In this episode of the NCS Podcast Hot Topics series, host Rich Choi, MD, is joined by Chiara Robba, MD, PhD, to discuss the article "The Effects of Milrinone Systemic Administration on Cerebral Perfusion as Measured with Computed Tomography in Aneurysmal Subarachnoid Hemorrhage Patients," published in Neurocritical Care.
Their conversation explores the complex, multifactorial pathophysiology of delayed cerebral ischemia following aneurysmal subarachnoid hemorrhage and the potential role of intravenous milrinone as a rescue therapy. In this episode of Hot Topics, Dr. Nicholas Morris interviews Dr. Giuseppe Citerio, professor of anesthesia and intensive care at Milano-Bicocca University, about the ORANGE study, which investigated the neurological pupil index (NPI) for prognosticating outcomes in patients with acute brain injury. The study, involving 13 centers in Europe and North America, found that repeat abnormal NPI values, including zero, were predictors of poor outcomes. The study collected data every four hours for seven days and used a sophisticated statistical model to account for longitudinal changes. Despite initial expectations, the study did not find a strong correlation between the NPI and ICP, suggesting NPI should not be used as a surrogate for ICP. Future research may focus on developing a standardized response to low NPI values.

Electroencephalography (EEG) is an important and relatively inexpensive tool that allows intensivists to monitor cerebral activity of critically ill patients in real time. Seizure detection in patients with and without acute brain injury is the primary reason to obtain an EEG in the Intensive Care Unit (ICU); and in response to the increased demand for EEG, advances in quantitative EEG (qEEG) created an approach to review large amounts of data instantly. Today, rapid response EEG is now available to reduce the time to detect electrographic seizures in limited-resource settings. In this week's MasterClass, our hosts Dr Jon Rosenberg and Dr Stephan Mayer are joined by Dr Lawerence Hirsch, Co-Director of the Critical Care EEG Monitoring Program at Yale School of Medicine to discuss the evolution, value, and impact of brain monitoring with EEG in the critically ill.
The purpose of informed consent, as it exists now, is primarily to maintain patient autonomy, but additional benefits include the protection of patients, avoidance of fraud, and promotion of rational decisions, amongst others. It is now considered both an ethical and legal obligation which should, at a minimum, always include: 1) the nature of the procedure, 2) the risks and benefits of the procedure, 3) reasonable alternatives, and 4) risks and benefits of alternatives, with an assessment of comprehension of these elements (1). It is unknown whether decompressive craniectomy improves clinical outcome for people with spontaneous severe deep intracerebral haemorrhage. The SWITCH trial aimed to assess whether decompressive craniectomy plus best medical treatment in these patients improves outcome at 6 months compared to best medical treatment alone. The SWITCH Trial has come to an end and the findings published in The Lancet and in this week's HOT TOPICS episode, Dr. Nicholas Morris is joined by two of the lead authors, Prof. Jürgen Beck, MD and Prof. Urs Fischer, MD to discuss the trial.
Patients with acute intracerebral hemorrhage who are receiving factor Xa inhibitors have a risk of hematoma expansion. The effect of andexanet alfa, an agent that reverses the effects of factor Xa inhibitors, on hematoma volume expansion had not been well studied, until the ANNEXA-I Trial. As clinicians working with patients who have sustained an acute brain injury, we are naturally focused on caring for the injury itself. Until recent years, the clinical severity of brain injury was regarded as the main driver of prognosis and outcomes. However, we are beginning to understand how non-clinical factors, including the social determinants of health (SDOH), contribute to differential risk for all brain injuries including hemorrhagic stroke, and continue to impact our patients’ recovery even after injury. In this week's episode Dr Lauren Koffman is joined by Dr Nirupama Yechoor and Dr Rachel Forman to discuss how understanding the complex interplay between clinical and social determinants of health is crucial for clinicians caring for hemorrhagic stroke survivors, not only for recovery and secondary prevention, but also to effectively counsel families on primary prevention. Lauren is also joined by Mike Foster, a member of Yale's Stroke Patient Advisory Board who talks about his experience as a stroke survivor.

