Dentistry can be isolating—but it does not have to be.

Marc sat down with Dr. Faraj Edher on location at the BC Dental Study Club headquarters in downtown Vancouver.

Since our last conversation, the study club has grown to more than 3,500 members. But this episode is not simply about growth. It is about what becomes possible when education, community, leadership, and technology are all built around a clear purpose.

Faraj and I talk about:

  • Why teaching makes you a better clinician
  • What separates a great educator from an impressive one
  • How purpose turns competition into fuel
  • The hardest part of building any organization: people
  • Why long-term vision matters during short-term turmoil
  • Evidence-based dentistry without rigid dogma
  • Why work-life balance has to be personal
  • How AI has multiplied Faraj’s productivity

One thought stayed with me: we can only make so much of a difference with our own two hands. Education makes that difference exponential.

Episode 173 goes live Friday, July 24 at 4:00 p.m. PT.

In this episode of DentalBytes, Marc Wagner sits down with Jay Geier, founder of The Practice Growth Institute, for a wide-ranging and honest conversation about what it really takes to grow a successful dental practice.

Jay shares nearly three decades of perspective on the patterns he sees inside dental and medical practices — from overwhelmed owners and undertrained teams to poor patient experience, financial stress, leadership gaps, and the isolation many dentists quietly face.

The conversation explores Jay’s “Big Five” pillars of growth: human capital, marketing, space and equipment, clinical duplication, and financial discipline. But at the heart of the episode is one clear message: dentistry is a people business. Growth does not begin with more marketing, more gadgets, or more production. It begins with building a practice that is truly ready to serve patients well.

Marc and Jay also discuss the importance of team training, first impressions, office environment, case acceptance, mentorship, scaling beyond the owner’s chair, and why dentists need to get out of isolation if they want to build a healthier, more sustainable business.

This is a powerful episode for any practice owner who feels stuck, overwhelmed, or ready to take a more honest look at what their practice could become.

Key Takeaways

Growth starts with the patient experience.
Jay emphasizes that practices need to begin with the patient relationship and work backward from there.

Marketing only works when the foundation is ready.
Marc and Jay discuss how practices often miss major opportunities because the team, systems, and patient experience are not prepared to handle increased demand.

The “Big Five” of practice growth matter.
Jay breaks growth into human capital, marketing, space and equipment, clinical duplication, and financial discipline.

Your office environment sends a message.
From the front sign to the lobby to the front desk experience, patients are forming impressions before they ever meet the doctor.

Dentists need to stop operating in isolation.
A major theme of the episode is the danger of trying to figure everything out alone, especially while juggling debt, family, leadership, burnout, and business pressure.

Scaling requires letting go.
Jay talks about the psychological difficulty many dentists face when bringing in associates and learning to lead beyond their own chair.

In this episode of DentalBytes, Marc Wagner sits down with Flint Geier from the Scheduling Institute for a wide-ranging conversation about one of the most overlooked growth opportunities in dentistry: the front-end patient experience.

Flint shares the story of how the Scheduling Institute began, how it evolved from a cassette tape training kit into a major force in dental practice growth, and why the first phone call into a practice can make or break the patient journey.

Together, Marc and Flint explore the connection between marketing, new patient conversion, team training, leadership, systems, and the often-underappreciated role of the front desk. They discuss why dental practices can spend heavily on marketing but still miss opportunities if calls go unanswered, patient intake is inconsistent, or the team is not aligned around the full patient experience.

The conversation also dives into the “dirty word” of sales in dentistry, reframing it as ethical influence, patient education, and helping people make the right decisions for their health. Flint explains why everyone in the practice plays a role in creating trust, building confidence, and helping patients move forward with care.

They also touch on AI reception tools, the importance of combining technology with human service, and why new tools only work when they are part of a strong overall system.

Whether you are a practice owner, front desk team member, treatment coordinator, marketer, or dental professional looking to grow your practice in a smarter way, this episode is packed with practical insight.

Key Themes:
New patient conversion
Front desk training
Patient intake systems
Dental marketing ROI
Practice growth
Team alignment
Ethical sales in dentistry
AI reception and automation
Leadership and accountability
Creating a better patient experience

In this episode of DentalBytes, Marc Wagner sits down with Pat Lipovski from Envision Group for a wide-ranging conversation on what truly makes a dental practice thrive.

