You diagnose a rotator cuff, bursa or proximal bicep injury, so you focus your attention on that injury...

In essence this is really a strength and conditioning skill, and something you did very little of, if any, during University training.
So many therapists struggle with this...

One of the things that I see new grads do all the time is to try and sound smart.
I did the same thing as a new grad and at the time I was just doing what I had learnt.
You use big words and medical jargon when explaining a diagnosis to your patient.
But whilst what you are saying might make sense in your head, always remember you are a physio, and your patient is not.
So whilst you think you sound more professional and by using fancy language you can sound more experienced it is often at the expense of making your patient confused.

The infamous ‘dead leg!’

When you test a knee, shoulder, ankle or any joint or muscle range of movement (ROM), what are you actually looking for?
Obviously you look for the actual ROM and measure it in degrees...
Before confirming this to the opposite side, right?
But this is not the only thing you should be looking at!

You were told at Uni that come the end of your subjective assessment you should know what is going on with your patient and have a firm hypothesis...
Then you just use your objective assessment to confirm or deny this hypothesis.
Sounds easy right?
But in reality it rarely is as easy as that.
The subjective assessment is the first thing you do for a very good reason.
It is the most important thing you do, as everything you do after that, including your objective assessment, treatments and rehab is all dependant upon your subjective assessment.
So do a poor job with the subjective and it makes everything you do after that so much more difficult.

I do one thing with each and every patient I see that most therapists never think to do...
I assess their feet!
The test I do takes less than 20 seconds to do, and it is the second test I do, after assessing gait.
As simple and as quick as this test might be, it can be like gold-dust and give you the information you need to make sense of what is going on with your patient...
Identify movement compensations that you will need to fix...
So you can find, and fix, both the site and source of your patients symptoms.

I only use one special test at the shoulder.
It is one of the most simple tests you can do at the shoulder...
But it gives me valuable information as to the best way to manage my patient.
Too many physio's, from new grad's to experienced therapists, overcomplicate their shoulder assessments.
You think the shoulder is complicated...
But that does not mean your shoulder assessment needs to be complicated too!
Quite the opposite in fact.
The more simpler you can make your shoulder patient assessment, the more clarity you will gain on how to best manage your patients problem.
Learn which test I use with each and every shoulder pain patient I see...
And why some of the other special tests, that I guarantee you are using with your own shoulder pain patients, are a complete waste of time!

On the face of it the subjective assessment looks easy.
You ask your patient a series of simple questions...

Why do physio's overcomplicate things?
We make what should be a simple process, to assess, treat and rehab patients, far more complex than it needs to be!
Everyone is chasing the 1% er's.
Those magic tests, techniques and rehab exercises.
You know the ones...
They pop up on your instagram account almost daily.
These techniques and exercises can be great and can help your patients...
But only if you understand how to use them and only use them when you have found a patient problem that requires that specific solution.

Probably the biggest area I see new grads struggling with when rehabbing their patients, is knowing when to progress (and regress) rehab.
If you progress any patient too quickly, regardless of their injury, they will break down.
Their symptoms will flare up and they will be back in your clinic asking you why.
They might get bored and stop doing their rehab or even worse...
Just not come back for their next appointment because they do not think you are the right person to help fix their problem.
Putting together a rehab plan for even more complex pain patients does not need to be rocket science.
You just need to know the next logical step your patient needs to take to get them back to full fitness, smoothly, timely and without the fear that they will break down again.

I used to think that strength needed to be the focus of every rehab programme I wrote.
I made the mistake of on spending too much time trying to get my patients and athletes stronger…
Thinking it would ‘fix’ everything.
It helped a lot of patient but, there were always some, that I could just not seem to help...
No matter how hard I tried or how much stronger they got!
They’d usual be progressing well through their rehab, but then would break down.
This was usually when I progressed them to higher level activities like running or allowed them to return to sport.
Their symptoms would come back quick, often much faster than they had gone away and often much worse…
And I had no idea why!
There is much more to rehab than just getting your patient, or athlete, strong.
Thankfully I worked out what I had been missing.
This meant I was able to stop patients breaking down and it changed my whole philosophy about how I now rehab the patients and athletes I see.

Just because you have been taught a certain way to do something and it is what you have always done, does not mean it is right!
This is never more apparent than with manual muscle testing!!!
You know that Oxford Strength Scale you use...
Where you score patients from 0-5 having tested their muscle strength...
You use this all the time, right?
But did you know it is was actually designed to assess neuro patients NOT MSK patients...
And that it has major flaws in it's application...
Which can be a major reason why you are not able to find the right diagnosis and be able to help your patient get back to full fitness!

