In the old days, everything was on the doctor, and in those days, it was mostly physicians. Doctors did house calls, in-office care, hospital care, and everything else.
Those doctors would probably look at today's healthcare system with awe and amazement and a decent amount of head-spinning. That's because our understanding of the human brain and body and our focus on prevention has expanded greatly. There is now so much more to do for any given patient that it's nearly impossible for one person to take care of it alone. Any healthcare worker knows this just by looking at their overflowing inbox.
The obvious solution to this conundrum is team-based care. But, the term team-based care is thrown around so flippantly in healthcare that it has lost much of its meaning. Building strong patient care teams is a critical step in preparing for the future of healthcare, decreasing burnout, and improving patient outcomes.
The Sheep and Shepherd Model turns the hierarchy intrinsic to healthcare on its head and systematizes the team-based care concept. But if it were that easy....everyone would already be doing it.

This installment will come at a time where many providers may find themselves pushed into the deep end of telehealth. This is a round table interview with four medical doctors about providing telehealth. Some have just started and some have been providing telehealth services for a few years. As a disclaimer none of these conversations are meant to provide medical advice, diagnostic advice, etc. it’s surely operational and process advice only.
Synopsis: As we grapple with the COVID-19 pandemic, we have been challenged to change the way we think about how healthcare is delivered. Because of social distancing guidelines and people’s overall desire to limit their contact with others, telehealth exams have emerged as one of the most efficient ways to deliver medical care to patients without physically coming into contact with them. Telehealth is the distribution of health-related services and information via electronic communication technologies.
This podcast offers insight into telehealth visits through an interview with four physicians. These doctors explain what a telehealth exam looks like as well as the obvious and subtle differences between telehealth exams and in-person exams. They share how they handled their own skepticism during the transition to seeing patients via telehealth and how they moved to see patients who presented with more complicated symptoms via telehealth. They also share how they helped their patients deal with their own skepticism about this new way of seeking care. In short, this transition brought these providers back to the basics: trusting their gut.
Telehealth exams are an innovative way to deliver medical care to patients without the added risk of an in-person exam during a pandemic. Synopsis: We are in unprecedented territory with COVID-19, and it is crucial to remain innovative in our ability to combat it in the healthcare setting. This podcast offers insight into how our habits can help or diminish our ability to do this. Through discussing strategies around communication, scheduling decision-making, and coaching, we want you to be better equipped to combat COVID-19 in a healthcare setting.
When communicating, make it a habit to relay to people what is in it for them, and connect the communication directly to the person. Joining behaviors and outcomes can alter people’s perception of communication and will help interest them in hearing what message follows. Additionally, your discussions must make people feel supported and acknowledged. Lastly, always make a habit of communicating clearly.
After communication, this podcast evaluations scheduling decisions. Does your patient need to come in for their follow-up, or can their needs be met through a virtual visit? The habit is meeting face to face, but by making this decision with patients, we can change that habit and prioritize in-person access for groups that need it most.
Lastly, this podcast offers insight on how to coach to be successful. With the vast amount of change happening right now, staff must be coached through these changes. Once you decide to do a new process, communicate well, guide your team through the transition by leading through example, and giving feedback to ensure improvement and success

The line between integrated health and behavioral health can be a blurry line for some organizations. Hear first-hand in this podcast how Hunter Health Clinic does integrated health from Director of Integrated Care, Kaitlin Boger. Kaitlin walks us through the difference between Behavioral Health (BH) and the BH Integrated care model and its evolution over time. Additionally, how Hunter provides access to patients needing behavioral health services.
Kaitlin describes the way in which their integrated care model reduces the patient wait time between seeing the medical provider and the behavioral health specialist. Rather than seeing patients after the provider, integrated care consultants began seeing patients before the medical provider, which she realized allowed them to provide better care as they often uncover more details from each patient.

In this special Leadership Series, we interview Amy Feimer, CEO of Hunter Health, a FQHC and Urban Indian Health Program in Wichita, Kansas. We learn how Amy and her team achieved a 36% reduction in their patient cycle time (70 to 45 mins), a 99% charting completion rate, a 48% reduction in their abandoned call rate (25% to 13%) and an increased staff engagement score in the 89% percentile.
This episode highlights ways to improve both provider and staff satisfaction in ambulatory care. Amy describes the ways in which her organization improved upon their abandoned call rate, their cycle time, and their documentation process and turn around time. Additionally, we hear insights she has had on how to lead people through change and the ways in which coaching people on data can be beneficial.
Hunter Health reduced their abandoned call rate by redefining roles responsible for that rate and by using data to anticipate when peaks would require staffing shifts. Cycle time reduced nearly 35 minutes by implementing the DPI™ team dance workflow and a sheep and shepherd model of care. In this model and workflow, there is a designated lead in charge of directing workflow. This model also improved upon their documentation process as teams began charting in real-time.
Through these changes and the implementation of a validation process, the organization has jumped to the 80th percentile in staff engagement scores. Listen to this podcast to hear exactly how Hunter Health was able to implement these changes, as well as how it helped their staff.

