Minimal Residual Disease (MRD) testing is a more sensitive way to test for remaining disease following myeloma treatment. Its use could change the way experts treat multiple myeloma now and in the future. MRD testing may help determine which patients should stay on treament or which patients are able to stop if they stay MRD negative for a long period of time. It may help prevent over or under treating patients.

A wave of blood-based tests are being researched for myeloma care, and for good reason. Patients love the idea of replacing the painful bone marrow biopsy with a blood-based test, especially if it is equally effective. In addition to being less time consuming and painful, blood-based testing could be performed more regularly to provide more information to patients and their doctors. The types of test vary. They include mass spectometry testing, blood-based MRD testing and circulating tumor cell testing. Many of these tools are still only used in research, but they are getting closer to being used in the myeloma clinic. Dr. Noa Biran of Hackensack University joins HealthTree Podcast for Multiple Myeloma to discuss a completely new approach targeting CD38, which is found on most myeloma cells. The drug called modafakusp alpha (formerly known as TAK-573) uses a new method of targeting CD38 by combining a monoclonal antibody with interferon. Early trial results are showing that this treatment option could also work for patients who relapsed after anti-CD38 monoclonal antibodies like daratumumab and isatuximab. Inteferon is a human hormone that has been previously used in myeloma care because it can both kill the myeloma cells directly and engage the immune system to fight the myeloma, but it had many intolerable side effects. In this new form combined with modafakusp alpha, the side effects are significantly reduced. This new drug is now in clinical trials being tested with common myeloma therapies like dexamethasone, daratumumab, proteasome inhibitors and immunomodulators. Learn about this new treatment option in this informative show. Having new treatment options, especially after patients have relapsed after the three most common drug classes (proteasome inhibitors, immunomodulators and anti-CD38 monoclonal antibodies) is a blessing for relapsed or refractory patients.

A new approach in myeloma immunotherapies is in the works called Engineered Toxic Bodies (ETBs). Early on, Molecular Templates partnered with Takeda Oncology to begin development on an ETB therapy for multiple myeloma targeting CD38. Currently, Molecular Templates is moving this therapy forward. ETBs work differently from monoclonal antibodies because they move deep inside the plasma cell instead of flagging the cell's surface. They also don't rely on the body's own immune system (like T cells or NK cells) to kill myeloma. Learn more about this emerging therapy from Dr. Shaji Kumar on this fascinating program.

Join Dr. Dan Vogl and Dr. Yulia Nefedova to understand more about a new oral treatment for multiple myeloma called tasquinimod. This is a treatment that targets the tumor microenvironment by targeting myeloid-derived suppressor cells (MDSCs). MDSCs are a variety of cells that expand during cancer, inflammation and infection, and can suppress T-cell responses. They can stop myeloma killing-immune cells and at the same time form new tumor blood vessels that help myeloma spread in the body. The researchers are hopeful that tasquinimod will promote anti-tumor immune activity . 

The treament is being developed by a Swedish company called Active Biotech and tasquinimod is now being tested in combination with standard myeloma therapies like lenalidomide, ixazomib and dex. 

New treatments like these could be used in patients who have relapsed after standard myeloma treatment like daratumumab and could help alter the microenvironment that is causing myeloma relapse.

Dr. Irene Ghobrial joins Myeloma Crowd Radio to share a study to identify discoveries that could potentially inform COVID-19 treatment decisions and prevention policies in the IMPACT study. Myeloma patients are known to be immunocompromised and may be at higher risk for poor outcomes. 

IMPACT is a study to identify the prevalence and unique characteristics of COVID-19 in individuals with precursor conditions to hematologic malignancies and in healthy populations. It is enrolling individuals nationwide who are enrolled in the PCROWD and PROMISE studies.

All participants enrolled in IMPACT are screened using a blood test for active SARS-CoV-2 and/or antibodies. Participants are then followed and undergo additional blood collection and testing over a one-year period. The overall goal of IMPACT is to understand the short- and long-term impact of COVID-19 on the immune system of healthy individuals and individuals with underlying immune dysregulation.

Myeloma specialists and myeloma patients alike want to know which treatment will work best before they are taken. Dr. Keith Stewart and colleagues from the Mayo Clinic Scottsdale have developed a new test using a "direct-to-drug" screening that helps determine how certain myeloma cells (with specific biomarkers and pretreatment histories) will react to different myeloma therapies. The new test is flexible enough to accommodate new drugs as they become FDA approved and is a way to avoid using drugs that won't provide much impact. This flexibility is required with the rapidly changing environment in myeloma. 

