Time for the next team on our community healthcare journey, and today we’re focusing on pharmacy in the community and how those working in this area positively impact the daily lives of older people in our society.
We’ll spend some time thinking about the ways our patient, Jean, might interact with community pharmacy and we'll consider what daily life looks like for pharmacists and pharmacy staff in terms of distributing medications, rationalising treatment, providing addiction services where needed and offering consultations and lifestyle advice, amongst many other things.

Great to have you join us for another new episode of the MDTea Podcast, which today is all about community nursing. Outside hospital in the UK there are thousands of nursing staff who use their clinical skills and knowledge to support patients with long-term conditions and help maximise their independence.
Some of these will be district nurses and others will be nurse specialists in particular areas of medicine. This can include nurses specialising in diabetes, stroke, respiratory and cardiac care, tissue viability, mental health, palliative care and lots more.
Two of the roles that we see quite commonly in older patients are Parkinson's nurses and dementia nurses. We speak to two fantastic guests today to help us learn about these roles. Clare Addison is a vastly experienced nurse who is now living with Parkinson's disease, and Chris O'Connor is an admiral nurse in Surrey who spends his time caring for patients and families affected by dementia. Lots of wise words to take on board from both of them.

We know as worth previous teams that these set ups can have a whole host of names- proactive care team, anticipatory care hubs etc. You may not have access to services like this locally but it remains useful to see how everyone works together to support patients in the community.

Today we’re talking about 2 hour response teams who work in the community supporting admission avoidance where possible. Typically they work with patients for short periods of time 24-72 hours where a person is at risk of an acute admission or readmission to hospital.
Teams have a multitude of names, rapid response, crisis team, urgent community response and more so to keep things generic and applicable to you where ever you are we’ve used the phrase 2 hour response team. Do go and find out what services near you are called and how they work!

General practice is often the first port of call for people with healthcare needs in the community, and it's that world that we'll be exploring today.

We’re back! After months of preparation and planning, we’ve got an amazing new series of the MDTea Podcast for you to enjoy. After taking a journey in Series 11 where we met each member of the hospital MDT (along with our patient, Jean) we’re ready to go again! And this time we’ll be joining Jean as she is discharged back into the community, and meets lots of the teams involved with her community healthcare.
On this first episode we’re considering reablement and what exactly that means for patients like Jean, who are continuing their healthcare journey outside the hospital setting. We’ll explore the concept of reablement, discuss the roles of people involved in this area, and find outhow reablement teams encourage patients to take the lead in regaining independence at home post-discharge.
We have a few new features for this series too! Having a substantial, nutrient-rich diet is as important for older people as it is for anyone else, and malnutrition can be a real problem as people age. In this episode, we explore what malnutrition is, why it affects older people so much, and the approach dietitians take to try and boost the nutritional intake and general wellbeing of our patients.
We speak to two vastly experienced dietitians, David and Lorna, and hear in particular about the work they do with the older patients that they see. Welcome along for another exciting episode of the MDTea Podcast! Today it's Episode 7 and we’re spending some time thinking about the amazing work done with older patients by occupational therapists (OT's).
OT's are problem solvers, and play a crucial role in enabling patients to carry out meaningful activities (or "occupations") and recover a degree of quality of life following illness. This can involve setting targets for patient goals such as using the shower, brushing their hair, making a cup of tea, managing their front door steps safely or even walking their dog, amongst many many other things.
On today's episode, we hear from eight brilliant OT's - Paul, Katie, Oran, Alanna, Suzanne, Bex, Louise and Rebecca - who tell us all about what life is like for them on the job, and why they wouldn't want to do anything else.

Physician associates first appeared in the UK in 2003, and have been growing in number since then. In this episode, we’re delving into the daily lives of doctors. And we’re about to discover how many different types of doctor can come into contact with an older patient in hospital. The ones we’re going to meet in this episode include an A&E doctor, an orthopaedic surgeon and a group of elderly care medics. All with the same goal in mind…managing the acutely unwell patient, maximising their comfort, planning for the future, and getting them safely back into the community again where possible. But this can be tricky when frail patients present with a whole host of conditions. How do doctors manage these complex patients? Is there a way to treat someone as a whole person rather than getting different specialists involved for every individual body system? And what exactly is it that makes a good elderly care doctor?
As the average age of our population increases, all doctors coming into the profession need to realise that a passion for elderly care is essential. After all, older patients will almost certainly make up the majority of their work, regardless of their specialty. And the guests on this episode want you to realise that this is something to be embraced, not frightened of.

We’re taking a closer look at a day in the life of a paramedic in our first episode of Series 11. Maybe the picture that springs to mind when you hear the word ‘paramedic’ is of an ambulance driving past you with its siren blaring. Or perhaps they might be people that you come across from time to time as they hand over unwell patients in the Emergency Department. But what do paramedics do? What does their typical day look like? How many older patients do they come into contact with? Are there any non-ambulance opportunities for them? And what exactly is it that they love about the job?

The MDTea podcast started out six years ago, with us wanting to create a place where we could foster multiprofessional learning around the care of older patients. And after 100 episodes that is still very much our aim. But we’ve decided to do something unique and exciting for Series 11 to explore what it really means to be part of the team.
We want to spend time over the next ten episodes exploring what each of our colleagues in the MDT do on a daily basis to provide older patients with the best possible care. What is the first thing they do at work each day? What is their favourite thing about their job? What are the challenges and rewards? And how does their job help serve our older population?
It’s important we ask these questions. Because despite our best efforts to work as a fully integrated team on the wards, we all know that it is easy to become trapped in our own little bubbles. Both in the separate training we receive depending on our professions, and the areas on the wards where we tend to work only with other people doing the same role as us.
And so the problem remains. Lots of us don’t know what our colleagues in the MDT actually do. Which is why we’re spending this new series looking at “A Day in the Life” of each of the vital healthcare professionals responsible for managing older patients coming into our hospitals.
Our 100th episode! This week we have a think about the (complex) topic of immigration law, the EU settlement scheme and the right to reside in the UK - and how these rules and processes affect older adults uniquely, particularly with regards to access to health and social care.