Many new grads struggle to get on top of patients with hip pain.
You know the story...
Your patient is progressing well, their hip pain has gone, they have full hip range of movement and can squat and lunge pain-free...
You even got them to do some higher level rehab like some hops, and they did these easily and pain-free.
But...
No sooner had their symptoms gone away, as soon as you you allowed them to start back in the gym or start running, they break down again!
How frustrating is this, for both you and your patient!!!

Most physio's overcomplicate treatment and rehab for lower back pain problems.
The lower back can seem a complex and confusing area of the body, especially when the so called 'experts' in lower back pain cannot agree on the best way to care for patients with lower back pain.
You have probably heard them telling you so many different things...
From core stability to trunk strength...
Treatment for the diaphragm and rib-cage...
Plus so many other things, all explained as the 'holy grail' to treating back pain.
But despite all the evidence and research into the lumbar spine, and even if you follow the so-called 'experts' advice, it can still be very hard to get on top of patients with lower back pain, right?

You have seen it all over instagram but what is all this rib-cage stuff about?
I'll be honest I was a bit skeptical at first...
Until I started to use this stuff and see how quickly it could change patient symptoms...
And keep them away!
Is use it a lot with patients with lower back pain...
But it has also been a game-changer for patients with pubic overload, hip impingement and especially with overload injuries like anterior knee pain, patella and achilles tendon issues and those patients and athletes that struggle with recurrent hamstring and calf tears.

Poor communication can completely undo all your hard work you have done with your patient.
Even if you nail your diagnosis...
And in your head you how exactly what your patient's treatment plan is going to look like...
If you cannot get this information across to your patient then this does not matter and it can be a recipe for disaster.
If your patient does not have a good understanding of their problem and how your rehab programme is going to them from A to B...
From injury and back to full health...
Then patient's will lack faith in you and what you are saying.

You know how important your red flag questions are...
But what do you actually do when a patient answers 'YES' to one of those questions?
Beyond the most obvious, like a lower back pain patient with cauda equina symptoms or unexplained calf pain following a long haul flight (DVT)...
9/10 patients with red flags do NOT need an urgent onward referral to A&E!
There is so much mixed evidence out there regrading red flags and how to manage patients reporting these 'sinister' symptoms...
It can feel overwhelming and confusing about what to do next?

Or do you acknowledge the symptoms, document your findings and start treating and rehabbing your patient?
This can feel like a big decision and it is, and is one decision you do not want to get wrong.

You already know that objective assessments are very important as they help you check how your patient is progressing...
And help guide the rehab process.
You might think then, that completing more testing is therefore better....
But this is not the case!
Too much testing, the wrong type of testing or testing at the wrong time in the rehab process can be a complete waste of time...
And worse than that, actually cause injury!
It is hard to know what to do as everywhere you look on social media you see some physio using some fancy gadgets and tech to test someone or something, using force plates, nordboards, hand help dynamometers and all sorts of other equipment.
If there is something that can be tested, you can bet that someone has designed some sort of gadget to test it!

Not every shoulder pain patient needs rotator cuff exercises!
You already know this right, because you have no doubt seen patients where these type of exercises did not help.
They might have even flared up your patients symptoms!
So why are you still using them?
Probably because that's what you were told to do at University, right?
You have a patient with shoulder pain...
You give them some 'rotator cuff' rehab...
Maybe even some 'scapula stability' and their pain goes away, they get better and job done.
That is how the story is meant to go...
But rarely it does and for most therapists, even those with loads of experience with shoulder pain patients. the shoulder is a joint physio's really struggle with.

Do you rush to get your patient on the bed...
Eager to assess their knee, ankle or hip pain?
STOP!
By doing this you are missing some really simple yet vital information.
Using simple movement testing you can observe how your patient is moving...
And you can identify any compensations they may have developed as a result of their injury.
These results can not only help you to correctly find the right patient diagnosis...
But can also tell you what to do next i.e. what treatment techniques and rehab exercises you need to prescribe to 'fix' your patients actual problems...
And all before you get your patient on the bed and without a special test in sight!

Many therapists struggle with ankle syndesmosis injuries...
Often incorrectly diagnosing them as lateral ankle injuries, like an ATFL or CFL injury, but this is a BIG issue...
As they need to be managed very differently!
This area of the ankle is often glossed over at Uni so it is no great surprise you might struggle to differentiate syndesmosis injuries to other lateral ankle injuries.
The ankle is a relatively simple joint, that moves up and down, and the mechanisms of injuries are often clear for both lateral ankle (inversion) and syndesmosis injuries (dorsiflexion and external rotation)...

