In this episode of JCMS Author Interviews, host Dr. Kirk Barber welcomes Dr. Hélène Veillette, Division Head of Dermatology at CHU de Québec–Université Laval, to discuss the article she co-authored with Dr. Louis Deschenes. “Skin Picking Disorder: A Canadian Retrospective Study.” It features in the May-June issue of the JCMS.
Dr. Veillette shares findings from a retrospective review of 82 hospitalized patients diagnosed with skin picking disorder, highlighting the high rate of psychiatric comorbidities, challenges in classification, and the gaps in current dermatologic management. Together, she and Dr. Barber explores how dermatologists can better identify, understand, and manage this complex psychocutaneous condition.

Morphologies included excoriations, crusts, plaques, and nodules. * Diagnostic challenges: * Review of DSM-5 criteria for skin picking disorder and the ongoing debate about its classification under obsessive-compulsive–related disorders. * Treatment and management: * Discussion of first-line therapies like SSRIs and the over-the-counter option N-acetylcysteine (NAC); the importance of multidisciplinary collaboration; and why empathy and follow-up are as critical as prescriptions. * The role of dermatologists in psychodermatology:* * Dr. Veillette and Dr. Barber emphasize that dermatologists must engage with psychocutaneous conditions rather than refer them away — combining clinical skill with compassion to support these often-suffering patients.

In 2015 Canadian Geographic recognized Ray as one of Canada’s Top Explorers. In 2024, Ray was awarded the Sir Christopher Ondaatje Medal for Exploration. He has run 17,000+ km across the world’s deserts, and unsupported expeditions in some of the coldest places on the planet.

"We have such good treatments today for kids who are suffering from pediatric psoriasis and other inflammatory conditions. And honestly I think the drugs we have now, the biologics in particular, are safer than some of the drugs we used in the past, and definitely more effective."

He received his medical degree from the University of Montreal. After graduation he completed his dermatology residency training at the University of Montreal Hospital Center and Sainte-Justine Hospital Center.

On this episode of JCMS Author Interviews, we take you to Verona, Italy for a conversation with Dr Paolo Gisondi about a very important topic: The Metabolic Aspects of Psoriasis. The conversation is based around Dr Gisondi's article in the May/June 2022 edition of the Journal of Cutaneous Medicine and Surgery titled: "Non-Alcoholic Fatty Liver Disease Is Associated With Reduced Glomerular Filtration Rate in Patients With Chronic Plaque Psoriasis."
As Kirk says about this conversation, "It's fascinating to learn how Dr Gisondi changed his practice over time. He's got an expertise and interest in the metabolic area surrounding psoriasis, and really has informed the rest of us internationally. He's made a difference."
Dr Paolo Gisondi is an Associate Professor of Dermatology and Venereology and President of the Dermatology and Venereology Residence Program at the University Hospital of Verona, Italy.
"Empathy goes a long way in making the patient increase their adherence to our treatment options, our management plans."
Dr Marlene Dytoc joins Kirk to discuss her letters on why empathy is an important tool in dermatology practices. They appear in the Sept/Oct 2022 and Jan/Feb 2023 issues of the Journal of Cutaneous Medicine and Surgery. She and Kirk talk about why empathy helps improve patient outcomes, why it is compatible with efficiency, the many ways doctors can use empathy, including everything from helping patients prep ahead of time for visits, their check-in, wait time, dealing with staff and the appointment with the doctor. They also discuss the Eli-Lilly's LEAP Program (Learning Empathy and Efficiency in Practice). As Kirk points out, empathy is all about the words he says at the end of each episode: "Be good to each other."

"Think about draping with everybody, especially younger females. Think a little bit about how you are moving that drape around, whether you need more than one drape, whether you have the patient move the drape. Just thinking about the patient adjusting their own clothing, it kind of makes sense from a dignity point of view."

If dermatologists want to reach young people, they can find them on social media, especially the streaming video site TikTok. In this fascinating conversation with Kirk, Dr Shari Lipner, chair of the public education committee at the American Academy of Dermatology, discusses her analysis of dermatologists on TikTok. She argues that more Dermatologists need to be on social media sites like TikTok to reach younger people, and to counter the large amounts of misinformation about dermatology that exists out there. Dr Lipner also discusses the dangers of sponsored content and the impact that can have and advocates for greater education on responsible use of social media, both for dermatology residents and in CME.

