Make sure you don't miss out on a ticket for this year's IEMTA ASM in RCSI's Albert Theatre, April 27th.

This time is always busy for our trainees with interview preparation so Leah took the opportunity to sit down with Dr Andrew Neill, consultant in Emergency and Intensive Care Medicine in the Mater Hospital to discuss the controversial topic of REBOA.
They discuss the UK-REBOA trial and the evidence behind the technique and why we should be cautious about it's used in the Irish trauma system in particular.

Ever wondered how to pursue an EM fellowship after finishing your SpR training? Is it worth it, and if so what are the benefits of doing a fellowship? We’re very excited to welcome back a familiar voice to our bonus episode this week to answer these questions and more!
TCR alumnus Dr Deirdre Breslin joined Geriatric Emergency Medicine (GEM) lead Dr Conor O’Gara to chat all things fellowships, frailty and finding out what to do with your interest in GEM.
Dr Breslin is an EM consultant in the Mater Misericordiae University Hospital (MMUH). She has a special interest in GEM, having worked as the TCR production lead in GEM and completed a GEM fellowship prior to taking up her consultant post. Her insights and tips make this both an interesting and important discussion.

With over 3900 neck of femur fractures (NOFs) reported in Ireland in 2021, these fractures represent a significant burden of morbidity and mortality, particularly for our older patients. Early recognition coupled with meticulous management, both in the ED and beyond, is instrumental in ensuring optimal outcomes.
Orla Kelly puts TCR newcomers Conor O’Gara and Jane Cassidy through their paces with today’s case before we sit down with our Adult in the Room - Dr Mustafa Mehmood, Consultant in Emergency Medicine in St James’s Hospital, and architect of the department’s ‘Code Hip 240’ protocol.
As always, don’t forget to follow us on socials on instagram, twitter and facebook to keep updated with all things TCR. Coffee in hand? Right then, let’s get to it!
Ever wondered how to make a referral to The Irish Paediatric Acute Transport Service (IPATS) when working in a tertiary hospital? What is the criteria for transferring a child, when can it be done by IPATS and when should the transfer be organised by the referring hospital? What do you need to have prepared before sending a child and how should you prepare to expect the unexpected?
We thought we’d start off the new year as we mean to go on with a paeds bonus episode on the first day of 2024! In this episode EM consultant Dr Conor Davis talks all things paediatric transfers and answers the above questions and more. And seeing as we are thecase.report, we might have even managed to give an EM consultant their own case to tackle!

We're in the Paediatric section of the TCR Emergency Department where Mo, Stephen and Rachel are concerned about a five day old baby. GBS sepsis is no joke, and our team bring us through the management of one of the most daunting cohorts of all: the septic neonate.
Our Adult in the Room Dr Rasha Sawaya reviews our work, and also talks about feeding issues, what to do when your initial management doesn't work, and gives some tips and tricks for ED NCHDs who are graduating to more senior roles.
Join the conversation on X, Instagram, and Facebook. We've got some great infographics and notes to compliment your learning that you can find at thecase.report.
This will be our last episode of 2023, but we've got an incredible PEM bonus episode coming early in the new year.
And as always:
"May your coffee be strong, and your rounds grand."
Time to enter the Echo Chamber! Dr Mai Nguyen joins Callum to discuss eFAST in Trauma in this bonus episode. Following on from a very detailed discussion of all things trauma in lasts week's main episode, we're diving into the specifics of using POCUS in trauma, and importantly how to do it effectively and efficiently.
Dr Nguyen is a Consultant in Emergency Medicine at University Hospital Limerick where she is the Director of Ultrasound Training for Emergency Medicine, an EGLS instructor and a Level 1 Ultrasound Trainer. In short, the perfect guest to run through all things eFAST. If you didn’t want to build your POCUS skills before, you sure will after this episode!
Listen along with the show notes and get involved in the conversation on Twitter and Instagram.
From the seatbelt sign and it’s association with intra-abdominal pathology to the “Hateful Eight” to the electrolyte breakdown in red cells, Callum, Eoghan and our AITR Dr Sean Croughan are doing a deep dive of managing the unstable trauma patient in this month's episode of thecase.report. Oh, and don't forget about distracting injuries!
Following on from our second South African Special Episode last month, we’re back in Ireland in a rural ED. Eoghan, the only Reg working in ED this evening gets a pre-alert about a female involved in a single vehicle road traffic collision. Follow the case from zero-point survey to a detailed debrief with the show notes online.
As always, don’t forget to follow us on socials on instagram, twitter and facebook to keep updated with all things TCR. Right then, let’s get to it!
Improvisation is the buzz word in this thrilling second bonus episode centred on South Africa (you can find the first here). We were delighted to sit down with TCR veteran Dr Jimmy Lee, Consultant in Emergency Medicine in University Hospital Galway. With a passion for Global Health & Humanitarian Medicine, Dr Lee recalls his unforgettable experience completing his Trauma and Resuscitation fellowship in Chris Hani Baragwanath Hospital in Soweto, South Africa.
With over 2,000 inpatient beds and a dedicated trauma unit, Barra hospital ranks as the 5th largest hospital in the world. Coupled with its location in an area with unfortunately high rates of violent crime, and variable reliability and access to resources, working in the trauma unit poses unique challenges not often faced here in Irish EDs.
Dr Lee’s experience and pearls of wisdom are not to be missed. Right then, let’s get to it. Get ready to pivot, and pivot fast!
In our first bonus of the season, Leah sits down with Dr Jamie Kearns and peppers him with questions. Can he withstand the onslaught?
Jamie is a Consultant in Sports and Exercise Medicine at UPMC Sports Surgery Clinic and Head of Medical at Munster Rugby. He was awarded FFSEM (Irl) in 2017 and was elected to the Board of the FSEM in 2018.

Don't miss this as Leah put's the controversies to Prof Morris and gets some well needed answers.

She has multiple publications in the world of female athletic medicine and concussion.

Thank you all for joining us for our very first episode of TheCase.Report! We’re delighted to have you with us as we set off on this very exciting journey! This month we’ll be looking at (what else!) COVID-19.
For our first segment, Mohammed, Karl and Safras will be going through a case and discussing the issues it raises. Our adult in the room this month Dr. Cian McDermott will check our work after!
Then we’ll be joined by Prof. Dara Byrne, who’ll help us think about simulation, and it’s varied uses during this pandemic.
Dr. Una Kennedy will talk to us about wellness in EM and will share some great tips for looking after ourselves and each other.
Sources & resources will all be linked in the show notes, in-line for the case material, and Sim & Wellness resources all the way at the end!
Let’s get to it!
Everything is changing. These days it feels like every day brings a new unexpected challenge. But it’s exactly in times like these, when everything is in flux, that we need to utilise, expand on and showcase the dynamism of our specialty.
From IEMTA (The Irish Emergency Medicine Trainees’ Association), comes TheCase.Report, a new Emergency Medicine podcast focussing on clinical challenges, and putting a spotlight on issues important to the specialty, and to all of us as EM providers.
We’re kicking things off with, you guessed it, an episode all about covid-19 on May 11th. As if you don’t hear enough about it…
But we’ll make it different, we’ll make it fun, and we’ll make it worth your while!