Pat brings decades of experience helping leaders and teams envision, adapt, and achieve more than they believed possible. Together, Marc and Pat explore the people-first side of dentistry — from leadership and team engagement to patient experience, personality dynamics, and the systems that keep a practice aligned.

The conversation dives into why strong leadership is less about control and more about developing confidence, loyalty, and purpose within the team. Pat also shares why financial incentives are often only a short-term fix, while culture, communication, emotional intelligence, and a shared practice vision create the foundation for long-term success.

They also touch on the importance of building a “practice playbook,” understanding personality types at the front desk and throughout the clinic, and creating a workplace where team members feel like they are part of the answer — not just following instructions.

Whether you are a new graduate, a practice owner, a clinic manager, or a team member looking to create a better work environment, this episode offers practical insight into the human systems that shape great dentistry.

KEY THEMES / TAKEAWAYS

• Dentistry is still a people business first. The mouth matters, but the mind of the patient — and the mindset of the team — comes first.

• Strong practices are built on two core pillars: bedside manner and systems.

• Great leadership can be measured by one simple question: who is actually following?

• Financial incentives may help temporarily, but they rarely fix deeper leadership or culture challenges.

• Younger generations want to feel heard, valued, and connected to purpose.

• A strong practice needs a shared plan, a common language, and a documented playbook.

• Personality dynamics matter — especially at the front desk, where patient experience begins.

• Emotional intelligence is not “fluffy.” It is becoming one of the most important skills in leadership, patient care, and team retention.

• “People work harder when they are part of the answer.”

• The best practices do not chase perfection. They focus on learning, growth, consistency, and steady improvement.

AI is quickly becoming one of the most important conversations in dentistry — not only from a clinical perspective, but from an operational and patient experience standpoint as well.

In this episode of DentalBytes, Marc Wagner sits down with Tyler McClendon for a live in-studio conversation about the future of AI, digital workflows, patient intake, practice efficiency, diagnostics, treatment planning, and the evolving role of technology inside the dental practice.

The discussion explores how AI may help reduce missed calls, improve patient responsiveness, support the front desk, and create more streamlined workflows, while also recognizing that dentistry remains, at its core, a people business.

The technology is changing quickly, but the practices that succeed will be the ones that use these tools to enhance the patient experience, support their teams, and create more meaningful human connection.

“Dentistry will always be a people business, the question is where technology fits into that experience.”

“AI is not replacing the front desk. It is helping support the workflow in front of them.”

“The future of dentistry is not just about technology. It is about how that technology improves the patient experience.”

“The patient experience starts long before someone sits in the chair.”

In this episode of DentalBytes, Marc Wagner sits down with Gurtej from White Coat Financial for a candid and practical conversation about the financial realities facing dentists today.

From younger dentists questioning whether clinic ownership is still the right path, to the rising pressure of debt, technology costs, lifestyle inflation, insurance decisions, and practice acquisition, this conversation cuts through the noise.

Gurtej shares why many dentists are rethinking the traditional “owner or bust” mindset, how financial peer pressure can lead to poor decisions, and why wealth management should begin much earlier than most people think. The episode also dives into practice due diligence, incorporation, insurance mistakes, the importance of reputation, and how having a clear financial plan can reduce stress and help dentists focus on what matters most: their patients, their families, and themselves.

A grounded, honest, and timely conversation for dentists at every stage of their career.

In this episode of DentalBytes, Marc Wagner sits down with Dr. Amreesh Khanna, a Calgary-based dentist, entrepreneur, educator, and founder involved in both Adept Dental Education and OraQ AI.

Dr. Khanna shares his journey from dental school at the University of Alberta, to a general practice residency in New York, to practice ownership in Calgary — including the major challenge of surviving the Calgary flood just one year into owning his first practice. That experience became a turning point, pushing him deeper into the business side of dentistry, leadership, mentorship, and data-driven practice growth.

The conversation dives into the importance of mentorship, the difficulty of implementation after continuing education courses, and why dentists need support not only clinically, but personally and professionally. Dr. Khanna also shares how his education platform helps clinicians align their personal, professional, and financial goals while building confidence chairside.

Marc and Dr. Khanna also explore the rapidly growing role of AI in dentistry, including clinical decision support, data interoperability, patient communication, medical-dental integration, privacy, compliance, and the future of AI-powered healthcare. Through OraQ AI, Dr. Khanna is helping clinicians better understand patient risk, communicate more clearly, and make more informed decisions without replacing the human clinician at the centre of care.

