Chris wrote a recent article, entitled The bizarre campaign against Physician Associates, in which he argues that the interests of doctors are being elevated above the interests of patients. Chris talks about he sees this as the driving force for the recent anti-PA campaign.

In this episode, we talk to Dr Richard Marks, anaesthetist in London and co-founder of Anaesthetists United (AU) . They are now in a legal battle with the General Medical Council (GMC) over the way that regulation for PAs and AAs has been introduced.
Specifically, AU feels that delegating the setting of scope of practice to individual hospitals, rather than having a nationally-set standard, is a mistake and puts both patients and Associates themselves at risk.

In this episode we talk to Matt Rai, a senior PA who has recently been focusing his efforts on making sure that PAs (and AAs) are getting additional support and guidance for their well-being during what has been a challenging time for MAPs.
Whilst simultaneously working clinically in primary care and as an Assistant Professor at the University of Birmingham, Matt has devoted much of his spare time to brainstorming and planning how he can have a positive influence on the MAP community.
Matt is running a series of wellbeing retreats for MAPs, the first of which is taking place at the Little Earth Centre in Wolverhampton on Saturday 25th May.
This day is the first in a series of planned retreats designed to gather the MAP community and focus on community connection and well-being.

Shayan Ashjaei is a Neurology Physician Associate, based at King’s College Hospital. I asked Shayan all about his background in neuroscience, his personal connection to wanting to work in neurology, and what he loves about the speciality.
Shayan passionately explains his choice of specialising in neurology and provides a glimpse into his daily activities on the ward, which involve delivering exceptional patient care, participating in exciting procedures, and serving as an educator.
Shayan is able to work on a number of clinical procedures including lumbar punctures, skin and muscle biopsies. Shayan has also developed, implemented and delivered regular lumbar puncture teaching and is trained to perform greater occipital nerve (GON) blocks for acute neurology patients with occipital neuralgia, cluster and cervicogenic headaches. Shayan has also worked on a “neurology virtual clinic” - a unique virtual post-discharge clinic for neurology inpatients’ investigations, to improve follow up and the safety of patient care.

A trade union is a group of employees who join together to maintain and improve their conditions of employment. The typical activities of trade unions include providing assistance and services to their members, collectively bargaining for better pay and conditions for all workers, working to improve the quality of public services, political campaigning and industrial action.
Nearly seven million people in the UK belong to a trade union. There is a trade union for doctors - the British Medical Association (BMA) - but there isn't yet a trade union for physician associates. I spoke with Susan Singleton, who has begun to make progress towards setting up a PA union about her plans.

The Academy of Medical Royal Colleges (the Academy) is the membership body for the UK and Ireland’s 24 medical royal colleges and faculties. It sets the standards for the way doctors are educated, trained and monitored throughout their careers. The Academy also plays a leading role in clinical quality, public health and works closely with organisations such as the NHS, the General Medical Council and patient groups on projects designed to improve the quality of care.
I was lucky enough to speak to Dame Helen Stokes-Lampard, who is currently the Chair of the Academy of Medical Royal Colleges and previous Chair of the Royal College of General Practitioners.
I spoke with Helen all about physician associates, and how the Royal Colleges might support the development of the profession in the future. We also touch on a range of other subjects, including the use of social media for professionals.

Last week, there were lots of headlines about the Medical Doctor Degree Apprenticeship. I wanted to find out more about this apprenticeship, and how it might help grow the future medical workforce and attract and recruit from a wider pool of people in the local community.
The Apprenticeship gives individuals, who for a multitude of reasons may have been unable to attend university full time, a new route to train as a doctor. Or it may be a method of career development for physician associates who want to train to become doctors.
To find out more, I spoke with Dr Sanjiv Ahluwalia - Head of School of Medicine and a General Practitioner. Sanjiv told me more about the Apprentice will be employed by a healthcare provider organisation. Apprentices study flexibly alongside work, so they can put newly acquired knowledge and skills into practice immediately, for the positive benefit of service users and patients. Medical Doctor Degree Apprentices will do the same training, at the same high standard as those taking traditional educational routes.

