Hunter shares her long history of migraine headaches since childhood, her side-effects from medication often bad enough she chose to deal with the headaches, her inability to focus, exercise and remain socially active due to the pain, and finally where she found relief. Dulcy shares her 4 month history of neck and shoulder pain after a fall, her inability to sleep, sit, and function, the unfortunate failure of traditional medical care and physical therapy, the serious nature of her outlook due to her medical doctor’s comment on the MRI findings and the potential of the possible future surgery and how Kristina’s care and guidance provided relief she doubted she might ever experience. In episode 373 of Season 11 we're joined by Rick, a 67 year old male retired from the telecommunications industry who began having bilateral thigh pain and inability to walk. He was diagnosed by his doctor of having spinal stenosis and told he could try physical therapy but may need surgery. We also speak with Susan Wood, his physical therapist who made some observations of his history and his physical examination which didn't match up with the classic stenosis presentation. She and her therapist, Stephen, share her outlook before having seen him, how she felt when she received her diagnosis, and Stephen's observations which led him to believe she might be able to recover without the need for the suggested knee surgery, the ultimate outcome and more. In episode 371 we’re continuing with our series of patient experiences with their therapist. This week Deb Collins, of Montreal, Quebec Canada joins us with her therapist Michael Dyck of Active Solutions Physiotherapy. Deb shares her struggle with a wide range of pain after a motor vehicle accident and a long list of practitioners and attempts to try to resolve it without success, until she began working with Michael. This is a great illustration of the benefits of an attentive clinician and patient committed to the process. This week on MCF!
In episode 370 we're joined by another patient, Julia, and her therapist, Julie Marley. Julia shares her story or progressively worsening bilateral hip pain and inability to walk. She explains what she had tried, how she struggled to even find a doctor with an available appointment. Her therapist, Julie, shares her examination of Julia and what tipped her off as to the most effective means of relief for Julia. Not all patients resolve from a movement and posture exercise and education program and this case highlights how a skilled clinician and a sound movement exam can still serve to be helpful in this situation.
In Episode 369 we’re joined by Emily Ledford and Betsy Scudder, both of the Washington DC metro area. We’re highlighting another patient experience this time a personal trainer with an acute onset of knee pain, a positive MRI showing a meniscus tear and an orthopedist who implied surgery was inevitable. We’ll hear Emily's story and her perspective and Betsy’s view of Emily's prognosis once she identified a familiar response commonly associated with good outcome.
In episode 368 we’re beginning season 11 and we’re highlighting actual patients who have a remarkable story and noteworthy results through a thorough movement and symptom assessment and treatment process and when possible we have their therapist join us too. We’re allowing the experiences and perspectives of these patients to reinforce the potential that exists with a proper mechanical exam and through a sound biopsychosocial approach for common musculoskeletal conditions. Our first patient is Makesh and he’s accompanied by his physical therapist Dr. Rhad Brown. Makesh struggled with persisting leg pain, sciatica as it's commonly called, which failed to resolve with conventional medical care. You’ll hear him describe a previous episode of leg pain involving his other leg and how that led to surgery, what he had been told about this new condition and how he had little hope that he might be able to avoid a second surgery. He’ll share how he discovered he could get control over his pain and, with some education from Dr. Brown and his own stick-to-itiveness, he is fully recovered and staying that way more than 2 years later. For Makesh, he tried the McKenzie Method of Mechanical Diagnosis and Therapy and it worked like magic.

Dr. Matthew Grimm is an Orthopedic Surgeon in New Orleans, practicing since the past 23-24 years. He has his own practice with three other surgeons. Matt is the only Orthopedic Surgeon in USA who is a Cert. MDT.
Professional Journey
Initially Matt studied biochemistry, but always wanted to be a physician. He talks about his journey of relocating to Louisiana and working as a chemist for Marathon Oil, meeting his current wife who is a physical therapist and going back to school to study health-care and eventually become an Orthopedic Surgeon.
Medical School Experience
Matt talks about how in medical school there was no education about mechanical pain and symptoms. He talks about the influencers that led him to specialize in orthopedics, his initial training, private practice and patient assessment methods prior to learning MDT.
Exposure to MDT
Matt talks about his first impression about McKenzie therapy being back extensions, which he now knows to be untrue. His exposure to MDT was through his wife, Lynn Grimm who after a hiatus from practicing physical therapy returned to work and underwent training and certification. He gives a patient example that had failed all kinds of conservative interventions including injections. That patient was assessed and treated during an MDT extremity course and was classified and managed as a hip derangement with great and rapid success which surprised him.
Eventually realizing that patients have mechanical diagnoses rather than anatomical is what got Matt to pursue MDT courses and get certified in 2019.
Envisioning Ideal Orthopedic Care
Matt feels that with his certification hopefully he might be able to influence other MD’s to MDT. He is of the belief that if more physicians are exposed to MDT it will impact healthcare costs in a positive way. Matt’s surgery hours have decreased by 50% since application of MDT principles in his practice, and it has not impacted his revenue.
Change in Practice
Matt now pays attention to movements and also the possibility of spinal involvement in extremity pain while assessing patients in his clinic. He also advices his patients on posture correction. Secondary to a high volume practice he is unable to go in a detailed assessment but uses basic MDT principles for a quick assessment.
Patient Example
Matt talks about patients presenting with acute kyphotic deformity and his management approach using a mechanical approach in his clinic. He also gives an example of a patient with knee pain responding to lumbar spine.
Orthopedic and Physical Therapist Communication
Matt talks about how he looks at the patient’s physical therapy notes if there is a MDT assessment form available, but otherwise does not look at therapy notes. He usually depends on information and feedback from patients to determine further management. Since use of MDT approach in his practice and referring patients to physical therapists practicing MDT his selection process for surgical candidates has improved along with his success rate.
Relation Development between Orthopedic Doctors and Physical Therapists
Matt does not know of any sure way of improving relationships between the orthopedic surgeons and physical therapists. He feels that with time things will change with more physicians acknowledging MDT since the method is so effective. He also proposes providing physicians with CME’s for MDT courses which are board certified.
Self Advice
Matt feels that knowing what he knows now, he still wouldn’t want to change anything. His ethics have never changed but over a period of time with new knowledge his practice have evolved.