This is a thoughtful, forward-looking conversation about where dentistry is heading — and why collaboration, education, ethical innovation, and optimism matter more than ever.

A great conversation on this episode of DentalBytes with James Neuber of Ocean Ceramics and Dr. Bradley Bishop, Certified Specialist in Prosthodontics at the Legato Centre in Kelowna, BC.

We discussed the foundations of successful smile cases — understanding patient expectations, planning for longevity, managing function, working closely with the lab, and why comprehensive dentistry is about so much more than esthetics alone.

One of the biggest takeaways: great outcomes begin with listening.

This episode also touches on the transformational side of dentistry — the impact a smile can have on confidence, identity, and quality of life.

In this solo episode of DentalBytes, Marc reflects on the journey of building the podcast, the lessons learned from years of conversations across the dental industry, and what really drives successful practices.

This episode explores why dentistry remains, above all, a people business — from the patient experience to the front desk, from trusted mentors to the teams and partners that help practices grow. Marc dives into the often-overlooked importance of front-end staff, the value of strong recall and review systems, and how the right technology can be a true game changer when it fits the practice and the team behind it. He also shares why education, communication, and investing in people are still at the heart of long-term success in dentistry.

A thoughtful, honest episode for anyone in dentistry trying to build something meaningful while keeping people at the center of it all.

It’s PDC season—and time is flying. In this quick, last-minute catch-up, I’m joined by Dr. Bruce Ward, President of the Canadian Dental Association, and a longtime leader within the Pacific Dental Conference organizing community.

We start with what Dr. Ward has been seeing on the national stage, including recent collaboration with dental association leadership from the U.S. and Mexico, and how Canada’s Dental Care Plan is being viewed internationally.

Then we dive into the heart of it: PDC—why it remains such a uniquely balanced conference with education, exhibit-floor energy, and a social atmosphere that brings teams together. Dr. Ward breaks down what’s new for this year, including the scale of the meeting (hundreds of sessions, a sold-out floor, and thousands expected to attend) and the behind-the-scenes logistics that make the show work.

We also touch on the broader shift happening in healthcare: the growing recognition that oral health connects directly to overall health, and how collaboration across professions is becoming more real (and more necessary) than ever.

If you’re heading to PDC—whether you’re a practice owner, a team member, or a first-timer—this episode is a great primer on what to expect, what’s changing, and how to make the most of the biggest dental meeting in the country.

Robert Hawkins from Aurum Group for a practical look at why the lab remains one of the most crucial pieces of modern dentistry—and how Aurum is evolving to meet the future.

Marc and Robert unpack how the lab is more than “fabrication”: it’s a clinical partner that impacts case planning, predictability, esthetics, materials selection, turnaround, and the final patient experience. As dentistry becomes more digital, the lab’s role becomes even more important—bridging technology with real-world execution and quality control.

They also explore a bigger shift happening right now: the growing intersection of medicine and dentistry. Robert shares how Aurum is leaning into that direction—including the development of an imaging centre designed to elevate diagnostic inputs, strengthen treatment planning, and help clinicians see more clearly before they treat.

From there, they dig into how labs are streamlining workflows, improving communication, and reducing friction from intake to delivery. Robert breaks down what’s real (and what’s not yet) with emerging tech like 3D printed crowns, where ceramics still lead, and how innovation is being applied responsibly—not just for speed, but for outcomes.

You’ll also hear about Aurum’s investment in training and collaboration through the Hyo Maier Centre for Dental Excellence and hands-on “Innovation Tours” designed to strengthen digital workflows between practices and technicians. If you want a grounded, forward-looking conversation about where labs—and dentistry—are headed, this one’s for you.

Joined by Josh DeVries (CEO & Co-Founder, Intiveo) to unpack why “patient engagement” has become commoditized—and why the real win in 2026 is designing the full patient experience: acquisition, engagement/loyalty, and appointment completion.

We break down the execution gap inside practices, how to spot bottlenecks that quietly kill production, why Josh’s concept of “micro-agreements” reduces friction, and what it really takes to get patients to show up prepared (hint: it’s more touchpoints than you think). We also get practical on what Intiveo is built for—automating, tailoring, and reporting on key points in the patient journey, reaching patients through the channels they actually use (text/voice/email) , and even improving communication with non-patients like parents/caregivers and referring practices through Intiveo Connect—so practices can reduce no-shows, protect chair time, and deliver an experience patients can feel.

