Dr. Braude discusses the steps involved to reach this point, as well as the obstacles faced when building their PECMO program. All of this hard work resulted in the first prehospital ECMO Cannulation in the USA late last year.

Richie McHale with Geisinger LifeFlight joins us for this awesome case review of a patient he flew 5 years ago. Carbonyl Diflouride is the toxic chemical that is commonly inhaled when "huffing", a practice that is common to induce euphoric like effects. This chemical causes a massive depletion of Magnesium, and can lead to Cardiac Arrest. Join Richie while he walks us through this fascinating case, and discusses the massive amounts of Magnesium this pt required!

Today we get a chance to talk to Paul Reeves and Wayne Papalski about their SAR programs in US and Australia. Paul is an Australian search and rescue intensive care paramedic working in Broome in western Australia and Wayne is a US Navy search and rescue paramedic and a member of the ECHO military team. This was a great discussion that started out about SAR and SAR programs and transformed into a discussion of training, education and combating complacency. It was a pleasure to speak to these two and we cannot wait to do it again.
On this episode of the ECHO Rotor Wash Podcast, we speak with Flight Nurse Ryan Dale from Orlando, FL. Ryan has a fascinating background; as he flies, works in the ICU, and is a rep for Abiomed. He has some absolutely fascinating insight on the care and transport of patients that are on one (or more!) cardiac assist devices. Check it out!

2. The ability to turn down a flight for safety reasons is your right and everyone’s responsibility. This right should exist equally within everyone’s job description.

3. No matter your job title you have the absolute right to query a mission AND each other.

4. Safe operations of an aircraft is the entire crew’s responsibility. Safety is an active process, speak up.

Today we get a chance to catch up with John Hamilton of Austin Travis County to discuss how they integrate the medical crew into the cockpit and truly prescribe to making everyone in the aircraft aircrew and not just self-loading baggage. From integration into the startup procedure on the aircraft to formal training on how to manipulate the autopilot to get the aircraft on the ground in the event the pilot becomes incapacitated the training all members of the crew receive is phenomenal and truly focused creating a cohesive crew. Plus, we get a chance to hear about their new AW169 for our friends at Leonardo Helicopters. For the record we are jealous!!
ECHO Rotorwash member Joey Loehner spent 17 days in the middle of nowhere for Burning Man 2019. While running the on-site EMS system for the event, he connected with some incredible folks from around the Air Medical Industry. The ECHO Rotorwash Team felt that these folks had some awesome thoughts on leadership, the industry, and patient care. A departure from our normal content, this episode is a conversational format with some interesting members of our industry. We hope you enjoy!

Today we head back to camp to hear Dr. Ben Lawner of Alleghany LifeFlight discuss how as critical care transport professionals are often tasked with airway management in a patient who is in need of emergent resuscitation because they are teetering on the edge. Whether it is trauma, sepsis, and other critical conditions they all necessitate a heightened awareness of the risks of intubation and positive pressure ventilation. He reviews the latest literature and helps us all consider "best practices" with respect to optimizing the crashing patient with concurrent respiratory failure so they have a parachute to arrest their clinical momentum as we manage their airway.

"We did not do enough”, this statement was made by a Dispatcher in the AfterAction Report following the mass shooting at the Pulse Nightclub, Orlando, Fl. It is an expression of psychological trauma that is echoed in us all at one time or another. Intrusive memories of the traumatic event or flashbacks are hallmark signs of PTSD, and preventive strategies are lacking. This presentation will examine recent studies that indicate the therapeutic value of video games as a cognitive vaccine.

Every person has different things that motivate them to be engaged in their job and the importance of being engaged with and learning what motivates your employees (there are several different motivators, rarely is it money!). Even prior to the interview, it starts with the right high-performance culture with in your program.

Flight Paramedic Joey Loehner presents his talk entitled: Hyperthermia, Excited Delirium, and Dantrolene - A Rave New Treatment
Joey will provide a brief overview of common party/rave drugs, the effect they have on users, and types of patients we treat at our large music festivals (150,000 attendees). This will be followed by how/why we treat the way we do, evidence to support our treatment modalities, and how to effectively transport these patients. We will then discuss what can be done by Flight Crew to maximize the benefits of these treatments in the transport environment.

Flight Paramedic and Life Force Air Medical Base Educator Joshua Piotrowski presents his talk entitled: Under Pressure
After this lecture, you will have a different respect for the venous system and understand the fine balance needed between vasopressors and fluid resuscitation to return the body to a more normalized state. We need to change our perception of the heart to realize that it does not dictate
the amount of blood pumped into circulation. It is our goal to put some more balanced tools in your toolbox for treating hemodynamically unstable patients.
During this episode, we go back to ECHONooga where Rachel Tester, an expert in IFR infrastructure and John Fetzer, an experienced IFR pilot, were interviewed in regards to the new pilot track that was offered at the conference. We discussed some information in regards to ECHO and public safety aviation pilots and how we want to continue offering support and industry specific information to them.

concepts of intrinsic and extrinsic pathway trigger a migraine? Not anymore!!! Who, when and how
to reverse will be covered as well as the most important question....”How do I do that?!”
Dr. Bill Hinckley will bring his talk, Master of the Massive UGIB.
We’re experts in managing hemorrhagic shock from trauma, and we know that in bleeding trauma
patients, the care that we bring to the patient is at least as important as the rapid transport we
provide to the Level 1 Trauma Center. I get stoked about massive UGIBs in the same way that we all
get fired up to take care of that MVC patient with severe TBI and florid shock. Why? Because I know
there’s very little the accepting GI doc and intensivist have that my partner and I don’t have in terms
of definitive care for that massive UGIB patient. I know, and you will too, that in a short period of
time we can have a huge impact on that patient’s morbidity and mortality. And because I’m anosmic,
so the aroma of melena doesn’t bother me in the least. Even if you’re not anosmic, I think I can get
you pumped up about taking care of massive UGIBs too.

Natalin Hammond is the widow of HEMS Pilot Chad Hammond who was killed in the March 26th, 2016 crash along with flight paramedic Jason Snipes, flight nurse Stacey Cernadas and patient Zach Strickland. During this episode, Natalin explains the gaps in support for the air medical community when disaster occurs. We are also joined by ECHO founder and president Jon Kaminski who explains what the FAST team does and how we can better support the crews and their families. Jon also details goals that ECHO has going forward to further assist families of the fallen.

Dr. George Koenig presented at the ECHO Annual Conference in 2017 addressing trauma and the standards to what is done during the "critical times" after injury to a patient. Dr. Koenig will discuss current standards and trends that he has seen as a medical director in the prehospital environment. He is a graduate of The Philadelphia College of Osteopathic Medicine (PCOM) in 2003. He also holds a Masters degree in Biomedical Science from PCOM. He is an Assistant Professor of Surgery in the Division of Acute Care Surgery: Trauma, Surgical Critical Care and Emergency General Surgery of Sidney Kimmel Medical College at Thomas Jefferson Univeristy. He is the Associate Medical Director of JeffSTAT.
