Pain Reframed is a podcast for physical therapists, occupational therapists, physical therapy assistants and other health care providers about pain and how pain is managed in our society. Pain is a large issue right now and we will bring powerful and helpful insights on how we can manage pain in a much more efficient and effective way. Pain Reframed is committed to changing language and practice patterns across disciplines and points of care in order to reduce the unnecessary suffering that results from the propagation of misinformation, insufficient understanding of the pain experience, and the over-medicalization of pain in society. Dr. Tim Flynn and Dr. Jeff Moore will share their expertise, as well as the expertise from others in the medical and psychological fields, about pain management and our roles in dealing with pain. We want to create a sustainable, multi-disciplinary, international conversation on pain.

On this week's episode of Pain Reframed, David Clawson, MD, joins Tim and Jeff to discuss holistically approaching neck and back pain and spinal deformity, breaking down how Dr. Clawson has used that approach his field of physical medicine and rehabilitation. Dr. Clawson categorizes the threats that patients are up against -physical and mental, then dives into the review of systems to inform and determine his plan of care. Dr. Clawson also discusses in depth how to transition from the physical evaluation to the mental or spiritual evaluation that plays such a large part of pain.

On this week's episode Liz and Jeff speak with Dr. Kirsty Bannister, a renowned researcher on conditioned pain modulation, about what we know, what we’re studying, and what we can look forward to in the future of this topic.

Kirsty completed BSc, MRes and PhD degrees at University College London and and Imperial College before returning to UCL for a decade-long postdoctoral research position in the laboratory of Professor Anthony Dickenson. In 2017 Kirsty gained her faculty position upon joining King’s College London where she now runs her research group investigating the central modulation of pain. Specifically Kirsty's research team bridges the gap between bench and bedside pain research by conducting exploratory experiments that seek to molecularly, anatomically and/or functionally define descending control pathways in rodent and man using electrophysiology and human psychophysics respectively. Kirsty has led multiple symposia at world pain congresses including IASP and EFIC and she also performs Section Editor duties for multiple pain-related journals.

In this episode, Liz and Jeff chat with Walt Fritz. Walt has been a physical therapist since 1985 and has integrated manual therapy into that model ever since 1992. He trained with many traditional tissue-based instructors, including myofascial release and craniosacral therapy, but became dissatisfied with the narratives used to describe the work and effects and began to move into a more biologically plausible explanation. While continuing to use a hands-on approach that visibly resembles those earlier models, he has neutralized the implied message by now referring to the work he uses and teaches as "manual therapy." Building on concepts firmly grounded in the biopsychosocial model, as well as the possibly undervalued third leg of the evidence-based practice model (patient preferences and values), his model of intervention and professional education stresses a patient-led experience. He believes that manual therapy need not be a passive modality, one that fosters patient dependency, but a subset of therapeutic intervention that reduces fear of movement and encourages self-efficacy, one well-grounded in evidence-based practice.

Dr. Justin Dunaway joins Pain Reframed this week! Justin has extensive experience in outpatient orthopedics, not only in the United States, but also in Haiti. Justin and his wife, Dr. Morgan Denny, founded and run the organization STAND Haiti. The STAND Haiti Project establishes permanent access to orthopedic rehabilitative services in the country of Haiti through direct patient care and clinical training of its citizens.

Through STAND Haiti, Justin and his team go to Haiti multiple times a year to see patients who have a significantly different relationship with healthcare and different level of resources and education than in the United States. Interestingly, the difference of access to resources and education brings up some fascinating discussion. Jeff, Justin, and Tim share their thoughts on these differences and how having a “blank slate” in treatment and care can actually be beneficial.

Justin shares his experience in going through the Therapeutic Pain Specialist Certification program and how it helped him and changed the way he manages pain. The certification was instrumental in teaching Justin ways to take the ideas on pain management and apply it in a framework that works for him and his patients.

On the other hand, Anna is not attacking doctors as she believes they are “trapped in the system” and she, therefore, has a great deal of empathy for them. Her desire is to help them to get them out of the bureaucracy in order to solve the problem for patients. This is a fantastic and intriguing episode worth your time!

Dr. Judy Grisel is a behavioral neuroscientist whose research focuses on determining root causes of drug addiction. She studies sex differences in the role of stress and endorphins on drug reward and works to identify innate factors that contribute to individual differences in the liability toward addiction.

In addition to being a professor and researcher, Judy is a New York Times bestselling author. Her book, Never Enough, uses her past as an addict to explore the neuroscience behind how drugs work, the changes they cause in the brain, and where we might be able to find the cure to today's addiction epidemic. It's a powerful piece that we recommend for all clinicians who work with current or recovering addicts.