‘Why didn’t they teach us this at Uni?’
This was a question I got asked by a therapist on a course I was teaching on just last week.
He was talking about how University had missed the bit about how to properly design and implement a rehab plan for a patient.
University teaching typically teaches you a very basic way to assess, treat and rehab your patients...
In the textbook way.
But if you have ever seen a patient in real life, you will know that these textbook patients do not actually exist!
You were taught that if your patient had [x] issue...
You would do treatment technique [y]…
Or rehab exercise [z], then their problem would go away.
It would be nice if it was that simple, but clearly it is not.
Every single patient you see is different.
They have a different story, different goals, previous injuries and non-msk issues that can affect their rehab.
Even if patients seem similar in how they present, they will never be the same...
And as such require an individual treatment and rehab plan to fix them up.

Picking the right exercise in hamstring injury rehab can be tough.
How do you know what exercise is best?
Is it a romanian deadlift, some form of hamstring bridge, a slider....
Maybe it's nordics?
It must be nordics, right, because everyone is using them?
Improvements in hamstring strength link to reductions in hamstring injuries, that is clear from the current evidence base.
You increase the capacity of a muscle via strength training, then it has a higher threshold before it breaks, which seems pretty obvious when you think about it.
But if it was just as simple as improving strength, why is it that even some of the strongest athletes and patients still break down?
Improving strength of the hamstrings, or any other muscle for that reason, can actually make some patients and athletes worse...
And actually increase their injury risk!

Why is it that some hands-on treatment techniques work really well with your knee pain patients...
Yet the same techniques do nothing for other patients with almost identical symptoms?

If you are a physio it seems you have to love or hate manual therapy.
Which camp are you in?
I'm in neither camp, and probably sit somewhere in the middle.
I see the value in manual therapy for SOME patients and not others...
And use it differently with different injuries and how I use manual therapy changes as the rehab plan progresses.
How I now use my hands-on treatments is very different to when I started out as a new grad physio.
I made a lot of errors, mainly because I did not truly understand what I was actually trying to do (hint - I was not breaking down scar tissue or smoothing out muscle tissue like I thought I was!).

Everyone forgets about the big toe!
You have no doubt heard people talk about 'triple extension' in rehab...
And the ability to extend the hip, knee and ankle at the same time.
This combination of movements is important as this needs to happen to allow the lower limbs to move in a way to allow us to walk, run and jump.
But aren't we forgetting something...
So shouldn’t it be quadruple extension?
When running we clearly need the hip, knee and ankle to extend...
But what about the big toe?
The last joint in contact with the floor prior to push off when we walk, run or jump, is the big toe.
So surely the ability or inability of the big toe to extend is equally as important as extension of the ankle, knee and hip joints above, right?
To highlight just how important the big toe is to movement, pain and function take a listen to this week's podcast episode.
In the episode I talk through a case study of a pro footballer that I worked with a couple of years ago, unable to play due to athletic groin pain.
The key to getting rid of his pain and getting him back on the pitch?

We all question if we might be good enough to do our jobs, at some point in our careers.
I bet you have already done this yourself, especially if you are just starting out as a new grad physio.
Feeling inadequate and questioning whether or not you have what it takes to do your job well and help the patients and athletes you are working with can be overbearing...
Laying in bed at night and worrying about your next day at work, hoping that that new patient is not too complex...
Crossing your fingers that last week's patient with lower back pain comes in tomorrow and reports they feel better.
Whilst this is normal and everyone tells you not to put too much pressure on yourself, that does not change those feelings of inadequacy and incompetence.
Imposter syndrome is real and is not a nice feeling.

You can do all the tests in the clinic...
But is your patient really ready to go back to return to higher level activities like running or sport?
Do you sometimes second guess yourself and give your patient the green light to go back to these sort of activities but deep down you are just not sure how they will go?
This is a really common with new grads, but can be fixed up by asking your patient one simple question.
You should ask this simple question before you plan on allowing your patient to return to those higher level activities...
Which might be a return to running or sport...
But could also be your patients job, the gym, hiking or any other activity.
Failure to ask this question can lead to poor patient outcomes and a return to higher level activities for patients that are not ready.
And we all know what happens then!

Put simply, there are two specific points in rehab that therapists mess up...
One is early in the rehab process...
The second later on, and prior to a return to higher level activities like running.
If I was a betting man (I am by the way!) I would bet that if your patient is not progressing…
Or even worse, their symptoms have flared up, that it will be at one of these two specific points in the rehab process.
The worst bit...
Most therapists are making errors at these crucial points in their rehab programmes without even knowing.
And this makes sense because if you don't know, it is easy to push your patients too hard…
Causing a flare up in their symptoms.