In this podcast, we are joined by two executive leaders of an organization out of Indianapolis: Dr. Dawn Haut and Mendy Rosa. Dawn, the CEO, and Mendy, the COO, talk us through their organization’s culture change over the last few years and the way in which they, as leaders managed the change with staff. Using their ‘just do it’ mentality, Dawn and Mendy were able to improve access to their patients and become the ‘Yes’ clinic.
In addition to specific actions taken by Dr. Haut and Mendy, and their health center, we hear their methods for sustaining the innovation they have achieved. This consists of staff accountability, a transparent sharing of data, and finding leaders that can change with the culture. Both leaders also discuss the importance of preparing staff for change and tips for leaders to prepare themselves for change, such as gaining or retaining humility and leaning into the ever-changing industry.
We also hear Dawn and Mendy's favorite book recommendation, "No Ego" by @Cy Wakeman, and how they applied the book's principles to their own cultural change efforts.

In this episode, we have the privilege of speaking with Dr. Tim Spurrell, who shares with us his reflections and thoughts on how to work through burnout. He shares his journey and insights which make this episode unique because we leave the theoretical behind and get to hear a real boots-on-the-ground example.
Synopsis: In this podcast, hear directly from a provider about how to work through burnout. Tim Spurrell is a board-certified OB-GYN and a Medical Director to many clinics. Dr. Spurrell discusses how his objective is to make a lot of diagnoses, rather than one particular diagnosis while still delivering quality care.
By focusing on what he can change, Spurell found ways to combat burnout. Often practitioners are frustrated because they don’t see much of their work coming together outside of the diagnosis, and there is little connectivity within the office/health center. Combat this feeling by creating a robust team environment within the center--A team environment that is more than just a “team” in name, but rather one that can be “felt.” Spurrell created a team with his medical assistants. Not only did he feel more connected to staff and patients, but this change also helped him gain more control of his life as he had previously thought he had less power in the office than he wanted. The newly designed team dynamic between him and his assistant had him leaving the office on time, enjoying his full lunch break, and relieved him of much of the burnout feeling he was having.
Spurrell continues to discuss the ways in which he created this dynamic with his medical assistant and the areas in which he has delegated work to his assistant to minimize burnout. If you are feeling burnt out or a lack of connectedness to your practice or patients, this podcast will offer ideas and methods to combat both of those feelings.
During this installment, we wanted to provide the "moon" for encouraging people to “shoot for the moon”. In this episode, we are joined by the iconoclastic innovator that brought you Coleman Associates, Roger Coleman. Roger joins us to talk about innovation and share experiences with excellent innovation and his encounters with ‘pirate innovators.’ Though touched on in Podcast #5, Roger refreshes our memory of the four stages of innovation: catalytic change, acquired adaptability or acquired agility, preeminent performance, and inspiring innovation.
Roger highlights examples he has seen in his career including the example of Steve Jobs/Apple computing in which he has inspired innovation to drive change and witnessed success centered around patient care. Through these examples, we learn that innovation magic can happen when you have the right people with varying expertise on a team. Additionally, top management plays a critical role in refreshing the values of the organization and driving patient-centered innovation.

During this installment, we are going to review the steps to becoming an innovative health center. For those who have listened, you may have asked yourself, "What is so innovative at reducing no shows?"
Synopsis: This podcast reviews the steps to becoming an innovative health center. While many of us think about what health centers are doing that is working and try to conform to those methods, we overlook why it is so essential for health centers to try new approaches. When conforming, you conform to the expectations of today. So what happens if expectations are different tomorrow? You are out of step again.
Though change may create chaos within an organization, humans are adaptive animals, and their habits are 85% of the formula for change. To become an innovative health center, your organization must flex a new muscle and learn new practices. This podcast describes how these habits can are established gradually to ensure successful implementation in the long run.
The stages of innovation discussed in this podcast are Catalytic Change, Acquired Adaptability/Agility, the Preeminent Performance, and the Inspiring Innovation. The catalytic change is the first in a series of changes that get staff excited and moving. The second stage consists of a bunch of changes happening concurrently acquiring a new sense of adaptability. The Preeminent Performance stage comes when the organization consistently used data tracking and analysis to steer performance and future performance. In the final stage, Inspiring Innovation, we see real innovation and the implementation of new processes on an ongoing basis.
