“With AI you can you lose the humanity?” -Summary

Nils and Matt discuss a follow-up statement and question that was posted after the last episode on AI in psychiatry. The listener states “AI must be flexible and compromise…it can cause more harm to the consumer”. This may cause an AI to be ridged and not adapt but stick to what they are programmed to find.

Can you study humanity and can you replicate it in AI?

People are not great at guessing expressions and we actually practice and teach skills on labeling emotions. AI might be able to actually read an emotion more effectively.

SEXBOTS

We then dive into the future when it comes to sex and sexual health with sex robots. Texas recently had a news article on a brothel that was staffed by sex robots. This new technology has already made it into the mainstream and the idea was even in the movie Lars and the Real Girl starring Ryan Gosling.

We then talk about this article here on the reduction of sexual intercourse in our youth. Nils ask Matt on why he thinks sex may be decreasing. Matt talks about technology might be decreasing the need for interpersonal relationships. Easy access to technology gives us instant gratification and we may not seek it from face to face interactions.

“Are homeless people dangers?”

There are many stigmas around people experiencing homelessness that leads to ideas that they are dangerous. Although we would never recommend approaching strangers for no reason, someone who is experiencing homelessness is not dangerous because they are homeless. Someone who is living on the streets are more likely to be victims of crimes.

“What do you wish patients did during an appointment, what are your pet peeves?”

Dr. Google is Nils’s pet peeve. He would prefer someone come with their major complaints ready to talk about. Try writing down what you want to discuss with your doctor, it can help the appointment go smoother.

“Is schizophrenia multiple personality disorder?”

Schizophrenia:

Schizophrenia is a chronic and severe mental disorder that affects how a person thinks, feels, and behaves. People with schizophrenia may seem like they have lost touch with reality. Although schizophrenia is not as common as other mental disorders, the symptoms can be very disabling.

Dissociative identity disorder:

Formerly known as multiple personality disorder, this disorder is characterized by alternating between multiple identities. A person may feel like one or more voices are trying to take control in their head. Often these identities may have unique names, characteristics, mannerisms and voices. People with DID will experience gaps in memory of every day events, personal information and trauma. Women are more likely to be diagnosed, as they more frequently present with acute dissociative symptoms. Men are more likely to deny symptoms and trauma histories, and commonly exhibit more violent behavior, rather than amnesia or fugue states. This can lead to elevated false negative diagnosis.

“What is Schizoaffective Disorder?”

Schizoaffective disorder is a chronic mental health condition characterized primarily by symptoms of schizophrenia, such as hallucinations or delusions, and symptoms of a mood disorder, such as mania and depression.

Borderline Personality Disorder (BPD):

Borderline personality disorder is a mental illness marked by an ongoing pattern of varying moods, self-image, and behavior. These symptoms often result in impulsive actions and problems in relationships. People with borderline personality disorder may experience intense episodes of anger, depression, and anxiety that can last from a few hours to days.

Bipolar Disorder:

Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in mood, energy, activity levels, and the ability to carry out day-to-day tasks.

There are four basic types of bipolar disorder; all of them involve clear changes in mood, energy, and activity levels. These moods range from periods of extremely “up,” elated, and energized behavior (known as manic episodes) to very sad, “down,” or hopeless periods (known as depressive episodes). Less severe manic periods are known as hypomanic episodes.

Listener Questions this week.

Charles in Sarasota, FL asks, “ How often should you see a doctor?”

Nils dives into this question. You should try to have a yearly checkup but don’t go looking for problems that are not there. If you take too many screening tests you will eventually find something positive. It is normal to have abnormal findings. What happens if you find an abnormal finding? Well, you will have to take more tests, which can cause more stress.

Sarah in Toronto, CA asks, “What movie closest represents being a cop and being a doctor?”

Nils shows his age with this question as he answers shows from the 70s. Nils feels that police is like the TV show is Barney Miller. Matt states the closest to police work is End of Watch and Super Troopers. The officer’s characteristics are very common in how officers act and interact. Those are two far ends of police movies but have some common traits seen in policing.

Nils asks if there are cop movies that are not accurate and Matt talks about the Departed and Training Day being good movies but very extreme and far-fetched.

