This study of 801 multiple myeloma patients found that atrial fibrillation occurred in 5.5% within 90 days after autologous stem cell transplantation—nearly double previously reported rates—indicating disease‑specific risks. AF strongly predicted mortality from non‑relapse causes and may serve as a marker of physiological vulnerability, emphasizing the value of expanded pre‑transplant cardiac assessment and monitoring.
Polygenic risk scores showed associations with several cardiometabolic outcomes in breast cancer survivors but did not improve prediction beyond traditional clinical factors. Their performance varied across ancestry groups and was weakened in patients exposed to cardiotoxic therapies, highlighting limits of current genetic risk tools.

This study analyzes over 2,600 transthyretin V142I variant carriers in the Million Veteran Program and finds significantly elevated risks of heart failure, cardiomyopathy, arrhythmias, neuropathy, and musculoskeletal complications compared with matched controls. Researchers used genome‑wide CRISPR/Cas9 loss‑of‑function screens in cardiomyocytes to identify RARA and SPNS1 as key modulators of doxorubicin cardiotoxicity, highlighting pathways that influence both injury susceptibility and intracellular drug handling. Activation of RARA reduced toxicity without impairing anticancer effects, while SPNS1 loss caused harmful drug accumulation, offering new mechanistic insights and potential therapeutic targets.

Left atrial dissection is a rare, but potentially serious, complication that most commonly arises following mitral valve surgeries. In this report, we describe an unusual case of left atrial dissection that occurred after multi-valve surgical replacement in a patient with hypertrophic cardiomyopathy. While permanent pacemaker placement was required due to recurrent episodes of complete heart block, the patient was otherwise managed safely using a conservative approach without surgical re-intervention.

Antiphospholipid syndrome (APS) is known for thrombotic events and pregnancy complications, but its association with myocarditis is rare and poorly understood. In this case, a 44-year-old female presented with acute chest tightness, elevated cardiac biomarkers, and severe anemia. Diagnostic workup revealed ST-segment changes on ECG, preserved left ventricular function on echocardiography, and myocardial inflammation on cardiac MRI, suggestive of acute myocarditis. Positive antiphospholipid antibodies confirmed APS. She was managed with close multidisciplinary follow up. This case highlights the role of cardiac MRI in diagnosing myocarditis early, guiding management strategies, and the need for heightened clinical suspicion in APS patients presenting with cardiac symptoms.

Intravenous leiomyomatosis (IVL) is a rare benign uterine growth that extends into the venous system. This case describes a 48-year-old female who presented with 1 month of chest pain and dyspnea on exertion. She was found to have a right atrial mass that transiently crossed into the right ventricle with associated moderate tricuspid regurgitation. Computed tomography revealed a uterine mass with contiguous intravascular extension through the inferior vena cava terminating in the right atrium. This case of symptomatic tricuspid valve obstruction due to IVL illustrates the importance of maintaining preload. Chest and abdominal imaging may be necessary to identify the source of the mass and ensure appropriate surgical planning.

Large lead-associated vegetations carry a significant risk of pulmonary embolism with hemodynamic instability when transvenous lead extraction is attempted. In this case, a 54-year-old male came to the hospital with fever and dyspnea. He was found to have MSSA bacteremia and a large RV lead vegetation on echo. He had multiple co-morbidities and was a high-risk surgical candidate. He underwent successful vegetation aspiration using the Flowtriever system, followed by endovascular lead extraction.

Pulmonary arterial hypertension (PAH) has a diverse range of etiologies and carries significant morbidity and mortality. Here, a 45-year-old female presented with shortness of breath and was found to be in acute right heart failure due to PAH. Exam demonstrated features of scurvy. She initially required vasopressors, supplemental oxygen therapy, and PAH therapies but rapidly improved with vitamin C supplementation.
Julien Dreyfus, MD, PhD, JACC: Case Reports Associate Editor, is joined by author Khalid Shakfeh, MD, discussing this study from Shakfeh et al presented at ACC.25 and published in JACC: Case Reports. A 56 year old man with chronic chest pain syndrome presented with sudden onset of weakness. Evaluation for a stroke revealed a large mass in the right atrium measuring approximately 2.2 x 2.0 cm immediately above the tricuspid valve annulus and adjacent to the atrio-ventricular groove on TTE. Cardiac MRI ruled out intracardiac mass, but rather demonstrated an RCA fusiform aneurysm. Coronary CTA characterized this as two fusiform RCA aneurysms. A LHC confirmed this. Giant coronary aneurysms appearing as a large space-occupying intracavitary cardiac mass are rarely diagnosed. Escalating multimodal imaging is essential for accurate diagnosis and surgical planning. Kawasaki disease can have a late presentation and should be considered in the differential diagnosis of coronary aneurysms. A multimodal imaging approach is essential for accurate diagnosis and management of giant coronary aneurysms.
Andrea Scotti, MD, JACC: Case Reports Deputy Editor, is joined by authors Richard Carrick, MD, PhD and Drew Bidmead, BS discussing this study from Carrick et al presented at ACC.25 and published in JACC: Case Reports. Left atrial dissection is a rare, but potentially serious, complication that most commonly arises following mitral valve surgeries. In this report, we describe an unusual case of left atrial dissection that occurred after multi-valve surgical replacement in a patient with hypertrophic cardiomyopathy. While permanent pacemaker placement was required due to recurrent episodes of complete heart block, the patient was otherwise managed safely using a conservative approach without surgical re-intervention.
Mary Norine Walsh, MD, MACC, JACC: Case Reports Deputy Editor, is joined by author Dena Hayes, MD, discussing this study from Hayes et al presented at ACC.25 and published in JACC: Case Reports. The diagnosis of cardiac sarcoidosis (CS) is often challenging, particularly in atypical cases. The authors describe a case of a previously healthy 33-year-old woman who was found to have a biatrial mass and evidence of a diffuse inflammatory or neoplastic process on multimodality imaging. Percutaneous biopsy of the cardiac mass was performed and histopathology revealed granulomas consistent with CS. This case adds to the growing number of reports of CS presenting as an intracardiac mass.