I sat down with Elise Poirier, a digital workflow specialist who’s lived on both sides of the fence—clinical dentistry, digital systems, and now helping bridge the gap between the practice and the lab.

In this conversation, Elise breaks down:

• Why new tech fails when training + workflow alignment aren’t built in (hello “expensive coat hanger” 🫠)
• Why labs aren’t “a checkpoint”… they’re a true partner in predictable outcomes
• And the big one: how we can run consultations in a way that builds trust, simplifies options, and creates a clear next step (instead of overwhelming patients with dental-speak).

Also… we had to talk about the Ocean team dynamic—different strengths coming together like a full-on Megazord. 🌊⚡️

If you’re building a more predictable digital workflow (or trying to tighten up patient communication), this one will hit.

In this landmark episode, Marc Wagner sits down with the legendary Dr. Howard Farran, founder of DentalTown, renowned dentist, speaker, entrepreneur, and unapologetically candid voice in the dental industry. If you’ve ever wanted to hear the raw, unfiltered truth about what it really takes to thrive in dentistry — not just clinically but financially, mentally, and spiritually — this is the episode to lock into.

Dr. Farran brings fire and wisdom to every minute of the conversation, covering everything from the psychology of fear and survival, to burnout, to the difference between dentists who make it and those who stay stuck. This is not another fluffy motivational interview. It’s a wake-up call, a roadmap, and a reality check for anyone in the dental field.

Key Topics Discussed

🧠 Fear, Scarcity & Survival Instincts in Dentistry
•Why humans are hardwired to focus on negativity — and how dentists must overcome this bias to grow.
•The tribalism and scarcity mindset that hold professionals back from seeing abundance.

🏢 Private Practice vs DSOs: Why the Debate Misses the Point
•Dr. Farran shatters the “evil DSO” myth and explains how group practices can be the best training ground for young dentists.
•Real-world breakdown of DSO demographics (most are 4–9 locations, not corporate monsters).
•The myth of “I don’t take insurance” straight out of school — and why it’s financial suicide.

💰 What Thriving Dentists Actually Do
•Why your location and demographics are 80% of your success.
•The importance of extended hours, emergency ops, and marketing funnels.
•Why too many new grads are “too good” for real experience and miss out on the grind that builds greatness.

📈 Hard Truths About Dental Burnout
•Burnout isn’t from dentistry — it’s from moral injury and being in the wrong work environment.
•You didn’t go through 8 years of school to be treated like a production robot.
•“You’re not burned out… You’ve just never been shown how to lead.”

👥 Mentorship and Moral Injury
•Why having a true mentor — someone who protects your soul, not exploits your labor — is more important than your first salary.
•How to identify toxic practices before you sign — including turnover red flags, manipulative treatment planning, and ego-driven owners.

🦷 Mastery Through Reps, Not Conferences
•Forget flying across the country to a fancy CE course if you haven’t even read the textbook.
•“You don’t learn to do implants by reading about them — you learn by screwing up the first 100.”
•The pattern of success: read the literature → practice hands-on → embrace failures → level up.

📞 Your Front Desk is Losing You Millions
•The highest paid role in every other industry is sales… so why is your receptionist your lowest paid, least trained employee?
•Why call conversion is the number one ROI lever in your practice.
•Dr. Farran’s data: DSOs like Heartland convert 4/5 calls into booked appointments — what’s your rate?

🤖 AI, Automations & the Next Frontier
•Why automating reception calls with AI is the worst mistake you can make.
•The dentist of the future will run lean teams with custom-built automation, but still invest in human connection where it matters.

This one spans the globe — literally. Craig joined me from the future (Australia!) for a deep dive into one of the most overlooked (yet critical) conversations in dentistry: exit planning.

With 25 years in M&A and a clear mission to help dentists build businesses worth buying, Craig lays out:

✅ The 11 ways to exit your practice (you probably only know 2)
✅ How to turn admin chaos into backend structure
✅ Why internal staff ownership can be a game-changer
✅ And how AI, automation, and even rollups are transforming dentistry — globally

Oh — and we also covered dental robots, co-op strategies, mental health in retirement, bolting on breweries (yes), and how to reclaim time and purpose in your career.

📈 If you’re a practice owner thinking about what’s next — whether it’s tomorrow, or 10 years from now — this is your roadmap.

In this powerful and purpose-driven episode, host Marc Wagner welcomes Wayne Rees, a long-time innovator in dental imaging and the driving force behind the VELscope oral cancer screening system. With over 35,000 units in operation globally, VELscope is transforming how dental practices detect early signs of oral cancer, dysplasia, bacterial load, periodontal disease, and even signs of sleep apnea—using a safe, blue light fluorescence system.

