Life is filled with inevitable trauma, death, and tragedy, even if you've had a seemingly normal or average life. This is because of a significant divide between what your life is like and how you actually feel. It's not just about having a good or bad life; internal variables can dramatically impact someone's emotional experience. I battled depression for 40 years, 98% of it was misery.

I used drugs, alcohol, and chasing short-term joy as a way to escape.

Now, I feel good 90% of the time and have even started helping others with mood and anxiety disorders through various programs and clinics.

To manage my depression effectively, I've made six key changes in my life. I want to tell you about how to use each of these effectively.

Remember, these worked for me. That doesn't mean that all (or any) of these will work for you in your context. But, I believe they will definitely help almost anyone with severe depression.

  1. Maintaining a consistent sleep schedule

The outside world is wildly mistaken about self-harm - and those who engage in it.Contrary to popular belief it isn't a cry for attention or something only the severely depressed do. In this video I'm debunking the misconceptions associated with self-harm. I also share why this act is usually shrouded in secrecy, shame, and embarrassment, and why it's challenging for individuals to seek help or openly discuss their experiences. Self-harm is one of the most stigmatized and misunderstood mental health symptoms. It is rarely for attention-seeking but primarily for emotion regulation. 03:29 Self-harm behaviors are expressions of overwhelming emotional distress and a lack of support or coping tools. It's similar to other emotion regulation behaviors like excessive alcohol consumption or impulsive behaviors. People seek a way out from intense unhappiness.10:10 There are alternative ways to manage emotional distress without resorting to self-harm. One approach is to use intense sensory experiences to trigger the brain's pain response without causing harm. For example, you can use extreme flavors like spicy or sour foods to divert your focus from emotional pain. Additionally, using temperature extremes, such as holding something warm or cold, can simulate the physical sensation without causing long-term harm.

Life is getting harder, and it can feel absolutely exhausting to do even the most basic of activities. This is due to the finite nature of willpower and our constant battle against high-stimulation activities, especially smartphones. To regain control you have to take charge of your inner ecosystem.Let's talk about that.

Grief isn't one-size-fits-all, and it's the nuance between the types that causes many people to stay stuck. There are two types of grief: regular grief when you lose something you had, and ambiguous grief when you realize you'll never have something you always wanted. Ambiguous grief is real and can be just as painful and I'm going to help you walk through that in this episode.Get my book: For When Everything is Burninghttps://bit.ly/forwheneverythingisburning Connect with me on TikTok:https://www.tiktok.com/@dr.scott.eilersSee it on YouTube:

https://www.youtube.com/channel/UCogWI9kVVvmRRPzzQBrOwzgHere's what you'll find in this video: 00:00 Two Types of Grief: Regular and Ambiguous There are two types of grief - regular grief, which occurs when you lose something you once had, and ambiguous grief, which is the realization that something you always thought you'd have will never come to be. Ambiguous grief can be as devastating as regular grief, and it often goes unrecognized by the world. It's like mourning the loss of an alternate version of yourself that will never exist. 04:02 Healing from Grief Both regular and ambiguous grief can heal over time through a combination of time passing and the expression of grief. It's important to feel and express your emotions naturally. Grief doesn't follow a linear path and comes in waves, so it's crucial to allow these emotions to come and go. Avoid rushing the healing process and remember that it's okay to take emotional breaks when the grief fades temporarily. 08:15 Ceremonies and Closure While traditional ceremonies like funerals help provide closure for regular grief, ambiguous grief often lacks such closure. To cope with ambiguous loss, consider creating your own form of ceremony or acknowledgment to signify the finality of your dream or expectation. Closure is essential to moving forward and preventing prolonged denial and limbo states. 12:15 Avoiding the Pitfalls of Comparison Comparing your ambiguous grief to others' situations can lead to negative emotions and hinder your healing process. Avoid the impulse to compare, as it will only make you feel angry and hurt. Each person's experience is unique, and comparing yourself to others won't bring you closer to healing or closure. Fight back against the urge to compare your situation with others.

This response is a product of our brain's inability to distinguish between chronic stress and imminent danger. 05:14 The Nature of Freeze Response The freeze response is an involuntary reflex, not a conscious choice. It occurs when the midbrain, convinced of imminent danger, redirects resources away from the frontal lobe, affecting functions like emotion regulation, behavioral inhibition, and complex social engagement. This response can be triggered by overwhelming stressors. 13:00 Effective Strategies During Freeze Response Motivation through pressure or consequences is not effective during a freeze response. If you don't feel excited about the future, it's hard to want to do much of anything. When depression and anxiety are severe, our lives often fall into a black hole where we do little, feel little, and remember little.I barely remember anything about my late teens, and it's mostly because I barely did anything worth remembering. Now that I'm in a better place mentally, I can look back on that period with a different perspective. These feelings, or lack of feelings, don't have to ruin your life. There's a loophole that can still make life feel worth living even in your darkest times, and I'm going to teach it to you today.