Tim and Jeff are joined by Dr. Roger Chou on this week's episode of Pain Reframed! Dr. Roger Chou is a Professor in the Departments of Medicine, and Medical Informatics & Clinical Epidemiology at Oregon Health & Science University (OHSU) School of Medicine, and Staff Physician in the Internal Medicine Clinic at OHSU. He has served as Director of the Pacific Northwest Evidence-based Practice Center since 2012. He has conducted systematic reviews in a number of areas, including chronic pain and musculoskeletal conditions, screening and prevention, diagnostic testing, and prognosis. He has served as Director of the American Pain Society clinical guidelines program, the GRADE methodologist for the World Health Organization’s Division of Reproductive Health, is a member of the Cochrane Back Review Editorial Board, and co-chair of the National Quality Forum Musculoskeletal Standing Committee. Dr. Chou is on several journal editorial boards and is an author on numerous scientific articles published in peer-reviewed journals.

We welcome Dr. Eric Christensen to Pain Reframed! We’re extremely excited to have Eric on the show. Not only is Eric a friend and colleague, but he’s the owner of Chandler Physical Therapy in Chandler, AZ and author of the book “Breathe Better.” Eric, Tim, and Jeff will be discussing Eric’s book and how and why Eric wrote and uses the book for patient care.

Eric graduated with his Doctorate of Physical Therapy program from Regis University in 2009, where he published a journal article on the effect of whiplash on the cross-sectional area of the muscles surrounding the throat as well as presenting at multiple conferences. He graduated from Colorado State University with a degree in Exercise Science and a focus in Sports Medicine. In practice, Eric enjoys working with all populations of patients in the orthopedic and neurological field. He has specialized in custom orthotic prescription and casting, vestibular reprogramming, and manual therapy. Eric takes a developmental approach to rehabilitation utilizing the Selective Functional Movement Assessment. He uses functional movement patterning and manual therapy to return patients to prior levels of function.

Today, we have another story from the “patient’s” point of view! We welcome Jenn Shelton, a world-class ultramarathoner, to Pain Reframed to discuss her story and experiences with injury and pain. She has a very fascinating story about how she experienced pain, in a way that’s a little different than what you might expect!

Science News and the media shared some recent research on what morphine does in terms of pain reactivity as well as what is happening with post-surgical opioids. We welcome Dr. Peter Grace to discuss these findings and to unpack this information to help us in this battle against the opioid crisis. Peter has a balanced viewpoint that should give us some good information to consider, going forward.

Additionally, Tim, Jeff, and Peter discuss Peter’s current research on understanding the mechanisms of pain and the role of peripheral immune cells and neuropathic pain. They also discuss the transition from acute to chronic pain and the effect of opioids and morphine substances on moving into that persistent pain pattern.

Peter is an Assistant Professor in the Department of Critical Care Research, at the University of Texas MD Anderson Cancer Center. His research aims to understand the neuroinflammatory mechanisms that drive chronic pain, in order to identify new treatment strategies.

Today, we are looking at large data sets, from a population standpoint, asking if there are ways we can figure out why people actually seek out healthcare and solutions to their pain. We welcome Dr. Derek Clewley, from Duke University, who has been researching behavioral attributes towards healthcare services. This week, we welcome Dr. Sara Lazar, a neuroscientist from Harvard University. Her research focuses mainly on yoga, meditation, and mindfulness-based cognitive therapy to ascertain neural changes that occur with these types of therapy. Dr. Lazar has studied a wide-variety of problems including bipolar disorder, persistent pain, and depression. She will share some exciting information about the research of the cognitive therapies and their effects on persistent pain.

Dr. Lazar’s research results suggest that meditation can produce experience-based structural alterations in the brain. They also found evidence that meditation may slow down the age related atrophy of certain areas of the brain. It’s very exciting research and we hope you enjoy this discussion!

We welcome back, to Pain Reframed, Dr. Beth Darnall, who is a pain psychologist, clinical scientist, and Clinical Professor at Stanford University. Beth has such a passion and commitment to making a difference in the management of persistent pain as well as reducing the opioid epidemic.

In episode 28 with Beth, we discussed the fact that we tend to ignore the pain psychology of patients. Today, we’re continuing that discussion and getting also into the topic of pain catastrophizing and what to do about it.

Beth has a focus on pain catastrophizing as well as treating some of the key issues that prevent patients from succeeding with other pain treatments. It’s our second Pain Reframed PSA! Today, we’re discussing an article titled “Physical Therapy as a First Point of Care to Treat Low Back Pain” by Dr. Bianca Frogner (link below). Specifically, there’s also discussion relating to opioids being the first point of contact and care for many patients.

Obviously, a huge goal of Pain Reframed is to keep individuals away from opioids as much as possible. This article showed that individuals who saw a PT first had a significantly lower percentage of opioid use. Enjoy this episode and we hope you check out the article as well!

We welcome Ash James to Pain Reframed! This week, we are talking about reducing fear, encouraging a variety of movement, and getting to the root of what’s going on with a patient’s body. We want to ensure patients don’t catastrophize, be afraid of their bodies, nor let negative thoughts and other things get in the way of living and enjoying life and full movement and ability.

Ash has worked in various sectors of musculoskeletal physiotherapy, including private practice, professional sports and occupational health. He is now a Clinical Lead for a large Occupational Health Company, runs external courses and workshops, and is looking at ways to improve the care of people within the community. Ash is also currently studying for his PhD with his research focus on the prevention of occupational back pain.