This makes me feel kinda old!
15 years ago I was just about to start my physio journey, at the third attempt, and begin my studies at the University of Bradford.

Why is it that patient’s struggle with certain rehab exercises?
You pick the right rehab exercise for your patient…
You tell them what it is, you might even demonstrate it...
But whatever you do they just can't seem to get it!
It might be a simple squat, deadlift or a hip hinge exercise like an RDL but however hard you try you struggle to get your patient to do the movement pattern well...
Your patients hips, arms and legs are all over the place, resembling something more like an octopus than a human being!
Even worse is when patient’s come back for their next appointment, and when you ask them to demonstrate the rehab they have been doing as part of their home exercise programme...
It is NOTHING like the exercise you first gave them!
Sound familiar?
Your ability to teach simple movements like a squat, deadlift or hip hinge is vital if you want to use these exercises, or any variation of these exercises in your patient's rehab programmes.
The right rehab exercise is great...
But it is not right if your patient can not perform it properly.

You might want to think twice about using this common hip exercise!
The glute bridge is probably the most used hip rehab exercise in physio.
But did you know that most therapists use it incorrectly...
Often with patients that do not need an exercise like the glute bridge!
Even worse than that...
The glute bridge can actually be detrimental to some patients, especially when you consider how most therapists coach this exercise when they instruct their patient to 'push through their heel.'

Making sense of your lower back assessments and making the right diagnosis is difficult enough as a new grad...
But taking a patient with back pain through a full rehab plan, from start to finish, without flaring them up can be even harder!
Depending who you listen to some therapists tell you that hands-on treatments are really effective for lower back pain...
Others tell you to avoid manual therapy with vengeance!
Then where do you start with your rehab?

Patients presenting with hip pain can sometimes be tough to get a handle on.
Often they might improve to your hands-on treatments and early rehab exercises…
But as soon as they go back to things like the gym, running or sport the hip gets irritated and your back to square one!

The hip is one of the most difficult joints to treat because it can be so easily aggravated.
Just about every type of movement, like any form of squat, lunge, running, even walking.
To make matters worse even simple tasks like your patient putting their socks or shoes on, sitting, driving and even sleeping can also set the hip off!
But there is one simple thing you can do to make a quick change to hip pain.

Sat in a supermarket cafe back in 2010 a short conversation as a new grad physio changed my career.

You do not need me to tell you how important your patient assessment is.
The assessment is at the start for a very good reason...
As it informs everything else you do after that.
It gives you your diagnosis...
Helps inform your patient prognosis...
And tells you what treatments and rehab to prescribe your patient.
Even as an inexperienced physio and even with complex cases, you should always be able to identify a probable cause of your patients symptoms after your subjective assessment.
Next you can use your objective assessment to prove or disprove this.
The subjective assessment is so important.
I'd go as far as saying it is probably the most important part of what you do as a therapist, because everything else you do after this is reliant on a good subjective assessment.
But it is something that so many new grad physio's struggle with.

Assessing swelling during your knee pain assessments is something I am sure you already do...
But are you missing the main point of this simple assessment?
You might use tests like a patella sweep or patella tap test to assess for the presence of knee joint swelling...

The key to success with any patient is being able to clinically reason what you are doing.
It is being able to make sound judgements based on the findings of your clinical assessment and making the right call with a patient’s diagnosis and prognosis.
This can be hard as a new grad, especially if you are seeing injuries that you might not commonly see, or injuries that you have not had much experience with in the past.
The physio inside you always wants to give a positive spin on each and every injury you see, to give good news and a rosy prognosis…
But be careful as this might sometimes catch you out!

Being able to clinically reason what you are doing in your patient assessments, treatments and rehab, is key to getting consistent patient results.
Without it...you are just guessing!
We talk about clinical reasoning ALL the time in the therapy world...
Yet most therapists, even experienced ones, do a really poor job in actually applying this practice to their day to day work.
Do you sometimes go through your assessment and even if you are unsure what the diagnosis is, carry on regardless?
Do you prescribe rehab exercises or using treatment techniques just because you are comfortable with them…
Or use techniques or exercises just because they have worked well in the past with similar patients...
Or use random exercises with your patients just because they appear popular this week on Instagram?
You are not alone...
But you should never start treating your patient until you are clear about what you are actually trying to fix.

