What is CCI?
Craniocervical instability or CCI is when the upper neck levels of the spine are unstable (1). This is usually C1-C2, but can also be C0-C1. There are a number of strong ligaments that hold this area together which can be injured or loose and there are a slew of measurements used to determine if CCI is present. Common symptoms can include headaches, dizziness, imbalance, upper neck pain, visual disturbances, rapid heart rate, and various pains elsewhere, just to name a few...

Craniocervical instability is a very misunderstood concept because it’s been made so complex with different measurements and aggressive treatments. Hence, I’ll be writing a series of blogs to try to help patients understand various concepts. Today’s entry is about how CCI measurements relate to instability.
What is Cranial Instability?
Craniocervical instability has many names including CCI, AAI (atlanto-axial instability), CCJ instability (cranciocervical junction), etc…The basic concept is that the ligaments that hold the head on to the spine have become loose and there is too much motion of the head on the neck (1). Note that CCI stands for Craniocervical INSTABILITY, meaning that this is a movement-based concept. This extra movement can cause everything from pressure on the lower brain, brainstem, and spinal cord to irritated nerves, beat up upper neck joints, and damaged tendons.....

I’m getting older, which is the “aging” part of the title. In addition, my family has a history of heart disease, so I wanted to start getting proactive about my ticker. This began a journey that begins with a new Apple Watch for Christmas and a discovery that rocked my world. In addition, let’s dive into the newly emerging world of smart device health monitoring and how that could eventually save many lives.
My Family Heart History and Smart Health Devices
I drew my heart genes from a very bad deck of cards. My dad had cardiac bypass surgery in his 60s and his father perished from a sudden death heart attack. On my mother’s side of the family are many heart stents and then, oh, by the way, her dad also perished from another sudden-death heart attack in his 60s. I guess the good news is that I did hear from a friend in medical school that he had a worse cardiac history. All of the men in his family had died in their 40s and 50s from a heart attack.
So what’s the good news about my bad genes? We’re entering a brave new world of constant health monitoring that will change everything. In the next 20 years, we’ll see a range of devices that exist in the background of our lives that are always looking for small problems while...

I get asked all the time by patients how best to control inflammation. I have a specific protocol I personally use to control my old man’s tendency for that dangerous slow burn that we all experience as we age, so I want to share that with you. Let’s dig in.
What You Shouldn’t Use
The poet laureate William Carlos Williams, a physician, once wrote: “I’ve seen death come on slow as rust, or as quickly and suddenly as a doorknob come loose in the hand…” I may be paraphrasing, but the point here is that one of those doorknob moments is a sudden-death heart attack or stroke. Regrettably, a sure-fire way to achieve that sort of surprise death is by taking NSAID drugs.
NSAIDs are the common anti-inflammatory drugs you see in the grocery store or by prescription:
Many of my patients take these drugs to function. However, they all have serious cardiac effects that can double to quadruple your chances of dropping dead and being that guy or gal with the loose doorknob in the hand (1). Hence, how does this aging athlete control inflammation while avoiding NSAIDs?

It’s new year’s resolution time and that means that the health clubs will be packed for the next few months. Many people these days find a personal trainer at their new health club or through a friend. The great news is that a trainer can push you much harder than you would usually push yourself, so it’s often easier to see results with a personal trainer than without. However, trainers can also mess people up with injuries or pain, so what can you do?
My Experience with Personal Trainers
Many years ago I began working out with a personal trainer. The great news was that the synergy of him pushing me and me wanting to impress was a great way to make me work twice as hard with the trainer as I ever would alone. However, all of that effort soon lead to several new back and neck injuries, so it always seemed to be two steps forward and one back. After a while, like many people, I gave up on the trainer as I was tired of getting injured.
A Core Stability Checklist
For anyone who has any type of chronic neck or back issue, one of the problems is that almost always there is too little strength in the core muscles. These are the muscles that help stabilize the spine from the neck, through the upper back, and into the lower back.