Law in Order has some accuracy but is very far fetched. Detectives do not solve crimes in an hour and not many people actually run from police. The psychiatrists in the show are often portrayed as bad people.

Brooklyn Nine-Nine shows some similarities to police work. Employees are in the precinct hanging out and complaining about the drama that is happening.

So what do you think is closet for doctors? Scrubs is one of the most accurate ones. It is tedious and they show them doing random little things like drawing blood. Most of the doctor shows is a doctor fighting the systems or doctors finding some rare disease, which is just not common.

ER was one of the most accurate shows for the time. The setting of the actual ER and the hierarchy of resident vs. attending were very accurate.

“Engaging people with first episode psychosis” and “What happens if an officer is mentally ill?”

Nils and Matt start this episode off answering questions around law enforcement and if they are prohibited from having a firearm if they have a mental illness. We then listen in to Melissa Garzon MD and Caroline Bonham MD as they talk about first episode psychosis on the CIT ECHO.

Lisa wrote us on Facebook after reading about Miami-Dade’s new unit around firearms and people living with a mental illness. You can see that article here.

She asked. “I have question what happen if the cop has mental illness or PTSD? Another question about HIPPA: they have to be compliment to HIPPA.”

Officers are normal citizens and the laws around prohibited people with firearms will apply to law enforcement as well. If an officer is adjudicated mentally ill or if they are convicted of a felony they would not be allowed to possess a firearm.

Having a mental illness alone is not a reason to be prohibited from a firearm. Seeking treatment for an illness will not prevent you or make you give up any rights.

Law Enforcement is not bound by HIPAA. HIPAA is the Health Insurance Portability Accountability Act and it apply to organizations that are billing for mental health care. Law Enforcement does not bill for insurance but they do respond to medical related calls.

Is there a difference between Crisis Intervention Training vs. Crisis Intervention Team (CIT) training?

Matt Tinney and Ben Melendrez discuss training around crisis intervention. What do you use in your program or training?

How do you view your training? Is your training part of an overall program or is your training a stand-alone training?

Remember a true CIT program is more than training. A big component is looking for resources to be part of the team and to help those in crisis. Training may be a critical part of your CIT program but it should not be the main part. Include your community experts in developing your training.

What is your favorite shift for work and training?

Ben and Matt both talk about 10-hour shifts and if your training should reflect the work schedule of the students. Should you put on classes that are 10-hour days if your students are working 10-hour days? Do you currently provide training for all shift and hours?

Officers make bad audience to outside presenters.

Prepare your outside presenters on what to expect when training law enforcement. You may need to work on your common language to not lose your audience. Officers as students sometimes appear distant as students but they are paying attention.

How do you check your presenters for bias towards law enforcement?

Check in with presenters beforehand. Get to know them and what their agendas may be. Look over their presentation and curriculum to see what the objects are and discuss what will work with law enforcement vs. what will alienate them.

Should students have laptops and phones out during class?

Ben speaks about why students should be allowed with their electronics. This generation is used to electronics and in college settings most students have laptops open. Officers are used to having to work on their computer while listening to the radio and another outside stimulus.

How to check if you should take a specific class or if it is a good class.

  • Ask for the curriculum
  • Ask to see the latest reviews
  • Ask others who have taken the class if they like it

Dr. Nicole Duranceaux is a psychologist with the New Mexico VA Health Care System who specializes in post-traumatic stress disorderand co-occurring substance use disorders. During her talk Dr. Duranceaux reviews some misinformation about PTSD, and provides key points about interacting with someone diagnosed with PTSD, as well as self-care tips for individuals who work in public safety.

Brian Stettin from the treatment advocacy center stopped by the CIT Knowledge Network to give a talk about the history of AOT. This is a unique law that directly impacts communities and public safety.

The Treatment Advocacy Center was founded in Arlington, Virginia, by E. Fuller Torrey, MD, in 1998. Dr. Torrey had worked for 15 years at a Washington, DC, clinic for homeless people with severe mental illness and authored Out of the Shadows: Confronting America’s Mental Illness Crisis about the criminalization of mental illness.