Most therapists I speak to think they have a problem with their treatment and rehab programmes.
You think you need to learn more skills...
So you go on courses to learn loads acupuncture, dry needling, ART or some other 3-word treatment technique or...
You start using using blood flow restriction training or some other 'sexy' rehab protocols or a new exercise that you have just seen your favourite physio using on Instagram.
Sure some of these techniques and rehab exercises might help.
But they will only help your patient if you give your patient the right intervention...at the right time.
Most therapists do NOT have a problem with their treatment and rehab programmes.

Getting your foot in the door in sport is tough.
You are in direct competition with 1000's of therapists, for the same job roles.
The same can also happen in private practice and in the NHS.
The demand for good new grad jobs, far outweighs the number of jobs out there.

I learnt this the hard way.
Earlier in my career working in sport this usually resulted in a barge of questions and shouting from an angry head coach...
Or patients becoming frustrated when I could not fix them after the first few sessions, so they stop turning up for their sessions in the clinic.
I did not appreciate the important of something so simple and this led me to a lot of problems!
I used to think it was the head coaches or my patients fault...
But it was in fact my fault!
I was making a simple error that I see being made every day by therapists just like you with their own patients...
That is stopping you making the progress you want with your patients and getting them back to full fitness.

Do you worry when your patient reports they have neck pain?

As simple as the knee might appear, I am sure you have had some patients with knee pain that you have found tricky to deal with.

Finding the time for CPD can be challenging but it is something you know you need to do to stay up to date with the latest evidence and continue to grow as a therapist.
University only teaches you so much.
Hopefully it taught you how to be safe and competent with your patients…

The physio room is NOT the place your patient needs to be if they have something sinister going on and it is your job during the assessment to work this out.
Failure to identify possible sinister problems is not great for you or your patient as you might start treating something you should not.

Working with real patients in the real world is a far cry from treating your classmates in a practical session or when you were out on placement.
Then you were still learning...
And you had the fall back and 'excuse' to get things wrong.
But once you qualify that all changes.
Patients expect you to fix them and they do not care if you qualified 10 months ago or 10 years ago...
They just want you to tell them what is causing their back pain and how you are going to fix it.
Lower back pain is something you will see more than any other injury.
It's the most prevalent MSK in the world so it's an area of the body you need to know how to manage.

You may have heard about physio's assessing the rib-cage as part of their patient assessments.
This might seem complicated...
But it's really not.
If you can understand what the ribs do when we breathe in and out, and how this relates to movement at the lower back...
You will be able to see how poor rib-cage movement can heavily influence lower back pain.
This is really important because a failure to assess and treat a rib-cage problem might stop you getting your lower back pain patient pain-free or keeping their pain away once they have left your clinic.

If you get to the end of the subjective assessment and you do not have a firm hypothesis of what might be happening, then something has gone wrong.
Even with complex cases, or patients with more than one issue, you should be able to identify a probable cause of the patients problem...
Then you can use your objective assessment to prove or disprove this cause.
The subjective is so important, probably the most important part of what you do as a therapist, because everything else you do after this is reliant on a good subjective assessment.
But it is something that many new grads struggle with.

There are many things that I would do differently if I had my time again.
Everything I do now is a million miles away from what I did when I first graduated back in 2010.
I guess looking back I was just doing what I had been taught at University and using the stuff I had picked up on my placements.
But I quickly learned that when I first graduated what I knew at the time was not going to cut it in the real world.
My private practice patients were paying me their hard earned cash to get fixed…
And the athletes I was working with wanted to be back on the pitch ‘yesterday’ and the pressure from the coaches was immense.
It was a sharp learning curve and littered with mistakes.
Since graduating back in 2010 I have learnt so much.
Looking back, 99% of the problems I had as a new grad were brought on by myself and were unnecessary.
If I had my time again I would do things very differently.

As a physio there is not much worse than patient’s not doing their rehab.
You give your patient the exercises to help them get better…
But for whatever reason they don’t do them.
Maybe it’s a lack of time…
Or they forgot what you told them to do…
But whatever it is, you know that if they don’t start buying in to the rehab plan, then they are not going to improve.
And these type of patients are always the same patients that start questioning you why they are not getting better after the first few sessions!
Doing your hands-on treatment techniques on a patient can help reduce pain and improve range of movement, sometimes even improve strength, but it is the rehab that fixes the problem 9/10 and stops the problem coming back.
Maybe you label your patients ‘lazy’ and question if they actually want to get better.
After all, if they did surely they would do their rehab exercises like you asked.
But in most cases the reason patients do not do their rehab is not their fault…
But your fault!

