This episode of The SHEA Podcast was created with the SHEA Pediatric Epidemiologists and Antibiotic Stewards (PEAS) group specifically for Ronald McDonald House, but the principals would apply to any resident care facility. As of June 2025, the world is experiencing a significant resurgence of measles. The US has documented 1,319 cases across 40 jurisdictions, marking the largest outbreak since 2020. In Europe, there were 12,694 confirmed cases over the last 12 months of reporting – the highest since 1997. Southeast Asia is also heavily affected and, Australia faces its worst outbreak since 2019. Canada has over 3,977 cases reported nationwide – the highest number since the disease was declared to be eliminated in 1998. Together, Matthew Chater, CEO of the Ronald McDonald House in Southwestern Ontario, and Dr. Ayelet Rosenthal from Lurie Children’s Hospital of Chicago representing SHEA’s PEAS group, walk through a case study on what to do if someone infected with measles visited or stayed in a Ronald McDonald House.

Overview: Antimicrobial Stewardship is well established in many inpatient settings; however, outpatient programs are not as common despite high volume of inappropriate prescribing. Literature is lacking to help guide the best interventional strategies. Additionally, patient volumes are high and time to decision making is fast in the ambulatory and ED settings. Thus, strategies that are successful inpatient may not be feasible, successful, or sustainable in the outpatient setting. Finally, hospitals fund stewardship programs through pharmacy department resources or perhaps infection prevention or quality, but what about in the outpatient setting what are sources of funding?

Summary: With the recent availability of BioFire FilmArray Assay, there is now the ability to detect 22 gastrointestinal pathogens from a single patient sample and obtain results within an hour. However, there have been multiple recent publications that have argued it has limited utility at 72 hrs after admission and upon repeat testing. Additionally, there are questions regarding what to do with unexpected results. This has led to some institutions limiting the availability of the assay, and others providing extensive education to clinicians in the hope that it will curb ordering of the assay. This podcast will focus on the utility of the assay, its strengths and weaknesses and whether or not clinicians should have unlimited access to ordering it.

Summary: Procalcitonin has been shown in numerous trials to reduce unnecessary antibiotic use associated with sepsis and respiratory infections and is a diagnostic tool that antibiotic stewardship programs can use to improve antibiotic use. Summary: Availability of urine cultures has led to a culture of culturing. As a result, over-treatment of asymptomatic bacteriuria has become common. Additionally, especially in catheterized patients, this may be leading to an increase of labeling of “catheter associated urinary tract infections.” This episode will focus on strategies to better target patients for urine cultures, when appropriate.

Summary: Under-diagnosis of Clostridioides difficile infection leads to worse patient outcomes and increased transmission. However, with sensitive nucleic acid amplification tests, over-diagnosis of C. diff has become more common. Specifically, overdiagnosis of asymptomatic C. diff colonization could lead to increased costs, over-reporting of hospital-acquired infection rates, and further disruption of patients’ microbiomes if treated. This podcast will focus on finding the sweet spot for C. diff testing.