Dr. Sanjeev Bhattacharyya is an associate editor of JACC: Case Reports. He is also a consultant cardiologist & honorary senior lecturer at University College London Hospital and Barts Heart Centre, St Bartholomew's Hospital, London, UK. His main sub-specialty interests are advanced heart failure, valvular heart disease and advanced echocardiography. His research interests are focused on the application of advanced echocardiographic techniques and other modalities to better characterise and identify the optimal timing of intervention in heart valve disease. Dr. Bhattacharyya was chair of the research committee and is an active member of the education committee of the British Society of Echocardiography. He is a member of the Royal Society of Medicine cardiology council.
Dr. David Fischman is Professor of Medicine at the Sidney Kimmel Medical College of Thomas Jefferson University and is the Co-Director of the Cardiac Catheterization Laboratory and Director of the Interventional Cardiology Fellowship Program at Thomas Jefferson University Hospital. In addition to direct patient care, he is actively involved in clinical research and has been the principal investigator of many clinical trials of new and innovative technologies in the treatment of patients with heart disease. He has received countless accolades for patient care and clinical research and has been the principal author of numerous manuscripts including the landmark trial on coronary stenting, the STRESS Trial. He serves as a peer reviewer for numerous journals and is active on social media. Board Certified in Interventional Cardiology he is a Fellow of the American College of Cardiology, American College of Physicians and the Society for Cardiovascular Angiography & Interventions.

Dr. Mladen Vidovich is Professor of Medicine at the University of Illinois at Chicago, He is Chief of Cardiology at the Jesse Brown VA Medical Center in Chicago. He is an interventional cardiologist with main interests in radial coronary angiography and intervention, intracoronary imaging (OCT, IVUS, NIRS) and quality improvement. He is Governor American College of Cardiology for Department of Veterans Affairs and Chair of the ACC Federal Section. Dr. Vidovich is member of the standing Cardiovascular Committee at the National Quality Forum (NQF). He has published numerous manuscripts and two books on Radial Angiography and Catheterization. He has designed a dedicated radial ventriculography catheter that is currently in clinical use.
Dr Eric Bates is professor of internal medicine and cardiology at the University of Michigan. He is also the deputy editor for JACC Case Reports. Dr Bates graduated from Princeton University and the University of Michigan Medical School. He trained in internal medicine and cardiology at the University of Michigan Hospitals. His major clinical interests include acute myocardial infarction, acute coronary syndromes, and coronary artery disease. His research efforts have focused on fibrinolytic and catheter-based reperfusion therapy for acute myocardial infarction and cardiogenic shock, antiplatelet and anticoagulant therapy for coronary thrombosis, and coronary artery revascularization.
Dr Bates has served on the steering committees for several important international multicenter randomized clinical trials. He serves on the editorial boards of the American Journal of Cardiology, the European Heart Journal, the Journal of the American College of Cardiology, JACC-Cardiovascular Interventions, the American Heart Journal, Circulation, Circulation-Cardiovascular Interventions, Catheterization and Cardiovascular Interventions, Cardiology Review, and Cardiology News, and is an associate editor for ACP Journal Club.
Dr Bates was Vice-chair for the 2011 ACCF/AHA Guideline for Percutaneous Coronary Intervention Writing Committee. He is a former member of the ACC Board of Governors and Board of Trustees, the AHA Committee on Scientific Sessions Program, and the ABIM Interventional Cardiology test committee.

Dr. Sofian Johar is Associate Editor for JACC: Case Reports and Consultant Cardiologist in Al-Muhtadee Billah Hospital in Tutong, Brunei (2016). He specialized in electrophysiology and devices in Royal Brompton Hospital Sydney Street, London, UK and Kings College Hospital Denmark Hill, London, UK.He has BA(Hons) in Neuroscience Class: II.1 from the University of Cambridge (1996) and completed his MB BChir from the University of Cambridge. In 2001, he obtained MRCP (UK) Royal College of Physicians London and in 2007, he received his PhD in Cardiovascular Medicine from the Kings College London, UK. Complementing his academic background, Sofian also has a Certificate of Completion of Training in Cardiology, UK (2012) and is a Fellow of the Royal College of Surgeons and Physicians of Glasgow (2014).Sofian was very active and has made many achievements throughout his career. Some of these include the BJCA-Cordis International Research Awards – Finalist (2005), British Society of Echocardiography Accreditation in TTE (2009) and EHRA Accreditation in Cardiac Devices Level 2 (2015). He has produced several publications and contributed chapters in books that were published internationally. He is also an advisor to the RIPAS Hospital Resuscitation Committee.Through this podcast, Dr. Johar takes us through his career pathway, explains why he decided to move back to Brunei and he gives advice to young fellows.