Personalized care in myeloma is especially tricky because of the differences in each myeloma patients' cells. A single patient can have an average of 5 different "types" of myeloma at diagnosis and those types can evolve over time. This is called heterogeneity. Add to that the large number of potential treatment combinations, and it becomes an art and a science to treat myeloma patients. This new test will help provide much needed guidance. Isatuxiab (Sarclisa) by Sanofi Genzyme is a new CD38-targeting monoclonal antibody that is now FDA approved to be combined with pomalidomide and dexamethasone. In the same drug class as daratumumab, isatuximab is different in its mechanism and is now being used in additional clinical trials in combinations such as isatuximab/carfilzomib/dex and isatuximab/bortezomib/dex. Antibody testing for COVID-19 is rapidly evolving on a daily basis. Dr. Stephen Russell is developing a new blood-based antibody test to identify neutralizing antibodies that can determine a person's immune defense against SARS-CoV-2. 

The project is a collaboration between Dr. Russell's company Vyriad and the pharma company Regeneron, and wants to make the tests commercially available through major labs soon. Dr. Russell notes that there are many different antibodies, some of which can neutralize the virus. And that those antibodies are the important ones that can tell if a patient will be protected against reinfection or not. He started looking into ways he could respond to the virus in February and a clinical trial quickly opened to study the test. How much does a cancer cell weigh? And why does it matter? As cancer cells die, they change their weight in a matter of hours. A new device called the Suspended Microchannel Resonator (SMR) is a new technology that can measure a small change in the weight of a single cancer cell. The SMR machine can detect this change and tell whether a cancer cell has died of "natural causes" or if it was killed by a cancer drug. This allows rapid assessment of whether an individual patient is responding to a cancer treatment or not. This wraps together everything about a cancer patient (such as their myeloma genetics, environmental and other biological factors) and analyzes the response to treatment, making it truly personalized medicine.  A company called Travera is now working to commercialize this technology developed in the Manalis Laboratory in the Koch Institute for Integrative Cancer Research at MIT. It was recently tested in multiple myeloma in a small study at Dana Farber. Cellectar Biosciences is working on testing a radiosensitive therapy in blood cancers including multiple myeloma. Their product, CLR 131, targets the myeloma tumors by delivering a cytotoxic radioisotope, iodine-131 directly to the tumor cells. Phase I study results showed an overall mean survival rate of 26.2 months for relapsed/refractory myeloma patients (who had received on average 5.8 prior lines of therapy). The drug was given as a one or two-dose treatment as a single agent, given during a 30-minute infusion. These results are promising when compared to recently FDA-approved myeloma treatments. 

A Phase II study is now open using CLR 131 and dexamethasone for blood cancer patients including: multiple myeloma (MM), indolent chronic lymphocytic leukemia (CLL)/small lymphocytic lymphoma (SLL), lymphoplasmacytic lymphoma (LPL), marginal zone lymphoma (MZL), mantle cell lymphoma (MCL), and diffuse large B-cell lymphoma (DLBCL) who have been previously treated with standard therapy for their underlying malignancy.

Irene Ghobrial, MD of the Dana Farber Cancer Institute and Ivan Borrello, MD of Johns Hopkins are leading the largest study on precursor myeloma conditions that will include 50,000 patients and family members. The goal of the study is to detect myeloma early and see why and how patients who have MGUS and smoldering myeloma progress to active myeloma. The study is also seeking to understand the genetic, molecular and immune factors that lead to disease progression with the hope that this knowledge could stop disease progression and improve survival with effective strategies.

The Dream Team will also identify lifestyle and demographic factors that contribute to disease progression, such as obesity and race.

CAR T therapy is becoming a hot area of early clinical trials for multiple myeloma patients, but most are personally customized for each patient. A company called Cellectis has developed an off-the-shelf CAR T that every patient can use. Instead of being an "autologous" CAR T treatment (where you use your own cells), this is also called an "allogeneic" CAR T, or a "donor"CAR T.

Many of the current CAR T therapies are going after a target called b-cell maturation antigen (BCMA). A new monoclonal antibody called Isatuximab is advancing through clinical trials and is getting closer to FDA approval. This antibody targets CD38. Targeting CD24 to Eliminate Myeloma Stem Cells

Myeloma stem cells (or early progenitor cells before they become plasma cells) may not be killed with current myeloma therapies and could be the cause of relapse. Dr. Zhan has identified the presence of CD24 as a strong candidate as a cancer stem cell marker and believes that high CD24 levels lead to more aggressive myeloma and earlier death. Why do some multiple myeloma patients relapse early after stem cell transplant while others stay in remission for many years? The immune system may be a key factor.  

