In today's episode we have Lindsay Castiglione sharing her incredible perspective & knowledge with us about breastfeeding multiples. Although there has been great growth toward society supporting & normalizing breastfeeding, there is still a huge need for resources in the multiples community.
There are a few areas of early lactation that are vital to success with multiples, but these areas are not often covered in-depth during traditional prenatal breastfeeding classes. We discuss how so many multiples born preterm or premature, so we need a solid focus on bringing in a milk supply even if our babies are not strong, effective nursers. Often this means completely bringing in a supply with a hospital grade pump, or if babies are older, letting them practice but immediately following with a short pumping session to stimulate supply.
We talk about preterm babies (with 50% of twins born between 35-37w, triplets and even earlier) and the common issues like low blood sugar, jaundice, and constant fatigue common with this early birth. When not talked about enough prenatally, and when experienced immediately postpartum, these situations can feel like an enormous crisis. Instead, we can understand the causes, and be armed with a plan of how to manage without compromising supply in those early days.
Lindsay also shares her fascinating story as a mom of 2 sets of twins. Through a lot of trials and tribulations she was able to breastfeed both sets! Lindsay now runs the Full Hearts Collaborative, teaching classes to parents expecting multiples, combined with her private practice specializing in NICU, Exclusive Pumping, and Multiples lactation care.

Today we are joined by Karlee Vincent, founder of the Traveling Milk Truck. Karlee is a millennial mom who has co-managed technology company events for 100,000 attendees, while struggling to breastfeed in airplanes and hotels on the road. Lisa Myers is a working mom of 2 sharing with use her journey through breastfeeding. With her daughter she made it to six months of breastfeeding and supplementing with formula. Then with her son she found out he had a tongue tie and ended up getting it treated. Lisa also shares with us what it was like to return to work. From her experiences, she learned a lot about storing breastmilk and has now created a revolutionary product for storing breastmilk.
Born in Canada, Lisa spent her childhood on a 200-acre hay and cattle farm outside Spokane, Washington, where she and her brother helped out with harvesting crops, caring for the animals and other seasonal work.
Several years later, Lisa was all grown up and running political campaigns in New Jersey and serving as an assistant district attorney in Philadelphia.
Lisa got married, started a family and works for an international law firm. Like almost every mom Lisa knows, she found breastfeeding incredibly difficult for a million reasons. For her, these included working in a male-dominated sector, some long work days and travel away from her son.
She soon realized she had no way to keep her milk safely stored without awkward walks down the office hall, juggling obvious bottles of breastmilk and pump parts. And she needed a new way to travel for work: A big ol’ cooler, bottles, and ice packs simply did not fit in her big bag already full of mediation binders, exhibits, a suit, computer, and...the pump.
A typical work trip saw Lisa having to dispose of 13.5 ounces of precious breastmilk at the airport. Her market research turned up nothing that could meet a mom's basic travel, working, and pumping needs. She got down to work inventing a solution to solve the problem simply, elegantly, and affordably. She was soon able to file for a patent and begin working with an amazing team to make Ceres Chill available to any mom in search of a better way to achieve their breastfeeding goals while continuing to be a great professional success.

On this episode, we invited Keira Chojnacki to share her breastfeeding journey with her four kiddos. She has been a breastmilk donor, and also a milk donation recipient. Her journey is very unique and diverse.
Keira became a mom at 20 years old. Right after her first baby was born, the first latch was magical. She had a fairly easy breastfeeding journey with her son and was able to donate 400oz of milk when he son was just 3-4 months old.
When her son was 1 year old, she decided to get a breast reduction and continued to give her son the stored breastmilk she had saved up after that. Two years later, Keira gave birth to her second son who ended up in the NICU for an entire month. She started out exclusively pumping and was able to make enough milk for him. She ultimately weaned at 3 months though because it was too difficult to maintain pumping with her two children and everything she had going on in her life.
With her daughter, Keira really wanted to try breastfeeding again. This time around her baby wasn't getting enough milk and Keira could only pump about 1/2 ounce at a time. It was then that Keira decided to seek out donor milk and she was able to provide her daughter with donor breastmilk from the time she was 2 weeks old to 13 months!
Keira is now a mom of 4, with her current son being 5 weeks old. Her priority is to enjoy her baby since she knows this is her last. She's learned to reset her expectations and is fine with breastfeeding, exclusive pumping, or donor milk.