It's easy to lose hope. It's easy to feel like you've tried everything. And maybe, to some degree, you have. Perhaps you hold all of the pieces needed for recovery, but haven't ever put them together correctly.I think we've done an unfortunate job of over-complicating mental health. We've developed a complex set of mutually exclusive theoretical orientations to explain why people become mentally ill and how we can best treat these conditions. None of these theories can be objectively examined in any real detail, and all are completely unique to the world of mental health. Getting your brain to work right and reducing symptoms of depression and anxiety is really fairly simple. First, you need to understand some basic principles of brain health. These are certain things that all people need regardless of who you are. We all need a certain amount of high-quality sleep, regular intake of good food, daily physical exertion, and careful management of substance intake. Second, you need to understand your individual needs. Certain aspects of life are more essential to some people than to other people. You have to know your mind and what it needs. Once you understand these universal and individual needs, you need to meet these needs consistently, skillfully, and frequently. Do this and your mental health WILL improve dramatically. Note: I mis-speak a couple of times in this episode and say "duration and time." These are the same thing. It should be "duration and frequency."

Depressive episodes often start with nothing more than a bad day or two. We don't feel like ourselves so we don't engage in our normal activities. After a few days of just "getting by," our perception of ourselves starts to change. We find it harder to keep the negative thoughts and feelings at bay as the evidence against the shame and the worthlessness feels further and further in our rear view. We begin to align with the beliefs imposed upon us by our depressive thoughts, and every day feels a little bit darker and a little bit heavier. This is a depression spiral.

The way we spend our time when we feel depressed is often an attempt at damage control or harm reduction. We feel unable to do the things that normally provide us with feelings like joy, connection, or accomplishment. We lose our ability to move forward and we cling desperately to whatever progress we've made since our last depressive episode, trying with all our might to avoid sliding back into that pit. But when we're depressed, neutral days are losses because of the pessimistic, nihilistic mindset forced upon us by our unrealistically negative mindset. We need to achieve positive moment to get out of the rut.

In order to kickstart your recovery from a depressive episode, you need to start making time investments. When we're depressed, we often make the mistake of trying to do whatever feels best in the moment because we're desperately searching from some degree of positive emotion. While completely understandable, these behaviors are ultimately ineffective because they often do little for us in the moment anyway as we are unable to enjoy them due to our anhedonia, but they also give us no momentum or challenges to the depression the next day or the day after. They do nothing to interrupt the spiral. In order to get out of the backslide you need to engage in behaviors that will pay off today, tomorrow, and into the indefinite future and you need to stack as many of these as possible until your depression finally relents under the weight of your opposition.

If certain sounds enrage you, sounds that most people aren't bothered by or don't even seem to notice, you may have something called Misophonia. Misophonia literally means "hatred of sound" and people with misophonia experience intense anger responses to sounds like chewing, breathing, tapping, and other repetitive noises.

This is a newer term, and we don't yet know if it is its own condition or if it's a symptom of something else. It does seem to have a very high overlap with anxiety. We believe that the symptoms are caused by hyper activation of the fight or flight response in the autonomic nervous system.

Treatment options are limited at this time to avoiding triggers (unrealistic in my opinion), treating the underlying condition, and general brain health interventions like sleep hygiene and exercise. I've also had some success with exposure and response prevention.

Lastly, I discuss the important of understanding this condition and having conversations about it with people you care about to preserve the health of your relationships.

If therapy and medication aren't getting your symptoms of depression and anxiety where you want them, there's another option. It's free, it doesn't require the help of another person, and it has no side effects.

It's physical activity. Exercise. Movement. Whatever you want to call it.

Meta-analyses of over 1,000 studies demonstrate that physical activity works at least as well as therapy and medication in reducing symptoms. It actually works better in terms of immediacy of effect and long-term outcomes.

That being said, regular physical activity is a challenge for anyone, and symptoms of depression and anxiety increase that challenge. But you probably don't have to exercise nearly as much as you think...

Have you ever wanted to go to sleep and just never wake up?

Do you wish something would happen to you today to put you out of your misery?

These are all forms of something we call passive suicidal ideation or passive SI. Millions of people experience this, yet almost nobody talks about it and even many mental health professionals don't really understand it.

