Drs. Ron Waksman and Spencer King continue their discussion of late-breaking trials presented at ACC.21 Virtual. This time, they focus on studies of antiplatelet regimens in patients with acute coronary syndrome and antithrombotic regimens after transcatheter aortic valve replacement. They also talk about the frequency (or lack thereof) of updates to guidelines.

Drs. Spencer King and Ron Waksman discuss whether authors who involved in developing guidelines should have conflicts of interest with industry. A recent article reported that 80% of European guidelines writers have conflicts. Does this bias guidelines or do these authors have more experience and insights that can strengthen these documents?
Dr. Paul Ridker, of Brigham and Women’s Hospital, Boston, discusses with Dr. Ron Waksman results from the RESCUE trial, which randomized patients at high atherosclerotic risk with stage 3-5 chronic kidney disease to receive ziltivekimab, a novel interleukin-6 ligand inhibitor (7.5 mg, 15 mg or 30 mg) or a placebo. The study, presented as a late-breaking trial at ACC.21 Virtual, found that ziltivekimab markedly reduced multiple biomarkers of systemic inflammation and thrombosis known to promote the atherosclerotic process as compared to placebo.
Dr. José R. González-Juanatey, of University Hospital, Santiago de Compostela, Spain, discusses with Dr. Ron Waksman results from the REALITY trial, which randomized anemic patients presenting with myocardial infarction to a liberal strategy of red blood cell transfusion or a restrictive strategy. REALITY, which was presented as a late-breaking trial at ACC.21 Virtual, found that the restrictive strategy did not achieve non-inferiority at 1 year after treatment. This was against the pre-study hypothesis, which was that the restrictive strategy would be non-inferior and less costly compared to the liberal strategy.
Dr. Matthew Price, of Scripps Clinic, La Jolla, California, discusses with Dr. Ron Waksman 1-year clinical outcomes after transcatheter left atrial appendage occlusion (LAAO) for stroke prevention in patients with atrial fibrillation (AF). This report from the National Cardiovascular Data Registry (NCDR) LAAO Registry, which was presented as a late-breaking trial at ACC.21 Virtual, found that thromboembolic events were infrequent in patients implanted with the Watchman (Boston Scientific) LAAO device. Study investigators did recommend that clinicians should incorporate risks of bleeding and death unrelated to thromboembolism in the early post-discharge period when selecting a stroke-prevention strategy for AF patients who are unable to take long-term oral anticoagulation.
Dr. Eric A. Secemsky, of Beth Israel Lahey Health, Boston, discusses with Dr. Ron Waksman initial results from the SAFE-PAD study, which was presented as a late-breaking trial at ACC.21 Virtual. The observational study found that among more than 160,000 Medicare beneficiaries, in the treatment of peripheral artery disease, paclitaxel-coated devices were non-inferior to non-drug-coated devices with respect to mortality.
Dr. Birgit Vogel, of the Icahn School of Medicine at Mount Sinai, New York, discusses with Dr. Ron Waksman a study examining sex differences in the TWILIGHT trial, which compared patients at high risk of bleeding or ischemic events who received ticagrelor alone vs. those who received ticagrelor plus aspirin after both groups received 3 months of dual antiplatelet therapy post-percutaneous coronary intervention. The study, which was presented as a late-breaking trial at ACC.21 Virtual, found that women had a higher bleeding risk than men, but men and women had similar risk of ischemic events. The higher bleeding risk in women was mostly attributable to baseline differences.
Dr. Kreton Mavromatis, of Emory University and Atlanta VA Medical Center, discusses with Dr. Ron Waksman an analysis of the ISCHEMIA trial evaluating quality of life for patients who underwent complete revascularization with those who underwent more conservative treatment. This analysis, which was presented as a late-breaking trial at ACC.21 Virtual, found that complete revascularization generally resulted in greater improvement in angina-related quality of life than incomplete revascularization, particularly in patients who began the study experiencing angina on a daily or weekly basis.
Dr. William Schuyler Jones, of the Duke Clinical Research Institute, Durham, North Carolina, discusses the ADAPTABLE trial with Dr. Ron Waksman. The study, which was presented as a late-breaking trial at ACC.21 Virtual, found no difference in a composite outcome of death, myocardial infarction or stroke between patients assigned to a low dose (81 mg) vs. a high dose (325 mg) of aspirin with a median 26-month follow-up. Of interest was that patients were recruited and randomized remotely, with no in-person study visits, which helped the investigators recruit 15,000 patients during the COVID-19-related restrictions on travel and office visits.
Dr. Federico Asch of MedStar Washington Hospital Center discusses with Dr. Ron Waksman a study comparing the ability of human and artificial intelligence (AI) echocardiography analysis to predict outcomes in acute COVID-19 patients. The study, which was presented as a late-breaking trial at ACC.21 Virtual, found that AI-based analyses of left ventricular ejection fraction and left ventricular longitudinal strain showed a significant decrease in variance and accurately predicted in-hospital and follow-up mortality and follow-up mortality.