Wayne shares how his journey began over 35 years ago in imaging technology, leading to his current mission: helping clinicians catch disease early and save lives. From partnering with the BC Cancer Agency to collaborating with specialists like Dr. Samson Ng and Dr. Edmund Truelove, Wayne and his team have developed a next-generation VELscope featuring surgical white light, polarized light for clearer images, and seamless integration with mobile devices and second-opinion platforms like OralMedicine.com.

🔍 Key Topics Covered:

VELscope’s clinical evolution and why it’s more than just a cancer detector—it’s a visual guide to better diagnosis.
How the blue light fluorescence works by identifying non-fluorescent (dark) areas that may signal pathology.
Why annual oral cancer screening (or more for high-risk patients) is vital—and how most practices are still falling short.
The growing role of HPV (types 16 & 18) in oral cancer and how rising case numbers (60,000/year in North America) demand proactive care.

New use cases: VELscope for margin delineation during tumor removal surgery, confirmed by BC Cancer’s five-year, double-blinded study.

Real-world success stories where VELscope literally helped save lives—including cases where seasoned dentists discovered oral cancer in their patients for the first time.

The push toward AI-powered diagnostics, with collaborative efforts underway with University of Washington and BC Cancer Agency.

Be Okay with Not Okay: The Lost Art of Being Human

You ever have one of those days where your brain’s running diagnostics, but all it returns is: “System Overwhelmed – Try Snacks or Existential Reflection”?

Yeah. Same.

In a world obsessed with optimization, perfection, and productivity, sometimes the most rebellious thing you can do is… absolutely nothing. Or better yet—feel something.

Here’s the truth:

✨ Some days, you’re not supposed to “crush it.”

✨ Some days, the win is brushing your teeth and sending one email.

✨ Some days, the only thing getting you through is a walk, a meme, or yelling “I’m fine!” into the void (with questionable conviction).

And you know what? That’s not weakness. That’s being gloriously, beautifully human.

The goal isn’t to always feel good—it’s to feel real.

To be okay with not being okay. To sit with it. To feel the fog and know the sun still exists behind it.

So today, instead of striving to be perfect… try being present.

Let the cracked parts of your day breathe a little. That’s where the light gets in anyway.

In the latest episode of DentalBytes, host Marc T. Wagner sits down with veteran dentist Dr. Dennis Marangos to delve into the essential strategies for practice growth. This insightful discussion covers the critical importance of understanding your practice's numbers and building robust systems to support sustainable growth.

Dr. Marangos emphasizes that a solo dental practice can thrive with a patient base of 1,500, provided these patients receive exceptional care. But what does exceptional care entail? It means truly understanding each patient and becoming a partner in their oral health journey. This requires maintaining detailed records to cater to the unique needs and characteristics of each individual, as dentistry is far from a "one size fits all" profession.

Effective patient relationship management also involves recognizing the diverse communication styles of patients and tailoring interactions accordingly. While acquiring new patients is crucial, it only contributes to about 10% of practice growth. The real growth lies in expanding your scope of practice through continued education, embracing the latest technology, and incorporating innovative tools both in the front office and the operatory.

Dr. Marangos also highlights the importance of investing in the knowledge and skills of both yourself and your team. A well-trained and happy team is key to retaining long-term patients and ensuring their satisfaction. Viewers can sense insincerity, so it's important to be yourself and part of the community. Social media is about connecting with content, values, messages, storylines, and personalities. It's crucial to consider who you want to attract and who you're currently attracting, as well as the value you offer or how you give back to the collective. Do you have a system in place that automatically priorities tasks? Even better - Do you have a system in place that automates tasks that is based in artificial intelligence?

This is a total "no brainer"...get it? Why not let a software take care of low value tasks so that the team can handle more high level, practice building work?

View Details

The fear is that technology will take over jobs, in some cases that is true.

At the front end of a practice, there isn't enough time in the day to:

A: Identify every opportunity that exists, both internally and externally.

B: Prioritize tasks based on level of importance.

C: Manage recall to the full potential and to clearly see who needs to be contacted.

The list can go on...technology on the front end only enhances the teams ability to ultimately bring patients in and back to the practice. You may argue your system is full proof. But, if your system does not include modern-day software. You're losing opportunity.
