PBS listings can radically reshape practice and the August changes are all about precision. Rachael Babin from the Oncology Network sits down with Professor Craig Underhill to translate the latest Australian Pharmaceutical Benefits Scheme (PBS) oncology and haematology listings into what clinicians actually need: who’s eligible, what the key trials show and what to watch for once the script is written.
We start with vorasidenib, newly listed for IDH mutant astrocytomas and oligodendrogliomas. Alongside a powerful public advocacy story, we unpack the INDIGO phase 3 data and why a clear progression-free survival gain matters for patients facing long treatment journeys. We also get practical about toxicity, including fatigue and liver function abnormalities, and the reality that chronic low-grade side effects still require close monitoring and supportive care to keep people well on therapy.
Next, we move to selpercatinib for RET-mutated medullary thyroid cancer, a rare disease where “getting the target right” can be transformative. Craig talks us through the LIBRETTO-531 results, what “PFS not reached” signals, and why tolerability and discontinuation rates matter just as much as headline efficacy.
We then cover tafasitamab (anti-CD19) added to rituximab and lenalidomide for relapsed or refractory follicular lymphoma, including day unit visit burden, neutropenia risk and the size of the benefit reported.

Welcome to the latest Series of Supportive Care Matters, a podcast hosted byMedical Oncologist and International Cancer Survivorship Expert, Professor Bogda Koczwara AM.
"If it were a pill, we would all want it." This powerful opening statement captures the essence of ground-breaking research that's transforming our understanding of cancer survivorship care. The CHALLENGE Study has delivered what many considered impossible: definitive evidence that structured exercise significantly extends the lives of colorectal cancer survivors.
The results are nothing short of remarkable. Colorectal cancer patients who participated in a structured exercise program for three years after completing surgery and chemotherapy showed an 80% disease-free survival rate at five years, compared to 74% in those who received only health education materials. The results showed that structured exercise provides a significantly longer disease-free survival. Even more impressive, overall survival improved from 83% to 90% - a 37% decrease in risk. To put this in perspective, for every 14 patients who followed the exercise program, one additional life was saved.
What makes this intervention unique is its sophisticated approach to behaviour change. Participants received individualised exercise prescriptions targeting 150 minutes of moderate aerobic activity weekly, combined with regular supervision and motivational support. Exercise physiologists conducted environmental scans to identify accessible opportunities, established accountability through regular check-ins and helped participants overcome barriers to physical activity. This wasn't simply about telling people to exercise - it was about teaching them how to make sustainable lifestyle changes.
The implications for clinical practice are profound. To discuss this ground-breaking paper in detail, Professor Bogda Koczwara is joined by the Australian Principal Investigators - Professor Haryana Dhillon and Professor Janette Vardy.

In this eye-opening conversation, researchers Associate Professor Nick Hart and Dr. Andrea Smith (who herself lives with metastatic breast cancer) explore the unique challenges faced by those living with advanced cancer. While survivorship programs have traditionally focused on post-treatment care, metastatic patients are typically on treatment for life, navigating constant healthcare interactions, accumulating side effects and facing the certainty rather than just the fear of disease progression.
The discussion reveals how metastatic cancer patients have often fallen through the cracks – excluded from survivorship programs yet not appropriately served by palliative care services that focus primarily on end-of-life needs. Hart and Smith share their ground-breaking work developing the first international standards for metastatic cancer survivorship care, now translated into 14 languages to guide implementation worldwide. They emphasise that improving care requires action on multiple fronts: better education for healthcare professionals, redesigned care models, dedicated peer support programs and recognition that survivorship care is everyone's responsibility rather than the domain of any single provider.

The experts dissect the BREAKWATER trial, which shows promising benefits of combining encorafenib and cetuximab with chemotherapy for notoriously aggressive BRAF-mutant colorectal cancer. Perhaps most surprisingly, our panel discusses how an inexpensive, familiar medication – aspirin – could reduce colorectal cancer recurrence by 40% in patients with PIK3CA mutations according to the ALASCCA study. This finding highlights how molecular profiling is becoming essential across treatment stages, not just for expensive targeted therapies but also for optimising use of accessible interventions.
The discussion extends to exciting developments in pancreatic cancer with a novel pan-RAS inhibitor showing meaningful activity, and advancements in immunotherapy for MSI-high colorectal cancer, confirming combination therapy's superiority. This week we have a very special episode – we are exploring what happens when the oncologist gets cancer. Three fantastic guests share their unique, very personal, stories of cancer with us.
We start with a giant in the Australian oncology world – Professor Grant McArthur who is director of the VCCC. We hear from our friend Mark Lewis from the US who shares his experiences as a physician-patient who has multiple neuroendocrine neoplasia type 1. He is an avid Twitter user and even live tweeted his Whipple surgical procedure in 2017.
We close this special episode with an incredibly poignant and moving interview with Heidi Probst, Professor of Radiotherapy and Oncology at Sheffield Hallam University in the UK. In 2018 Heidi was diagnosed with stage 2b breast cancer and underwent surgery, chemotherapy and radiotherapy. This experience has had a profound effect on her work.

In this week's episode Rachael Babin from Oncology News Australia talks with Associate Professor Matt Naylor about his research on identifying aggressive prostate cancer and his role as a Research Integrity Advisor at Sydney University.
Matt and his team have shown that levels of the gene CBFβ are increased during prostate cancer and when they stop the gene from working, tumour growth and spread is dramatically reduced. In his latest project, Matt's team will investigate exactly how CBFβ functions and determine if levels of the gene can be used as a marker to predict if a patient will have an aggressive or slow growing form of prostate cancer.

In today's edition of The Oncology Podcast Rachael talks to Dr Michelle McDonald from the Garvan Institute about her research on bone metastasis and intravital imaging.
Dr Michelle McDonald is Group Leader of the Bone Microenvironment Group at the Garvan Institute of Medical Research. Her research career spans over 18 years, attaining her PhD in 2008. Her current research team aims to develop new strategies to target cells of the bone environment to prevent tumour growth both within bone and spreading from bone to other sites.
Using a specialised live animalimaging technique, Michelle visualises interactions between single tumour cells and bone cells, investigating how these interactions control the growth and behaviour of tumour cells in bone. Her work has recently attracted a number of competitive grants and spans collaborations with academic and industry partners internationally.

Eva Segelov is a Medical Oncologist and Director of Oncology at Monash University and Monash Health, Melbourne, Victoria, Australia. She is lead author of the paper "Practical considerations for the management of cancer patients during the COVID-19 pandemic" (pending publication) which offers guidance for cancer clinicians, focusing on areas in medical oncology affected by COVID-19, including communication and the psychosocial impact of COVID-19 on cancer patients, as well as tumour specific considerations.
Eva has an interest in academic clinical trials in GI and breast cancer and has been a member of the AGITG since 2003.