In today's episode, I discuss how passive SI differs from intrusive thoughts about death, how it's not a reliable indicator of the severity of depression, why it should always be taken seriously, how it can be treated, and where it comes from. I also have a very important question for you to answer if you experience passive SI.

I know everybody wants something that works quickly and changes things dramatically. Maybe you’ve tried some of those ideas, and if you have what you’ve probably found is that they work really well for some fairly brief period of time before fizzling out completely. If you're sick of getting your hopes up for strategies or treatments that don’t work in the long run, this episode is for you.

The strategy I’m going to teach you today isn’t fast, it isn’t magic, and it isn’t terribly exciting. But if you execute this strategy correctly, it can turn you into a relentless depression-crushing machine. It can give you a tank to ride into the greatest battle of your life, slowly but consistently crushing your mental health symptoms under your tireless treads.

It’s called “marginal gains” and it’s a proven process in sports and business. It’s based on the work of cycling coach Dave Brailsford and the Kaizen Six-Sigma philosophy of management. I can tell you from personal experience that it also works in mental health. I hope you enjoy!

Anger, more than other emotions for most people, is a feeling we generally try to avoid. Anger is seen as unsafe, destructive, and out-of-control. And is certainly can be these things.

But anger is not inherently more damaging or dangerous than any other feeling. It’s simply part of the spectrum of human emotion, and it plays a critical role in our relationships with ourselves and with others. Anger is also something we tend to feel more often when we’re depressed or anxious.

Any emotion can destroy your life if you don’t know how to handle it safely, even pleasant feelings like love or pleasure. So many of us are taught incorrect strategies to manage our anger, and that’s what leads to these problematic outcomes. That’s why skillfully managing anger is the topic of this week’s YouTube video and podcast episode.

OCD is one of the most misunderstood and mischaracterized mental health diagnoses, and the term "OCD" is often used to describe patterns of perfectionism, rigidity, and high achievement that typically have more in common with generalized anxiety disorder.

Exposure and response prevention is a treatment for OCD involving intentionally creating situation which trigger your intrusive thoughts but using coping skills rather than compulsive behaviors to reduce the distress caused by the exposure.

In this episode I discuss OCD and ERP with Alexandra (Alex) Reynolds, an OCD peer support specialist who has some amazing insights to offer. I know that you will benefit from her wisdom and experience.

One of the most common questions I hear as a clinical psychologist who specializes in depression in anxiety is “how do I deal with the guilt and the same that I constantly feel?” It’s a tough question that doesn’t have a single, easy answer.

However, for many of us, much of our guilt and shame comes from distorted, unrealistic thoughts. Our minds only see the mistakes we’ve made, the areas we’ve fallen short, and the regrets from the past that we can’t let go of.

If you can learn to challenge these thoughts and see yourself more accurately, much of the guilt and shame melts away as it no longer has a platform to stand on.

In this episode I interview executive functioning and nervous system regulation coach Sasa Harper. I’ve known Sasa for a while, and she’s what would be called a peer support specialist in my world, meaning she is someone who has struggled with executive functioning and nervous regulation, is now in a place where she is doing well in these areas, and teaches other people how she got there. While Sasa tends to focus more on neurodivergence like ADHD and autism spectrum disorder, nervous system dysregulation and problems with executive functioning are also common components of mood and anxiety disorders so our work has a lot of overlap. Today we discuss how procrastination often results from unmet needs or excess stress rather than laziness or lack of motivation, as is commonly believed, and how challenging your inner critic, practicing relaxation techniques, and rewarding yourself appropriately are MUCH more beneficial tools for challenging procrastination and increasing productivity than putting more pressure on yourself.--- Support this podcast: https://podcasters.spotify.com/pod/show/scott-eilers/support

Symptoms like depersonalization, derealization, dissociation, delusions, and hallucinations can be incredibly disturbing. They are often thought of as being signs of a psychotic disorder or severe trauma, but if these symptoms only occur during depressive episodes, they may actually be part of a mood disorder. We can also experience disconnections from reality because of intense psychological distress, including periods of prolonged or significantly heightened anxiety. Dissociation is somewhat like the mind’s version of going into shock. It’s a temporary anesthetic to protect us from the immense emotional pain that we’re experiencing. The prefrontal cortex, the very front most region of the brain, is responsible for our orientation to person, place, and time. When activity in the frontal lobe decreases, we lose some (or all) of this ability to orient to these anchors. This can be terrifying or disturbing if you don’t know what it is or don’t realize that it’s temporary and something that is, to some degree, supposed to happen.Understanding what all of these processes are and the neurological reasons for them makes them a lot less scary.