From the outset my new grad journey might look like a dream...
Gaining my dream job as the Head Physio at the Leeds Rhinos less than 15 months after graduating.
At the time they were the top team in the country, having just one that years Grand Final and having won 4 of the last 5 championships.
This was a big job!
But it wasn't all rosy, far from it.
Those early times as a new grad were littered with errors, mistakes and those imposter feelings we all get as a new grad.
I remember each and every mistake I made like it was yesterday.
Those feelings of being inadequate and incompetent I'm not sure you ever forget how bad they make you feel.

What is the best way to manage an athlete or patient following a concussion?
All the recent evidence around concussion points towards a safe, but early return to exercise, and not a prolonged rest period, that was previously prescribed.
It is important that the exercise you prescribe does not heighten patient symptoms, especially in the early stages, but like any other injury we need to load the injured area to help it recover (in concussion this means the brain).
During the early stages of recovery following a concussive injury, moderate rest is still important, but what the evidence is showing is that this period of ‘rest’ is becoming shorter and that longer duration rest can actually cause more harm than good.
Concussion research is ever evolving so if you work with deal with concussions then keeping up to date with the current evidence is key.

When you use a standing forward flexion test or toe touch, what do you actually look for?
Pain?
Restriction?
Quality of movement?

As a new grad you will make mistakes.

MRI’s are thought as the ‘gold standard’ to make an accurate diagnosis but they might actually cause more problems than solutions!
One big thing with any imaging is to remember that a scan gives you an image of anatomy and not function.
Any investigation you use is just part of your assessment, in no way shape or form should it replace the tests you do with your patient or athlete in the clinic room.

Making the decision to clear a patient or athlete for higher level activities like running or a return to sport can seem like a BIG call.
It is a big step in the rehab process but only if you haven't done the work beforehand, and your patient has not earned the right to progress to the higher levels of rehab.
Get this wrong and your patient will struggle, their symptoms will flare up and you may have to hold them back to let their symptoms settle...
Before you can get back into their rehab.
Get this wrong again and you may end up in that vicious cycle of flare up, rest, rehab, flare up, rest...
Which is highly frustrating for you and your patient, particularly if you can't put your finger on why this is happening.

The ankle is one of the most simple joints in the body and you see injuries at this joint all the time, however…
Many therapists often make a hash of their treatments and rehab, even with the most simple of ankle sprains.
One of the big issues I see is therapists missing important clues during the assessment.
During your assessment you need to gather all the important information from your patient to nail your diagnosis...
So you can be sure you are treating the right problem.
Because as good as your ankle treatments and rehab might be, if you are treating the wrong problem, the likelihood is your patient or player, will not get better.
Being able to differentially diagnose the ankle is the first step to a successful patient or player treatment plan.

What I do now is certainly very different to what I did when I first graduated.
The way I structure my patient assessments, plan rehab, talk to patients, the end stage rehab exercises and tests I use…
Pretty much everything I do is nothing like what I used to do as a new grad…
And certainly nothing like how I was taught at University!
11 years after graduating I have learnt a lot and continue to learn with every patient and athlete I work with.
I am still as passionate to get learn as I was back then, as I want to improve my skills as a therapist and continually strive to be the very best at what I do.
Looking back, most of the struggles and challenges I had as a new grad were unnecessary and if I had my time again I would do so much differently.

Think of your career journey like a car journey.
You want to get from A to B.
A is where you are right now.
B is where you want to get to (your dream job).
Whatever your career goal, I truly believe you can achieve it, if you are willing to put some work in, and if you follow the right path.
The hard work bit takes care itself.
If you are willing to dedicate some time and effort in to your learning and development this is a great start.
Following the right career path is not so easy…
Especially when you might not know how to do this.
It is not something you are taught how to do at University, so once you graduate you are left to try figure this out yourself.
This is tough and those early set-backs, like those failed placement or job applications, are a big reason so many therapists give up on their dreams so early in their careers.

‘’They got that placement because they know the physio there….
Or they only got that job because so and so said something or helped them out…’’
Right place, right time, that kind of thing.
I used to believe this myself.
But it is a myth and the biggest pile of nonsense relating to the job market…
And it is NOT the reason you can not get your foot in the door!

No matter which way you want your career to go or what your dream job is…
Making a fast start and ensuring you are heading in the right direction at the beginning of your career is vital.
However, most new grads waste the first 3-5 years of their careers, making little progress, largely because they have been told this is ‘normal’ and it is ‘what everyone has to do.’
This is not true!
Maybe you are a physio and like me, you were told that you should qualify get a job as a Band 5, go through your rotations for a few years, before deciding what area you want to work in.
Or your a sports therapist or rehabber and your currently working in a clinic or sports club, but just mainly doing massage…
You’ve been told you will start doing assessments and rehab soon, but 2 years later you are still waiting.
The best thing I did back at Uni was to totally ignore the career advice I was given.

