The Rehabilitation Research and Training Center on Neurogenic Lower Urinary TractDysfunction (NLUTD) was launched in 2019 and includes a diverse team of physicians(rehabilitation, urology, pediatrics), researchers, and people with NLUTD (more commonlyknown as “neurogenic bladder”). We are located across the United States and have a passion to advance science, health care, and quality of life around this common condition. Our podcasts are for clinicians, scientists, non-scientists, adults, kids, and everyone in between. We cover a wide range of topics, from the basics of neurogenic bladder and its management, to being in a relationship and dating, social justice, and even how your bladder is like a snowflake! In this episode of Bladder Buzz, Dr. David Ginsberg, Chief of Urology at Rancho Los Amigos National Rehabilitation Center, discusses bladder management options for individuals with Neurogenic Lower Urinary Tract Dysfunction (NLUTD) when oral medications are no longer sufficient. We explore how clinicians recognize when it’s time to consider next steps and review options such as Botox, neuromodulation, catheter-based strategies, and surgical reconstruction. Recurrent urinary tract infections (UTIs) are a common challenge for people with spinal cord injury, but new research shows that many suspected cases aren’t true infections. In this episode, Madison Hughes and Dr. Chris Elliott discuss how recurrent UTIs are diagnosed and managed in the spinal cord injury population, the role of urodynamics and bladder diaries, and why patient education and non-antibiotic strategies can make such a big difference.

Bladder symptoms are extremely common in individuals with MS. These symptoms include urinary frequency, urgency, incontinence or difficulty voiding or both. Voiding dysfunction (VD), a common neurogenic lower urinary tract symptom in individuals with multiple sclerosis (MS), often leads to urinary retention. The current primary management for MS patients with VD involves catheterization. Our team has explored the safety and therapeutic effects of a multi focal transcranial magnetic stimulator, a noninvasive, portable neuromodulator capable of simultaneously stimulating or inhibiting multiple brian regions and modulating the strength of their functional connections called Transcranial Rotating Permanent Magnet Stimulator (TRPMS).
In this study, MS women with VD underwent functional magnetic resonance imaging concurrently with a urodynamic study (fMRI/UDS) before and after ten 40-minute TRPMS sessions. Baseline fMRI/UDS scans identified regions of interest (ROIs) associated with voiding initiation, guiding microstimulator placement. Post-treatment, patients exhibited increased activation in these ROIs, a significant decrease in % post-void residual/bladder capacity (%PVR/BC), and reported improved bladder symptoms including more effective bladder emptying.

They discuss the important take-aways that people with spinal cord injury can consider when seeking medical attention for urologic issues, as well as considerations for clinicians who encounter spinal cord injury patients that are hospitalized or visit the emergency room with urinary tract infections.
