A huge amount of dental practice revenue doesn’t disappear because patients refuse to pay or because insurance “just won’t cover it.” It disappears because someone clicks an adjustment code and the money evaporates without a clear name, a written policy, or a second look. That quiet leak is what I call adjustment slop, and once you learn to spot it, you won’t be able to unsee it.

  • Cigna PxDx: 2023 class action (E.D. Cal.), citing ProPublica reporting — ~300,000 denials in two months, avg. 1.2 seconds per claim, "we literally click and submit." Court allowed the case to proceed March 2025 (abuse of discretion). This is litigation, not a fine — described on air as a lawsuit/court ruling.
  • UnitedHealth nH Predict: Class action (D. Minn.) alleging a 90% error rate on the AI tool and that ~0.2% of policyholders appeal. Court ordered algorithm disclosure in 2026. Allegations in active litigation — stated as such on air.
  • The "0.2% will appeal" motive appears in both the Cigna and UnitedHealth complaints as an allegation of intent.

I've had the opportunity to sit down and have a deep and enlightening conversation with Jennifer Lyman, a devoted dental hygienist with a heart for recognizing signs of sleep apnea in children. Elaine has 20 years of dental experience, with special emphasis on clinical laser applications, risk and disease management, and business-leadership workshops for dentists and dental hygienists. Elaine is multi-trained for surgery and hygiene and holds multiple fellowships for all types of lasers, including Waterlase Erbium, Deka CO2 & All Diodes. In addition to her role as a clinician, Elaine is an established global laser educator, published author, and clinical consultant for practices nationwide. Outside of work, Elaine enjoys spending time with her daughter, family, and friends hiking around their Arizona home.

As most of you know, Ericka has a dental management company that has a few offices under its belt. She recently purchased a dental laser for the full-time hygienist in the Carson, California office (Shout out to Alex! This training was a game-changer! As a management company, we thought we knew enough to improve hygiene performance. Friends! We could not have been further from that truth! Elaine taught us so much about speaking to the patient about the disease and not the laser in order to increase case acceptance. As a result, we are selling the laser to almost every patient that comes through our doors.