Concussion is a very talked about subject right now, especially in sport.
Almost daily there is a story about a current athlete struggling with concussion or ex-athletes suffering long term problems, like dementia and Alzheimer's, that are starting to be linked to previous incidence of concussion.
Despite their prevalence, most new grads do not know how to manage a concussion, largely because these injuries are not covered at University.

The Acromioclavicular joint (ACJ) is a relatively simple joint.
It has very little movement…
It joins the clavicle or collarbone to the acromion (shoulder)…
It has no direct muscles attachments and only two ligaments…
Yet ACJ injuries can be tricky to manage.
They can cause persistent symptoms and are at high risk of re-injury.
They are also often misdiagnosed, usually as glenohumeral joint (GHJ) injury or even a muscular problem like an issue with the upper trap or levator scapulae.
I made a big error as a new grad with an ‘ACJ’ injury with a pro rugby player (this was not good) and I’ll tell you all about this in this episode!
I’ll also share with you 3 vital mistakes that so many students and new grad therapists make that might be stopping you finding the right shoulder diagnosis.

Connect before you correct.
This is so important to the effectiveness of your patient assessments and your ability to find the right patient diagnosis.
You need your patients to tell you everything, like their injury history, past medical history, social and drug history, so you can get to the bottom of their problem…
But they are not going to do this if they do not feel safe and comfortable in your presence.
Regardless of what injury you are dealing with, you are always dealing with a person.
Person first, injury second.
Connect first….
Then you correct.
You might think this is harder than it sounds, but it is easy to do, and so important to the effectiveness of your patient assessments…

On this episode I had another special guest as part of my success series as I was joined by Gareth Robinson, sports physiotherapist and current personal therapist to one of the world’s top golfers.
We recorded this podcast as Gareth was away in Abu Dhabi on the golf circuit and we delved into what life is like as a therapist travelling around the world working with a top golfer.
Prior to his current role Gareth has worked in professional football with Middlesborough and Darlington and rugby with Yorkshire Carnegie and the Leeds Rhinos.
He also has a vast amount of private practice experience which included managing a busy physiotherapy department in a private hospital.
He took a different route into therapy, having initially started training at college to be a bricklayer before realising he had a passion for something else.

Knowing where to start with a patients rehab programme may sometimes seem hard.
What exercises do you pick?
How many reps or sets should your patient do?
What should I be working on; strength, hypertrophy, power...?
Is bodyweight ok or should they use weights?
All these questions may confuse you and make you feel unsure about whether or not you have prescribed the right exercises for your patient.
These are all questions relating directly to rehab.
So you might think they are rehab problems.
But often they are NOT.

Hip pain can be tough to get a grip a hold of.
I’m sure you have seen those patients that seem to get better, sometimes quickly, but then just as quickly, they regress.
Sometimes getting rid of their symptoms seems to be the easy part, as no sooner do these symptoms go away, they come back as patients get back in the gym or playing sport.
The hip is also tricky because it can also be easily aggravated with things like sitting, driving and even sleeping, as well as more active movements like squatting, lunging and running.
But there is one thing that can make a big difference to hip pain and not only help you quickly take away a patients hip pain, but also help keep it away.
It’s super simple and really obvious when you know it…

One of the most common lower limb misdiagnoses made by new grad’s involves ankle syndesmosis injuries.
Often these injuries are mistaken for lateral ankle injuries like an ATFL or CFL ligament injury.
And this is a big issue…
As the management of syndesmosis injuries and lateral ankle injuries, is completely different!
It’s kind of funny that I have put together a podcast episode, advising new grads about how to not misdiagnose these types of problems…
Because I did exactly the same myself as a new grad!
One of my most memorable *%$@ ups as a newly qualified therapist was missing an ankle syndesmosis injury.

Neck pain patients with positive red flag symptoms, like pins and needles and weakness, used to get my head in a spin as a New Grad.
I used to worry that I would make this kind of patient worse...
Unsure whether or not I needed to refer this type of patient to someone else or if I was safe to start treating them.
I had a complex neck pain patient walk through my door recently, who had been in car accident and had quite significant neck pain, and presented with several red flags, alongside lower back pain and a concussion.
He is a complex case, but through a simple, structured assessment we were able to rule out any serious pathology…
And we were able to make a big change to his symptoms both in clinic and over the next few days in the very first session…
By doing some really simple things.
The same things that you could do with a similar patient if they walked in your clinic door tomorrow.