In May 2014 Elisabeth Beraquit fell through the faulty doors of an elevator into an empty shaft, landing 30 feet below on top of the elevator car. Point-of-care ultrasound (POCUS) is increasingly recognized as an essential skill for critical care providers, intended to enhance clinical assessments to improve patient care. Beyond its adaptation for procedural guidance, bedside ultrasound assessments can provide critical diagnostic information in a timely manner to guide real-time decision making for the acutely ill patient. An accumulating body of evidence has shown that POCUS can improve clinical outcomes and lower medical costs.
Over the past decade, there has been widespread adaptation of bedside ultrasonography by various critical care specialties, with respective governing societies issuing guidelines and position statements encouraging the use of POCUS. However, the field of neurocritical care has yet to fully embrace this innovative field, and neurocritical care training programs vary in their efforts to provide accessible, well-structured educational programs for trainees.

Following their debate on approaches to early phase management of refractory status epilepticus (RSE), Salia Farrokh, PharmD is joined by Caroline Der Nigoghossian, Neurocritical Care PharmD, Renad Abu-Sawwa, PharmD, and Gabe Fontaine, PharmD to explore the benefits and harms/risks of early ketamine administration in refractory status epilepticus, as well as the emerging literature for the use of ketamine in pediatric neurocritical care.

Aneurysmal subarachnoid hemorrhage (SAH) may be complicated by delayed cerebral ischemia, which is a major cause of unfavorable clinical outcome and death in SAH-patients. Delayed cerebral ischemia is presumably related to the development of vasospasm triggered by the presence of blood in the basal cisterns. To date, oral application of the calcium antagonist nimodipine is the only prophylactic treatment for vasospasm recognized under international guidelines.In retrospective trials lumbar drainage of cerebrospinal fluid has been shown to be a safe and feasible measure to remove the blood from the basal cisterns and decrease the incidence of delayed cerebral ischemia and vasospasm in the respective study populations. However, the efficacy of lumbar drainage had not been evaluated prospectively in a randomized controlled trial until Dr Stefan Wolf and his colleagues at Charité – Universitätsmedizin in Berlin conducted their study. Dr Wolf joins Dr Nick Morris to discuss the study's results.

Multiple studies demonstrate that fever/elevated temperature is associated with poor outcomes in patients with vascular brain injury; however, there are no conclusive studies that demonstrate that fever prevention/controlled normothermia is associated with better outcomes. The primary objective of the INTREPID (Impact of Fever Prevention in Brain-Injured Patients) trial was to test the hypothesis that fever prevention is superior to standard temperature management in patients with acute vascular brain injury. Dr David Greer, Professor and Chair of the Department of Neurology at Boston University School of Medicine, joins Dr Nick Morris to discuss the results from the study, which have just been released.
Several measures of blood pressure (BP) control have been used including time to reach target BP, maintaining BP within target range, reducing BP fluctuation, achieving goals with monotherapy and avoiding multiple anti-hypertensive agents, achieving goals with minimum dose adjustments, avoiding discontinuation due to side effects, and preventing hypotension. There are multiple methods of quantifying these measures. The clinical relevance of various measures particularly relationship with acute kidney injury, and death or disability in patients with acute stroke or traumatic brain injury is not well understood. A better understanding of these measures is essential to identify the ideal therapeutic agent and for comparison of effectiveness between various antihypertensive agents. Dr Nick Morris is joined by Dr Ali Seifi, Director of Neuro ICU at University of Texas Health whose session at the Meeting focused on the need to review existing data to evaluate the relationship between various measures of BP control and acute kidney injury and death or disability in stroke patients.

Acute ischemic stroke is a major burden for healthcare providers around the globe. This presentation will explore recent updates in the medical literature related to acute ischemic stroke in a dialogue format. Specifically, the speakers will review the place of tenecteplase as a choice of fibrinolytic for stroke including current efficacy and safety data. In addition, the management of hemorrhagic conversion and selection of antiplatelet medications for secondary prevention of stroke will be reviewed.

It is unclear what threshold for early neurologic deterioration or improvement following intracerebral hemorrhage may predict long-term outcomes for these patients. Using data from the PREDICT and VISTA cohorts, Dr. Yogendrakumar and his team sought to evaluate what change in NIHSS is sufficient to predict long-term disability in patients with ICH. Drs.