Dr. Hyo-Soo Kim, of Seoul National University Hospital, discusses the results of the HOST-EXAM trial with Dr. Ron Waksman. The study, presented as a late-breaking trial at ACC.21 Virtual, showed that clopidogrel monotherapy in the chronic maintenance period following percutaneous coronary intervention and 6 to 18 months of dual antiplatelet therapy reduced cardiovascular risk by around 27% compared to aspirin monotherapy.
Dr. Gilles Montalescot, of Pitié-Salpêtrière Hospital in Paris, discusses the results of the ATLANTIS trial with Dr. Ron Waksman. The study, presented as a late-breaking trial at ACC.21 Virtual, showed that ultrasound renal denervation, as compared with a sham procedure, reduced blood pressure at 2 months in patients with hypertension resistant to a standardized triple-combination pill.
Dr. Rupert M. Bauersachs, of Klinikum Darmstadt and University of Mainz, Germany, discusses the results of the latest update from the VOYAGER PAD trial with Dr. Ron Waksman. The study, presented as a late-breaking trial at ACC.21 Virtual, showed that rivaroxaban significantly reduced the otherwise high total event burden for peripheral artery disease patients undergoing lower-extremity revascularization by 14%.

This trial showed no significant reduction in risk of cardiac events with a new fish oil medication, omega-3 carboxylic acids (CA) as compared to a corn-oil placebo in those patients with existing heart disease or who were at high risk of heart disease due to other medical conditions.
Drs. Ron Waksman and Pierre Amarenco discuss a subgroup analysis of the THALES study, which was presented at the American Heart Association Scientific Sessions 2020 virtual conference. This analysis showed that ticagrelor added to aspirin reduced the 30-day risk of recurrent stroke by 27% in transient ischemic attack (TIA) and minor ischemic stroke patients with ipsilateral atherosclerotic stenosis of cervicocranial vasculature.

In our latest COVID-19 Focused Discussion, Drs. Aloke Finn and Ron Waksman discuss STEMI and microthrombi in COVID-19 patients. In a paper recently published in Circulation, Dr. Finn and co-authors presented the case of a 43-year-old woman who died of COVID-19 at a hospital during the height of the initial surge of infections in Italy. This case showed the potential for microvascular thrombi to cause acute myocardial infarction in the setting of COVID-19, showing a novel mechanism by which the disease can cause cardiac injury.

In the latest of Dr. Ron Waksman’s COVID-19 Focused Discussions, Dr. Manan Pareek discusses Yale University’s COVID-19 registry. While the registry’s focus is cardiovascular variables, Yale has collected data on a number of non-cardiovascular factors and outcomes as well. Data on the first 485 patients were presented at the European Society of Cardiology (ESC) 2020 virtual congress. The data show that these patients had a high burden of cardiovascular risk factors, including diabetes, obesity, hypertension and hyperlipidemia, and that nearly half had a history of cardiovascular disease.
Drs. Ron Waksman and Spencer King discuss Dr. King’s latest monthly column for Cardiovascular Revascularization Medicine (CRM), titled “Freedom Is Not Entitlement.” In this column, which is scheduled to appear in the December issue of CRM, Dr. King says that “freedom” to not wear a mask in public during the COVID-19 pandemic actually infringes on the freedom of others to not be infected with the novel coronavirus. Drs. Ron Waksman and Renato Lopes discuss the results of a study presented as a late-breaking trial at the European Society of Cardiology 2020 virtual congress. This study examined whether medical therapy or more or an invasive strategy was better for patients with intermediate heart failure (ejection fraction of 35% to 45%) and stable ischemic heart disease. When compared with patients with ischemia but without heart failure, stable ischemic patients with heart failure saw a benefit from invasive intervention.

When a patient suffers a STEMI, should clinicians revascularize only the culprit lesion or any lesions that the patient has? This has been an ongoing debate for some time in the field. Dr. Spencer King says complete revascularization is a good goal, but it is not necessarily critical – or better for the patient – that all lesions be revascularized immediately after treating the infarct-related vessel.