David Chung, MD, PhD of Memorial Sloan Kettering Cancer Center was recently selected as a Myeloma Crowd Research Initiative (MCRI) award recipient.

The goal of the MCRI is to help optimize myeloma therapy for individual patients. Dr. Chung's research will review patients who have undergone stem cell transplant. He will study three different parts of the immune system after transplant - T cells, lymphocytes and myeloid derived suppressor cells (MDSCs) and will compare early vs. late relapse. He will then combine that immune system signature with the myeloma genetics of each patient to understand the impact they have together.

Stand Up To Cancer has announced a $10 million award to a Stand Up To Cancer Dream Team focused on revolutionizing the treatment of multiple myeloma, an incurable blood cancer of plasma cells, through early detection of precursor conditions before they turn into full-blown disease. 

The team will be led by Irene Ghobrial, MD at Dana-Farber Cancer Institute (DFCI) in Boston and Ivan M. Borrello, MD, of Johns Hopkins University School of Medicine. 

The hypothesis of this proposal is that early detection of MGUS/SMM in a high-risk population, along with a good understanding of the molecular and immune factors that lead to disease progression, will lead to effective strategies that intercept disease progression and improve survival.

The Dream Team proposes to conduct a screening study of individuals over the age of 45, who are at a high risk for having MGUS or SMM, such as African-Americans and individuals who have a first-degree relative that has been diagnosed with a plasma cell disorder. This study will be called the PROMISE study. We will focus on these populations because they are two to three-fold more likely than others to have these precursor conditions.

The Team expects to screen 50,000 individuals to obtain 3,000 MGUS/smoldering myeloma cases to intensively study and follow over time. The Dream Team will study this group in an effort to define biological characteristics that will help to identify which patients will benefit from particular therapies. These biological characteristics include inherited mutations, acquired mutations, and immune factors. The Dream Team will also identify lifestyle and demographic factors that contribute to disease progression, such as obesity and race.

The Dream Team will use this information to develop new therapeutics that that can be used to prevent myeloma from progressing.

At last year's ASH meeting, there was excitement around a new antibody drug conjugate targeting a protein called BCMA from Glaxo Smith Klein. Results from their Phase I study showed that patients who had relapsed immunomodulators (like Revlimid), proteasome inhibitors (like Velcade) and even monoclonal antibodies (like daratumumab), responded when taking this drug alone (60% response rate).

Researchers found that expression of MUC20 could help predict how sensitive or resistant patients would be to proteasome inhibitors and found that MUC20 protein could be potentially impacted by an already FDA-approved drug offered by Amgen.

Adequate treatment of multiple myeloma at diagnosis is a key issue for patients because their first remission is typically the longest and most durable. Dr. Luciano Costa of the University of Alabama joins Myeloma Crowd Radio to discuss a trial testing some of the best therapies for myeloma up-front: lenalidomide, dexamethasone, carfilzomib and daratumumab followed by a stem cell transplant. The same four drugs will be used after transplant as "consolidation" therapy.

The goal is to understand how much treatment could be needed to get patients into a complete remission status where no myeloma is detectable (also called minimal residual disease negative or MRD status). The four drug combination will be used before transplant. If, after the transplant, the patient still has detectable multiple myeloma, they will proceed to a series of consolidation blocks, up to three, consisting of four cycles of the KRdD at specified dosages and time frames. Patients who still show detectable disease will be given more consolidation therapy while those who are MRD negative will stop treatment.

Natural killer cells are the body's way of eliminating cancerous cells. When a myeloma patient's immune system is too weak, the natural killer cells can't effectively do their job to eliminate myeloma. A study is now being done to test donor natural killer cells in relapsed myeloma patients. The natural killer cells are collected and combined with IL-2 to help them grow and expand. Elotuzumab is used to help the NK cells migrate to the myeloma tumor and has anti-myeloma effect on its own. Myeloma patients can't wait until blood-based "liquid biopsies" replace the dreaded bone marrow biopsy. Painful and intrusive, the bone marrow biopsy is today's "standard" of myeloma testing to determine levels of myeloma disease burden in patients. These biopsies need to be performed at diagnosis, relapse and repeated every so often to determine response to treatment. Researchers at the Dana Farber Cancer Institute and the Harvard School of Medicine are creating a blood-based test that could provide easier measurements of disease burden. Sugar molecules could act as a smoke screen for multiple myeloma cells, essentially hiding them from the immune system.  The surface sugars, called sialic acids, can mark the cancer cells as “self” cells, giving the immune system the signal to ignore them. 