Think about going to see your doctor.
And before you even get chance to explain what is going on…
The doctor just gives you some pills and shows you the door.
You would hope this would never happen.
Clearly the doctor needs to ask you some questions and examine you to diagnose your issue and THEN…
Prescribe you with the right medication or advice for your illness.
But most therapists do this very same thing day in and day out.

Impingement is NOT a diagnosis.
I hold my hands up and admit that I used to think it was...
And worse still, I admit I gave it this diagnosis to 100's of patients and the athletes that I worked with.
Not only is this diagnosis label highly inaccurate...
Giving impingement as a diagnosis is actually detrimental to your treatment plan and results in reduced adherence to a patients home exercise programme!

Have you noticed that most injuries you see are not quite as simple or clear cut as you were told at University.

When it comes to the shoulder and special testing, the biggest mistake that I see New Grad’s make, is doing too many tests…
And worse than this…
Relying too much on special testing to make a diagnosis.
This is a big problem because most of the special tests at the shoulder are $*&£.
They have poor sensitivity and specificity and as such, are not great tests to use to make a correct diagnosis.
If you are relying too much on special tests during your assessments, finding a clear diagnosis at the shoulder, is almost impossible.

Why is it that shoulders are so difficult to treat and especially, tricky to diagnose?
The reason…
99% of New Grads (likely you too) are greatly overcomplicating the assessment.
This leads to overwhelm and uncertainty about the diagnosis…
Leaving you feeling confused, as your subjective and objective assessments don’t match up.
You may think that the shoulder is complicated and as a result you need some ‘magic’ assessment formula to find the right problem.
But it’s actually the opposite.

He had depression and I didn’t know.
We were 4 months in to a 9 month rehab programme and he had been struggling for the last 3 months.
I had spent 1-2 hours with him almost every day of the week with his post op shoulder treatment and rehab yet…
I was totally unaware of what was going on in his head.
As a health progressional I was meant to help him.
Looking back I let him down.

This worry is common but results in many therapists applying a very conservative rehab plan, sometimes even prescribing ‘rest’.
But this approach is usually not the best way and often leads to poor patient progress and results.
You need to be mindful of serious pathology and use your RED FLAG questioning to screen all patients with lower back pain.
However, you also need to know that most patients need and will benefit from your hands-on treatments and the rehab you prescribe.

The knee itself is a relatively simple joint.
It’s a hinge joint, it flexes and extends (it also internally and externally rotates) but lets’ keep it simple.
The strong muscles of the quads, hamstrings and calves support the knee alongside the passive structures like the ligaments and menisci.
As simple as knee might appear, some patients with knee pain can be tricky to deal with.
You can no doubt think of a knee pain patient you have seen before that didn’t go as you had hoped…
…maybe you struggled to settle their knee pain after a period of rest…
…maybe that patient struggled with their rehab and some of the exercises you gave them made them symptoms worse…
…maybe they went back to running and their symptoms came back…
…maybe you worry sometimes that you might miss an important injury like an ACL.
Despite being a ‘simple’ joint the knee can be challenging.

When rehab doesn't go well it is frustrating for your patient and for you as their therapist.
A common time to encounter rehab problems is when patients go back to higher-level activities like running, the gym, back to work or return to sport.
Why is it when some of these patients and athletes return to these activities they break down?
You think you have progressed their rehab well…
…you have taken them from the bed to standing rehab, added resistance or gym-based exercises…

Patients with knee pain, generally do very well, until…
They go back to higher-level activities like the gym, running or sport.
Once activity levels are increased their pain often returns, leaving the patient and the you, the therapist, frustrated.
However, if you understand the importance of the calf and its role in knee biomechanics then you will be better equipped to manage any patient with knee pain.

Knowing when to allow your patients to return to higher level activities, like the gym, running and sport can be a difficult decision.

I hold my own hands up and admit I gave ‘sciatica’ as a diagnosis commonly as a New Grad Physio.
Problem is, it isn’t even a diagnosis and more importantly than that, giving ‘sciatica’ as a diagnosis may be causing you to focus on the wrong problems, leading to poor treatment effects and ineffective patient outcomes.
In episode #17 learn why sciatica and other diagnoses like shoulder impingement, are poorly used and what you should focus your attention on instead.
This is a very different way of approaching the diagnosis of any injury but looking at your patient’s injury in this way will help you start getting better patient results.

The ability to deliver a clear, concise clinical message to your patient is key to ensure your patient fully understands the problems they have, their prognosis and how their proposed treatment plan will progress.
Regardless of how great your patient assessment, hands-on treatment techniques and treatment plan might be, all this great work can be undone if you cannot get your clinical message across well.