Sometimes, there are pain areas that people aren’t always comfortable discussing. Even further, it’s easy for us to compare, for example, our hands to others around us. But some areas… well, let’s just say, we’re not going around comparing those! So we need a safe environment for patients to feel comfortable and confident getting the treatment they deserve.

We have a very special and fascinating episode for you this week! Mark Cummings joins us to share his story and also his recent journey of pain. Mark also, specifically, shares how his base level of pain increased significantly and immediately after seeing an image of his fracture, continuing to show how cognitive our pain really is.

Mark is an amazing musician who has a passion for the guitar: specifically his 1980s Martin. Mark also has Multiple Sclerosis and is very close to the release of a new CD, featuring incredible musicians from around Colorado. Sometimes patients will hear mixed messages and contradictory advice, which can be a huge problem in their treatment and faith in the system, especially when they have high concern and fear levels. Thankfully, Dr. Becky Vogsland has been working with an amazing team to create a system where a common message is focused on to truly help patients. Becky’s story and their results are phenomenal!

Becky has practiced at the Minneapolis VA in the areas of traumatic brain injury, polytrauma, orthopedics and chronic pain since 2007. To better serve her patients, Becky has obtained certifications as an Orthopedic Clinical Specialist, Certified Manual Trigger Point Therapist, Certified Spinal Manual Therapist, and in Rehabilitation Pilates and TRX. She is an adjunct faculty member at St. Catherine University teaching about traumatic brain injury and headaches.

Welcome back to Pain Reframed! This week, Dr. Kory Zimney joins us from the University of South Dakota (Tim’s old stomping grounds where he grew up!). Dr. Zimney has been practicing physical therapy since 1994, following his graduation from the University of North Dakota with his Masters in Physical Therapy. He completed his transitional DPT graduate from the Post Professional Doctorate of Physical Therapy Program at Des Moines University, Class 2010. He moved into full time academics in 2013. Past clinical work experiences have been with various community-based hospitals working in multiple patient care areas of inpatient, skilled rehab, home health, acute rehab, work conditioning/hardening and outpatient. He also was department manager of rehabilitation services at St. Luke's Health System in Sioux City for 8 years. He also teaches nationally with International Spine and Pain Institute.

We are going back to the patient’s perspective this week on Pain Reframed! The Real Brian joins Dr. Jeff Moore and Dr. Tim Flynn to discuss pain and the intense story behind Brian’s experience in dealing with pain and a surgery that didn’t go exactly as expected.

Brian discusses his journey with back and leg pain, starting in 2002, and how it affected him throughout the last decade. In 2013, his lower disc ruptured and he, at that point, had to get “emergency” surgery to remove the disc fragment from pressing up against and traumatizing the sciatic nerve. The surgery should have been a success (with a 1% chance of complications) but the surgery left Brian with no nerve signal down into his leg. Brian shares the story of his journey from debilitating pain to surgical complications to how he has overcome these limitations today.

Additionally, Brian, Tim, and Jeff talk about the transparency of medical professionals in needing to own up to their mistakes and find solutions to the problems at hand. They also discuss the power of physical therapy and the incredible success of pain management from, not only a physical side, but also a mental side.

Sandy is instrumental in the areas of pain management as well and will be discussing her thoughts and expertise on this topic.

Welcome back to Pain Reframed! This week, we welcome Dr. Anna Lembke who is the Chief of Addiction Medicine and Assistant Professor of Psychiatry and Behavioral Sciences at Stanford University School of Medicine. Anna is also the author of the powerful and eye-opening book “Drug Dealer, MD.”

Dr. Lembke shares her professional journey and what led her to write “Drug Dealer, MD.” The premise behind her book is extremely jaw-dropping and shows a very real perspective to what happens between Doctor and Patient in the medical profession and in terms of the opioid crisis. She reads a passage out of her book and she, Tim, and Jeff discuss just what that looks like and what we, as PT’s need to do to fix this pain problem and help our patients!

On the other hand, Anna is not attacking doctors as she believes they are “trapped in the system” and she, therefore, has a great deal of empathy for them. Her desire is to help them to get them out of the bureaucracy in order to solve the problem for patients. This is a fantastic and intriguing episode worth your time!

Dr. Justin Dunaway joins Pain Reframed this week! Justin has extensive experience in outpatient orthopedics, not only in the United States, but also in Haiti. Justin and his wife, Dr. Morgan Denny, founded and run the organization STAND Haiti. The STAND Haiti Project establishes permanent access to orthopedic rehabilitative services in the country of Haiti through direct patient care and clinical training of its citizens.

Through STAND Haiti, Justin and his team go to Haiti multiple times a year to see patients who have a significantly different relationship with healthcare and different level of resources and education than in the United States. Interestingly, the difference of access to resources and education brings up some fascinating discussion. Jeff, Justin, and Tim share their thoughts on these differences and how having a “blank slate” in treatment and care can actually be beneficial.

Justin shares his experience in going through the ISPI Pain Certification program and how it helped him and changed the way he manages pain. The ISPI Pain Certification Program was instrumental in teaching Justin ways to take the ideas on pain management and apply it in a framework that works for him and his patients.