Dr. Michael O'Dwyer is Director of the Blood Cancer Network Ireland and Professor of Hematology at the National University of Ireland Galway. Dr. O'Dwyer has expertise in the glycosylation process as it relates to multiple myeloma. The glycosylation process (or the reaction when carbohydrates is attached to other molecules) is a process that produces DNA, RNA and proteins. This normal process is altered in multiple myeloma and could cause changes in cell signaling, adhesion and drug resistance. In this show we will explore how this affects multiple myeloma, if food intake is related to the presence of sugar molecules and how it is tested for in myeloma.

In 2015, Dr. O'Dwyer received a Clinician Scientist Award worth €1.7m from the Health Research Board to establish a translational research programme in multiple myeloma at NUI Galway. Many patients are familiar with the monoclonal antibodies daratumumab and elotuzumab. These target a specific protein on the surface of myeloma cells. A new approach is in progress to target both a protein on the surface of myeloma cells (BCMA) while also activating the immune system's T cells. Can the genetics of myeloma or other clinical factors predict who will progress from early conditions like MGUS and smoldering myeloma to active myeloma? Dr. Elisabet Manasanch of the MD Anderson Cancer Center is hosting a study in partnership with Signal Genetics/Quest to determine what is meaninfgul (or not) in determining who is most likely to progress and is thus most likely to receive treatment. Dr. Manasanch is also studying the new monoclonal antibody Isatuximab in a clinical trial for high-risk smoldering myeloma patients. Today's myeloma therapy can help patients gain remission, but the combinations can cause their own side effects because they target normal cells as well as cancer cells. Investigators at the University of Iowa including Dr. Guido Tricot and Dr. Frank Zhan have tested mega-doses of Vitamin C in combination with the standard chemo melphalan and found that the Vitamin C can kill myeloma cells while not affecting normal cells. About 10-20% of multiple myeloma patients will develop a condition called amyloidosis during the course of their disease. Amyloidosis is a disease caused by protein buildup in organs such as the heart, kidneys, liver or intestines, leading to organ damage and complications associated with some multiple myeloma treatments. There are three major types of amyloidosis: primary, secondary, and hereditary.  Each type of amyloidosis is classified by its underlying causes and the type of protein that accumulates in organs.

Primary amyloidosis is the most common form and occurs with multiple myeloma. The cause is unknown but the disease starts in the bone marrow. Fragments of abnormal antibodies (called light chains) stick to each other and accumulate in organs in the body.

For the last 50 years, myeloma studies have relied on myeloma progression measurements like complete response (CR) or progression free survival (PFS) to tell whether new treatmets are working (or not). Myeloma patients are now living longer because of new treatment advances, which is a huge blessing. But it is also a challenge for research. If the average patient is living 8-10 years, how can studies be performed and show results in the 2-3 year range so that the new drugs can be approved quickly? 

Minimal residual disease testing can detect very low levels of disease and being MRD-negative is linked to patients living longer. Researchers would like to use MRD testing as a study measurement. One of the two projects funded by the Myeloma Crowd Research Initiative is an immunotherapy called MILs (marrow infiltrating lymphocytes). For high risk myeloma patients, their T-cells are taken out, grown up a hundred fold in the presence of their tumor and then given back a post-transplant. Dr. Borrello gives us an update on his open clinical trlal for high risk patients and shares how is also using the MILs with allogeneic transplant and other future combinations (like with checkpoint inhibitors) and in earlier settings. This immunotherapy can be used in almost 100% of patients with fewer side effects than other types of immunotherapy approaches like CAR T cells.

At the University of Michigan, Dr. Craig Cole is working on a collaboration with the MMRC to perform genomic sequencing on 500 hundred relapsed and refractory myeloma patients over a two year period. The goal is to share each patient's genomic profile to identify patient-specific treatment options. The study will be open at 11 myeloma academic centers to begin, with more to open in the future. The tissue samples collected at these centers are shipped to the University of Michigan Comprehensive Cancer Center, which then evaluates the alterations of 1700 genes. The study aims to find the specific mutations that are "actionable," or that have a drug that can be used. The study goal is to make myeloma treatment more precise for a disease that varies widely from patient to patient.