Dr Charles Samuels is the medical director of The Centre for Sleep & Human Performance in Calgary, Alberta. He is a clinical Assistant Professor in the Faculty of Medicine at the University of Calgary. And a past President of the Canadian Sleep Society.
Dr Samuels is also currently the lead investigator in a long-term research project exploring the impact of rotating shift work on the health and performance of police officers. Italian Dermatologist, Dr Simone Garcovich, joins Kirk to discuss what he learned from his detailed, four year, 870 patient study of pediatric HS that is featured in the March-April 2022 issue of the Journal of Cutaneous Medicine and Surgery.
HS is uncommon in patients of pediatric age, and the differentiation with adult-onset disease is controversial. Treatment of pediatric hidradenitis suppurativa is scarcely standardized, and specific guidelines are lacking. As Kirk describes it, Dr. Garcovich's article "is a call to action for people to start thinking about this early."
Early diagnosis is key, as is diet, weight and many other factors they discuss, along with use of biologics and surgery, in this fascinating conversation.
Dr. Simone Garcovich is a dermatologist and faculty member at Catholic University of the Sacred Heart and Gemelli University Hospital in Rome, Italy.

Dr Mark Kirchhof is back on the podcast, joining Kirk to talk about his very topical article from the March/April Edition of the Journal of Cutaneous Medicine and Surgery entitled: "A Survey of Topical Cannabis Use in Canada."
That survey found that about a quarter of dermatology patients had used a topical cannabis product, with about 27 percent of those using creams.
Kirk and Mark discuss the high instances of self medication by dermatology patients using topical cannabis for everything from eczema, to pain and aging. They talk about the implications of the overall lack of regulation or medical trials surrounding the use of Cannabis, and the side-effects from cannabis products.
Dr Kirchhof is the Division Head of Dermatology in the Faculty of Medicine at the University of Ottawa and The Ottawa Hospital.
His co-authors on this article are Dr Megan Lim and Dr Farhan Mahmood.

A Systematic Review of Efficacy and Safety of Monotherapy and Combination Therapy With Biologic for Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis
Kirk Barber is joined by Dr Marisa Ponzo to discuss her article in the Nov/Dec issue of the Journal of Cutaneous Medicine and Surgery, a deep and thorough investigation of treatments for Stevens-Johnson Syndrome and Toxic Epidermal Necrolysis. Dr Ponzo runs a specialty clinic that reviews these cases and is responsible for the treatment of this very rare disease, at St Paul's Hospital in Vancouver, which has become a referral centre for this condition. She's also the division head of Dermatology at St. Paul's and a community dermatologist at West Dermatology.
Dr Ponzo's co-authors on this article are Dr Muskaan Sachdeva and Dr Khalad Maliyar, both from the faculty of medicine at the University of Toronto.

"Superficial Granulomatous Pyoderma Gangrenosum Involving the Face: A Case Series of Five Patients and a Review of the Literature."
Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis that presents as an inflammatory ulcerative disorder. It most often affects the lower extremities but can involve other areas of the skin. While, involvement of the face has rarely been reported. Dr Alavi and Kirk discuss five cases of unusual ulcerating and vegetating facial plaques mimicking halogenoderma and blastomycosis-like pyoderma.
Dr Alavi is a consultant dermatologist and associate professor of dermatology at the Mayo Clinic in Rochester, Minnesota.
Her co-authors are Eran Shavit, Michael Cecchini, James J. Limacher, Scott Walsh, Ashely Wentworth and Mark Denis P. Davis.

Dr John Arlette joins Kirk Barber to discuss his article, "Prevention and Management of Filler Induced Iatrogenic Stroke of the Eye," from the September/October edition of the Journal Cutaneous Medicine and Surgery.
In this fascinating and important conversation, Kirk and John discuss the rare but potentially life altering instances of blindness caused by filler injections. In the past few decades, minimally-invasive esthetic treatments and the use of injectable Hyaluronic Acid Gels and other filling agents to treat facial esthetics have increased dramatically. Although extremely rare, a filler can cause ocular and orbital ischemia by retrograde flow from the ophthalmic artery when injected in any of the anastomoses of the face. Once filler reaches the central retinal artery, blindness is inevitable, and no treatment is effective. Kirk and John discuss what to do when faced with this rare incident, and the best ways to prevent it.
Dr. Arlette is a dermatologist and clinical professor in the department of surgery, division of plastic surgery at the University of Calgary.
Dr Arlette's co-author on this article is Dr Vivian Hill, professor of ophthalmology at the University of Calgary Medical School, along with Dr Michael Ashenhurst, and Dr Kailun Jiang.

Dr Raed Alhusayen joins Kirk Barber to discuss his article, "Overview of Ultrasound Imaging
Applications in Dermatology," from the September/October edition of the Journal Cutaneous Medicine and Surgery. Currently, the most readily available technologies used by dermatologists include dermoscopy and photography. But ultrasound is emerging as a useful, non-invasive tool, which can help with early and more accurate diagnosis. In this fascinating conversation, Raed and Kirk talk about why complete visualization of lesions is critical for the accurate diagnosis and management of dermatological diseases, including Hidradenitis Suppurativa (HS), connective tissue diseases like morphia, and in malignancy cancer screenings and surgical excisions.
Dr Raed Alhusayen is assistant professor at the University of Toronto, an associate scientist at Sunnybrook Research Institute and past president of the Canadian HS Foundation.