First, as a late baby boomer, I have to ask, what’s up with this butt implant thing? I see many celebrities right now with really small bodies and really big butts. Why is this a thing? If you’re wondering what your doctor is doing asking about celebrity butts, that’s a great segway into this week’s Aging Athlete series. Let’s dive into why keeping your butt muscles big as you age will help protect your hips and back. Let’s talk about how how losing your butt is just a sign of aging, not a must!
The Old Flat Butt
First, I feel like a 3rd grader even using the term “butt”, but hey we’re all adults, right? Let’s all admit it, we all notice that our older friends just don’t have that same butt that they did when they were younger. Is this a real thing? Yep. In fact, a research group in 2015 looked at 185 gluteal regions on MRI and found that not only was there a trend toward shrinking butt muscles with age (gluteus minimus and maximus), but that as the muscles atrophied, their tendons ended up getting frayed and torn (1). Meaning older people had flat butts with more tendon tears in their gluteal muscles.
We also know that the butt muscles are stabilizers of the hip joint and that in people with hip arthritis, these muscles don’t work as well (2). We also know that atrophy (shrinking) of the gluteal muscles happens in patients with low back pain (3,4). Hence, a strong butt is likely associated with protecting the back and hips.

One of the most problematic areas for all aging athletes is the ITB, yet most of us have no idea why this spot is always tight and that it has to do with a muscle called the vatus lateralis. I’m an aging athlete who gets orthobiologic therapies to stay active with a family history of obesity, heart disease, and type 2 diabetes. Meaning, I got dealt a bad hand in the grand poker game of genetics. Recently some groundbreaking research came out on Metformin and how it may slow aging, which coincided with my doctor wanting me to take it. So did I take it? Should I take it? Should you take it?
Metformin and Reversing the Body’s Biological Clock
I have had a love-hate relationship with Metformin ever since my doctor (yes we doctors have doctors too) said I might consider taking it for blood sugar control. Why? I pride myself on a really good diet, but I’ve had a blood sugar control problem since I was a kid. So should I take it or chuck it? This new study caught my eye.
Ther new study is the TRIIM trial which tested nine white guys who were about my age (1). They took Human Growth Hormone, Metformin, and DHEA. I had tried HGH, but thought that it whacked out my blood sugar a bit, which is why it was used here as it’s a blood sugar stabilizer. The HGH has the impact of stimulating cellular growth and the DHEA increases testosterone levels.....(read original post here).

This is my weekly “Aging Athlete Series” tip. This series is for people in their 30’s and up who want to stay active as they age. Those who want to be that guy or gal who is in their 70’s or 80’s who are still doing cool active stuff. This is my third episode: The Core Before Everything.
The Problem?
Lots of middle-aged and older people get hurt when they join a gym or cross-fit class. In my experience, this often happens because they’re not working on the core first. The core is made up of those muscles that stabilize and support the spine and trunk. Without a strong core, the extremities have a poor base of support which leads to injuries.
Here’s a simple visual to get a sense of how critical your core muscles are. How does lifting 30 pounds over your head on the shore compare to lifting the same weight while standing in the middle of a small rowboat? You might imagine that doing that while stabilizing yourself and preventing yourself from falling into the water would be tough, even risky. The difference is the intrinsic stability of the shore versus the boat. Your core is the same. Lifting weights or doing cross-fit with a weak core is also a prescription for a similar disaster.....

Man, it’s tough getting old. Your body aches more, stuff that used to just magically heal doesn’t or takes a long time, and injuries from long ago tend to talk to you. Big toe numbness – while it might not be particularly common, or even that concerning—it’s just a toe after all— when your big toe goes numb, it can certainly get your attention. And for good reason; it’s usually an early warning sign that something more significant is going on. You should be especially concerned if it’s happening often or in on-again-off-again episodes and with certain activities.