In this episode I spoke with the UK’s first PA in hospice medicine, Jovy Giles, about tough workplace experiences, going after your goals and getting into the heart of why palliative medicine feels like the ‘medicine we are supposed to do‘.
In the words of Prof Baroness Ilora Finlay arguing for the Health & Care Social Bill change before palliative care was finally recognised as a UK citizens’ right in March 2022: “The other truth, that everybody is born, was recognised by requiring every part of the UK to have maternity services. Hayley Arceneaux works as a physician assistant at St. Jude Children's Research Hospital. As a child, Hayley was treated at St Jude's Hospital for cancer. Now a fully qualified PA, Hayley works with children in paediatric oncology.
At age 29, Arceneaux was the youngest American to travel to space, when she became an astronaut on SpaceX's first private spaceflight Inspiration4, and became the first human in space with a prosthetic leg bone after surviving bone cancer.

This might be the most important episode I think I've ever made of the podcast. In it, I am lucky enough to speak with Liz O'Riordan, who worked as a consultant breast surgeon, until she was diagnosed with breast cancer herself.
Liz talks frankly about what it is like being on the other end of treatment, as a patient with expert knowledge of the disease, how we can look after our colleagues at work, and how we should talk to patients about diagnoses and dying.

Albie James is a PA employed by Alliance for Better Care, GP Federation, and works in a split role with Surrey and Sussex Healthcare Trust. This was part of wider integration work, implemented to explore potential benefits of working across organisational boundaries.

Clare Barton is an Assistant Director of regulation at the General Medical Council. Clare is currently responsible for leading the programme to bring Physician Associates and Anaesthesia Associates into the regulation of the GMC. The role of the GMC is to protect patients and improve medical education and practice across the UK.

Clare has a background in education and regulation having previously worked at the University of Paisley and the Association of Certified Chartered Accountants. She joined the GMC in 2006 as Head of Applications, overseeing the teams that deal with applications from doctors all over the world to join the Register. She then became Assistant Director Revalidation, Licensing and Specialist Applications. She was responsible for the process required for those doctors seeking to gain entry to the specialist and GP registers. She oversaw the revalidation team which receives recommendations from Responsible Officers and makes the revalidation decisions for all doctors registered with a licence to practise. She also had responsibility for the team that dealt with applications to leave and restore to the register and to give up a licence.

Physician Associates can play an important part in relieving a proportion of the clinical burden on GPs and their wider staff, but only with the right training and support in place. In this episode, we chat to Sheffield based PA Ria Agarwal who is an experienced primary care PA. Ria and her team have published a couple of papers recently about an innovative preceptorship year that they have launched to help newly qualified PAs transition into working in primary care effectively, and the levels of supervision that PAs might need as they begin working in GPs surgeries.

More about Professor Phil Begg
Professor Begg has been a clinician for almost 40 years, and is an international leader in his clinical field, with his main interest in audiological and vestibular medicine.
He leads a research team currently working on a £1.5 million NIHR funded study developing a totally implantable cochlear implant, his research was recently featured in a BBC documentary ‘Surgeons at the edge of life’. He has published over 100 papers and spoken at conferences internationally for over 30 years.
Professor Begg is a lifelong clinical educator, and has held a variety of academic appointments including past Associate Dean at the University of Wolverhampton, where, with his team, he developed and launched the first PA programme in the UK in 2004. Phil has been part of the small group of dedicated educators and practitioners that started the development of the Physician Associate in the UK over 18 years ago. He is also an examiner on the UK National PA exam.
In 2008, he advised the South African Government in the development of education programmes for the Clinical Associates and he has assisted a number of international developments of new PA programmes, he is currently working with colleagues in Switzerland to introduce the PA concept into the Swiss medical system.
He is one of the four original Founding Trustees of the International Academy of Physician Associate Educators (IAPAE), in 2007.
He has served as past President and Chairman of International Academy of PA Educators and the UK & Ireland Board for PA Education. He is currently Director at Large for Europe and the UK for IAPAE.
He was also privileged to serve for 10 years on the Council for Assisting Refugee Academics (CARA), rescuing at risk academics from conflict areas across the world and placing them in a safe university in the West, until repatriation was an option.
He is proud to hold an academic position as full visiting professor at the University of Kentucky (USA), along with honorary positions at the Universities of Birmingham (UK) medical school, Aston University and Brunel University (UK) in medical robotics.
He is also an Executive Director at the Royal Orthopaedic Hospital in Birmingham Europe’s largest single specialist hospital for orthopaedics and a national center for Orthopaedic oncology.