Nanotecnology is the study of very small things - won to the atom and molecule level. Researchers at Washingon University have been given a prestigious grant by the NIH to study nanotechnology in cancer and more specifcially, in multiple myeloma. Nanotechnology can deliver a higher concentration (by a hundred fold) of drug in a more targeted way. Myeloma stem cells are believed to cause the growth of multiple myeloma. Many myeloma treatments kill the myeloma plasma cells, but according to William Matsui, MD of the Johns Hopkins Kimmel Cancer Center, these treatments may not affect the myeloma stem cells. Dr. Matsui recently presented results from a study he did with colleagues at Johns Hopkins testing a new monoclonal antibody with lenalidomide and dex. Just with len/dex, he saw myeloma stem cells increase by 2.5 times, but when the new monoclonal antibody was added, it decreased the myeloma stem cells by half. But in some patients, the M-protein gets lost, making it difficult if not impossible to detect myeloma levels without a bone marrow biopsy. Dr. James R. The past is impressive for this complex cancer, but there is more work to do. Dr. Ken Anderson helped lead the way for the discovery of new developments over a prestigious career of 40 years. He shares with us a brief look back and what he sees for the road ahead towards a cure.

Dr. Anderson is the Kraft Family Professor of Medicine at Harvard Medical School as well as Director of the Lebow Institute for Myeloma Therapeutics and Jerome Lipper Multiple Myeloma Center at Dana-Farber Cancer Institute. He is a Doris Duke Distinguished Clinical Research Scientist and American Cancer Society Clinical Research Professor. After graduating from Johns Hopkins Medical School, he trained in internal medicine at John’s Hopkins Hospital, and then completed hematology, medical oncology, and tumor immunology training at the Dana-Farber Cancer Institute. Over the last three decades, he has focused his laboratory and clinical research studies on multiple myeloma. He has developed laboratory and animal models of the tumor in it is microenvironment which have allowed for both identification of novel targets and validation of novel targeted therapies, and has then rapidly translated these studies to clinical trials culminating in FDA approval of novel targeted therapies. His paradigm for identifying and validating targets in the tumor cell and its milieu has transformed myeloma therapy and markedly improved patient outcome.

JSK is the first in its class with a new and unique method of killing multiple myeloma. It has a broad-spectrum anti-cancer activity, especially for myeloma and AML (acute myeloid leukemia) but is not toxic for normal cells. Dr. Carmen Baldino of Jasco Pharmaceuticals and Dr. Kelvin Lee of the Roswell Park Cancer Institute are working on a potentially curative solution for patients with this high-risk translocation using a PIM2 Kinase inhibitor called JP-11646. Myeloma is not a single disease. Two types of high-risk myeloma have swapped chromosomes (14;16) and (14;20) have notoriously short survival times with conventional treatments and are found to be resistant to bortezomib, one of the standard myeloma therapies. Dr. Frank Zhan, MD, PhD of the University of Iowa searched a genetic database to identify 100 genes uniquely deregulated for these two types of myeloma. Based on database matching to find compounds that induce or reverse desired gene changes, he found 5 targets to affect the c-MAF and MAFB genes that are disregulated by these translocations. They found that only Alsterpaullon (ALP) significantly inhibits MM cell growth with “spike” expression of either c-MAF or MAFB. ALP is a cyclindependent kinase (CDK) inhibitor and blocks cell-cycle progression. However, the reasons for its antiproliferative effects is unknown. The research will test whether ALP is able to disable c-MAF/MAFB signaling events, thus rendering MM responsive to chemotherapeutics.

Join us for our second interview on Myeloma Crowd Radio highlighting the new Myeloma Crowd Research Initiative on high-risk myeloma. As part of this series, Dr. Guenther Koehne, MD, PhD of the Memorial Sloan Kettering Cancer Center will share his use of a T cell vaccine with an allogeneic transplant to provide outstanding results, even in plasma leukemia patients. His research pulls out T cells, marks them with a tag to target the WT1 protein, and are given back. He will describe how this works, the impact he has seen so far, the stage of his clinical trial and the work left to do to make this an available therapy for high-risk myeloma patients. 

Dr. Guenther Kohene, MD, PhD is Medical Director of the Cell Therapy Laboratory in the Bone Marrow Transplantation Laboratory at Sloan Kettering. He is also Assistant Member and Assistant Attending Physician in the Allogeneic Bone Marrow Transplantation Service. He is Assistant Professor of the Joan and Sanford Weill Medical College of Cornell University. He leads research at the BMT Department/Immunology Program to develop adoptive immunotherapeutic strategies for post-transplant blood disorders. He has particular expertise in the creation and monitoring of antigen-specific T cell responses in these patients. He is the Principal Investigator in active clinical trials using adoptive cell therapy following allogeneic stem cell transplants for multiple myeloma and plasma cell leukemia patients. He obtained his medical degree at the University of Hamburg, Germany and has been at Memorial Sloan Kettering Cancer Center permanently since 2005.