Families call him Dr. K., and on today's episode we have Dr. Pejman Katiraei to talk about a holistic approach to pediatrics with a focus on tongue ties. Specifically, we're discussing the domino effect they cause later in life--including failure to thrive, oral aversions/extreme picky eaters, sleep apnea, development of palate, and even what looks like ADHD and anxiety.
Dr. K graduated from his residency in 2006. From 2004-2005 he realized there has to be more to the story than what he was trained to do. He was fortunate to see a lot of complex cases, great training, but saw things that didn’t add up. One particular case was an 8-10 year old boy who had horrible colitis. Steroids didn’t work and doctors wanted to remove colon. Ultimately he knew there was more to the story than a major surgery.
Dr. K is an integrative pediatrician, with a focus on severe behavioral issues in children. He works with more than just diet, the gut, and supplements because his clinical experience has taught him that only 25% of kids got better with that approach. He was really awoken to what happens in the mouth 4 years ago.
Digestion begins in the mouth—chewing, breaking it down, swallow begins there. If you don’t know how to chew your food, how does the rest of digestion work? It’s not meant to work with unchewed food. The pancreas’ job isn’t to chew food—it’s to take already broken down food and further unpack that. Tongue tied children cannot chew food well (rotary chewing is lacking).
Can the child take a chunk of steak and chew it? If not, they have a “broken eating mechanism”. If eating is difficult or something isn’t happening properly, your brain learns those foods are dangerous. Then you have a picky eater.
Poor digestion and food intake means a lack of micronutrients, lack of fiber, changes in gut microbiome, lack of diversity. The immune system then becomes activated to these foods because they’re not more digested by the time they hit the gut. This ultimately causes a phenomenon known as “sickness behavior”—this is the brain’s response to immune activation—brain fogginess, feeling “sick”.
Once these kids are labelled with ADD, AHHD, depression, or anxiety, they lose their self-esteem and feel like they cannot function normally.
When parents know what to look for, it will become clear as day:
We shouldn’t approach this from fear or “doom and gloom”—tongue tie isn’t a damning diagnosis. How you approach this depends on how preventive you want to be. Tongue tie procedures are a surgery—risk/cost benefit.
What is it worth to prevent these issues? We must honor where the parents are at and supporting their choices and needs.
Lastly, the tongue helps us breathe—it pushes the palate and teeth out. Choking, gagging, snoring can all be related issues, and later in life we see sleep apnea or upper airway resistance syndrome.

In this episode, Jacqueline Kincer, IBCLC reviews the most pertinent evidence to date regarding COVID-19, current vaccines for COVID-19, and breastfeeding. In June 2020, WHO (World Health Organization) recommended that mothers should continue to breastfeed, and that the benefits of breastfeeding substantially outweigh the potential risks for transmission.
A study done in May took breastmilk samples from 43 mothers that were negative for the COVID-19 virus but 3 mothers tested positive for viral particles by RT-PCR from those three infants one infant tested positive but it was unknown if they were breastfed. The other two tested negative as one was breastfed and the other newborn was fed expressed breastmilk after viral RNA particles were no longer detected.
RNA detection in the breast milk is not the same as the virus for transmission of COVID-19 to occur it would require replicative and infectious virus to be able to reach target sites in the infant.
Secretory IgA (the most abundant type of antibody in breastmilk, making up 90% of breastmilk antibodies) has been detected in breastmilk of mothers with previous COVID-19 infection.
The CDC says it is unclear as to where mothers with covid can spread virus to babies in breast milk but current evidence suggests that this isn’t likely. To date there is no SARS-CoV-2 that has been isolated from breast milk and there is no documented cases of transmission of infectious virus to the infant through breast milk.