"Theoretically you would not want to give the vaccine within two weeks of having an injection of a biologic, but that's more of a theoretical risk. I think the risk of getting COVID is far greater."

Telehealth brings both challenges and advantages for Dermatologists. To explore how to work telehealth to your advantage, Dr Kirk Barber talks with three experts in this field; Dr. Harvey Lui is Professor of Dermatology at the University of British Columbia, Dr Regine Mydlarski is Associate Professor of Dermatology at the University of Calgary, and Dr Steven Feldman is a Professor of Dermatology at Wake Forest University in North Carolina and a leading expert in the use of empathy in the successful treatment of patients.

Post-inflammatory hyperpigmentation (PIH), when the skin becomes discolored or darkened, is a common consequence for people of color receiving laser treatments. It's a major challenge for Dermatologists and one with no clear solutions. Dr Vincent Richer of Pacific Derm in Vancouver, joins Kirk to discuss the article he co-authored with Dr. Ian Wong in the Jan/Feb 2021 issue of the Journal of Cutaneous Medicine and Surgery, "Prophylaxis of Post-Inflammatory Hyperpigmentation From Energy-Based Device Treatments: A Review." As Dr Richer explains to Kirk, the article is an attempt to "look into the science behind what supports all the 'witchcraft and wizardry' that we do either before or after our treatments, to try and prevent (PIH) from coming."Dr. Vincent Richer is a medical and cosmetic Dermatologist and holds positions as Clinical Instructor and Director of Continuing Medical Education at the Department of Dermatology and Skin Science at the University of British Columbia. He is motivated by getting optimal outcomes for his medical and cosmetic patients through customized care. This includes using the power of light to treat and enhance skin health, such as ultraviolet phototherapy to treat psoriasis or eczema, lasers and injectables to selectively improve skin appearance as well as photodynamic therapy to treat precancerous lesions.

Severity measurement scales for atopic dermatitis are important in clinical trials as objective outcome measures and they are often required for government and private insurance plans. These scales are sometimes underused by clinicians for a number of reasons, including time constraints and lack familiarity. Dr Patrick Fleming talks about how there has been an increasing number of studies reporting the successful use of biologictherapy in patients with PRP, a relatively rare skin disease, however the data on the efficacy and safety is limited. His objective, along with his colleagues, was to evaluate the existing evidence for utilizing biologics, whether alone or in combination with established systemic therapies, in patients with treatment-resistant pityriasis rubra pilaris.

Dr Catherine McCuaig joins Dr Kirk Barber, Editor-in-Chief of the Journal of Cutaneous Medicine and Surgery, for a fascinating discussion about her article in the July/August edition of the Journal. It is a retrospective single-centre study of children with IBD under anti-TNF treatment between 2013 and 2016. Forty patients with CSE related to anti-TNF were referred to Dr McCuaig's pediatric dermatology clinic. A control group was randomly selected from patients receiving anti-TNF for IBD, who were referred to the dermatology clinic for other conditions unrelated to anti-TNF.
Dr. Catherine McCuaig is a pediatric dermatologist at Montreal's Ste Justine University Hospital Centre, and a clinical professor of dermatology and pediatrics at the University of Montreal.

Dr Regine Mydlarski joins Kirk Barber, Editor in Chief of the Journal of Cutaneous Medicine, for a fascinating snapshot of what Canadian Dermatology looks like today and what changes need to be made to meet the evolving needs of patients across Canada. As Chair of the Dermatology Working Group at the Royal College of Physicians and Surgeons of Canada, Dr Mydlarski led a comprehensive and objective analysis of the state of dermatology in Canada. This resulted in two articles in the JCMS, one in 2019 looking at where Dermatology has come from in Canada, and one in the May/June 2020 issue making recommendations on how Dermatology should look to the future. Kirk and Regine discuss how changes in population needs, demographics, technology, an emergence of novel therapeutics and other factors are shaping the future direction of dermatology.
Dr Regine Mydlarski is the Chair of the Specialty Committee in Dermatology for the Royal College of Physicians and Surgeons of Canada and Assistant Professor of Medicine and Section Chief of Dermatology at the Cumming School of Medicine at the University of Calgary.

They discuss Dr Lui's article, "Developing a Patient Charter for People Living With Conditions, Diseases, or Traumas Involving the Skin" in the March/April edition of the Journal of Cutaneous Medicine and Surgery. At the center of the discussion are the 8 principles for providing and receiving professional services for the skin in the healthcare setting as laid out in the Patient Charter, and why it's important that the Canadian Skin Patient Alliance Board was the driving force behind the charter.
